Those who succeed will have the vision to adapt towards the future state and the ability to implement those needed changes. Jack Kolosky Board Chair Chapters Health System Chapters Health System is a great example of an organization that will do well in the future. The Chapters Health board, our CEO Kathy Fernandez and her leadership team have strengthened us so we can continue the compassionate service to so many patients and families in our region. Chapters Health leads the region in post-acute care. Post-acute care includes services that support the patient and family as they recover from illness or manage a chronic condition or disability. This includes, of course, the hospice services for which we are most known. Chapters Health has always recognized the benefits of high quality, coordinated, post-acute care for our patients, families, and our healthcare partners. Chapters Health’s role as the postacute care provider of choice will meet the needs of our changing healthcare system. Let me tell you about an example from Highlands County: A father, receiving care at the Somers Hospice House in Sebring, was still able to take a significant role in his daughter’s wedding plans. His daughter’s boyfriend first worked with Good Shepherd Hospice staff to stage his marriage proposal in the gazebo on the hospice house campus, so our patient could give his blessing 2 to the couple. Later, the hospice staff was able to manage the father’s pain to the point where he could practice some dance steps, don his tuxedo and perform a father/daughter dance at a local hotel. This allowed the family to experience that time-honored tradition by videotape at the wedding reception, even though our patient had passed away one month before the ceremony. This is just one story of the care that supports patients and families. The National Hospice and Palliative Care Organization will feature the video of the father/daughter dance in an upcoming public service campaign. Chapters Health has exciting developments on the horizon. We look forward to the progress of our state-of-the-art Lakeland Hospice House, which opened in early 2014. We will see the benefits of concurrent care for pediatric patients, which will allow children diagnosed with lifelimiting illnesses to have curative treatments, while also receiving compassionate hospice services. On behalf of our entire board of directors, it is a privilege to be part of an organization that the community turns to in their times of need. We are proud that the companies of Chapters Health System make a difference in so many lives as they face serious illnesses. C E O’ S R E P O RT As I celebrate my 20th year with Chapters Health System, I look forward to this opportunity to reflect over the past year. As you are well aware, the healthcare landscape is a fluid, shifting environment, and the many recent changes have been at paces not previously seen during my tenure. Healthcare providers continue to deal with challenges associated with decreasing reimbursement along with the implementation of new and/or re-interpreted regulations. This combination would typically overwhelm many other companies. But Chapters Health System has a history of identifying industry trends that will impact the organization, developing strategic plans to address these issues and establishing programs that keep the company strong and moving forward. At the federal level, we are seeing moves to continue to reduce reimbursement, continue to promote high levels of regulation and increasing proposals to combine services in a managedcare type model. These include plans like The U-Shaped Curve – where hospice payments will be adjusted to pay more at the beginning and end of a patient’s hospice stay and less in the middle; Related vs. Unrelated – where the amount of care considered to be “related to the terminal diagnosis and related conditions,” and therefore, covered under the hospice per diem has increased; Medicare Carve-in – where hospice care would be included in the payment to Medicare Advantage (MA) providers requiring hospices to compete and negotiate rates for that care, and in which MA providers might decide to provide variants of hospice services within their companies, limiting traditional complete hospice care. At the state level, in 2013 we directly felt the effects of some of the changes in the industry. With the rollout of the Statewide Medicaid Managed Care program, future funding for PACE (Program for the All Inclusive Care of the Elderly) and Nursing Home Diversion was discontinued, resulting in the closure of our programs. We continue to watch this implementation closely to determine its effect on our ability to continue to provide hospice services to this at-risk population. All of these state and federal initiatives threaten the ability for hospices to continue to provide the high