WELCOME TO CENTENNIAL BANK AND TRUST

Centennial Bank and Trust (CBT) is now part of Heartland
Financial USA, Inc. (For more information about Heartland,
see page 19 of this Guide.)
On June 10, 2016, we will begin the final step of conversion
to the updated systems and processes. We are working
together to bring you all the strengths of our combined
team, along with other key advantages that have made
Centennial Bank and Trust such a great place to bank.
The following pages will help you quickly identify
information needed to understand changes that might alter
the way you interact with our business services. In addition,
this Business Services Guide was created to provide:


Important dates that may require action on your part.
Included in this packet is the Business Deposit Accounts
booklet that provides the new Deposit Account Agreement,
Disclosures and fee schedule that will be effective June 13,
2016.

Conversion weekend begins when online banking services become unavailable.
Centennial Bank mobile application (app) will be unavailable beginning at 11:00 a.m.
Centennial Bank Bill Pay will be disabled at 11:00 a.m.

Account Statements. Account statements will have a new
look and continue to provide 12 check images per page.
There will be a special statement cut on June 10, 2016. If
you are on a month-end statement cutoff, you will receive
two statements for the month of June.
Closely review the first statement received after June 13,
2016, and notify our Customer Care Center at 844.392.7796
if you have any questions.
Existing Checks. Good news! Continue to use your
existing check stock. Your CBT routing number for check
transactions will not change. New check orders will be
updated automatically with the new name and logo.
Electronic Payments and Deposits. Electronic deposits
and payments will process without disruption. Continue
to use the CBT routing number for your ACH payment
instructions. Wire payment instructions will change
beginning June 13, 2016. Details are provided in the Wire
Transfer Payments section on page 14 of this Guide.
Recurring external transfers established outside of The
current Online Banking Service to transfer money to your
account at another bank will be converted and should
continue without interruption.
Overdraft Privilege. On June 13, 2016, all business
checking accounts, except IOLTA and IBRETA checking,
will have an Overdraft Privilege (ODP) limit of $750. More

the Business Deposit Accounts disclosure booklet enclosed
with this Guide.
Posting Order. Payments will continue to post to your
account in the same order:
▶ ATM Transactions
▶ Debit Card Transactions (posted according to the time
the final authorization was processed).
▶ Wire Transfer
▶ ACH Items
▶ On-us Checks Presented for Cash
▶ Other Checks Presented for Payment
Checks will post in numerical order from lowest to highest
check serial number.
Reporting of ACH Addenda. Advice of ACH Addenda
information will be converted to reporting through the
Federal Reserve Bank’s FedEDI service. This is a new service
available to Centennial Bank and Trust business customers.
FedEDI will send an email notification when your designated
account receives a CTX, CCD+ or IAT transaction. The email
will contain a link to FedEDI, and within the online service
you will have the ability to print or download transaction
and addenda detail.
Some transactions may have a slightly different description on your account statement or
displayed through Business Online Banking (your new online banking service). If you have
questions about a transaction description, please contact your Banker or the Customer Care
Center at 877.280.1859.
If you would like more information about FedEDI, please
contact your Banker.
NOTE: If you receive healthcare ACH payments with an
addenda record, you will no longer receive the paper
notice after June 10, 2016.
We will provide you with the FedEDI service to receive the
addenda information associated with those payments.
If you need access to healthcare ACH addenda records,
contact your Banker or the Treasury Management Support
Center at 844.392.7796 to set up this service.
Returned Deposited Items. Beginning June 13, 2016,
all deposited items returned to your CBT account will be
charged back to the deposit account. We will continue to
process items on accounts with special instructions for
chargeback and redeposit, according to those instructions.
Stop Payments. The expiration of stop payment orders will
be six months, effective June 13, 2016. Stop orders on file
prior to this date will retain the five-year expiration.

IMPORTANT ACTION REQUIRED: Call CBT Customer Service at 303.680.1600 or your local
Banking Center to verify the phone number on file for your account. We will use this number in
our identity verification process when you log in to the new Business Online Banking service
beginning June 13, 2016.

