Despite the development of neonatal intensive care and use of modern
generation antibiotics mortality and morbidity from neonatal sepsis remains high,
particularly in the very low birth weight infants. In this group it remains around 18 to
20% in the most advanced countries. In fact in the extremely low birth weight
infants it has slowly crept upwards.
There are many reasons for this continued rate of mortality and morbidity.
These include immunological immaturity of the preterm infant, our inability and
uncertainty in diagnosing signs and symptoms of sepsis early. Plus the lack of
highly sensitive and specific biomarkers and our own inadequate management
practices.
Added to these difficulties is the increasing incidence of microbial resistance and
lack of development and availability of newer antibiotics.
Given the above, and our better understanding of the simultaneous activation of
pro-inflammatory and anti-inflammatory mediators in sepsis, researchers,
clinicians have attempted to develop adjuvant therapies exchange transfusion,
use of
haemopoeitic stimulating factors but what has been most fruitful has been the
use of various immune-modulatory therapies that work by regaining the deranged
immunological balance is sepsis.
This presentation after briefly reviewing the epidemiology, pathophysiology of
neonatal sepsis will discuss the evidence on the use of immune-modulatory
therapies in neonatal sepsis.

Autoinflammatory syndromes
Sanaa A. Mahmoud, MD, PhD
Clinical Professor of Pediatrics and Allergy and Immunology (2015- 2020) at the University of
Pikeville, Kentucky College of Osteopathic medicine Allergist/Immunologist at the Allergy Asthma Center , Somerset,KY
Part time: Neonatlogist: McLaren McComb, Mount Clemens, Detroit, Michigan
In the last 10 years, a new category of immunologic disease, known as
autoinflammatory diseases, has
been created to account for disorders that did not fit well into the classical disease
groups. The recent recognition of a new class of immunologic disorders known as
autoinflammatory diseases has changed the way we look at pathologic
inflammation and allowed for a better understanding of the mechanisms
regulating the innate immune system.
Although patients with these disorders can present with classic rheumatologic symptoms,
including inflammation
Involving the joints, skin, muscles, and eyes, the underlying inflammatory
mechanisms do not involve autoantibodies
or antigen-specific T cells. Some patients also display episodic or precipitated symptoms,
including urticaria and conjunctivitis, similar to those seen in allergic patients, but there is
no evidence of IgE-mediated inflammation.
Recurrent fever is a common feature of both autoinflammatory disorders and infections
caused by immune deficiency.
Autoinflammatory disorders are primarily driven by the innate immune system and
include rare inherited syndromes and more common and genetically complex
inflammatory diseases.
Understanding of the cause of inherited autoinflammatory syndromes has led to
effective targeted therapy with significant clinical effect. However, Colchicine is still the
standard of care for FMF and is used widely around the world. IL-1 is one of the central
mediators of autoinflammatory diseases and is the target of 3 approved biologic
therapies, including Anakinra, Rilonacept, and Canakinumab.
This disease category has now expanded from the original rare, inherited, recurrent fever
syndromes to include much more
common diseases. Translational research concerning the single gene
autoinflammatory disorders has not only improved our knowledge of pathophysiology
but has rapidly led to improved diagnosis and therapy of the patients with these diseases
and holds promise to improve future treatment. This presentation with briefly highlight
pathogenesis, clinical picture, diagnosis and management of some of those disorders

