All for only $2000.00 Please Call: 1 (888) 360-5550 *Special Includes One-Piece C&B and Ball Attachment Implants (Includes Housing and O-Rings) and One-Piece Implant Kits The Tatum Times Page 2 www. tatuminstituteusa..com The Tatum Times Volume 6, Issue 1 A Simplified Approach to the Edentulous Maxilla By: Dr. James Gibney Utilizing a combination of sinus grafting, bone expansion and conventional dental implants, a technique is described for the implant supported fixed restoration of the fully edentulous maxilla in patients with the combination of a Division A, B, or C-w premaxilla and SA- 3 or SA-4 maxillary sinuses. Dr. James Gibney Figure 1 Pre-operative condition 9-08-00 Sinus Graft Surgery The patient is prepared for surgery using surgical aseptic technique. An IV is established and appropriate sedative drugs are administered. Local anesthetic is administered to achieve anesthesia for the surgical procedure. A palatally oriented crestal incision is made with mesial and distal release incisions. A full thickness flap is elevated to expose the lateral wall of the maxilla. Using a #8 surgical round bur with copious irrigation, an oval shaped window is created in the lateral wall of the maxilla without penetrating the sinus lining. This window is gently fractured inward and the sinus lining meticulously elevated off the inferior, lateral, medial, anterior and posterior walls of the maxillary sinus without tearing. A collatape dressing is placed in the superior aspect of the site and the site is grafted with irradiated cancellous bone plus 500 mg of amoxicillin (unless penicillin allergic). Primary closure is obtained with 4-0 vicryl sutures. The procedure is repeated on the opposite side. Implant Surgery – Anterior Maxilla Bone expansion is used to place four “D” implants in the positions of teeth positions 6, 8, 9 and 11. Osteotomy sites for four implants are created using a series of specially designed instruments. Implant site preparation is initiated by the use of mallet to drive a #15 scalpel blade, penetrating the soft tissue and bone in the desired implant locations. These incisions are expanded by tapping a channel former to the full length of the planned implant. The osteotomy sites are finished using a series of progressively larger socket formers to form receptor sites that match the size and shape of the proposed implants. The coronal threads of each implant are covered with a mixture of osteogen and blood and the Figure 2 Bone Expansion Instruments implants are inserted. Sutures are rarely required. The existing full upper denture is modified to avoid contact with the implants for the four month integration period. Implant Surgery – Posterior Maxilla After a four month healing period, additional implants are placed into the grafted posterior maxilla using the same bone expansion technique. These implants are placed in the positions of teeth 3, 5, 12 and 14. At this time it is also possible to load the four anterior implants. Appropriate abutment posts are cemented with resiment cement and prepared with diamond burs under copious irrigation. An eight unit acrylic provisional restoration is fabricated with single cantilever pontics in the positions of 5 and 12. ***When space allows additional implants may be placed. In this case an additional implant was placed in the position of tooth #4 at a later appointment. Figure 3 Immediate Post-op Posterior Implants 7-20-01 Page 3 The Tatum Times www.tatumsurgical.com Restorative The maxillary posterior implants are allowed to integrate over a six month period. Appropriate abutment posts are cemented with resiment cement and prepared using classic crown and bridge techniques. A full arch hydrocolloid impression is made and the case mounted on a semi adjustable articulator. A six unit maxillary anterior fixed bridge and two three unit posterior fixed bridges were fabricated and cemented with glass ionomer cement. Post op photos and radiographs were taken to document the case. A comparison of radiographs taken in 2002 with those taken in 2011 reveals no detectable bone loss. Page 4 The Tatum Times Volume 6, Issue 1 How Dirty is your Patient’s Toothbrush? Could you ever imagine using dirty silverware, dirty cups, dirty plates, or dirty bowls without cleaning them before eating or drinking? Of course not! Why not clean your toothbrush? Unlike most of the other things you put in your mouth every day, your toothbrush has tightly packed bristles which form areas that hold on to moisture, provide areas for germs, mold, bacteria, viruses and yeasts to grow and flourish which can cause illnesses like flu, cold, and diarrhea and can be harmful to your health. Scientific studies have shown that toothbrushes are an ideal breeding ground for harmful bacteria and germs. Cold and flu viruses, even bacteria that cause gingivitis, travel from toothbrush to toothbrush when stored closely together. This promotes re-infection and cross contamination of bacteria that is on the toothbrushes which can affect your immune system and lead to harmful diseases. This especially happens when the fine bristles in your toothbrush make tiny incisions in your gums injecting harmful germs directly into your bloodstream. Bacteria and harmful germs are introduced into your toothbrush in many ways. Some bacteria comes from your own mouth, others are airborne, and a lot of bacteria is placed on your toothbrush from water droplets that are launched form the toilet when you flush. Whichever way bacteria and harmful germs get into your toothbrush, the germs are there to stay and can be harmful to your health. If left unchecked the bacteria will multiply, filling your toothbrush with colony after colony of potentially disease causing germs. By the time you replace your toothbrush there could be thousands, if not millions, of bacteria in your mouth every time you brush.