Online Banking (your new online banking service). If you

Allpoint has more than 55,000 surcharge-free ATMs in
the United States, Canada, United Kingdom, Puerto Rico,
Australia and Mexico. Please visit www.allpointnetwork.
com to download a mobile app or find ATM locations.
ONLINE BANKING SERVICES
BUSINESS DEBIT CARDS
If you have (a) Visa® Debit Card(s) for your business to
access your deposit account(s), the daily withdrawal limits
may change.
Unless you are otherwise notified, beginning on June 13,
2016, the daily limit for an ATM withdrawal will be $500. The
daily limit for point of sale (POS) merchant transactions will
be $2,500. Contact your Banker to discuss other options if
you prefer different daily purchase limits on your card(s).
Debit cards with a savings account as the primary account
will be replaced with an ATM Card. If you wish to keep your
Debit Card, you will need to assign a checking account as
the primary account. Your Banking Center can assist you to
make these changes.
ATM NETWORK
The surcharge-free ATM network will be changing from
MoneyPass to the Allpoint® network beginning June 13,
2016. Debit and ATM cards will no longer be surcharge-free
with the Pulse, Cirrus, or MoneyPass ATMs after this date.
7 ONLINE BANKING SECURITY
Business Online Banking offers enhanced out-of-band
security to verify the identity of individuals accessing
account information.
On June 13, 2016, when you log in to Business Online
Banking, you will be asked to select either your business or
mobile phone number to receive an out-of-band voice or
text message to verify your identity. A verification code will
be sent to the phone number selected. This code is required
to complete the login process.
This out-of-band security code replaces the personal image
(watermark) and security questions used in The current
Online Banking Service.
BUSINESS ONLINE BANKING
(formerly the Centennial Bank Online Banking Service)
On Friday, June 10, 2016, the current online services will
be put in inquiry mode after 4:00 p.m., as we complete the
upgrade of our online banking and processing systems.
The NEW Business Online Banking system will be available
after 8:00 a.m. on Monday, June 13, 2016.

CBT’s new Business Online Banking provides you with new
and innovative features. We are committed to delivering
a richer online experience with more flexibility, reliability,
security and control.
4. Enter your new temporary password: Last four (4) digits
of your Social Security Number or TIN tied to your
primary business account.
ACCESS TO BUSINESS ONLINE BANKING
6. Enter your secure access code and register your personal
computer.
You will be able to log in to Centennial Bank and Trust’s
new Business Online Banking anytime after 8:00 a.m. on
Monday, June 13, 2016:
1. Go to CentennialBanking.com and click on the Login
box located on the home page.
2. Use the Business Online Banking section in the middle
of the login pop-up.
3. Enter your current username, which is case-sensitive.
NOTE: When entering your current Online Banking
ID, please use the unique username you set up, rather
than the 12-digit number (beginning with 8200). This
username would have been created by you, under
the Options tab.
5. You will be prompted to select how your secure access
code will be delivered (SMS text or voice call).

8. Read and accept the Terms and Conditions for Online
Banking.
9. You’re in! At this time you may set up any automatic
account alerts, future dated or external transfers you
had previously, as those will not be converted.
NOTE: Business Online Banking will retain more history
for your online access! In addition to the converted
history from May 1, 2015, history will continue to build
for up to 24 months.
As best practice, we encourage you to download your
history prior to June 10, 2016.
However, if you have always used this 8200 number
and did NOT set up a unique username, the number