RSV prophylaxis in preterm babies
Medhet Ezzat
Consultant Neonatologist and Honorary senior clinical Lecturer, Aberdeen University
Bronchiolitis is the most common acute lower respiratory tract infection of infancy
and causes approximately 3% of all infants to be admitted to hospital 1. The hospitalisation rate for bronchiolitis is higher in certain, including those born prematurely 1.
Prophylaxis with a humanised monoclonal antibody (Ig) to the virus associated
with most cases of bronchiolitis (Respiratory Syncytial Virus, RSV) is licensed in the
UK for use in high risk infants. The approval was based on the IMPACT study2
which demonstrated that RSV-Ig reduced the relative risk for a hospitalisation with
bronchiolitis by 55% among ‘‘high risk’’ infants and by 78% among infants born
prematurely without bronchopulmonary dysplasia (BPD) when compared to placebo. The absolute risk for a bronchiolitis admission associated with Palivizumab
treatment is 5%1. Despite this promising intervention against a common and
sometimes serious infection, a consensus statement and systematic review recommend that RSV-Ig prophylaxis should be confined to ex-premature infants with
BPD 1 3. Additionally, the UK Department of Health states that prophylaxis is indicated in some
Infants born up to 34 weeks gestation
(https://www.wp.dh.gov.uk/immunisation/files/2012/07/chap-27a-dh_130131.pdf)
.The question “should Palivizumab prophylaxis be given to ex-premature infants
without BPD?” remains unanswered although in practice treatment is not given in
this scenario.
As part of an annual audit of bronchiolitis admissions to our unit we have collected details of infants admitted between the winters 2004/5 and 2010/114. Here
we determine the number of prematurely born infants who did not develop BPD
who would require Palivizumab prophylaxis in order to prevent a single bronchiolitis
hospitalisation. We also tested the hypothesis that ex-premature infants without

The World Health Organization (WHO ) declared on Monday 18 January, Zika virus
infection which spread rapidly, to 21 countries in the Americas since May 2015, is
due to the prevalence of Aedes aegypti mosquito.
The mosquito-borne Zika virus, which has been linked to brain damage in
thousands of babies in Brazil, is likely to spread to all countries.
WHO director general, Margaret Chan called Zika an “extraordinary event” that
needed a coordinated response.
She advised pregnant women to consider delaying travel to areas affected by
Zika and to seek advice from their physician if they are living in areas affected by
Zika, as well as protect themselves against mosquito bites
The incubation period (the time from exposure to symptoms) for Zika virus disease
is not known, but is likely to be a few days to a week.
The most common symptoms of Zika include fever, skin rashes, conjunctivitis,
muscle and joint pain, malaise, and headache
These symptoms are usually mild and last for 2-7 days.
Zika virus usually remains in the blood of an infected person for a few days but it
can be found longer in some people.
Prevention can be done by using insect repellent-, wearing clothes (preferably
light-coloured) that cover as much of the body as possible using physical barriers
such as screens, closed doors and windows; and
It is also important to empty, clean or cover containers that can hold water such
as buckets or flower pots , so that places where mosquitoes can breed are
removed.
The CDC now is advising pregnant women to consider delaying travel to
Barbados, Bolivia, Ecuador, Guadeloupe, Saint Martin, Guyana, Cape Verde and
Samoa. Those regions have been added to the Level 2 travel alert that already
includes Brazil, Colombia, El Salvador, French Guiana, Guatemala, Haiti,
Honduras, Martinique, Mexico, Panama, Paraguay, Suriname, Venezuela and the
Commonwealth of Puerto Rico.

Neutrophil counts should be carefully evaluated in premature and critically-ill
neonates. Although neutropenia is usually benign and runs a self-limited course in
most neonates, it can be prolonged and constitute a serious deficiency in
antimicrobial defense in some infants. We will try to highlight on the causes ,
diagnosis and treatment .

It is noticeable that food allergy is on the rise and is becoming of a concern to
the practicing pediatricians. Factors responsible for food allergy in children are
poorly
Understood. One possible contributing factor is the early use of medications, e.g.,
antacids that are liberally prescribed early in life for symptoms of acid reflux
Excipients are all of the substances found in pharmaceuticals other than the
active ingredients. These substances are added to “aid the manufacturing
process, to protect, support or enhance stability, or for bioavailability or patient
acceptability.”1 Some pharmaceutical excipients are foods or, more often,
substances derived from foods, raising the possibility that these substances would
pose a hazard to patients with food allergy
In most circumstances these medications should not be routinely withheld from
patients who have particular food allergies because most will tolerate the
medications uneventfully. However, if a particular patient has had an apparent
allergic reaction to the medication, potential allergy to the food component
should be investigated.
This presentation will provide attendants with a recent update on some of those
studies