quality, holistic and cost effective patient care that is the hallmark of the hospice benefit. These types of proposals have the potential to severely limit the choices seriously ill patients have when needing to select the most appropriate end-of-life care. We have seen this before in the acute care, home care and nursing home industries. You can be reassured that Chapters Health System is taking a leadership role, is at the table for these discussions and will continue to be at the forefront of this revolution. Chapters Health has senior leadership involved at the state and national levels with the National Hospice and Palliative Care Organization through our memberships on the Regulatory, Public Policy, and Hospice Action Network (legislative affairs) committees; the Florida Hospice and Palliative Care Association Public Policy committee; and the National Hospice Work Group, a professional coalition of executives from the nation’s largest and most innovative not-for-profit hospices. In addition, Chapters Health has a full-time lobbyist to ensure our voice is heard. Chapters Health System is taking a leadership role…and will continue to be at the forefront of this revolution. Even with the challenges presented in 2013, we have implemented programs and changes that will allow us to provide the holistic, multidisciplinary patientcentered care that will continue to be the foundation of our organization for years to come. It’s safe to say that our challenges will continue into 2014 and beyond. The Center for Medicare and Medicaid Services implemented a 2 percent across-the-board payment reduction, effective April 1, 2013, that reduced the company’s payments by $1.7 million. The state of Florida unexpectedly did not approve additional funding to expand the number of participants within our PACE program. Without the ability to expand this valuable service, the program could not become financially viable, now or in the future. Therefore Chapters Health System had to make the difficult decision to close the program. The total financial impact associated with closing the PACE program was approximately $2.8 million. Despite these challenges the total margin for 2013 (which includes investment earnings and development activity) was 4.8 percent. Even with these challenges, net assets increased by $6.9 million. Net cash provided by operations was $9.6 million. The Development Department generated $2.1 million, net of expense, from contributions and thrift store sales. I am glad to report that we continue to have a strong balance sheet and cash position. Current assets exceed current liabilities by 1.5 to 1.0. Unrestricted cash and investments represent over 237 days of operating cash requirements. Total assets are $159.2 million and net assets (equity) total $108.8 million. In 2013, the Company funded $16.1 million in capitol expenditures, including $11.0 million on the Lakeland Hospice House and related campus. As a not-for-profit health system, the Company serves hospice patients and families regardless of their ability to pay. During 2013, more than $3.3 million in services were provided as uncompensated care for this reason. In addition, the Company provides a broad range of unfunded services designed to meet the supportive needs of patients, families and others in the community. These include extensive bereavement services, caregiver services that permit patients to remain in their homes, pet and music therapy, education programs for medical students from the University of South Florida, medical fellowships and ongoing community education. During 2013, expenditures for these programs exceeded $800,000. In keeping with our goal to provide high-quality, cost-effective support for patients, families, co-workers and care partners, the Care Navigation Center improved overall productivity through the creation of systematic reporting. This resulted in reduced labor time and increased overall reporting accuracy, while allowing the expansion of information collected without adding staff. The Care Navigation Center continued progress toward a centralized scheduling model by assuming the scheduling of Good Shepherd Hospice Medical Services staff for existing patients. This process incorporated system analysis, identification of a sub-set of high-risk patients and logistics programming to enable the more rapid assignment of medical staff visits and better control travel costs, while maximizing resource utilization and proper billing standards. Other major Care Navigation Center 2013 accomplishments include: • An overall quality rating of 97.8 percent for data entry accuracy, an increase over the impressive 97.4 percent rating achieved in 2012. • The promotion of two high-performing Care Navigation representatives to other Chapter Health programs, taking with them to their new assignments the foundation of process and quality