The User ID and password that you use for logging into the HealthTeam network will vary, depending on your location and the device you are using; however, you will always use your unique Centricity EMR User ID and password to log into Centricity EMR 9.2. If you have difficulty logging into either the HealthTeam network or Centricity, call the Health Information Technology (HIT) Help line at (517) 355-6531. Logging into Centricity EMR 9.2 1. With a network connection established, double-click on the Centricity EMR 9.2 icon on your computer Desktop. The Centricity EMR 9.2 icon has a white background, with a blue picture of a woman and child on it text “Centricity EMR” directly below. 2. The login screen opens, where you will enter your Centricity EMR User ID and password. Your Centricity EMR ID typically consists of the first 6 letters of your last name, followed by the first 2 letters of your first name. 3. Type your password in the Password field. 4. Tab to the Location of Care field, or click directly in that field. The system authenticates your ID and password, then provides the Location options to which you have EMR access privileges. Select the appropriate Location of Care for your current working session. If you work in more than one HealthTeam location, be sure to verify that the correct Location is selected each time you log in. TIP: If your password becomes highlighted in blue after you’ve tabbed to the Location of Care field, you have entered it incorrectly. 5. Click OK , or use the [Enter] key of your keyboard. Enter your User ID and Password, then select the appropriate Location of Care. NOTE You must enter both your User ID and password correctly before you are given Location of Care options. Once your ID and password have been authenticated, the Locations of Care for which you have access rights become available. At that point, be sure to select the Location of Care appropriate for the current work session. Michigan State University EMR 2 Exiting Centricity EMR 9.2 On the far right side of the Centricity EMR 9.2 toolbar is the Exit button. When you are done working in Centricity EMR, click the red Exit button to log out. This logs you out of the EMR and takes you back to the login screen. To complete the logout process entirely, click the “X” in the upper right corner of the login screen. SECURITY Do not ‘minimize’ your Centricity session when walking away from the computer. Either lock the screen by using the Exit button, or log out entirely. Turning off the power to your computer monitor does not meet security and confidentiality standards, either. Also use the Exit button if you want to keep your Centricity EMR session active, but need to be away from your computer temporarily. Clicking the Exit button locks the screen, therein prohibiting unauthorized access to the system. This protects your own Centricity EMR identity, while at the same time safeguards the confidentiality of HealthTeam patients. Changing your Centricity 9.2 Password For security purposes, occasionally you may be required to change your password. When doing so, the new password must not closely resemble your name or your Centricity EMR User ID. 1. With a session active, click the Exit button, located on the far right side of the Centricity EMR toolbar. 2. Click the New Password… button. 3. Enter your existing password in the Current Password field. 4. In the New Password field, type the password that you want to use from this point forward. 5. Type the new password once again in the Confirm New Password field. 6. Click OK . 7. Click OK again to return to your Centricity EMR Desktop, or click Exit to close out of the program. Michigan State University EMR 3 Returning to your Desktop If you lock the screen by clicking the Exit button during an active Centricity EMR 9.2 session, then want to return to that session, first verify that your Centricity EMR ID appears in the User ID field, then type your password in the Password field. Click OK , and the program will open to your Desktop or a particular patient’s chart, depending on where you had been in the program when you initially exited, and if there is a checkmark () in the Return to [xxx] field. PASSWORD POINTERS Your password must contain both numbers and letters, and be eight or more characters in length. Centricity EMR 9.2 passwords are case-sensitive, so pay close attention to your use of upper- and lower-case letters. When changing your password, the new one must be different from your four most recently-used passwords. Michigan State University EMR 4 CENTRICITY EMR 9.2 DESKTOP YOUR DESKTOP’S COMMON FEATURES Your Desktop Summary is the first screen you see when beginning a Centricity EMR 9.2 session. At the top of the screen is the Title bar, in blue. Beneath the Title bar is a menu bar, and directly below that is the Centricity EMR 9.2 toolbar. Use the menus and toolbars to quickly navigate the various screens and their components. Title Bar Menu Bar Toolbar The Centricity EMR 9.2 Desktop Summary Title Bar The Title Bar displays your name and location, as well as the current date and time. Before documenting patient information, make sure that your name appears in the title bar, and not that of a colleague. If another name appears in the title bar, click the Exit button, then log in using your own Centricity EMR User ID and password. Also, if you work in more than one HealthTeam location, make sure that the location displayed in the title bar is appropriate for your current session. If it is not, click the Exit button, then log back in under the Location of Care. Menu Bar The Menu Bar offers several functions throughout Centricity EMR 9.2, and the options in the menus vary, depending on the where you are at in the program. A few of the options available in the various menus are: Actions | Find Patient… Ctrl + F Options | Quick Text… Help | Help on this Screen F1 Actions | Alerts/Flags Options | Display… Help | What’s New Options | Out of Office Assistant… Help | Centricity Library Ctrl + P Actions | Print… Ctrl + P Michigan State University EMR 5 Centricity EMR 9.2 Toolbar Use the toolbar to move from one area of Centricity EMR 9.2 to