HOW TO DOWNLOAD TRANSACTION HISTORY
Cleared Check Images. On June 13, 2016, you may not
have immediate access to cleared check image history. We
will build the check image history from May 1, 2015, to the
current date within 30–45 days after conversion.
As we continue to build the transaction and image history
available through Business Online Banking, you will have
access to 24 months of history—more than is currently
available through the current Online Banking Service.
The current service will be available in Inquiry Mode only
after June 13, 2016, for 30 days, allowing you access to
check images and eStatement history.
1. Log in to the current Online Banking Service.
2. When viewing your account, go to Transactions >
Download > Select Account, Date Range and Format.
Debit Card Functionality. Access to activate, reorder or
close a card through online banking will not be available
after June 10, 2016. These tasks can be efficiently handled
by contacting the Customer Care Center at 844.392.7796.
To report a lost or stolen card call 800.554.8969.
FEATURES AND FUNCTIONALTIY
eStatements. Though this is not a new service, you will
need to read and accept the eStatement disclosure after
your initial login to confirm your enrollment.
Alerts. Alerts are notifications sent to you when certain
events occur on your accounts. Your alerts will not be
converted.
Customers currently enrolled for eStatements will have
all accounts automatically enrolled for eStatement at
conversion.
Re-establish Alerts after your initial successful login on or
after June 13, 2016. When you set up the new alerts, you
may select to receive them as an SMS text message, phone
call or email.
eStatements help protect your financial data, are available
online for up to 24 statement periods and help to avoid fees
associated with a paper statement, if applicable. It is also
one of the most secure methods to receive your financial
data.
We also offer Security Alerts to help ensure your online
access is secure.
9 Protect your data and your business. If you’re not using
eStatements now, sign up through online banking after
conversion.
Export to Accounting Systems. Export account transaction
detail into current Quicken, QuickBooks, CSV or Excel
formats, which allows you to quickly create files for upload
into other financial management systems.
Customers using QuickBooks WebConnect to download
account transaction history from CBT will need to modify
QuickBooks settings.
NOTE REGARDING DIRECT CONNECT: CBT customers
will not be able to use Direct Connect to automatically
download information from Business Online Banking to
Quicken. We regret the inconvenience this may cause.
WebConnect or the export options discussed above can
be used in place of Direct Connect.
An instruction booklet is available on our website
(CentennialBanking.com/quicken-quickbooks) to guide
you through the update of QuickBooks settings. Contact
the Customer Care Center at 844.392.7796 if you are
a QuickBooks Online user and wish to use automated
account aggregation.
Loans. You will have the same functionality with regard to
viewing loan balances and loan activity.
Stop Payments. Online stop payment orders will not
be available after June 10, 2016, though we plan to reintroduce this functionality in the near future.
NOTE: The following will not convert from the current
system. You will need to re-establish these items in the
new system after you have logged in on or after June 13,
2016.

ONLINE BILL PAY
Prior
to June 10, 2016, we suggest you review any
bills due June 10-13, 2016, and consider making these
payments in advance.

Please know that every attempt will be made to properly
convert all your current payees. On Monday, June 13,
2016, we recommend reviewing all your payments and
payees to ensure all were converted properly. If any are
missing, you can re-establish them at this time.

Bill Pay customers will now select the due date.
Items can be processed the same day (with a cut-off time
of 2:00 p.m.).

New!
eBills allow you to receive bills online and set up
automatic payments. You may enroll each eBill within
Online Banking beginning Monday, June 13, 2016.
Payees eligible for the eBill service will have a link under
the payee’s name when you view your list.
MOBILE BANKING
On Monday, June 13, 2016, download the new Centennial
Bank and Trust Savvy™ Mobile Banking application anytime
after 8:00 a.m.
Use the QR Code below or visit the Google Play Store™ or
Apple® App Store to download the app to your smartphone.
To find the app, search "Centennial Bank and Trust mobile".
MOBILE DEPOSIT
You will be able to continue to deposit checks via mobile
remote deposit. Eligible checks can be deposited up to 6:00
p.m. for credit to your business account the same business
day.
You may enroll your business for this new feature after the
conversion is complete on June 13, 2016.

