REAL Services, Inc.
Supporting Independence for Life for over 40 years.
www.realservices.org
The REAL Services Story
REAL Services was created
in May of 1966 as part of planning
for the future of older adults who
were displaced when the
Studebaker Plant closed. In 1968,
REAL Services designed the first
program in the entire United States
to establish nutrition sites to provide
meals and socialization for older
adults. This program served as a
model for nutrition assistance used
across the country.
Since that time, REAL
Services has established itself not
only as a state-wide leader, but an
example for the entire nation in
serving the aging community
through innovative ideas, service
with integrity, competent
management, ground-breaking
practices, and sound financial trust.
Today, REAL Services has
more than 20 distinct programs that
support individuals and families in
living independent and meaningful
lives. Our programs address the
needs of nutrition, self-sufficiency,
education, and protection for
30,000 people in northern Indiana
each year.
Community Services
programs, which support single
parents and low-income families,
are designed to break the cycle of
dependence on the welfare
system. Home-Delivered Meals,
Guardianship, Foster Grandparent,
In-Home Care, Caregiver Training,
// Page 1
Transportation, and Crime Victim
Assistance are some of our
programs that assist older adults
to remain independent and
connected to their community
Without REAL Services,
each year more than 5,000 older
adults would be at risk of moving
to a nursing home to receive the
care they need. There would be
2,000 people in our community
who would not have access to
food each day. The staff and
volunteers at REAL Services work
tirelessly to help people of all
ages, races, nationalities, and
income levels in northern Indiana
to find independence and
satisfaction in their lives.
The REAL Mission
Believing in the dignity of all people, the objective of REAL Services is to assist in
establishing a community in which those we serve can maintain their independence to
the maximum degree possible and find meaning and satisfaction throughout their lives.
To fulfill our mission, the programs we offer enhance quality of life. Additional programs
provide education for individuals and families through Options Counseling, Aging and
Disability Resource Center, Lunch n‘ Learns, Boomer Bootcamp, Chronic Disease
Management, Medicare Counseling, Financial Literacy, and more.
We believe in the inherent dignity of human
beings and will always have that in
mind in any action we take.
We believe that all people are important;
that every individual has equal rights
and must have equal opportunities.
We will take strong measures to ensure
that this belief is reflected in our
activities and that every individual with
whom we deal is treated courteously,
and with compassion and
understanding.
All affairs of this Agency will be conducted
under high ethical standards and with
personal integrity.
In discharging the responsibility for funds
under our stewardship we will:
•
Ensure that fiscal matters are
conducted with accuracy, in
accordance with principles of good
financial management and with
complete integrity.
•
Make certain that the maximum
amount of quality services are
provided for each dollar expended
with a minimum use of funds for other
expenses.
From the opportunities available to us, we
will use our best judgment in selecting
those which:
•
Are most needed by those we
serve.
•
Provide the best return services for
expenditures.
•
Are within our managerial
capacity.
Programs to meet needs will be of high
quality, developed in a timely
fashion, and well maintained.
We will face our problems squarely, be
diligent in seeking the most
beneficial solution, and courageous
in taking proper action regardless of
difficulties encountered.
We believe that any static organization
becomes stagnant and ineffective.
We also believe we face almost
endless opportunities to expand our
service to our clients. We will
constantly be alert to opportunities,
which under this philosophy and
within this policy may be available
to us.
We believe that good management must
quickly and effectively confront and
resolve situations which arise that
conflict with our mission. We pledge
that this will be done.
The operation of REAL Services is based upon a number of principles and beliefs.
Policies have been developed from these and when properly implemented they
will ensure the success and further growth of this Agency.

Caregiver of the Year for a
Disabled Person
Amy Adams- LaPorte
At the 2014 Age of Excellence awards ceremony, Amy Adams, of LaPorte
County, was awarded Caregiver of the Year for a Disabled Person. This acclaimed
award is given to an unpaid caregiver who provides compassionate care to a family
member or a friend who is less than 60 years of age.
Amy is a dedicated mother who raises funds by running in marathons. ―I hate to
run… but I hate the fact that my child can‘t run even more, so I run for her,‖ Amy
expresses. Her running has helped raise an impressive $17,000. She has hosted three
give-back nights and helped coordinate the annual ―Zumba for Retts.‖ Amy has sold tshirts, hosted a ―cocktails and creativity‖ night, held raffles, and has sold beautiful
ornaments, lanterns and homemade jewelry. Amy also diligently works information
booths and raises funds to pay for playground equipment.
Amy‘s next challenge is to push her disabled child in a brand new wheelchair as
they participate in the next 5K Dyngus Day Dash. She willingly does all this while she
takes her daughter to speech, occupational, and physical therapy and provides for her
ongoing care. The family is even participating in a study through a Chicago research
clinic to help understand the role genetics plays in her daughter‘s disease all with the
intent of helping others.
Amy has devoted herself to raising funds and awareness of the disease that her
daughter struggles with every day, all in the hope of fighting the disease by finding a
cure. Helping keep other families from the disease is her dream. Providing the best
possible quality of life for her daughter is her mission.