another. The most frequently used buttons on the toolbar are displayed below. The Desktop button; click this to return to your Centricity EMR 9.2 Desktop, where you can view your Flags, Documents, or Appointments. The Chart button allows easy access to patient charts. If you’ve not yet viewed a patient chart in your current EMR session, the Find Patient window opens when you click the Chart button. If you’ve already been in one or more patient charts during the session, clicking the Chart button returns you to the patient’s chart you were last in. Click the to see a list of your ten most recently-visited patient charts. Click the Appts button to navigate to the full Appointments window, where you can view multiple books concurrently, an entire week’s worth of appointments for a specific book, etc. Use the New button to begin a new Care Alert or Flag. (Care Alerts and Flags allow for communication between HealthTeam Centricity EMR users, in a secure environment). Print the day’s appointments, Flags, and other documents using the Print button. The options in the Print window vary, depending on the window you are in, and the object you have selected, when you click the Print button. Use the Help button to access the online Centricity EMR 9.2 Help utility. Click the Exit button to end your Centricity EMR 9.2 session, or to temporarily lock the screen. TIP Think of your Centricity EMR 9.2 Desktop as home base. Then, if you become disoriented while working in the program, just click on the toolbar’s Desktop button to return to home base (your Centricity EMR Desktop), readjust to the Centricity EMR environment, then continue your session. Michigan State University EMR 6 Today’s Appointments on your Centricity EMR 9.2 Desktop Summary If you are a scheduling Provider, your Appointments are displayed in the left column of your Desktop Summary. Open the patient’s chart directly from the Appointments section of your Desktop by double-clicking on that patient’s name. See the reason for the patient’s visit by single-clicking on the patient’s name in the Appointment area, then clicking the View Details button. Use the up and down arrows in the upper left corner of the Appointments windowpane to change the time increment displayed (15 minute intervals, 20 minute intervals, etc.). A black dot appears in front of a patient’s name in Double-click on an appointment to go directly to that patient’s chart. your list of Appointments when that patient is ‘arrived’ in your clinic. You can get a better look at your Appointments, or those of other Providers in your clinic, by clicking the Appointments button on the Centricity EMR toolbar. Using the Appointments feature is discussed in more detail later in this documentation. (See Viewing Appointments, page 29.) NOTE: The Appointments appearing in Centricity EMR 9.2 are driven by what is scheduled in Centricity Business; you cannot add, alter, or remove scheduled appointments directly from within the EMR. The Desktop Summary’s Right Windowpane – Alerts and Flags The upper right portion of your Desktop Summary page provides you with a glimpse of your Care Alerts and Flags, which are messages, much like email, sent between you and other HealthTeam Centricity EMR users. Alerts and Flags that you have not yet viewed are displayed in bold font, and those that you create and save as a reminder to yourself appear in italics. See Care Alerts and Flags (page 9), to learn how to use these tools to enhance your Centricity EMR 9.2 experience and facilitate communication in your clinic and HealthTeam-wide. Use the buttons to work with your Alerts and Flags on your Desktop. Double-click on a specific Alert or Flag to view it in its entirety. Care Alerts have a red “+” in front of them. Care Alerts and Flags can be prioritized as Normal, Important, or Urgent. Michigan State University EMR 7 The Desktop Summary’s Right Windowpane – Documents The lower right portion of your Desktop Summary page is devoted to patient-specific Documents. A few examples of documents are lab reports, transcription, phone notes, and office visits. Desktop tools help you manage your documents. Click on a column header to sort Documents using different criteria. A red ! indicates that a document requires your immediate attention. The document status appears in the far left column; a pencil means that a document needs to be signed. By default, documents on your Desktop are sorted first by Priority, placing those that are urgent at the top of the list. However, you can change the order in which they appear by clicking on any given column’s heading. The illustration above has documents sorted by date; hence, a small, grey arrow () appears in the Date column header. You can Open, Append, Route or Remove a Document from your Centricity EMR Desktop; use caution, though, when removing Documents, especially if they still require your attention! Document Status Classifications The status of a document tells users at what juncture of processing that particular document is at any given time. Status Meaning In Progress A Centricity EMR user is working in the document. Other users should not open the document until it is no longer considered In Progress. On Hold The document - a chart update - is waiting to be completed and signed. Example: a nurse begins documenting an office note, then routes the note to the Desktop of the Provider seeing the patient. When the document arrives on the Physician’s Desktop, it should be marked On Hold. Unsigned A document that has been imported, such as a transcription or lab result, has yet to be signed by its Responsible Provider or a different, appointed Centricity EMR user. Signed The document has been signed by a Centricity EMR user and is a permanent part of the patient’s chart. Signed A preliminary signature has been applied to the document, but a final signature is still needed. This designation is commonly associated with documentation done by Resident Physicians and Physician Assistants. Deciphering the Document Status TIP While the Documents Summary tab lets you see appointments, flags and documents in a single glance, use the full Appointments, Alerts/Flags, and