Annual Report
Mis s io n & H i g h l i g h ts
In 2012-2013 ShelterCare programs served 1,730 individuals in our community.
ShelterCare’s mission is to shelter and support families and individuals, providing each an
opportunity to live the fullest life possible in an environment that fosters well-being and success.
Families
Our programs supported 721 parents and children
in securing housing and stabilizing their lives.
New Start: Based on our experience that one or
both parents in many of the families entering our
short-term shelter were struggling with mental
illness, we initiated this new extended emergency
shelter program. Families in New Start live for
six months at Family Housing, benefiting from
group and individual counseling as well as case
management support to assist with housing and
job searches.

Mental Health
Nine ShelterCare programs offered housing and
case management to 966 adults with psychiatric
disabilities.
Community Health Worker: In partnership with
United Way of Lane County, in January 2013 we
hired our first Community Health Worker (CHW).
The CHW has helped facilitate appropriate and
timely care for our consumers, allowing them to
better navigate the healthcare system, identify
prevention and enhance the management of their
medical and mental health needs. Our CHW
served 20 clients in the first six months.
Electronic Health Records: In March 2013, all of
our programs successfully adopted – and adapted
– to a new universal electronic health records
system. This system will allow ShelterCare to
track treatment plans, progress and outcomes of
our consumers to streamline healthcare decisions,
facilitate the coordination of services, and
improve recovery times.
Brain Injury
The Uhlhorn Program provided housing and case
management to 43 adults who are recovering from
traumatic brain injuries.
Way Finding: In the past year, the program
engaged in community partnerships with the
University and others to expand skill building and
social opportunities for residents.

In 2012-13, ShelterCare launched the quiet phase of our first-ever capital campaign.
This campaign will transform an historic timber warehouse into a hub of services
for families, clients, volunteers and staff. PIVOT Architecture’s plans for this
project are simple, sustainable, and multipurpose. Owning the facility is fiscally
prudent, too – the monthly mortgage payment is less than our former rent, and
ownership gives ShelterCare control of its financial future.
Programs that have been housed in scattered locations, such as Homelessness
Prevention and our Housing Specialists, will come under one roof at the new
location, improving our capacity to provide services to individuals and families.

Annual Report 2013
Better Together
Reflections from Susan Ban
It has been an extraordinary year of growth and change! Our partnerships grew, both in number and in the
level of investment; and our programs expanded in new directions to meet the needs of our clients. Following
the leadership of our board, long-time friends and new supporters launched our first-ever capital campaign.
Construction is now underway to re-make a former warehouse into a new centralized hub of client services,
and we expect to open the doors in the fall of 2014.
In the spring of 2013 our discussions with PeaceHealth, Trillium, Lane County Human
Services and Lane County Community Health Centers culminated in an agreement to
initiate the Homeless Medical Respite program. This pilot program will extend medical
care to homeless patients who are discharged from the hospital into ShelterCare’s shortterm supportive housing with case management.
And, we formed new partnerships through two community wide calls to action:
“I ShelterCare about Homelessness” and “Healthy Homes.”
We do this work together – and as a community, we are all better for it. Thank you.
