Cushions are often lost or misplaced when wheelchairs are stowed out of the way at night. The
Stay-Put™ Cushion ends the worry of losing or
misplacing cushions. It’s a new spin on traditional
wheelchair cushions, adding a hinged center and
fastening straps that secure under the sling seat.
The result—a cushion that folds and stays with
the wheelchair when closed for storage. When
ready for patient use, simply open the wheelchair
and the cushion is already attached, properly
placed, and ready for use!

Sit-Straight™ Cushions combine a Wedge
Cushion and Solid Seat Insert, creating an immediate positive change in posture. The patient sits in
a proper posture without the tendency to slide
forward in his/her chair, allowing participation in
self-care and recreational activities. This cushion is
also beneficial to caregivers, as it frees them from
continually repositioning patients and helps eliminate the need for restraints to keep patients from
falling out of the wheelchair.

Foam Cushions are a quick and inexpensive
solution to conventional wheelchair sling seats,
which provide little-to-no comfort or support,
leading to poor sitting tolerance, slumped posture,
pressure ulcers, and skin shear.

“Slow recovery” is the phenomenon that sets a
T-Foam Cushion apart from other traditional
foam wheelchair cushions. T-Foam retains its
compressed shape long enough to allow the
person’s capillary vessels to nourish tissues on
the weight-bearing surface. When a person sitting
on a T-Foam Cushion shifts or lifts their body even
slightly, T-Foam then recovers its original shape.
T-Foam’s open-cell structure permits air to circulate
freely through the material to prevent heat buildup
and perspiration. T-Foam is also fire-retardant,
exceeding CAL 117 specifications. Our T-Foam
cushions come with a removable stretch knit
cover that won’t interfere with the skin-protecting
qualities of the T-Foam and is easily launderable.

Gel cushions minimize heat and improve sitting
tolerance. We offer smooth or checkerboard
T-Gel; the checkerboard version provides greater
air circulation. The choice of gel is important
because of the different benefits. Polymer gels
are dense, rubber-like sheets of gel that mimic
the skin-protecting properties of fatty tissue.
They are far superior at minimizing friction and
shear forces. Aqueous gels are viscous fluid and
encased in a bladder and set atop of (or sandwiched between) foam layers. Aqueous gels
offer moderate pressure relief and cooling
comfort and are often more economical than
polymer gels.
AliMed T-Gel Cushions have a premium
polymer gel pad covered in a removable stretch
knit cover.

Pommel Cushions help reduce the risk of
adduction or internal rotation contracture, while
limiting forward sliding in the wheelchair. All of
our pommel cushions start with a convex base,
filling the sling seat to create a level sitting
surface, and promoting upright posture, improved
comfort, and increased sitting tolerance. We also
offer a Release Pommel Cushion, where the
pommel drops away for easy patient transfers.

associated with sacral sitting include skin
breakdown at the coccyx and thoracic regions,
falls from the wheelchair, poor sitting tolerance,
and loss of functional abilities. By positioning the
pelvis in an anterior tilt with the knees slightly
higher than the hips, a wedge decreases pressure
on the sacrum and coccyx, and encourages
improved breathing, better head control, and
better positioning for self-feeding.

facility looking to combat potential pressure
issues in a low- to moderate-risk population,

Convex Base Anti-Thrust—T-Gel version, p. 29
Anti-Thrust Positioning Cushions have a
higher front surface and a lower rear surface to
help prevent forward sliding in the wheelchair. The
sloped region between the front and rear surfaces
is positioned in front of the ischial tuberosities to
keep the pelvis in a neutral position and help
support upright sitting. Anti-thrust protection can
be helpful when extensor tone is present. Antithrust cushions offer a comfortable slide-control
alternative to pommel cushions, especially for
male patients.

Specialty Gel-Topped Cushions feature an
added layer of T-Gel that mimics fatty tissue,

What’s in a Foam
Cushion?
AliMed’s foam cushions come in a
range of densities and with varying
degrees of support. Compared to other
materials, foam cushions are a quick,
inexpensive solution that helps improve
comfort, stability, and support that relieves
pressure points of tissue breakdown that
cause ulcers.
Different foams are often used in combination, layered for their various properties of
softness, evenly distributed support, and
memory. AliMed offers four types of foam:
Basic Foam (Polyfoam)
Open-cell Polyfoam has interconnected, air-permeable cells
that make it lightweight and
flexible, with both durability
and compressibility.
A Polyfoam cushion bears weight while still
maintaining its original shape. The comfort
and support of a Polyfoam cushion varies
depending on the foam density. Less-dense
foam has a softer feel, but bottoms out more
easily under greater weight. Denser foam
offers less cushioning, but is more supportive.
Polyfoam can be custom-cut to fit the needs
of the user. For custom cushion ordering,
see p. 16.
Latex Foam Naturally derived from rubber,
latex foam is known for its resilience. Among
the most comfortable of cushions, it is made up
of interconnecting air cells, which react to
changes in position, sustaining a high comfort
level. (See p. 5).
Independent Cell Foam
Easily distinguished by its
checkerboard appearance, this
cushion is made up of blocks
of Polyfoam that independently act as springs, working
to evenly distribute pressure away from
sensitive, bony areas. The cuts between each
cell maximize airflow and reduce friction against
the skin. (See p. 6).
T-Foam™ (Memory Foam)
This more complex foam
cushion has a higher viscosity
and density than Polyfoam or
latex foams, causing the “slow
recovery” effect—it remains
compressed temporarily when the user shifts
his/her body weight, providing pressure relief on
weight-bearing body tissue surfaces. This allows
air circulation to reduce the buildup of body heat
and reduces shock and shear forces.