Document Windows to get a comprehensive view of their respective components. This is especially important in clinics where the Comments field is used extensively during the document routing process. Michigan State University EMR 8 CARE ALERTS AND FLAGS Care Alerts and Flags are much like email, but they are maintained within the confines of Centricity EMR 9.2, so you can use them to communicate patient information with your colleagues in a secure, confidential environment. You can send Alerts and Flags to, and receive them from, anyone in the HealthTeam who has a Centricity EMR account. Alerts and Flags can be easily replied to, or forwarded to, other Centricity EMR users, as well. Viewing Care Alerts and Flags To view an Alert or Flag on your Desktop Summary, double-click directly on it; a split window opens, with your Alerts and Flags and their properties listed in the top, and the content of the selected message revealed down below. If the Flag that you double-click on is attached to your Desktop, the View Flags window opens in the foreground, with your Desktop in the background. If you double-click on an Alert or Flag that is attached to a patient’s chart, the View Alerts/Flags window opens, with the patient’s chart in the background. Only Alerts and Flags attached to that patient’s chart are displayed. A Care Alert attached to a patient’s appointment will open with that appointment in the foreground, and the scheduling provider’s Appointment book in the background. This Care Alert is attached to an appointment for Walter Caldwell, scheduled to be seen by Dr. Winston. When you double-click on the Alert from you Desktop, you are taken to Dr. Winston’s Appointment book, opened to the day of Walter Caldwell’s appointment, with that appointment outlined in yellow. Michigan State University EMR 9 The Alerts/Flags Tab To get the most comprehensive view of your Care Alerts and Flags, and to access those of other HealthTeam Centricity EMR users, click on the Alerts/Flags tab of your Centricity EMR 9.2 Desktop. There you will be able to see a list of your Alerts and Flags in the upper portion of the windowpane, and the content of the selected Alert or Flag in the lower windowpane. Viewing Alerts and Flags of other Centricity Users On the Alerts/Flags tab or your Centricity EMR 9.2 Desktop you can view the Alerts and Flags sent to other individuals and generic accounts in your clinic. Use the dropdown arrow to locate and select the User ID whose Alerts and Flags you wish to view. Alternatively, use the Binoculars to launch a full Centricity EMR search. NOTE: Since you can read the Alerts and Flags of other Centricity EMR users, keep in mind that others can read your Alerts and Flags, as well. You should not include information in an Alert or Flag that you would not want others to see. The Alerts/Flags Toolbar Use the various toolbar buttons of the Alerts/Flags toolbar to Reply to, Forward, Convert, Remove, or Hide an Alert or Flag. Replying to an Alert or Flag To reply to an Alert or Flag that is currently selected, click the Reply button. This causes the Reply to Alert/Flag window to open, where you can type your response, then send it back to the message’s original sender. If you want to keep a copy of the Alert or Flag on your Desktop even after you have replied to it, route it back to your Desktop when initially replying; otherwise, it is automatically removed from your Desktop. To do so, use the dropdown arrow by the To field to locate and select your own Centricity EMR ID, near the very top of that list. Michigan State University EMR 10 Forwarding an Alert or Flag Forward an Alert or Flag to another HealthTeam Centricity EMR user by clicking directly on the message, then clicking the Forward button. Select the person(s) to whom you are forwarding the Alert or Flag from the dropdown list of the To field, or use the Binoculars to locate and select someone else. Removing Alerts and Flags Once you have read an Alert or Flag and no longer need it, you delete it by first selecting that message, then clicking the Remove button. Hiding Alerts Clicking the Hide button, on the far right side of the Alerts/Flags toolbar, removes the selected Care Alert from your Desktop, but preserves it in the patient’s chart. Retrieving Care Alerts and Flags You can view hidden Care Alerts and removed Care Alerts and Flags while working on your Desktop Alerts/Flags window. Click the Organize button, located on the far right side of the Alerts/Flags toolbar. This opens the Organize Alerts/Flags window. Check View removed alerts/flags to view those messages which have been removed for less than 30 days. Check View hidden care alerts to see such messages hidden from your Desktop, but not actually removed from patient charts. Make other adjustments to how you view Alerts and Flags, if desired, then click OK . Organize Options The options available in the Organize window vary, depending on where you are at in Centricity EMR 9.2 when you click the Organize button. If you do not see the option to View hidden care alerts, you were probably in a patient’s chart when you clicked Organize. NOTE Care Alerts and Flags are only temporary and are not to be used to record information that should be part of that patient’s permanent medical record. If the content of a Care Alert or Flag needs to be saved in the patient’s chart, use the Convert tool to convert the Care Alert to a Phone Note or other document type that can then be signed and saved in the patient’s Centricity EMR 9.2 chart. Michigan State University EMR 11 Creating a Flag 1. To create a new Flag, click the New button of the Centricity EMR 9.2 toolbar. 2. Designate the recipient(s) in the To User field by using either the dropdown list or the Binoculars. By default, the dropdown list is comprised of the other individuals in your clinic. Also, at the top of the list you will see the names of those to whom you have recently sent either a Flag or other document. a. If the intended recipient’s name does not appear in the dropdown list of the To User field, click the Binoculars button. b. Either scroll through the list of Centricity EMR users on the Browse page, or go to the Search page and perform a search for the individual, typing in that person’s last name. 3. (Optional) If you select a Centricity EMR user to whom you really don’t want to send the Flag, select that user’s name in the recipient list, then click the button with the red “x”; this removes that individual from the recipient list. 4. Add the pertinent information for the Flag, such as the Priority and Subject, then type the message itself in the lower portion of the screen. 