training they had received during their tenures with the department. • Maintaining a perfect record of 0 percent external turnover of Care Navigation staff, resulting in lower labor costs and higher quality performance. • Leveraging technology to create a quickreference guide for clinical staff to more quickly 10 and accurately identify team assignments, reducing errors and costly delays that come with shifting team assignments. • Care Navigation Center management regularly scheduling visits with key care partners, designed to build relationships, identify important feedback and incorporate partners’ ideas for enhancement of support into day-today processes. • Development of an in-house screening tool allowing real-time qualification of in-coming calls to best present callers with the most appropriate information on Chapter Health programs and education on potential program options. Looking forward, the Care Navigation Center is developing a comprehensive referral tracking report, in collaboration with the Chapters Health Information Technology Department. When implemented in the first quarter of 2014, the realtime Referral Tracker Report will allow multiple program managements to see the patient referral progress – from the time of referral receipt through the submission of an admission package – via the web. Also in development is the Clinical Transitions Report, which is a time comparative of a patient’s individual health factors. Chapters Health will use these factors to track a patient’s evolution and to support program decision making. This report has a target implementation date of the second quarter in 2014. The staff and managers at the Chapters Health Care Navigation Center take great pride in these 2013 accomplishments. Our driving force is to support our patients from the first moments of contact and to continue that support via the Chapters Health System programs and services that deliver care in our communities. The department values its partnerships with co-workers and care partners, striving to provide outstanding customer service for patients and families coping with complex medical decisions. PA L L I AT I VE C A R E In 2013, Chapters Health Palliative Care welcomed its new medical director, Dr. Mary Alfano-Torres. Dr. Alfano-Torres is board certified in Family Medicine and Hospice and Palliative Medicine, and comes with extensive experience providing palliative care services in hospital and hospice settings. Throughout the year, the staff of Chapters Health Palliative Care worked to extend partnerships with care providers by developing and presenting a new model contract for shared savings with select current hospitals and a new hospital. In addition, Chapters Health Palliative Care began a complex data analytical project with BayCare Health System, which was modeled after a national study. The purpose of this project is to demonstrate the cost savings garnered by the provision of palliative care in the hospital setting. Chapters Health Palliative Care provides palliative care services in Hillsborough and Polk counties for adult and pediatric patients coping with complex medical conditions. Overall, the division saw a 19 percent increase in patients served, providing 5,536 palliative care visits for 1,814 patients in its two-county service area. In 2013, physicians and nurse practitioners completed 3,105 visits with adult patients at St. Joseph’s Hospital in Tampa and 285 visits at St. Joseph’s North in Lutz. The palliative care adult service line at Florida Hospital Tampa grew from 280 visits in 2012 to 565 in 2013, greater than a two-fold increase. In Polk County, at Winter Haven Hospital, adult palliative care visits saw a greater than three-fold increase, growing from 26 visits in 2012 to 84 visits in 2013. Pediatric palliative care services can provide parents and children with comfort and answers during difficult times. Through its partnership with St. Joseph’s Children’s Hospital, Chapters Health staff completed 167 pediatric palliative care consults for a total of 912 visits, resulting in a more than two-fold increase. A key element in the future of palliative care is the ability to offer services in the outpatient setting. Once again, Chapters Health Palliative Care is a leader in developing programs to meet patients’ needs by completing 584 adult visits in nursing homes, assisted living facilities, clinics and patients’ homes. With its expertise in addressing the issues of this patient population, Chapters Health Palliative Care will be at the forefront in delivering this emerging and important level of care. 11 P H A R M AC Y In 2013, the Pyxis® automated dispensing system was installed at each of our five hospice houses, a multi-department project that was successfully completed over the course of the year. The Pyxis® system allows staff members at all the hospice