Neonatal Septic Shock
Khalid N. Haque
FRCP (Lond, Edin, Ire), FRCPCH, MBA, DCH, DTM&H
Professor of Neonatal Medicine
Every year around 1.6 million newborn infants die from sepsis of these between
1-5% will develop septic shock with an associated mortality reported to peak at
around 71%.
Infection reflects a continuum from foetal inflammatory response syndrome to
sepsis, severe sepsis, septic shock and multi-organ failure and death. The difficulty
for the clinician is to recognise precisely the phase the neonate is at any given
moment as the neonate may move from one phase to another imperceptibly
thus making the diagnosis of shock difficult. However, our teaching has been that
“patients do not suddenly go into septic shock, it is the healthcare professionals
who suddenly notice it”.
Recent work has shown that neonates with septic shock demonstrate reduced
expression of genes representing key pathways of innate and adaptive immunity.
They demonstrate a predominantly down-regulated transcriptome that may
account for why more neonates develop septic shock than any other age group.
Whilst, genomic studies have improved our understanding of the pathophysiology
of septic shock in the new born, similar advances have not been made in
diagnosing septic shock early which is the hallmark of good management
leading to better outcome.
American College of Critical Care Medicine have up-dated their Paediatric
Advanced Life Support Guidelines for Management of Paediatric and Neonatal
Septic Shock emphasizing on early recognition, use of antibiotics, fluids, inotropes
and possibly hydrocortisone within the first hour of diagnosis. This will be discussed.
In this presentation I will discuss the genomic-based pathophysiology, clues to
early diagnosis and suggest a care bundle for the management of neonatal
septic shock.

Modifiable risk factors for preterm brain injury:
implication for neuroprotective
Medhet Ezzat

Neonatal Pain; Myths and Facts

The International Society for the Study of Pain defines pain as “an unpleasant experience, sensorial and emotional, associated with tissue damage, actual or
potential, described in terms of their damage.”
Newborn infants, particularly those born preterm, regularly are subjected to a multitude of diagnostic and therapeutic procedures that are painful but medically
necessary to their care also it was found that infants born at 25–42 weeks gestation experienced an average of 14 painful procedures per day during the first
2 weeks of life. The most frequently performed procedures were nasal, endotracheal, and nasopharyngeal suctioning followed by heelstick and intravenous and
nasogastric tube insertions.
Compared with older children, neonates exhibit greater hormonal metabolic and
cardiovascular responses to invasive procedures. Thus, pain management not
only makes the neonate’s pain more comfortable, it may also improve outcome.
Sedation masks the clinical response to pain but does nothing to minimize pain
and its physiological and metabolic effects.

Start healthy, start now” is a wake-up call for all of us and those whom we take
care of. it’s an invitation for a healthy lifestyle with a plan to have 30 minutes
every day for doing some moderate exercise, to keep your body mass index
within the proper limits, quit smoking for the sake of yourself and your dear
contacts at home, stick to healthy diet rich in vitamins and minerals, organically
grown vegetables and fruits without the use of fertilizers, chemical farming or
irradiation, and strike a balance in your diet to be 2/3 plant stuff and 1/3 animal
products with antioxidants and probiotics. a show of the super-healthy foods will

Weaning update
Ahmed El Beliedy
Professor of Pediatrics
Head of the Pediatric Department