5. Click Send , and the Flag is routed to the Desktop of the designated recipient(s). Select the Flag’s intended recipient(s). The content of your message goes here. Set the desired properties, including a Start and/or Expiration date, when applicable. Enter something brief in the Subject field. Send the Flag to the designated recipient(s), or Save as Draft to your Desktop. TIP A Flag directly related to a specific patient can be attached to that patient’s chart. When opened, a Flag attached to a chart opens in the foreground, with the patient’s chart opened in the background. Michigan State University EMR 12 Creating a Care Alert or Popup Creating a Care Alert or Popup in Centricity EMR 9.2 is much like creating a Flag; the main difference is that you must be in the patient’s chart to which you want to attach the Alert or Popup. 1. In the patient’s chart, click the New button, either on the main Centricity EMR 9.2 toolbar, or on the toolbar of the Alerts/Flags tab. 2. Select a Location from the options in the To Location field’s dropdown list. This does not actually send an Alert to the Desktops of everyone in a specified Location; it only appears on the patient’s Chart Summary tab when someone with privileges to that Location accesses that patient’s chart. 3. In the To User field, indicate if you want to actually send the Alert to the Desktop of one or more HealthTeam Centricity EMR users. 4. Set the Priority as is appropriate for the given instance. Priority options are Normal, Important, and Urgent. 5. Select the note Type: Care Alert, or Popup. 6. (Optional) Assign a Start date. TIP: Combine the Start date feature and the Save function to create a reminder that appears on your Desktop at a later date 7. (Optional) Assign an Expiration date. 8. Attach the Alert or Popup to the Patient’s Chart, Registration, or Appointments. 9. Type the content in the Message field. 10. Click Send . Or, if you need to stop entering the Message before having completed it, click Save as Draft , which will save your work and place the unfinished Care Alert or Popup to your Desktop. Popups and Care Alerts can be useful, but they should not be over-used, or HealthTeam Centricity EMR users may become desensitized to all Popups and Alerts, and miss something critical about a patient’s medical care. Michigan State University EMR 13 Disabling Popups At some point, you may want to turn off the Popup functionality. You can do so by checking Disable popup care alerts, in a Care Alert Warning window. Disabling Popups turns off the Popup functionality for all Popups in HeatlhTeam patient charts to which you have access – not just in a specific patient’s chart. Restoring Popups If you have disabled Popup Care Alerts and want to re-activate this feature, go to the Options menu, then select Display… In Display Options, check Restore warning message dialogs, then click OK . Converting an Alert or Flag to a Document If you want the content of an Alert or Flag to be a permanent part of a patient record, convert it to a chart document. 1. Select the Alert or Flag that you wish to convert to a Document, then click the Convert button. 2. In the Convert window, select the appropriate Document Type. 3. If necessary, change the Provider in the Provider field. 4. Enter a brief description of the message’s content in the Summary line. 5. Add additional text in the Note area, if desired. 6. Click Sign . This applies your electronic signature to the Document, and saves the document in the patient’s chart. Michigan State University EMR 14 DESKTOP DOCUMENTS Documents on the Desktop Summary Documents routed to you appear on your Centricity EMR 9.2 Desktop Summary. A particular document’s type and status, your clinic’s workflow and protocols, and your signing privileges determine how you work with that document. (For more on document status options, see Document Status Classification, page 8.) Viewing Your Documents on the Documents Tab While the information visible on your Desktop Summary is useful, you may find that the assorted views and tools available on the Documents tab of your Desktop are especially helpful. Click on the Documents tab of your Centricity EMR Desktop. A document that is selected in the upper portion of the Documents window is highlighted in blue, its properties are displayed in the middle of the window, and its content is shown in the lower portion of the window. A comment is displayed in the Comments field, in the center of the window, when the document to which it is associated is selected. Comments are entered when a document is routed. Comments are only visible on your Documents tab, and they are not saved as part of the document. The Documents Tab Michigan State University EMR 15 Change the order in which documents are sorted and displayed by clicking directly on a specific column header. Example: Click the column header “Status” to sort the documents by status. The various types of documents on your Desktop are shown on the left; click directly on a specific document type to filter out all other types. Example: Click on “Office Visit” to see only the Office Visit documents on your Desktop. The Full Document Viewer and Attachments Use the Full Document Viewer button, located below and to the right of the Comments field of your Documents window, to view a document and associated Appends in a separate, scrollable window. If the document selected has an attachment, use the Attach button to access that attachment. Viewing Documents on another Centricity User’s Desktop To view the documents sent to another individual’s Desktop or a generic Desktop in your clinic, use the dropdown arrow of the “View documents