house facilities to provide medications to patients through the use of automated dispensing cabinets. Pharmacists conduct a clinical review of each order received and then authorize the usage of all medications via an interface that connects the dispensing cabinets to the pharmacy’s computer system in Tampa. The automated medication dispensing units are designed to improve patient safety, track the usage of controlled substances, increase nursing staff efficiency and help prevent medication dispensing errors. A pharmacy efficiency initiative was implemented in 2013; this involved multiple changes in the pharmacy formulary, delivery schedule alterations and medication refill changes, all resulting in greater efficiencies while maintaining the highest quality of medication management. The efficiency initiative resulted in a cost savings of over $500,000. The per patient day medication costs for home care patients, decreased by 9.5 percent in 2013. Pharmacy completed a major upgrade to the Guardian dispensing software in the first quarter of 2013 with the implementation of the Order Management Module. This upgrade allows pharmacy to transmit and receive inventory and purchase data electronically from the wholesaler on a daily basis. In addition, all medication orders faxed, emailed or telephoned can now be stored electronically, as part of the patient profile charts within the Guardian system eliminating the need for printing and storage of paper documents. 12 Chapters Health pharmacists participated in hospice interdisciplinary group meetings on a regular basis. These meetings allow pharmacists to provide medication reviews at the team level, resulting in optimal patient-focused care. Over 1,438 recommendations for improving patient therapy were made in 2013 as part of these medication reviews. The Chapters Health Pharmacy Services staff filled approximately 156,000 prescriptions for LifePath Hospice and Good Shepherd Hospice patients in 2013. As part of the overall Silo Bend refurbishment, the size of the pharmacy’s facility was increased in 2013 to accommodate current and future needs. S TA F F I N G In 2013, Chapters Health Staffing Services provided specialized care to the patients of LifePath Hospice, Good Shepherd Hospice, Palliative Care and the Pediatric Partners in Care program. By the end of the year, Staffing Services employed 19 full-time, part-time and pool personnel who worked across Hillsborough, Polk, Highlands and Hardee counties to maximize efficiency and ensure a consistent level of care. This Staffing Services team includes physicians, nurse practitioners, a director of research, infusion nurses, respiratory therapists, chaplains, social workers and certified child life specialists, plus speech, occupational and physical therapists. In the past year, Staffing Services met an average of 94 percent of all subsidiary staffing requests. The majority of our employees are hospicecertified and all have certifications and/or licensure in their specialty areas. They also provide education and training about their specific areas of expertise to other Chapters Health employees. This team approaches the present and the future with a positive outlook and works together to provide seamless services throughout the Chapters Health post-acute care system. In the coming year, Staffing Services will continue to consistently meet the needs of a growing company by hiring highly qualified specialists who are determined to find new and better ways to care for those dealing with advancing age and/or illness. In March, the annual LifePath Hospice Golf Tournament was a sellout! Chair Herb Bell and his committee worked Island Night Chair Rick Lentz encouraged guests to open their hearts and open their wallets when sponsoring campers for Camp Circle of Love. 16 tirelessly to ensure that all participants enjoyed a memorable day on the links at the TPC. This outing generated more than $66,000. The Island Night Sponsor Preview Party was held in April at the new Cooper’s Hawk Winery and Restaurant. Guests enjoyed an evening full of memorable music, American Bandstand dancers and a couple of Mad Men look-a-likes that helped set the stage for the 2013 theme of “O What a Night.” In September, more than 75 guests enjoyed the Island Night Patron Party at the lovely home of Tate and Blake Casper. The evening included a special presentation to the Calvin Reed Family in recognition of their financial support. The Reed Family Foundation made a generous gift in memory of Calvin’s late wife, Charlotte P. Reed, who passed away under the care of LifePath Hospice. Island Night festivities culminated on September 29 for the 22nd annual fundraiser at Jackson’s Bistro. A record crowd of more than 600 guests attended the party, which was sponsored by SunTrust, with many dressing the part for the 1960s “O What a Night” theme. Chair