After your successful login to Business Online Banking, you
can find the Mobile Deposit enrollment located under the
Services tab. You must download and install the Mobile
Banking App to utilize Mobile Deposit.
Limits applicable to the number of items or the amount
deposited may change effective June 13, 2016. After this
date if you feel the limits are not sufficient, please contact
your Banker or the Customer Care Center at 844.392.7796
to request a different limit.
CUSTOMER RESOURCES / ONLINE TUTORIALS
We have prepared tutorials for Online Banking, Bill Pay
and Mobile Banking to guide you through the transition to
Centennial Bank and Trust’s new Business Online Banking.
These can be found online at CentennialBanking.com after
June 13, 2016. Click “login” on the home page and select
“Demo & User Guide” in the Business pop-up box.
Phone Support: To speak with an Online Banking specialist,
contact the CBT Customer Care Center at 844.392.7796.
NEW! 24-HOUR TELEPHONE BANKING
Centennial Bank and Trust’s automated telephone banking
service will assist you with general account inquiries such
as balances, deposits, transfers, etc. This service provides
24/7 access to your account and will be available beginning
June 13, 2016, by calling toll-free at 888.215.0125.
When you access CBT Telephone Banking, you will be asked
to provide the business TIN or Social Security Number
(SSN) associated with your business account. You will then
be prompted to establish a new PIN.
NEW! TEXT BANKING
Receive instant account information by sending text-based
commands from your cell phone. Get account balances and
receive recent transaction history without logging in.
Utilize the new Text Banking feature by enrolling through
Business Online Banking after June 13, 2016.
MERCHANT SERVICES
There are no changes planned for our merchant services at
conversion.
SWEEP SERVICES AND REPURCHASE
AGREEMENTS.
No changes will be made to these services.
WIRE TRANSFER PAYMENTS
Wire Request. Individuals authorized by your business to
create wire payments will have the following options to
submit wire instructions:
Incoming Wires and Wire Advice. Incoming wires will be
processed for same-day credit until 4:00 p.m. A wire advice
is generated for each incoming wire posted to your account.
▶ Via phone using a PIN.
Your business will begin to receive wire advices in the
mail after June 13, 2016, for wire payments credited to
your account. Mailed advices are generated at the close of
business and mailed the next business day.
▶ If you desire to initiate a wire request via telephone,
you will need to complete and execute a Call-In Wire
Agreement.
▶ PINs will be issued to those authorized in the Call-In Wire
Agreement. Phone requests to initiate a wire will not be
accepted without a valid PIN.
▶ If you initiate wires on the phone currently and your
business has not completed a Call-In Wire Agreement,
please contact your banker.
▶ In-person at a CBT Banking Center. Requestors will need
to be authorized for your business account.
▶ If you currently initiate wire instructions via fax, email,
scan or letter, contact your Banker to discuss new
security procedures and options.
A more secure and efficient method of receiving this
information is available with the option to receive advices
through fax or email. These electronic advices are generated
immediately upon the wire memo-post to the account.
To register for this free service to receive electronic advice
notification and discontinue the mailed advice, contact
your Banker to complete the Wire Notification Options form.
NOTE: The Fed reference number associated with a
wire payment is not reported in the transaction detail
through online banking. The Fed reference number and
full payment detail is available on the Wire Advice.

Incoming wires for CBT are routed through Dubuque Bank

We are excited to introduce new services to improve your
business banking experience with Centennial Bank and
Trust. You will have access to these services after June 13,
2016.
ACH Fraud Filter. An uncomplicated way to protect your
account against unauthorized ACH activity. You can decide
if you want to block:
▶ All ACH transactions,
▶ All ACH Credits,
▶ All ACH Debits or
▶ Establish the allowance of specific
transactions to post to your account.

After you have established the parameters to block
ACH activity on your account, ACH Fraud Filter works
automatically to return items without your intervention.
FedEDI ACH Reporting. Provides detailed transaction
information on CTX, CCD+, IAT and ACH return items
received to your account. When your designated account
receives one of these ACH types, FedEDI will send an
email notification. The email will contain a link to FedEDI,
and within the online service you can print or download
addenda detail on received transactions.
Business Credit Card Program. A new business credit
card program is available to help your business optimize
cash flow. This can add 25-55 days to your payables cash
cycle and pay no interest when you pay your balance in full.
There is no annual fee to use our card, and your business
earns reward points on qualifying purchases. GoToMyCard.
com provides no cost online access and eStatements.
Business Solutions Banking – more payment options.
In addition to the Business Credit Card program and
Business Bill Pay, our Solutions Banking provides access
to person-to-person payments, tax payments and payroll
direct deposit. More payment options for day-to-day cash
management of your business.
Retirement Plan Services2. Retirement plans should

allows employers the opportunity to offer a suite of

▶ Schedule recurring transfers and set up alerts.
▶ Review your Bill Pay payments and payees for accuracy.
▶ Sign up for eBills.
▶ All CBT Banking Centers will be open regular business
hours.
▶ Online systems become available after the open
of business. Sign in to Business Online Banking at
CentennialBanking.com after 8:00 a.m.