to” field. Use the binoculars, just to the right of the dropdown list, to locate a particular Desktop if it does not appear in the dropdown list. NOTE If you apply a filter to view only a particular document type on your Documents tab, be sure to click on “ All Documents” before moving on to another screen. If you don’t, you may overlook important documents on your Desktop. If you view documents on another user’s Desktop, set it back to your own Desktop before moving on to another area of Centricity EMR. Michigan State University EMR 16 THE OUT OF OFFICE ASSISTANT When a HealthTeam Centricity EMR user attempts to route a Document, Care Alert, or Flag to another user whose Out of Office Assistant has been activated, a pop-up box informs the sender that the intended recipient is not presently monitoring his or her Desktop. The sender can then elect to route the Document, Alert or Flag to the originally indicated recipient, or user the Do Not Route button and determine how best to handle the communication. Activating the Out of Office Assistant Turn on the Out of Office Assistant to let other HealthTeam members know that you are not in the office and monitoring your Centricity EMR Desktop. However, please note that activating the Out of Office Assistant does not prevent documents from being electronically routed to your Desktop via an interface, such as Sparrow labs, MSU Radiology reports, etc. 1. Logged into Centricity EMR and on your Desktop, go to Options | Out of Office Assistant… NOTE: Be sure to start this process while on your Desktop, as it is not available in the Options menu while in other areas of Centricity EMR. 2. In the Out of Office Assistant window, click to select “ I am currently out of the office”. 3. Type the content you wish to have displayed in the Out of Office Assistant in the Auto-reply field. 4. Using the checkboxes in the lower left worner of the window, indicate if you want Centricity EMR to send an Out of Office reply when receiving Documents, Alerts/Flags, or both. 5. Click Ok to save your work, close the Out of Office Assistant window, and return to your Desktop. Turning off the Out of Office Assistant Once you have activated the Out of Office Assistant, the next time you log into Centricity EMR you will be prompted to either keep the Assistant activated, or to turn it off. Yes will deactivate this feature, while No will turn it off. Michigan State University EMR 17 CHART NAVIGATION AND LAYOUT Finding a Patient Chart To locate a patient’s Centricity EMR chart, click on the Desktop toolbar’s Chart button; this should cause the Find Patient window to open. If, however, you have already been in a patient’s chart during the current session, you may need to click on the Find Patient button of the Charting toolbar, located on the far left side of that toolbar (from inside another patient chart). 1. The default search criteria in the Find Patient window is Search by: Name, but you can also use the dropdown arrow to prompt Centricity EMR to search using other criteria. 2. Other than when the Search by: Birthdate option has been selected, you can then refine the criteria further by selecting the Search method most appropriate for that instance. 3. In the Search for field enter the first few characters of the last name, followed by a comma and the first few characters of the first name. Next, click the Search button, and all HealthTeam patients who meet the criteria that you have established will be listed in the lower portion of the Find Patient screen. NOTE: Be careful when searching for a patient whose real name includes special characters such as apostrophes, hyphens or spaces; these are usually - but not always - removed from names when patients are initially entered in Centricity Business (ex: “Smith-Jones, John” should be entered as “SmithJones, John”). 4. Centricity EMR 9.2 provides information in the Search results section to help you verify that you have, in fact, found the correct individual. Open the patient’s chart by either double-clicking on the patient’s name in the Search results, or click once on the patient’s name and then click the OK button. Tip for Keyboard Users: the keyboard combination [Ctrl + F] opens the Find Patient window. Michigan State University EMR 18 Returning to a Patient Chart Use the dropdown arrow () attached to the Chart button of the Centricity EMR toolbar to open a patient’s chart in which you’ve worked during a recent session. Your ten most recently accessed charts are displayed in the list; select a name from the list, and you’ll be returned to that patient’s chart. Opening a Chart from your Centricity EMR 9.2 Desktop Patient-specific documents such as lab results, phone notes and office visits that require your attention are routed to your Centricity EMR Desktop. When you double-click on one such document, Centricity EMR opens it directly in the patient’s chart. If you have difficulty double-clicking to open a document, click once directly on that document, then click the Open button. Once you have completed working in the patient’s chart, you can return to your Desktop by clicking on the Desktop button, located on the far left end of the Centricity EMR toolbar. Or, if you want to locate and navigate to another patient’s chart, click the Find Patient button to open the Find Patient search utility. THE PATIENT CHART The patient banner spans the width of a patient’s chart near the top of the screen, just below the Centricity EMR toolbar. This banner contains demographical information about the patient whose chart you are viewing, some of which remains constant from one HealthTeam clinic to another, and some that is clinic-specific. Furthermore, much of the information entered in Centricity Business drives what is displayed in the patient banner; as such, if you wish to change something appearing in the banner, it may need to be changed directly in Centricity Business. Verify patient information displayed in the banner before making changes in a patient’s EMR chart. Michigan State University EMR 19 The Summary Tab A patient chart is set up with tabs that define specific areas of the chart, each of which contains