Rick Lentz announced the event generated a recordbreaking $223,000. An impressive 66 percent of all LifePath Hospice employees helped provide unfunded and underfunded services by supporting the 2013 annual Employee Giving Campaign. The theme of the campaign, “Ignite the Light,” was very well received. Employees had an opportunity to view a video featuring their coworkers who shared their reasons for contributing. As a special thank you, employees who made a gift received a small pin featuring a solitary candle, symbolizing their commitment to ignite the light. A new fundraising event set sail in May to benefit LifePath Hospice and Suncoast Hospice: The Tampa Bay Regatta. Hosted by the Davis Island Yacht Club, the event featured a sold-out auction/ dinner and a full day of competitive racing. After the regatta, the DIYC presented a $15,000 check to LifePath Hospice, which represented one-half of the proceeds from this inaugural event. Margaret Dufney, Pat Linton, Connie Boos, Neva Reed and Ellen Reed at a Hospice Women of Philanthropy networking event. The Hospice Women of Philanthropy continued to grow its membership. The group held several recruitment gatherings and now includes 61 members. Their annual education meeting featured Kathy Fernandez, CEO and President of Chapters Health, who spoke about “The Business of Hospice.” The group funded its first project, a video for Camp Circle of Love, which had its debut at the Muvico Theater in Centro Ybor. In addition, the video was honored as a finalist for a “WEDU Be More Award” in the “Be More Relevant Best Use of Video” category. One hundred thirty six volunteers wrapped more than 4,000 gifts at gift wrapping stations placed throughout the Citrus Park Mall for 12 days in December. In December, the annual Lights of Love observances were held at the LifePath Hospice South Tampa office, the Sun City Center Hospice House and the Melech Hospice House in Temple Terrace. The leaf is in memory of his late wife, Dr. Gwendolyn Stephenson, who received end-of-life care from LifePath Hospice. This year, the third location in Brandon Staff and volunteers celebrate at the 2013 ribbon-cutting for Life’s Treasures Thrift Store in Brandon 18 The Gap provided the additional “muscles” needed to stock the store during the holidays. The hard work from volunteers and staff helped all three Life’s Treasures locations reach a record in gross sales – nearly $1.3 million. In February, models walked the runway at Christ the King Catholic Church wearing stylish outfits at the third annual Fashion Show and Boutique. More than 200 guests were treated to great shopping at the Marketplace Boutique, an incredible lunch and the fashion show, featuring 20 LifePath Hospice volunteers. News Channel 8 anchor, Gayle Sierens, returned to serve as celebrity commentator, with the event raising more than $17,000 to support LifePath Hospice. opened its doors in September. More than 80 pre-opening volunteers logged close to 1,700 hours to get the store ready. Staff and volunteers at Life’s Treasures in Brandon didn’t let a shortened year hold them back. By year’s end, more than 100 volunteers gave more than 6,000 hours and accepted 241 donations. The staff of Life’s Treasures thrift stores and the Chapters Health Development Department thanked volunteers at the second annual Volunteer Appreciation Luncheon in May. More than 100 volunteers attended the gathering, where they had an afternoon of food, fellowship and fun. Life’s Treasures in Carrollwood celebrated five years of supporting LifePath Hospice. With more than 120 men, women and high-school students logging over 17,000 hours, the store was able to process contributions from a record 1,400 donors. This year, the Carrollwood location partnered with The Red Hat Society and Aston Gardens for fashion shows and provided designer jewelry, stylish accessories and fashionable clothing for sale at Island Night. Life’s Treasures thrift stores said goodbye in 2013 to two special friends – volunteers Mary Raines and Edward “Ed” Schwartz. Mary began volunteering at the South Tampa store in 2005 after her husband passed away under LifePath Hospice care. Since 2009, Ed volunteered at the Carrollwood store, where he joined the team that keeps the store’s book section in precise order. Life’s Treasures is extremely thankful for Mary and Ed’s contributions. The original South Tampa location of Life’s Treasures boasted 132 volunteers in 2013, who donated nearly 20,000 hours of support. That dedicated team processed items from 1,300 donors. The South Tampa store benefited from the support of many community donors, including items donated by The Chislers and the Old Seminole Heights Great Neighborhood Yard Sale. In addition, employees from TECO and The staff and volunteers at the three Life’s Treasures locations work tirelessly in support of the