INPATIENT SERVICES u family guide Welcome to ChildServe! Our organization has a rich history of serving children and young adults for more than 80 years. Through ChildServe’s inpatient services, we care for children with intensive rehabilitation, complex respiratory, palliative care, and comprehensive medical needs. We serve children and young adults from birth through 21 years of age, and we work to promote a great life for each child while they are with us. ChildServe’s Stelter-Fisher Transitional Care Unit is a bridge between hospital and home, offering your child a place to recover following an illness, injury, or surgery. Our staff are experts in the field of pediatrics, and they are dedicated to providing you with education and training you will need to be successful at home. Our equipment and facilities are specifically designed for children and young adults, and we pride ourselves on offering child-focused, family-centered care. From our therapy gyms, aquatics center, family room, and toddler room, we are confident that your child will benefit from ChildServe’s nurturing environment. Originally founded in 1928 as a convalescent home for children, today ChildServe provides specialized pediatric health care services to more than 2,500 children in Iowa. Through the support of private insurance, public funds, and the generosity of donors, ChildServe offers accessible, family-centered care unique to each child’s needs, hopes, and dreams. tessa’s story Walking may be a simple activity for most adolescents, but for Tessa it was a challenge. Tessa has Spina Bifida and also suffers from a congenital hip deformity which made walking very difficult and painful. If her condition went untreated, she would have eventually lost the ability to walk altogether. Because of this, Tessa underwent acetabuloplasty surgery, a procedure in which the edge of her hip socket was built out to improve stability and keep her hip from dislocating. Such a complex operation required significant rehabilitation, which Tessa received at ChildServe. She arrived in a full hip/leg cast and once it was removed, she received intensive inpatient occupational and physical therapies. ChildServe’s therapists worked with Tessa to regain her strength and improve her balance so that she could receive the full benefits that her surgery afforded her. After eight weeks of focused treatment, Tessa left ChildServe walking on her own, an activity that she and her family will never take for granted. Our Approach Child-Focused, Family-Centered Care At ChildServe, you will be involved in every aspect of your child’s care. ChildServe staff will partner with you to develop goals and plans to meet those goals. We are committed to helping your child achieve the best quality of life possible. We know that “quality of life” means different things to different people, so we work to fulfill the goals of your child and family. Pediatric Focus ChildServe knows that children and teens have needs that are different from those of adults with similar conditions. All of our equipment is designed specifically for children, and every member of our staff has specialized training to understand children’s growth and development. Training and Support for the Transition Home In addition to the care of your child, staff provide one-on-one training for your child and your family members so you can learn how to manage your child’s needs after discharge. Qualified staff help arrange for equipment and technology needs at home, ensuring that you will be more prepared to care for your child. clap-out: A ChildServe tradition! Our Inpatient Services Pediatric Rehabilitation ChildServe’s pediatric rehabilitation services help children and young adults restore function following an accident, illness, or surgery. Our therapists have extensive training and experience in working with pediatrics, ensuring that your child will receive treatment that is developmentally appropriate for him or her. ChildServe offers comprehensive rehabilitation services, including: • • • • • • • • • Physical Therapy Occupational Therapy Speech Therapy Recreational Therapy Child Life Services Aquatics Music Therapy Adaptive Equipment Rehabilitation Nursing Our experienced team of therapists serves a wide range of rehabilitation needs, including: • Acquired brain injury (traumatic and non-traumatic) • Orthopedic rehabilitation (traumatic and post-surgical) • Multiple fractures • Spinal cord injury • Stroke • Burns/wounds • Spasticity • Feeding disorders Complex Respiratory Did you know that even children with respiratory equipment may benefit from aquatic therapy? ChildServe’s inpatient respiratory services offer a continuum of care for children with complex respiratory needs. Our respiratory therapists specializing in pediatrics provide 24-hour specialty care for a wide spectrum of respiratory illnesses. ChildServe’s team works with primary-care physicians, pulmonology subspecialists, community providers, and other specialists to create personalized care plans that will meet the unique needs of each child. Our respiratory services provide: • All levels of ventilator support, both invasive and non-invasive • Sophisticated monitoring systems to best track respiratory status • All modalities of respiratory care including, but not limited to, tracheostomy care, vest therapy, inhaled medication, and airway clearance therapy • A focus on individualized respiratory therapy and social interactions • Ventillator weaning services for appropriate patients Palliative Care ChildServe’s pediatric palliative care offers a total approach to care for children, embracing physical, emotional, and spiritual elements. Palliative care is appropriate at any time during an illness and can be provided at the same time as treatment that is aimed to cure. The palliative care team’s purpose is to: • Advocate for the child and family • Collaborate with the child’s providers • Help the family make treatment choices • Help the family find support ChildServe’s unique facility, staff, and expertise are particularly suited for children with the following illnesses: • Life-threatening conditions for which curative treatment may be feasible but can fail • Conditions where early death is inevitable but where there may be long