important patient information. The first ‘page’ of the chart is the Summary, where you can view multiple facets of the patient chart in one screen. Use the tabs to move from one area of the patient’s chart to another. A pencil indicates that there is at least one unsigned document in the chart, even if it isn’t easily seen in the Documents list on the Summary tab. The Patient’s Summary Tab Content of the Summary Tab Problems Signed Problems on the patient’s active Problem list are displayed; double-click directly on a problem to see its full details, including associated assessments. Medications Signed Medications on the patient’s active Medication list are displayed; double-click directly on a medication to see its prescribing details, including refills given. Flowsheet The flowsheet View displayed on the full Flowsheet tab is shown in brief, with the latest signed observations included. Documents Both signed and unsigned documents in a patient’s chart are listed in the Documents area of the Summary tab, and they can be sorted by clicking on one of the column headers. Drug Interactions Centricity EMR checks for medication interactions, adverse reactions, and contraindications; if any such issues are detected, an alert icon is displayed on the Summary tab, between the Medication and Alerts. This display varies by the type of interaction or adverse reaction, and may also include medication/diagnosis interactions. Hover over the icon to see details. Allergies Allergies and Adverse Reactions recorded in the patient’s chart are displayed in the Allergies section of the Summary tab. Hover the mouse over a particular entry to see details of that allergy or adverse reaction. Directives Signed Directives on the Active Directives list are displayed. Care Alerts Care Alerts appear on the patient’s Summary tab; hover over an Alert with the mouse to see details of that Alert, or double-click to see more on the Alerts/Flags tab. Note: Flags linked to a patient are not included in the Care Alerts region of the Summary tab. Registration Notes Information entered in the patient’s Registration Notes field is displayed. Michigan State University EMR 20 The History Tab While the information displayed on the Summary page is valuable, much of the time you will find it necessary to see more detailed information about the patient. The History tab provides space dedicated to the display of narrativetype data, as well as numeric-based data, recorded in the patient’s EMR chart. For example, the illustration here shows a patient’s Past Medical History, Social History, and Family History. Use the View dropdown list to change the historic data displayed. The History Tab The Problems Tab On the Problems tab you can see details and assessments of problems recorded in the patient’s chart. You may choose to view only the patient’s current problems by selecting Active Only, or to see All problems that have been recorded for the patient in the past yet are no longer active. Centricity EMR displays inactive problems highlighted in gray. Select a problem, then click the View Problem Details button to see that problem and it’s comments and assessments in a separate window. An Alerts/Flags icon, along with respective numbers, appears on the Problem tab when the one or more Alerts and/or Flags are associated with the patient. A small Drug interaction display is added to the Problems tab, when applicable. Michigan State University EMR 21 Select a problem from the list in the upper portion of the chart, and its details appear in the lower portion. Double-clicking directly on a problem in the upper portion of the Problems window opens the Related Document Viewer window in the foreground. Related documents are those that contain the originally documented Problem, or assessments and modifications of that Problem. The Medications Tab On the Medications tab you see the medications recorded in the patient’s chart and associated details. View only medications the patient is currently taking by selecting Active Only; or, select All to see medications that the patient is currently taking and those he or she has taken in the past. An Alerts/Flags icon, along with respective numbers, appears on the Medications tab when there are Alerts and/or Flags presently associated with the patient. Another small display is added to the Medications tab when the system detects Drug interactions in the patient’s electronic medical record. Michigan State University EMR 22 Prescribing details of the medication selected on the left side of the Medications window are displayed on the right, including relevant dates, persons involved in the prescribing process, instructions, comments, and any refills on record. When you select a medication in the left portion of the Medications tab, the prescribing details of that medication are listed in the right column. The Alerts/Flags Tab The Alerts/Flags tab is comprised of Allergies and Adverse Reactions, Directives, Care Alerts, and Flags. You can choose to see Active Only or All for both Allergies and Adverse Reactions, and Directives. You can view Care Alerts and Flags for Today or Anytime. The Alerts/Flags Tab The screen size available for the three sections of the Alerts/Flags tab can be changed by placing your mouse pointer directly on one of the horizontal lines between two sections. As the mouse touches one of these lines, it is shown as two small horizontal lines, each with tiny arrows; when you see this instead of the basic mouse pointer, click and drag the line up or down, as desired. Michigan State University EMR 23 Allergies and Adverse Reactions Allergies and Adverse Reactions documented in the patient’s chart are listed near the top of the Alerts/Flags window, along with their respective details. Also, a drug interaction alert is displayed in the upper right corner, when applicable. Directives In the mid-section of the Alerts/Flags window you’ll find the patient’s Directives. To view the document in which a particular Directive was recorded in the patient’s chart, double-click on that Directive. Care Alerts and Flags All active Alerts associated with the patient are displayed on the Alerts/Flags