I (We), tenant, authorize Andco Management, Ltd. To sublet apartment #___________
At ________________________________, Chicago, Illinois.
Monthly rent rate of $__________.
It is understood that the tenant is not released from the original terms of the existing lease
agreement for the above mentioned apartment. It is further understood that tenant shall remain
liable for the rent payments for the balance of the lease term, in addition to return the apartment
to the Landlord in good condition, reasonable wear and tear expected.
The security deposit shall remain in the Landlord’s possession until the subtenant moves out of
the above mentioned apartment at the expiration date of lease. At which time, the security
deposit shall be forwarded to the original tenant at the address below.
Tenant shall advertise at their own cost. Tenant assumes responsibility for showing the
apartment to prospective tenants. Tenant shall be contacted directly by prospective tenants to
view the apartment.