Client Story
Lori wears a necklace given to her by a friend that reads, ‗she thought her world was
ending and then she turned into a butterfly.‘ ―That‘s me,‖ Lori confirms. She compares herself
to a butterfly frequently due to the major transformation that has taken place over the past
five years. With a very friendly personality and a terrific attitude, Lori‘s life has recently taken a
turn with her good days outnumbering her bad days. ―REAL Services has made an extreme
change in my life,‖ she says with a smile beaming across her face. Lori explains that being
with REAL Services for the past five years has significantly altered her life for the better.
At only 49 years old, Lori suffers from a number of serious health issues that have given
her complications throughout her entire life; severe obesity being her main hindrance. At 500
pounds only five years ago, Lori was wheelchair bound, homebound, and generally
bedbound. She knew that she needed help but did not know how to get it. Lori‘s nurse
practitioner at the time referred her to Amy at REAL Services. ―I can tell you exactly where I
was when I met Amy on the phone. I was on Route 20, headed towards Meijer. She called me
and I could hardly hear her, so I pulled over into the Marathon station and she said I‘m going
to get you some help.‖ Lori explains that from that point on, her life was different. Once REAL
Services entered her life, Lori received in-home services and a home attendant to help her
bathe, get to the doctor, prepare meals, and take medication. She was also able to receive
a life-line button, a lift chair, and diabetic shoes. However, the biggest blessing that Lori
believes she has received through REAL Services is her gastric bypass surgery that took place
July 16, 2013. Now at a healthy weight of 193 pounds, Lori has her motivated attitude and
REAL Services to thank.
In addition to her weight loss goal, Lori also had a goal of becoming more physically
social. ―I wouldn‘t have been able to do it without you guys,‖ she says. REAL Services has
helped Lori become a member of support groups and an active member in her apartment
complex. Lori explains that she is preparing for her time with REAL Services to come to an end
within the next year. She is doing so by evaluating how she wants to further occupy her days.
Lori is sufficiently tech savvy and wants to give back by going to nursing homes to help the
elderly learn how to use computers. ―I‘m trying to pay it forward,‖ Lori expresses.
As the interview with Lori comes to an end in her apartment beautifully decorated
with butterflies, she illustrates her gratitude for REAL Services with tears in her eyes and
outstretched arms. Lori has gone through a transformation of a lifetime and has learned very
valuable lessons in the process. ―You can‘t let your feet get stagnant. You have to keep
transforming,‖ she assures. She is extremely thankful for the support REAL Services has shown
her in the past five years and is ready to live the rest of her life to the fullest.

portion of his accomplishments, Dr. Abel is truly an inspirational man in the Elkhart
community and the deserving recipient of the 2014 Hoosier Lifetime Award.

Nutrition awareness, healthy living practices, fall prevention, fitness and
chronic disease management are just a few of the topics presented
in communities throughout our area.
Home Weatherization
Families whose income is below 150% of
the poverty level are provided with
energy conservation measures to
lower fuel consumption and expenses.
Families enjoy a warmer home and
see, on average, a 34% reduction in
fuel consumption.
Hospital Transitions
As
// Page 11
hospitals respond to the need to reduce
readmissions as part of the changes in the
affordable care act, the Area Agency on Aging
has developed a program that helps in that
effort. Piloted and implemented with the help of
community partners, the effort has produced
results far better than other national models. By
contracting with hospitals and insurance
companies, this program is being offered as a
way of reducing costs and providing better care
to patients.

