Some appointments with the doctor are available up to two weeks in advance; the rest are available to book on the day of the appointment. Appointments to see the practice nurse can be booked up to four weeks in advance. Please make a separate appointment for each person needing to be seen. Transplants save lives Doctor’s Appointments When you ring or call in for an appointment, the reception staff will be doing their very best to give you your choice of day and time but, as with all systems, this may not always be possible. Once all appointments have been booked, we can then only see people with an urgent problem that cannot wait until another day. If this happens and you have an urgent problem we will offer you an appointment but we will not be able to offer a choice of time. Your telephone call to the practice may be recorded. Our practice nurse has been highly trained in the monitoring of long-term conditions such as asthma, COPD (chronic obstructive pulmonary disease), diabetes, heart and vascular disease and hypertension (raised blood pressure) and sexual health checks including chlamydia screening. If you have one or more of these conditions and need a review, you can make an appointment with the practice nurse instead of the doctor. Our practice nurse continues to see patients for: • Monitoring and repeat prescribing for contraception and HRT • Contraceptive advice • Coil checks • Wound care • Cervical smears • Pessaries • Travel advice and immunisation • Immunisation (including child immunisations) • Health checks and lifestyle advice, including weight management • Ear syringing • Spirometry •Over 75 Clinic - All patients who are over 65 are called annually if they are not seen on a regular basis for a health review. •Health Checks - All patients who have not been seen by a GP in the past three years are entitled to a health check. If the practice nurse has concerns over your condition she will involve a GP in further assessment and any changes necessary to your management. Cancellations If you cannot keep an appointment, please inform us as soon as possible so that time can be used by another patient. Telephone Consultations Some problems can be resolved over the telephone. Should you wish to speak to the doctor or practice nurse on the telephone, please telephone. The receptionist will ask you for your name, date of birth and telephone number and will arrange for you to be called back when the person you wish to speak to is available. Emergencies If you are confronted by a serious problem such as acute chest pain or severe bleeding, call an ambulance (tel: 999) before calling the surgery. Out Of Hours From 6.30pm to 8.30am and during weekends and bank holidays, medical services are available for urgent medical problems that cannot wait until the following morning. This service is provided by Care UK (0345 605 9999). Registering With The Practice Anyone who wishes to register with the practice should enquire at reception in the first instance. You will be asked to fill in a health questionnaire giving details of your medical history and lifestyle. Once registered, you will be asked to arrange for a health screening examination. You may register as a temporary resident if the duration of your stay in our practice area is for less than three months. Repeat Prescriptions Prescription requests can be made by telephone from 8.30am to 6.00pm Monday to Friday. Prescriptions are normally ready for collection after 2.00pm the following day. Alternatively, you can leave the request slip at reception. If you include a stamped addressed envelope we will send your prescription to you by post. If you are housebound, the local chemist will collect your prescription and deliver your medication to your home. Clinics Antenatal The practice provides antenatal care at the Innovations Children’s Centre on Cousin Lane. Tel: (01442) 251090 Baby Clinic The baby clinic is held every Wednesday morning between 9.30 and 10.30am. You can pop into this clinic for baby weighing and advice regarding your child’s health and care. You will automatically receive an appointment for immunisations and assessments. If you are unable to attend, please telephone the surgery to arrange an alternative date. If you need this service, please telephone the surgery and listen to the recorded message. Services Available At The Practice Home Visits The practice provides a wide range of services under the GMS contract. Patients are requested to telephone before 10.00am if a visit is required that day. Emergency visits only will be arranged after that time. Please give the receptionist as much information as possible to enable the doctor to allocate priority to house calls. Please remember that several patients can be seen at the surgery in the time it takes to do a home visit. The areas covered include Atrial Fibrillation, Asthma, Cancer, Cervical Screening, Child Health Surveillance, Childhood Immunisations, Chronic Kidney Disease, Chronic Obstructive Pulmonary Disease, Contraceptive Services, Coronary Heart Disease, Dementia, Depression, Diabetic Care, Epilepsy, Family Planning, Heart Failure, Hypertension, Hypothyroidism, Learning Disabilities, Maternity Services, Mental Health, Obesity, Palliative Care, Smoking Cessation Advice and Stroke/TIA. Childhood Immunisations The current recommended scheme is: 2 months Diphtheria, Tetanus, Pertussis, Polio Hib (Haemophilus) and Pneumococcal 3 months Diphtheria, Tetanus, Pertussis, Polio Hib (Haemophilus) and 4 months Around 12 months