Legends for Kids Scholarship Application Purpose: Legends for Kids awards scholarships to college-bound high school seniors who have demonstrated leadership/commitment in school athletics, their community and academic achievement. Criteria: The students eligible for assistance must be graduating high school seniors who reside in or around the areas outlined on the map attached and intend to continue their education at a two or four-year college or university in the fall directly after their graduation from high school. The student must have a cumulative grade point average of 2.5 or higher in high school. The student must have participated or contributed to their high school athletic program in any way. Guidelines: Scholarships will be awarded in two equal payments in August and January of the first year of posthigh school education and after an official transcript has been sent in for confirmation. Students must continue to attend classes and maintain no lower than a 2.5 grade-point average (or equivalent). Amount: Each scholarship awarded will be $5,000 to be paid in two equal installments directly to the academic institution. Deadline: The application deadline is Thursday, April 21, 2016 Renewal Process: This scholarship is not renewable. Only high school seniors intending to enroll in a two or four-year college or university are eligible to apply. Previous scholarship recipients cannot re-apply. Scholarship Criteria: 1.The student(s) must be a senior as defined by the high school. 2.The student(s) must have a minimum 2.5 GPA. 3.The student(s) must be nominated by school official (e.g. principal, athletic director, coach). 4.The student(s) must display high character and values that make them a positive role model within their respective community. The following guidelines will be observed by the selection committee: • Applications will be reviewed and scholarship recipients selected in accordance with the written criteria established. • All qualified applicants will be considered. • For each scholarship recipient selected all application criteria, questions and appropriate paperwork must be completed. Legends for Kids is now accepting applications for the Legends for Kids Scholarship. This scholarship seeks to identify the most outstanding graduating senior(s) and to grant an award to be used for higher-level education. The $5,000 prize was created in 2016 by Legends for Kids, Hy-Vee Food Stores, Sanford Health and First Premier. The award recipient must be a graduating senior. Additionally, the award recipient will have demonstrated exceptional character and leadership in furthering his/her own progress and in enriching the lives of others, especially in athletics and school involvement. Those interested in nominating a student(s) for this prize need to submit a nomination packet to Sanford Health, by no later than April 21, 2016 at 5 p.m. The application criteria should address the ways in which the student exemplifies this award. Specific examples of activities and achievements are especially helpful to the selection committee. Finalists, who are selected by a committee designated by the Legends for Kids Board of Directors, will be notified in May of 2016. The group of applicants from which the recipients are selected must be sufficiently broad as to be considered a charitable class.
