Page 4 of 4 Memorandum To: Scott Konopasek CC: Carlos Webb, Sara Brady From: Travis Ebbert Date: 7/5/2016 Re: Voting System Incident Report Contra Costa County used a two card ballot for the June 7, 2016 Presidential Primary. Only the card with the Top Two Contests was designated to increase the total number of ballots cast. On election night it was discovered that the total number of ballots cast from the polling places (M100 scanners) were aggregating the total number of cards scanned (Top Two Cards plus Presidential Cards) and reporting that as the total number of ballots cast. After Election Night all ballots from the polling places were rerun on our central count tabulators (M650) and the discrepancy was resolved. Travis Ebbert Election Processing Supervisor