LifePath Hospice mission. A group of dedicated women met for lunch for the first time in early 2013, where they discussed ways to strengthen hospice-related programs through the power of their joint contributions. That initial meeting led to the formation of the Hospice Women of Giving. Charter members Margy Allen, Linda Brummer, Pat Couturier, Denise Crenshaw, Sylvia Falcon, Anna Clare Green, Laura Hawley, Pat Sanchez, Betty Miller, and Cindy Sternlicht held recruitment events throughout the year, growing the giving circle to 25 enthusiastic members. Through an effort chaired by member Judy Joiner, the Hospice Women of Giving has expanded into the Winter Haven community and has continued to attract new members to this vibrant group. Also in March, Florida’s Natural Charity Classic teed off at the Lake Wales Country Club. Ten Good Shepherd Hospice volunteers donated their time at the LPGA Symetra Tour event, with tournament proceeds benefitting Camp Brave Heart. In March, the Gibbons Pathway Society luncheon held its inaugural luncheon at the Lakeland Yacht and Country Club, where 16 members heard a presentation from Good Shepherd Hospice physician, Dr. Brendan Flynn. Dr. Flynn discussed why he became a hospice physician and shared meaningful stories from patients and their families. The Gibbons Pathway Society is a planned giving membership for donors who have Through their gifts to the Shining Star program, patients and families honored 133 Good Shepherd Hospice employees in 2013. The program Denise Crenshaw and Suzette Jones share generated more than $41,000 laughter at the Hospice Women of Giving by recognizing employees’ reception at the Lakeland Yacht Club extraordinary commitment to quality patient care. Staff members received special recognitions and hospice teams celebrated their accomplishments. In April, long-time Lakeland resident Stephanie McLean joined the Good Shepherd Hospice Development team. Stephanie’s major focus is building the relationships that will secure ongoing support for the new Lakeland Hospice House. The new Lakeland Hospice House construction inspired a group of local community leaders to form the Lakeland Hospice House Community Advisory Committee. The committee provides advice and counsel to Good Shepherd Hospice staff on issues relating to the early 2014 opening of the Lakeland Hospice House. The group hosted a wellattended community educational breakfast, as well as holding a variety of small events at various locations to educate the community on the services and purposes of Good Shepherd Hospice. Several members took part in an educational video, providing moving testimonials on their personal hospice experiences. Walsh by Cindy Sternlicht, Ken Walsh and Mike Walsh Volunteer Services Office In Memory of Jim Green by Anna Clare Green and Family 28 CONTINUED To keep the community up to date on the progress of the Lakeland Hospice House, Good Shepherd Hospice published a bi-monthly newsletter, filled with educational information, inspiring stories and interesting facts. The newsletter was mailed to approximately 20,000 residents in the Lakeland area, helping to raise community awareness of this important project. Good Shepherd Hospice employees once again set a record for shining bright by giving to the 2013 Employee Giving Campaign. More than 76 percent of all employees made a gift to help underfunded and unfunded services, which include patient care, hospice house care, and bereavement programs. Staff connected with the theme of the 2013 campaign, “Ignite the Light,” and the campaign’s launch included a video appeal from co-workers who shared why they individually ignite the light. Each employee who contributed received a collectable pin as a token of appreciation. Harvest for Hospice, the annual Good Shepherd Hospice fundraiser in Sebring, brought out the community to the Chateau Élan in October to support the Bud and Donna Somers Hospice House. Two weeks before the event, event chairs Suzette and Cliff Rhoades hosted an exclusive patron party at their Lakeview Drive law office. The night of the event, more than 300 guests enjoyed entertainment from the Hot Sun Quartet and sampled delectable treats from some of the area’s finest restaurants. The year came to a close with the annual Lights of Love ceremonies at the Bud and Donna Somers Hospice House in Sebring, the Forsythe Hospice House in Auburndale, and for the first time, at the new Lakeland Hospice House. This popular event brings the community together to remember and honor loved ones with music, remembrances and a moving tree-lighting ceremony. All of these events are important to the success of Good Shepherd Hospice. They provide a critical opportunity to engage our constituents, educate the community about hospice care and provide financial resources for unfunded patient and family programs and services.