periods of intensive treatment aimed at prolonging life allowing participation in normal activities • Progressive conditions without curative treatment options • Irreversible but non-progressive conditions with complex health care needs leading to complications and likelihood of premature death Comprehensive Medicine ChildServe’s inpatient comprehensive medicine services are designed to support children who need additional time to recover following a hospital stay. Our dedicated staff will work with you to establish an individualized care plan that will support the specific needs of your child. Whatever situation your child is facing, we can provide the medical support, training, and transition planning to help your child reach his or her potential. Once he was stabilized at the hospital, Cayden was non-responsive, unable to eat, speak, or interact. Because of the severity of his injuries, Cayden’s expected chance of recovery was grim. Cayden came to ChildServe for inpatient rehabilitation following his discharge from the hospital. After assessing him, ChildServe’s therapists and staff knew that he had the potential to regain his lost skills. They committed themselves to connecting with Cayden in spite of his injuries and limitations. Involving his grandparents in his treatment, ChildServe’s therapists used the bond between Cayden and his family to help him make strides in recovery. His treatment at ChildServe included physical therapy, occupational therapy, speech-language pathology, child life, recreational therapy, pet therapy, and aquatic therapy. This comprehensive approach to Cayden’s care allowed him to learn to walk again, talk again, and begin eating again. Through the love of his family, dedication of staff, and determination of his spirit, Cayden went home after intensive inpatient rehabilitation. Your Health Care Team At ChildServe, many people will play a role during your child’s recovery. Each member of your child’s health care team will be an important source of information and support. They wil bring their specific expertise to the treatment planning proces and will help you learn skills to meet your child’s needs. Health care team members may include: Child and Family You and your child are the most essential members of your child’s health care team. Active participation in treatment and rehabilitation is critical to the recovery process and will help your child successfully return home and to the community. Medical Director Our medical director provides leadership to our clinical teams and oversees the quality of care that will be provided to your child. Pediatrician(s)/Nurse Practitioner Pediatricians will oversee your child’s admission and treatment plan while at ChildServe. ChildServe provides 24/7 physician coverage, and your child’s pediatrician or other specialists are welcome to provide oversight during your child’s stay as well. Pulmonologist Pulmonologists, or physicians who specialize in the field of pulmonary medicine, will treat disorders relating to the lungs and respiratory system. Nurses Nurses will deliver care to meet your child’s daily needs including administering medications, feedings, and treatment. They have experience in rehabilitation and coordinate care with your child’s heath care team. Access Specialist Our access specialist will work with you through the admissions process and will communicate with all parties involved in your child’s care to ensure accuracy of information upon admission. Medical Social Workers Medical social workers will work with you to develop a discharge plan for your child. They will also assist in identifying and connecting you with community resources. Care Coordinators Care coordinators will work with you and your child to coordinate the plan of care from admission to discharge. Your Health Care Team Adaptive Equipment Technicians Adaptive equipment technicians will work with you and your child to find all of the equipment and supplies needed with onsite customization. Aquatics Coordinator Our aquatics coordinator oversees the aquatics center located on ChildServe’s campus which may be used to help your child participate in functional activities under the supervision of therapists or other trained staff. Child Life Specialist Child life specialists will work with your child to reduce anxiety and help to gain an understanding of his or her medical condition, procedures, and treatment. Dietitian Our dietitian is a food and nutrition professional who will work with your health care team to coordinate medical and dietary needs. Music Therapist Music therapists will use music creatively to help your child address social, emotional, or physical difficulties. Occupational Therapists Occupational therapists (OT) will help your child with upper extremity functioning, fine motor skills, and sensory motor integration. Orthotist Orthotists will design, develop, create, and fit custom braces to maximize your child’s physical function. Physical Therapists Physical therapists (PT) will help your child with strength, balance, coordination, and gross motor skills. Recreational Therapist Recreational therapists will help your child integrate skills learned in other therapies and apply them to leisure activities. Respiratory Therapists Respiratory therapists (RT) will help your child if he or she has breathing or respiratory difficulties. They will work to improve your child’s ability to cough, breathe, and resist infection, and will manage ventilator support when necessary. Speech Language Pathologists (Speech Therapists) Speech language pathologists will help your child with swallowing, oral motor skills, cognitive-linguistic skills, and communication. They will also work to identify alternative forms of communication when necessary. seth’s story Seth and his brother went out for a ride on the family’s ATV Ranger on Seth’s 13th birthday. During the ride the ATV flipped, landing upside down and rolling. Seth was life-flighted to the University of Iowa Hospital where it was determined he had a skull fracture and a brain bleed. Seth underwent surgery that evening, receivi ng 80 stitches and two drains of blood from his brain. Seth came to ChildServe after his stay at the hospital, unable to walk or talk. During his stay, he received