Client Story
A sign that reads ―Bonnie‘s Command Center‖ hangs in the independent living
apartment of 62 year old Bonnie, a REAL Service client since 2010. A handmade sign created
by her daughter-in-law because ―this is where I run my life,‖ Bonnie expresses as she stretches
her arms out in motion to her whole apartment. Living by herself in a comfortable one-room
independent living apartment, Bonnie declares how thankful she is to be living there on her
own and with frequent visits from her family.
Right away, Bonnie begins speaking about her former career as a nurse for 30 years,
her achievement of becoming the first hospice coordinator in the county, having worked for
the state board of health for a number of years, and teaching and setting up a nursing lab at
Ivy Tech for 18 years. However, her most important achievement that she speaks of is raising
her three sons. Bonnie‘s greatest joy in life are her three sons and four (soon to be six)
grandchildren. She expresses how much happiness it brings her when they come for surprise
visits and that she is able to see them frequently.
Bonnie suffered a stroke at 57 years old which has caused her health to progressively
decline since then. After her stroke, she was diagnosed with early onset Alzheimer‘s which
sentenced her to disability. Additionally she has left ventricular hypertrophy, chronic
obstructive pulmonary disease, chronic bronchitis, pulmonary hypertension; pancreas failure
has caused her to become diabetic, she has experienced renal failure, dissolving discs in her
back, shingles, gout, depression, anxiety, high cholesterol, and psoriatic arthritis. ―When you‘re
as sick as I am, your whole world changes,‖ Bonnie says. ―And to have REAL Services come in
and understand that, understand that your world is different, and wanting to make your world
the best that it can be.‖ Bonnie knows that she and her family would not be where they are
today if it wasn‘t for REAL Services. ―There are just no words to thank them for everything they
have done and continue to do,‖ she says as she shows her gratuity for the agency.
As the conversation with Bonnie comes to an end, she explains how she has learned
to cope and live with her disease and illnesses. ―I have learned more about grace and mercy
and what it truly means when you‘re by yourself, maybe laying in bed with your oxygen, on a
Sunday afternoon, remembering when you used to be outside with the grandkids. And I‘ve
learned to live with such a peace. My body is broken, but my spirit isn‘t.‖ As hard as her
struggles may be, Bonnie has strong faith that helps her through her difficult days. Knowing that
there is always support through her family and through REAL Services gives her a positive
attitude and encourages her to keep her head held high.

The Volunteer Group of the Year
Award St. Joseph High School
At the 2014 Age of Excellence awards ceremony, The Volunteer
Group of the Year Award was given to St. Joseph High School. The
qualification to be nominated for this award was a group that displayed an
astounding amount of dedication and commitment to volunteerism in
service to the elderly. The students at St. Joseph High School well deserve to
be awarded with this impressive distinction. They have had an enormous
impact on their community through numerous volunteer service projects.
St. Joseph High School has dedicated staff who work with their
students to provide an opportunity for them to give back to their community
while encouraging them to learn the honor and the joy of serving
others. Through the guidance of their Service Coordinator, the students
have provided over one thousand hours of service to aged and disabled
community members. Last year alone, the homes of 66 seniors benefited
from the hard work of these amazing young adults.
These days there are so many opportunities open to students and
demands on their time. Work, friends and family, sports, and of course their
studies are all competing for their full attention. However, these students
consistently make it a priority to serve their community‘s most vulnerable
citizens. Working on service projects which assist the elderly since 2008, this
group of over 300 teenagers have logged more than 8,000 hours of
volunteer time. All of their time and dedication makes them very deserving
recipients of the 2014 Volunteer Group of the Year Award.

Client Story
At 93 years old, Mildred has a sharp personality and a witty sense of humor
that will keep you on your toes. Living with her daughter Nadine, an interview with
both Mildred and Nadine illustrated their unbreakable bond and countless memories
they have together.
Despite her 10 years of living with Alzheimer‘s disease, Mildred still recalls her
time spent growing up on a farm in North Dakota, along with the memories of milking
cows and dressing her farm cats in doll dresses. She also treasures her time spent
teaching grades 1-8 in a one-room school house before settling down with her
husband and being a stay-at-home mother. Having lived a fulfilling life as a mother
and wife, Mildred‘s favorite pastimes were travelling the entire continental U.S.
(except Maine and Mississippi) and going to church every Sunday with her family.
As Mildred‘s primary caregiver, her daughter Nadine has a lot of input on the
caregiver‘s perspective in a situation such as this one. ―I am very grateful to be able
to continue my job and still have her here.‖ With a home care aid that comes
Monday through Friday and respite hours on Friday evenings and Sunday mornings,
Nadine and her husband are able to keep a full time job to still support their family.
Mildred, Nadine, and her husband are very thankful for REAL Services and what the
organization has done for them. ―The idea of keeping families together is so
important. And most of us can‘t do that without support,‖ Nadine says as she praises
REAL Services.
Mildred has been living with her daughter Nadine for 10 years and they have
been receiving services from REAL Services for about 2 ½ years. Bathroom
modifications, a lift chair, and home care aids have greatly assisted their care plan
goal of keeping Mildred in their home for as long as possible. ―I wouldn‘t want her
any place else,‖ Nadine expresses. REAL Services works diligently to accomplish a
client‘s goals and provide support in every type of situation. In this remarkable
mother-daughter relationship, REAL Services has been able to assist in continuing to