Around 13 months When abroad, check the quality of the drinking water. If in doubt, either drink only bottled water or use water purification tablets. Avoid ice in drinks as this may have been made from suspect water. Raw vegetables, salads and fresh fruits should be carefully washed in clean water. If in doubt, stick to freshly cooked food. Meningitis C Beware of the sun! Use a high factor sunscreen especially in the first few days of exposure. Children in particular should be closely monitored in this respect. Diphtheria, Tetanus, Pertussis, Polio Hib (Haemophilus), In hot climates, drink plenty of non-alcoholic drinks. If you are not passing water regularly you are not drinking enough. Meningitis C and Pneumococcal At least two months before departure, discuss any vaccination requirements with the practice nurse. Pack a small first aid kit containing adhesive dressings, insect repellent, antiseptic cream and water purification tablets etc. Your pharmacist can advise. If you fall ill on your return, don’t forget to tell your doctor you have travelled abroad. Hib (Haemophilus) and Meningitis C Measles, Mumps, Rubella (MMR) and Pneumococcal What To Do In Time Of Bereavement 3 years 4 months to 5 years Diphtheria, Tetanus, Pertussis and Polio plus second dose of Measles, Mumps, Rubella (MMR) What To Do If Someone Dies 13 to 18 years It is a legal requirement for a doctor to confirm that someone has passed away. There is no need to move the patient. If a doctor has recently seen the patient, a death certificate can normally be issued. However, in the event of an unexpected death, the doctor will need to notify the coroner. Tetanus, Diphtheria and Polio Influenza And Pneumonia Vaccine In accordance with Department of Health guidelines, we recommend an influenza plus pneumonia vaccination for patients with diabetes, chronic heart, lung or kidney disease, residents of nursing and rest homes and people aged over 65. The vaccination is available in the late autumn; please contact reception to book an appointment. If Someone Dies At Home Laboratory Specimens 3. Collect the medical certificate from the practice. (You will be told when this is available for collection.) This will not be possible if it is necessary to involve the coroner. Specimens are sent to the hospital Monday to Friday. If you are asked to bring a specimen for the lab please ensure that we receive it before 12 noon. 1. Telephone the doctor. They will visit to confirm that the death has taken place. 2. Contact the funeral director to inform them that their services will be required. If Death Occurs In Hospital Results Of Medical Tests 1. You may be contacted by a telephone call from the doctor or nurse, or by a letter asking you to make an appointment. 2. Collect the medical certificate from the hospital. Travel Advice Then… Our practice nurse will be pleased to give advice to those patients going abroad. Please allow adequate time in order for any vaccine(s) to become effective. Certain vaccines are chargeable; please ask at the time of booking. Well before travelling abroad, check with your travel agent or the tourist office/ embassy of the country you intend to visit on any special precautions you may need to take. The death needs to be registered at the Register Office for the area in which the death took place. If possible, this should be done by a close family member who should take with them the medical certificate and the deceased’s medical card, if this is available. The Register Office operates an appointment system and, to avoid delay at this difficult time, you should telephone to make an appointment. Standards Of Care The practice is devoted to achieving and maintaining a quality health service to meet your requirements. Complaints All NHS practices are required to operate a complaints procedure. Our aim is to give you the best possible service at all times. If, however, you do have a complaint, this should be made in writing to the practice manager. To give you the most appropriate care by suitably qualified staff. To provide you with emergency care when you need it. To refer you to a consultant acceptable to you when necessary. To give you access to your health records, subject to any limitations in the law. Your Responsibilities As A Patient To treat all staff with respect and courtesy at all times. Personal Details To tell us if you are unsure about the treatment we are offering you. It is most important that you inform us as soon as possible if any of your personal circumstances change; for example, you can change your name upon marriage, have a new telephone number or if you move to a new address. Please do note that if you move to an address outside the practice area, it will be necessary for you to register with another practice in your new locale. To ask for a home visit only when the patient is unable to attend the medical centre through illness or infirmity. Freedom Of Information The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. To request such a visit if at all possible before 10.00am. To ask for an out-of-hours visit only when necessary. To keep your appointments and to contact the medical centre in advance if you cannot. Zero Tolerance To take medicines according to the instructions and to only ask for a repeat prescription if you need one. The practice supports the NHS policy of zero tolerance with regard to violence or abuse directed towards the doctor, staff or others on the practice premises or other locations where treatment may take place.