intensive physical therapy, occupational therapy, speech-language pathology, child life, and recreational therapy. After four weeks of treatment, Seth left ChildServe walking and talking, wanting to get back to running track, fishing, and golfing. “When w e first go it was a li t here tt le overwhe We’d nev lming. er he suddenly ard of ChildServe , we were here thin and king, ‘Wow, all of these people a going to re take care o f u s.’” -Seth’s M o m, Anne What to Bring • Insurance cards • If you are not a parent, please bring legal guardianship documents • Clothing: 7-10 outfits for your child Your Child’s Stay Visitors General guidelines for visitors: • Regular visiting hours are from 8 a.m. to 8 p.m. Parents or primary caregivers are welcome at any time of the day. • People with colds, sore throats, or any contagious diseases should not visit the children. • Visitors may not consume alcohol in ChildServe facilities or on ChildServe premises. • A child over the age of 16 can visit on his/her own; anyone under 16 must have adult supervision. r o f s d r a rew : k r o w d har More than one therapist has gotten a pie to the face courtesy of children who met their therapy goals! we’re serious about fun. Thinking outside the box is something we do on a daily basis, especially when it means creating great life for a child. When Teddy’s favorite computer game was released while he was still in arm casts, our recreational therapy team and Electronics Specialist developed a pedal system so Teddy could play the game with his feet. Parking Parking is provided at Entrance B for visitors to the Transitional Care Unit. Overnight Stays Up to two parents or guardians may remain overnight with your child. You may come to see your child at any time during the day or the night to accommodate your schedule. No one under the age of 18 may stay the night. Please inform the nursing staff in advance (by 9 p.m. if possible) of your intent to stay. Private rooms provide a fold-out couch, and double rooms provide a recliner which may be used for sleeping. Meals The dining room (Spirit Cafe) is open to families and visitors. The cost of meals is nominal, but only cash and check are accepted (no credit cards). Vending machines are also available. Roommates/Changing Rooms Some of our children share rooms with another child, so great effort is made to match roommates based on compatibility. Every effort is made to keep moves to a minimum, and you will be notified prior to any planned move. Personal Items Please make sure that all personal items, such as clothing, blankets, and toys, are labled with your child’s name. ChildServe is not responsible for the security of personal items. Furnishings ChildServe will furnish each room with a bed, storage closet/dresser, and window coverings. ChildServe will also provide night stands, bulletin boards and TV’s in each room. Linens will be furnished and laundered by ChildServe. Any furniture addition to a child’s room must be approved by the ChildServe staff prior to use. Clothing Parents and families are responsible for providing clothing during their child’s stay at ChildServe. ChildServe has laundry services available to do all of your child’s laundry, including personal clothing and linens. ChildServe can not do laundry for family members. Toiletries ChildServe will provide basic toiletries to each child throughout his or her stay including toothbrush, deodorant, tissues, and hair care products. You are welcome to supply different toiletries. Yo ent the e xtra mile u -Katlin’s !” mom Breyten, born three months premature and diagnosed with Noonan Syndrome and chronic lung disease, spent the first nine months of his life in the neonatal intensive care unit at a local hospital. When he was stable enough to be discharged, he still required 24-hour nursing care as well as intensive therapy to help him grow strong enough to be weaned from the ventilator. Breyten’s parents brought him to ChildServe’s Transitional Care Unit, where he stayed while he recovered from two separate surgeries and utilized therapy services to build his strength. During that time, his parents learned to care for his tracheostomy, feed him through a g-tube, maintain his ventilator, handle his pulse oximeter alarms, and assist in his therapy. By the time he was discharged, Breyten was strong enough to successfully make the transition home—and had already gotten to know his homecare nurses and outpatient therapists. breyton’s story What Will My Stay Look Like? Program costs are authorized and funded through commercial insurance and Medicaid. Specific fees will be discussed during the intake/admission process. WHERE ARE SERVICES PROVIDED? Inpatient Rehabilitation Services are provided on the Stelter-Fisher Transitional Care Unit and Inpatient Therapy Wing at the ChildServe Center in Johnston. Other facilities on the Johnston campus may be utilized if it will be beneficial for treatment. WHEN AND HOW OFTEN ARE SERVICES PROVIDED? The frequency of any and all of the services listed above is dependent upon the specific needs of each child/young adult. Physical, occupational and speech therapy frequencies are determined by individual assessments and vary from one service a day to three services twice a day up to 6 days a week. WHO PROVIDES MEDICAL MANAGEMENT SERVICES? All children/young adults will have a primary physician or nurse practitioner to provide medical management. Families have the choice to select their own provider or utilize providers that routinely provide medical management at ChildServe. Consideration for discharge from inpatient rehabilitation will be based on the following criteria: • Plan of care (treatment) goals are achieved • Child no longer requires a daily therapy service • Progress towards goals has significantly slowed or stopped • The child’s violent or agressive behaviors pose a significant danger to self, other children, and/or staff and cannot be safely managed by ChildServe staff • The physician determines that services are no longer required HOW CAN MY CHILD ACCESS CHILDSERVE’S INPATIENT REHABILITATION SERVICES? Referrals to ChildServe can be made by a discharge planner, physician, case manager, or parent/guardian. To make a referral, contact our Access Specialist at 515-331-8106.
