How to get your mail-in rebate:
1) BUY at least one (1) case of any of the eligible products listed.
2) REDEEM ONLINE at diversey-cbrebates.com or MAIL completed Rebate Form with copies of dated proof(s) of purchase or Distributor velocity reports for full cases of eligible products
purchased between April 1, 2015 and June 30, 2015, postmarked no later than July 31, 2015 to:

Offer good in the United States only and is valid only on the products shown on
the rebate form. Offer expires 06/30/2015. No Minimum Rebate. Maximum rebate
$500.00. Allow 6-8 weeks for delivery of your rebate. Maximum one location
per rebate request. Each Commercial End-User location may submit up to two
rebates but may not exceed the maximum rebate amount. Requests from
individuals, groups, clubs or organizations will not be honored or returned.
Alteration or attempted alteration, transfer, sale or purchase of distributor invoices
or velocity reports is prohibited and constitutes fraud. Fraudulent submissions will
not be honored and may be prosecuted. Please make a copy of your rebate
submission for your files. Diversey and or its Distributors are not responsible for
any non-complying Commercial End-User Rebate submissions or for lost, late,
illegible, postage due or undeliverable mail. Non-complying End-User Rebate
submissions will not be honored and may not be acknowledged or returned.
Limit: Two rebates per location, as allowed by law. Not valid in combination with
any other offer. No Cash Value. Void where taxed, restricted or prohibited. Offer
not valid on cases purchased for resale, partial cases, purchases made by
non-commercial end-users, by direct buying Diversey customers, or purchases
bought under Diversey’s contract programs.