tab, regardless of the Location of Care under which they were created. (A user sees only Alerts linked to his or her own Location of Care in the Care Alerts region of the Chart Summary tab.) Also, any Care Alerts still active that you may have hidden from your Desktop are included in the default Care Alerts and Flags display on the Alerts/Flags tab. Tools with which you can address issues in a patient’s Alerts and Flags are available on the Alerts/Flags toolbar, and they work much as they do on your Desktop Alerts/Flags window. What’s the difference between Hiding a Care Alert and Removing a Care Alert? You can Hide a Care Alert only while working on your Centricity EMR 9.2 Desktop. You cannot Hide a Care Alert while in a patient’s chart. Hiding a Care Alert takes it off of your Desktop, but it remains unchanged in the patient’s chart to which it is attached. You can Remove a Care Alert while on your Desktop or in the patient’s chart. Removing a Care Alert essentially deletes it. A removed/deleted Alert is saved in the system for 30 days and can be retrieved, if necessary. After 30 days, the Alert is removed entirely from the system and can not be recovered. Michigan State University EMR 24 The Flowsheet Tab The Flowsheet tab provides you with different ways in which you can view patient information such as past and present height, weight and blood pressure, and lab results. The Flowsheet tab with the CBC View selected Use the View field’s dropdown arrow to change the displayed flowsheet View. At the top of the dropdown list are Views you have recently accessed, followed by a list of several other Views available. Alternatively, you can also use the Binoculars to search for a particular View. To restrict the data points displayed in a particular View, place a checkmark in the box to Use Date Range, which activates two date range fields, where you can enter desired start and/or end dates. Flowsheet Symbols Values are color coded to help you differentiate imported values, such as lab and pft results, from those entered by a Provider or staff member. Values displayed in black font are those entered by someone on the HealthTeam, such as vital signs, immunizations, and other information recorded in a chart update. Blue font is used to show lab values that are electronically entered into the system via an interface. For lab values that are found to be outside the set parameters of what is deemed normal, “” and “”symbols are added to show elevated or decreased levels, respectively. Single yellow arrows represent high or low values, and double reds represent panic highs or lows. A green “” indicates that more than one entry has been made for a particular value on the same day, while a red “” signifies the deletion of a value. If you single-click on a single value, details of that value are displayed near the bottom of the page. Double-click on a value, and its associated document opens in a separate window. Michigan State University EMR 25 The Flowsheet View/Graph Connection Graphing functionality is directly impacted by the flowsheet View set in a patient’s chart. If a particular observation is not included in the currently displayed flowsheet View, you cannot include that particular observation in a Graph. If this happens, click the Close button of the Graph Observations window, select a View that does contain the observations you wish to graph, then try the Graphing feature again. Flowsheet Courtesy While identifying one flowsheet View that best suits your needs and clicking the Set Attached View button seems like a good idea, doing so actually sets that View for the entire HealthTeam, which can be considerably inconvenient for others, and could lead to critical patient health information being overlooked. Documents Documents associated with a particular patient are visible on the Documents tab of that patient’s Centricity EMR chart. Working with the documents on the patient’s Documents tab is much like working with documents on your Desktop, but with additional functionality. The content of the document that you select in the upper windowpane is displayed in the lower windowpane. The content of the document selected in the upper windowpane is displayed in the lower pane. The Patient’s Chart Documents Tab In the upper right corner of the Documents window you will see the number of Alerts and/or Flags associated with the patient, as well as any system-identified Drug interactions. Just to the right of those displays you can place a checkmark to have the system Group [documents] By Date. Michigan State University EMR 26 The Document View If only “+All” appears in the upper left area, click the “+” to expand the view; then you can click directly on a particular document type to filter out all other documents, therein helping you to focus on one particular document type. NOTE: If you filter on a particular document type, be sure to turn off that filter by clicking the “- All” before navigating away from the Documents window. Otherwise, the filter remains in effect and you may miss critical documents of another type that require your attention. Sorting Documents Change the sort order of the chart documents by clicking on one of the column headers. A faint gray “” indicates on which column the documents are currently sorted. Viewing Attachments A document that has an attachment is shown in the Documents window with a paper clip. To view the attachment, first click to select the document in the upper portion of the Documents window. Then click the Attach button, near the middle of the window. Click to open the desired Attachment, and it will open in a new, separate window, with the patient’s EMR chart in the background. PRINTING IN CENTRICITY EMR 9.2 On occasion you may need to print documents in Centricity EMR 9.2. Printing is relatively the same throughout the system, though the actual printer selection process varies slightly from one HealthTeam location to another. PRINTING DOCUMENTS IN A PATIENT’S CHART To do so, navigate to the patient’s chart, then click the Print button. The items displayed and selected in the Print window are determined by the chart tab that you have open when you click Print. Printing the Chart Summary 1. On the Summary tab of the patient’s chart, click the Print button of the Centricity EMR toolbar. 