All returns must receive a RETURN MATERIALS AUTHORIZATION
(RMA) number from the DENTSPLY Canada Customer Care Department
prior to shipment. This RMA number must be clearly noted on the outside
of the shipping carton. If products are shipped without an RMA, the
product will be shipped back to the customer Returns must be sent to:

1. An RMA # will be provided once the following documentation is faxed
to Customer Care Dept:
- Proof of Purchase
- Serial#, lot# and/or expiry date (where applicable).
2. All products returned for credit or exchange must be of current design
in the original, unopened, sealed package and in sellable condition.
Damaged, opened, outdated, obsolete, used, “demo” products, or
products and parts purchased more than 6 months prior to the current
date are not returnable for credit or exchange.
3. A copy of the original invoice must accompany all product returns for
credit or exchange. Consumables must have
corresponding Lot and expiration dates. All Equipment must have
serial number referenced on invoice.
3. Any credit applied will be based on the original invoice price.
4. No discontinued products will be accepted for return.
5. Return Authorization and credit will not be issued for overstock/
inventory relief.
6. Products (except equipment) returned after 30 days are subject to
a minimum 20% restocking charge. Products (except equipment)
will not be accepted for return credit or exchange beyond 6-months
from the original invoice date. Equipment can be returned within 30
days of invoice date for credit, but is subject to a 20% restocking fee.
No unopened equipment will be accepted for return credit or exchange
beyond 30 days of invoice date.
6. Final approval for replacement or credit is at the sole discretion of
DENTSPLY Canada
7. R.M.A. # will be valid for a period not to exceed 30 days from the date
of issue. Merchandise received after expiration of the R.M.A. # will not
be accepted for credit.
8. Transportation charges both ways must be paid by the customer or
distributor on products, parts or accessories
returned for credit or exchange, regardless of the warranty period.
9. Opened packages of all equipment cannot be accepted for credit by
DENTSPLY Canada. Upon evaluation if equipment can be repaired,
standard repair and repackaging charges will be applied based on
product received. repair charges exceed warranty coverage, customer
will be notified and approval must be received before additional
repairs are made. There is a 90-day warranty on the repair parts and
labor for qualifying small equipment.
10. Custom, imprinted, made-to-order, or customer modified products are
not returnable for credit or exchange.
11. All product complaints are to be reported to DENTSPLY Canada’s
Customer Care Department with all relevant product information (i.e.:
Item#, Lot# and Expiry Date).
12 Any parts or products which fail in service after date of installation will
be repaired and returned in accordance with the terms of the product
warranty and require a Service Work Order Number.

Patient Selection
MTM® Clear•Aligner Service Center is recommended for patients who require minor orthodontic adjustments
to the anterior “Social Six” teeth. It can be used for making cosmetic enhancements, correcting orthodontic
relapse, preparing for restorative procedures and/or finishing other orthodontic treatments.
MTM® Clear•Aligner is indicated for the treatment of anterior tooth malocclusions in patients with permanent
dentition (i.e. all second molars). Utilizing a series of incremental minor tooth movements (torque, tipping,
rotation and bodily movement), MTM® Clear•Aligner sequentially positions teeth by way of continuous gentle
force.
It’s an ideal choice for patients who desire a quick, efficient and esthetic solution to their orthodontic needs. It
addresses the common minor anterior misalignments seen in up to 50% of the U.S. adult population**.