Client Story
―The doctor wanted to amputate‖ Jackie states with closed eyes, nodding her head
in disbelief. In August, 2008, Jackie‘s life changed instantly when she was in a serious car
accident which crushed her right ankle. ―A young man, coming from the opposite direction
didn‘t yield the right of way and he plowed right into me‖ she explains. Ten surgeries, six
months of therapy, and 58 additional specialty treatments later, Jackie is miraculously able to
walk again.
64 year old Jackie loves to cook. ―I was a cook at the Marriott for 20 years‖ she says
with a proud smile on her face. With countless hours of experience working the kitchen, the
restaurant, and the breakfast buffet, Jackie knew how to please guests‘ taste buds. ―I
celebrated 20 years at the Marriott June 25, 2008, and then an awful car accident happened
August 5 of 2008.‖ Jackie was forced to leave her beloved cooking career at the Marriott
due to the results of the car crash.
After a long process of ten surgeries on her ankle, Jackie had to endure six months of
therapy to learn how to walk again. Once she finally returned home, she soon developed
MRSA in her wounded leg. Her doctor advised treatments of hyperbaric oxygen therapy to
cure the MRSA and allow her leg to fully heal. Jackie needed to get to the wound center
every day for 58 days in order for her therapy to be effective. She did not have a car after the
accident, let alone was even able to drive. That is when Jackie‘s home nurse suggested
REAL Services transportation services. REAL Services provided transportation to Jackie‘s
appointment every day. ―My leg is healed because of everyday transportation‖ Jackie
confirms. Jackie now receives transportation from REAL Services every three months for
checkups with her doctor.
As well as the transportation services, Jackie began receiving Meals on Wheels meals
about a year ago. ―That was such a treat!‖ Jackie proclaims. Receiving home-cooked,
healthy meals takes a lot of pressure off of Jackie trying to find rides to the grocery store. ―I
feel like baking them [Meals on Wheels employees] 10,000 chocolate chip cookies and
taking them up there to show them how much I appreciate it.‖
The interview comes to a close with a thought on how Jackie‘s life would be different
if REAL Services was not a part of it. ―I‘m healthier‖ Jackie explains. ―And I feel secure in
knowing that when I have a new appointment, that I can have a way there. And to know
that there are responsible people driving. They‘re very good drivers.‖ Another smile stretches
across Jackie‘s face. ―There are so many people who depend on REAL Services‘ services, that
it‘s [much] needed and very appreciated. The families of the clients can rest at ease
[knowing] that there is an organization that cares and gives their all to help someone with
transportation or meals.‖
Even though Jackie‘s story is one that is difficult to fathom, her animated and warm
personality makes it easy to discuss. Despite all her struggles and difficulties, Jackie is a
positive and very optimistic woman. She cannot express enough how thankful she is for REAL
Services. With great appreciation, Jackie expresses, ―just make sure you put in there that I‘m
so thankful for them [REAL Services]. That‘s my main bottom line. So thankful for them being
how they are.‖

Volunteers are a vital part of REAL Services‘
mission. Volunteers work directly with the

administrative functions. The program recruits
and trains volunteers who assist an elderly
person by providing grocery shopping, meal
delivery, transportation, respite, friendly
visiting or home repair.
Weinberg Caregiver Connection Center
The Caregiver Connection Center connects
informal caregivers in the 46619 zip code
in St. Joseph County to a comprehensive
network of resources to assist them as
they care for our older adults. This is
accomplished through case
management, caregiver education, and
direct services.

Client Story
Living on a quiet and isolated street in Plymouth, Indiana, Sandy often finds
herself thinking about how thankful she is for REAL Services. ―I‘d be totally lost without
her,‖ Sandy expresses when speaking about her case manager, Allison. Sandy is a 56
year old woman who suffered from a massive stroke six years ago that impaired the
left side of her body. Having always been a very self-sufficient woman, Sandy finds it
hard to rely on others for help. ―I used to work 60-70 hours a week before I had the
stroke,‖ Sandy says. However, she also makes a point to say that she is always very
thankful for all the assistance she receives from REAL Services and her caregivers.
When Sandy had her stroke six years ago, she was left completely helpless
with no nearby family members to care for her. That is when REAL Services stepped in.
Immediately, REAL Services came into her home and assessed what needed to be
done. A homemaker, a bath aide, and a lifeline button were quickly arranged for
Sandy. Next began the home modifications that were essential for Sandy to stay in
her home. Her damaged electrical system was repaired, her water heater and
furnace was fixed to working conditions, and her outdated roof was patched.
Additionally, REAL Services was able to facilitate a loan from 1st Source Bank in order
for Sandy to have her well repaired.
Reminiscing on more positive memories, Sandy ended the conversation
expressing some of her favorite pastimes of going to the conservation camp in
Kankakee with her family. ―We would sit around the campfire, go fishing, play games.
It was all in all fun, like having a picnic 24/7.‖ Sandy‘s grateful attitude continues to
give thanks to REAL Services, and everyone that has made the organization possible.
She concludes by saying, ―I thank God for you, God bless you and thank you very
much. I wouldn‘t have made it without you.‖