2. The Print dialog box opens; because you first clicked on the patient’s summary page, Chart Summary should already be highlighted. 3. Type the number of copies you want to print in the Copies: field. Michigan State University EMR 27 4. Use the checkbox for Letterhead to indicate whether the header, which includes your Location of Care, should appear across the top of the printout. 5. (Optional) Click Preview... to see how the Chart Summary will appear when printed, then click Close to return to the Print dialogue box. 6. Click Print . 7. Click the Close button to close the Print dialog box and return to the patient chart. Printing a Specific Chart Document 1. In the patient’s chart, click on the Documents tab. 2. Select the document that you wish to print, then click the Print button of the toolbar. Because you first selected a specific document, Selected Document(s) should be highlighted. 3. When applicable, determine if you want to “ Print all internal image attachments at end.” 4. Indicate how many copies you want to print, and if Letterhead should be included. 5. Click Print . 6. Click Close . 1. Print Topics: Chart | Documents 2. Print Items: Selected Document(s) Printing Tip Medication, Alerts, and other reports can be printed via Chart | Clinical Lists in the Print window. The content of the report is determined by the settings of the respective aspect at that time. For example, if you have Active Only medications displayed on the Medications tab, when you print the Medications Report only those currently active in the patient’s chart will be included in that report. PRINTING OTHER DOCUMENTS IN CENTRICITY EMR You may want to print other information in Centricity EMR that is not directly associated with a patient. An example would be the appointments of a Provider in clinic on a given day. To print such documents, navigate to the area of Centricity EMR as is appropriate for what you wish to print, then click the Print toolbar button. Make your selections in the Print window, then click Print once again. Michigan State University EMR 28 VIEWING APPOINTMENTS If you are a Provider or Nurse who sees patients, appointments scheduled in the Centricity Business scheduling system under your name appear in the left column of your Centricity EMR Desktop Summary. (For more on appointments on your Desktop, see Today’s Appointments on your Centricity EMR 9.2 Desktop Summary, page 7.) APPOINTMENT BOOKS To view your Centricity EMR appointment book in detail, or to see the appointments of another Provider in your clinic, click on the Appointments button (Appts) on the Centricity EMR toolbar. This opens the full Appointments window. There you can see appointments on different dates, view appointments for an entire week, open multiple appointment books in your clinic, and customize the screen in various other ways. The Appointment Book Use the date feature on the left side of the Appointment book window to change the display date; the up () and down () arrows change dates one day at a time. The button with the three dots helps you to quickly jump to an exact date farther out. Click the Daily button to display the chosen day’s appointments in a single column. To display both the morning and afternoon appointments of a single day in two columns, click the Split button. To see a full week’s appointments, click the Weekly button. Michigan State University EMR 29 Click the Select View button to open the appointment books of other Clinicians in your clinic. o Choose the desired book or books from the Book(s) dropdown list. o To include canceled appointments and no-shows, click their respective boxes. o If you want your selections just made to be your default for subsequent Centricity EMR work sessions, click Save as Default . o Once you’ve made your selections, click OK and your settings will be applied to the Appointments window. WORKING IN AN APPOINTMENT BOOK When a patient is ‘arrived’ in the front office via Centricity Business, a black dot appears in front of that patient’s name in your Centricity EMR appointment book. To open a scheduled patient’s chart directly from the Centricity EMR appointment book, single-click on the entry in the Appointment book, then click the Open Chart button, in the lower left corner of the window. To view the details of a patient’s scheduled visit, first select the patient in the Appointment book view by single-clicking directly on that patient’s name, then click the Details… button, located in the mid-lower portion of the Appointment book window. View a patient’s past and future scheduled appointments by selecting the patient’s name, then clicking the View Appointments… button. By clicking the Status… button, after having selected a patient’s name in your Appointment book, you can see that patient’s current Appointment Status. HealthTeam patients are scheduled in Centricity Business; therefore, the Add Appointment… button is deactivated in Centricity EMR. Michigan State University EMR 30 GETTING HELP CENTRICITY EMR 9.2 HELP ONLINE When working in Centricity EMR 9.2, click on the Help menu to learn more about the program’s various features and functions. Select Help | Help Topics to access the full library of Centricity EMR Help, and to search for information on a specific word or phrase. In selecting Help | Help on this Screen, you get content-specific Help topics. For example, if you are on the Alerts/Flags tab of your Desktop and choose Help | Help on this Screen, Centricity’s Help window displays several entries relating to various aspects of managing Care Alerts and Flags from your Desktop. Additionally, in most screens the Centricity toolbar provides a Help on this Screen button, which also opens the screen-specific help page. This button is located on the far right side of the toolbar, just to the left of the Exit button. Use these tools at your convenience to become familiar with the various aspects of Centricity EMR 9.2 and how the program is used to document patient care in MSU HealthTeam clinics.