The nutritional status of children and mothers in our regions, as regard micro- and
or macro-nutrients are far beyond being satisfactory: micronutrients’ deficiencies,
macronutrients’ deficiencies as well as
overweight and obesity are so common . Such deficiencies can have a negative
impact on children later in life.
Nutritionally-speaking the first 1000 days, from conception till the age of 24
months, are very crowded and extremely important. They are so vital to ensure
proper growth and development, represent a critical window of opportunity to
impact the life-long health, and are an ideal time when epigenetics can work.
While all of us know the importance of encouraging breastfeeding since birth,
the importance of adequate complementary feeding of infants is increasingly
acknowledged. Complementary feeding is defined as the process starting when
breast milk alone is no longer sufficient to meet the nutritional requirements of
infants, and therefore other foods and liquids are needed, along with breast milk.
The transition from exclusive breastfeeding to family foods – referred to as
complementary feeding – typically covers the period from 6 - 24 months of age.
Unfortunately, the feeding practices of our infants are toddlers are suboptimal. In
this talk, the major nutritional problems/challenges at this vulnerable age group
will be addressed and a tentative plan of complementary feeding will be
outlined.

Dilema’s in the diagnosis of neonatal sepsis
Khalid N. Haque
FRCP (Lond, Edin, Ire), FRCPCH, MBA, DCH, DTM&H
Professor of Neonatal Medicine
As the signs and symptoms of neonatal sepsis are non-specific clinicians and
researchers have tried to find individual or group of biomarkers that could
diagnose neonatal sepsis early, accurately and as quickly and cheaply as
possible. This quest is of greatest relevance in neonatal medicine as neonates
as a group have the highest rate of infection compared to any other group save
elderly
immune-compromised patients. Finding such biomarkers is also important in
neonatology because for every one neonate with sepsis thirty babies are
prescribed antibiotics leading to antimicrobial resistance.
Conventional biomarkers like white cell count; C-Reactive protein, pro-calcitonin
etc, are useful but have either poor sensitivity or specificity. With advancement in
biochemical and genomic research, more sophisticated classes of biomarkers
are being identified. Researchers are seeking ‘novel’ and or specific biomarkers
that not only diagnose an individual disease such as sepsis but also monitor disease activity and give an indication of prognosis. With increasing advancement
in technology it is hoped that in the very near future biomarkers will likely be highly
specific, universally available and universally clinically applicable.
After discussing the criteria for an ideal or a group of ideal biomarkers and
reviewing the conventional biomarkers in routine use including the use of various
cytokines and chemokines this presentation will highlight the advances in
molecular diagnostic technologies such as genomic, proteomic technologies
and the use of Flight Mass Spectrometry to study metabolomics ending by
suggesting a panel of test that is currently most likely to be useful to most
clinicians who do not have access to genomic or proteomic technologies.

Is it gut or the brain?
Sanaa Shaaban

The task of the circulation is to supply the tissue with oxygen (recognised by Pflüger
as long ago as 1872)
Haemodynamic monitoring is therefore monitoring of whether the circulation is
performing its task of supplying oxygen to the tissues.
If it is, no further intervention is necessary.
If not, haemodynamic monitoring is required for diagnosis, treatment of the
disorder and monitoring the effect of therapy.

Enteral nutrition in newborn
Fluid Is A Drug “Fluid Overload In Critically Ill Children”
Mahmoud Tarek
Adel Reyad
Professor of Pediatrics - Benha University
Professor of pediatrics, Faculty of medicine, Ain Shams University
Intravenous Fluids remain one of the mostly widely used and abused medical
therapies. Intravenous fluids should be managed as DRUGS that have widespread
systemic effects. As with other drugs, intravenous fluids should be used with the
correct ‘DOSE’.
Pediatric intensivists should be alerted that fluid accumulation a potentially
“TOXIC” phenomenon that influences patient outcomes.
Fluid overload has been shown to be associated with worse outcome in critically
ill patients (e.g. mechanically ventilated patients, acute lung injury, sepsis &
septic shock, ...etc)

No population has benefited more from the development and advancement of
specialized nutrition support than pediatric patients.
•Today, neonates comprise the largest group of pediatric patients receiving
parenteral nutrition (PN).
•Nutrient needs of neonates differ substantially from other populations,
presenting unique challenges in optimizing nutrition care. •Continual reassessment and delivery of adequate nutritional support are imperative in order to
reach the
optimal growth rate for premature infants.
