RESPONSIBILITY
I hereby release Friends of Phoebie Animal Rescue Art Show (henceforth "the Show") the rights to display my artwork on July 27, 2015. I hereby
release any and all claims against Friends of Phoebie Animal Rescue Inc. (henceforth "Friends of Phoebie"), its Officers, associates, and sponsors for
damages and/or loss regarding my artwork; and I release, discharge, and agree to hold harmless the Show and Friends of Phoebie from any lost,
damaged, or stolen goods or personal items before, during, or after the time of the Show.
PHOTOGRAPHY AND USAGE WAIVER
I consent that the Show and Friends of Phoebie have full and unrestricted publishing and usage rights for any photographs (digital or print) taken of
me or any artwork submitted to the Show, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any
medium. I hereby release, discharge, and agree to hold harmless the Show and Friends of Phoebie from any liability by virtue of any blurring,
distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of
said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion
of privacy. I understand that any publication or usage of my artwork will credit me, the artist, by preferred publication name, year of completion,
and title of work.

RULES AND REGULATIONS
1. The artwork theme of the Show is: DOG- AND CAT-INSPIRED ARTWORK. All art must embody this theme to be considered for participation in
the Show. The overarching theme of the Show is “from shelter to home” but all dog- and cat-themed art will be considered.
2. All artwork must be original.
3. There is a limit of 1 artwork entry per person. There is no entry fee for the Junior Exhibit.
4. Entrants to the Junior Exhibit must be in middle school or of middle school age (homeschooled) to be eligible.
5. Artwork must be 2-dimensional, unmounted, and have a maximum size of 15 inches on a side, no minimum size. All mediums, including
photography and graphic art, are welcome.
6. All work must be submitted with an Official 2015 Friends of Phoebie Animal Rescue Art Show Junior Exhibit Entry Form that is fully completed.
Entries submitted without an official entry form or submitted with an incomplete entry form will not be considered.
7. All accepted artwork must ARRIVE at 169 Haviland Road, Queensbury NY 12804 by mail (addressed to Friends of Phoebie, c/o Summer Steves)
or delivery no later than 4:00 p.m. on Tuesday, June 30, 2015. No late entries will be accepted.
8. Selected entrants will be notified by July 13, 2015 that their artwork has been accepted and will be displayed in the exhibit.
9. Accepted artwork must remain on display for the duration of the Show.
10. Artists may collect their exhibited artwork(s) after the Show has officially ended. All artwork remaining after the Show must be picked up (at
the submission office) within fifteen (15) business days (weekdays only) of the closing of the Show, after which time your work will become the
property of Friends of Phoebie and Friends of Phoebie will have the right to discard, sell, or donate artwork as we see fit.

I give permission for my child/youth to participate in the Friends of Phoebie Animal Rescue Inc. Art Show. My signature below certifies my
agreement with the Terms, Conditions, and Policies outlined above as they pertain to my child/youth. In particular, I give my consent to the
Show and Friends of Phoebie to use my child's/youth's name, school, grade, age, and entry in the Junior Exhibit and for publications. I
understand that any publication or usage of my child's/youth's artwork will credit my child/youth (i.e. the artist) by name, grade, age, and title
of work. I understand that care will be taken in handling this artwork but I will not hold the Show, Friends of Phoebie, its Officers, associates,
and sponsors responsible for loss or damage to this artwork.

This is the OFFICIAL CALL TO JUNIOR ARTISTS FOR THE 2015 FRIENDS OF PHOEBIE ART SHOW!
At this time, we are accepting entries to become part of our Junior Exhibit. All artists in middle school or of middle
school age (homeschooled) are welcome to enter.
ABOUT THE ORGANIZATION
Friends of Phoebie Animal Rescue Inc. ("Friends of Phoebie") is a local, volunteer-based, non-profit organization devoted
to saving local homeless dogs and cats from being harmed and euthanized and giving them a second chance. We
retrieve stray, abandoned, neglected, and oftentimes abused animals from unsavory home environments, local medical
facilities, kill shelters, and surrendering owners and we devote our time and resources to providing them with veterinary
care, obedience training, and psychological and behavioral rehabilitation in an effort to eventually find permanent,
loving homes for them, regardless of how long it takes - sometimes weeks, sometimes years.
ABOUT THE SHOW
The Friends of Phoebie Animal Rescue Art Show ("the Show") is an exhibition appealing to art and animal enthusiasts
alike. We hope to bring our community closer together by celebrating our very own, local artists while simultaneously
increasing appreciation for rescuing animals and the positive impact that pets have on our lives. The event will be held
at Hiland Golf Club on Monday, July 27, 2015 from 4:00 p.m. to 7:00 p.m.
During the Show, members of the community will vote on their favorite works exhibited and prizes will be awarded at
the end of the event to the top entries. The event will also feature raffle items and door prizes. There are no entry fees
associated with the Junior Exhibit and junior artwork will not be for sale.
100% of the proceeds from this event benefit Friends of Phoebie Animal Rescue Inc. and go toward the care,
rehabilitation, and placement of local homeless dogs and cats.
HOW TO ENTER
1. Review the Rules & Regulations listed below.
2. Mail or deliver artwork with a fully completed entry form to Friends of Phoebie, c/o Summer Steves, 169
Haviland Road, Queensbury NY 12804. Please note that this address differs from our office and facility location
at 21 Blind Rock Road in Queensbury. For delivery purposes, the submission office at 169 Haviland Road in
Queensbury is adjacent to Hiland Golf Club. The office is open for business Monday through Friday from 8:00
a.m. to 4:00 p.m.

prints) taken of you at the event and for images of any artwork submitted to the Show for the purpose of

help, we can continue to save those in line to be euthanized, rescue those who are neglected and mistreated,
rehabilitate those who have lost trust in humans and their environment, train the disobedient, and love them all
because they deserve it.
