How to get your mail-in rebate:
1) BUY at least one (1) case of any of the eligible products listed.
2) REDEEM ONLINE at diversey-cbrebates.com or MAIL completed
Rebate Form with copies of dated proof(s) of purchase or Distributor
velocity reports for full cases of eligible products purchased between
January 1, 2016 and March 31, 2016, postmarked no later than

velocity reports is prohibited and constitutes fraud. Fraudulent submissions will not be honored
and may be prosecuted. Please make a copy of your rebate submission for your files. Diversey
and or its Distributors are not responsible for any non-complying Commercial End-User Rebate
submissions or for lost, late, illegible, postage due or undeliverable mail. Non-complying EndUser Rebate submissions will not be honored and may not be acknowledged or returned.