Fluid-Proof Brushed Flocked Covers
Velvety feel is soft against the skin. The urethane
inner coating and heat-sealed seams are fluidproof to protect the cushion from incontinence.
Covers are durable for long-term or heavy facility
use. Rear safety ties secure cushion to upright
supports for added security and fall protection.

elastic-yet-firm qualities of rubber latex,

Independent-Cell Cushions Aircell cushions have long been touted as the
leader in shear reduction and pressure relief, but
the high cost, complex setup, and ongoing maintenance needs make them a poor choice for a
general-purpose facility cushion. We created independent-cell foam cushions—a low-cost, easysetup product with many of the pressure- and
shear-reducing attributes of an air-cell cushion. The
70+ foam cells offer uniform pressure distribution.
The cells act as springs, adjusting to anatomical
contours and minimizing pressure over bony

COMFORT AND POSITIONING
Two important factors to consider when
purchasing a cushion are patient comfort
and positioning. To ensure that these
needs are met, AliMed offers cushions that
are made with slow recovery T-Foam™ for
reduced pressure points and added impact
protection, or in a wedge style to help
prevent sacral sitting.
AliMed T-Foam Cushions recover their
original shape at even the slightest shifting or
movement of the patient’s body, reducing
pressure points and pressure sores. When
subjected to sudden impact, T-Foam absorbs up
to 90% of impact shock and vibration. The
foam’s open-cell structure provides better
impact protection and pressure distribution than
regular open-cell foam, so that air can circulate
freely through the material to avoid heat buildup
and perspiration.
Wedge Cushions help prevent sacral seating by
positioning the pelvis in an anterior tilt with the
knees slightly higher than the hips. Pressure is
now decreased on the sacrum and coccyx, and
patients are now positioned for improved
breathing, better head control, and better
positioning for self-feeding. Wedge cushions
can also be used to keep users’ hips properly
positioned at 90° when seated in a reclined
position.

What is T-Gel™?
T-Gel is an elastomer gel—a solid sheet of
soft, rubber-like polymer that mimics fatty
tissue. With typical cushions, excessive
shear force leads to occlusion of blood flow,
which is seen as one of the biggest
contributing factors behind pressure sores
and discomfort. But T-Gel moves with the
skin to minimize the effects of skin-shearing

Sacral sitting can occur if a wheelchair
is too deep, too high, or uncomfortable.
If the chair is too deep, the user will
tend to slide forward into sacral sitting
because they find it difficult to maintain
the pelvis at the back of the chair. Sacral
sitting is the most common result of poor
seating provision, especially if the user
cannot reposition themselves.

When the NASA Ames Research
Center set out to find a way to
counteract discomfort for astronauts
who sit for long periods of time, they
invented a cushioning foam with memory;
that is, foam that recovers its original
shape. The foam’s unique characteristics
and remarkable temperature and
compression-rate sensitivity led to their
naming it T-Foam, or Tempur Foam
(temperature foam). When NASA made
T-Foam available to the public, AliMed was
first to use the technology. For more than
35 years, AliMed has provided the widest
selection of T-Foam products for medical
professionals and their patients today.
Why use T-Foam™?
“Slow recovery” is the phenomenon that
sets an AliMed T-Foam Cushion apart
from other foam wheelchair cushions.
T-Foam retains its compressed shape
long enough to allow the user’s capillary
vessels to nourish tissues on the weightbearing surface. When a person sitting on a
T-Foam Cushion shifts or lifts their body even
slightly, T-Foam then recovers its original
shape. When subjected to sudden impact,
T-Foam absorbs up to 90% of impact shock
and vibration. Because the foam’s cells are
interconnected so closely, T-Foam’s opencell structure provides better impact
protection and pressure distribution than
regular open-cell foam, and also permits air
to circulate freely through the material to
prevent heat buildup and perspiration. It is
also fire-retardant, exceeding CAL 117
specifications.

T-Gel is an elastomer gel—a solid
sheet of soft, rubber-like polymer

Gel-Foam Cushions
AQUEOUS GEL/FOAM
There are two standard types of
aqueous gel cushions—gel-encased
and gel-topped. Gel-encased cushions
have a gel bladder encased within the
foam and typically have two-tone covers.
The two tones act as a visual reminder to
staff/caregivers to regularly reposition
patients in their wheelchairs. By
establishing a schedule, staff members can
determine whether the patient has been
repositioned by which color is facing upward.
Gel-topped cushions have the gel bladder
sitting on top of the foam for lower contact
pressure on fragile skin and a cooler surface
to reduce perspiration and heat buildup.
Positioning Gel/Foam Cushions
(see pp. 20, 21) combine the comfort and
skin-protecting benefits of traditional gel/foam

SIT-STRAIGHT ™ CUSHIONS
A traditional level sitting surface
causes the pelvis to rest in a neutral or
posterior pelvic tilt. After a short time,
the user’s buttocks slowly slide forward
on the seat, resulting in sacral sitting.
Poor posture can increase the risk of
developing pressure sores. If the user
slumps forward into sacral sitting or they
have developed a scoliosis or pelvic
rotation, it will increase pressure on the
ischial tuberosities and sacrum.
Sit-Straight™—Promotes proper posture
Sit-Straight Cushions work by combining
the convex base design of the SSI with a
Wedge Cushion, creating a stable base to

adaptations for
stroke patients
For individuals who have had a stroke
and have not regained enough muscle
control to walk safely, a wheelchair is the
means to independent mobility.
However, wheelchairs were designed to
be propelled with two arms, and most
stroke patients have full use of only one.
Some patients can be taught to propel a
wheelchair using one arm and one leg, but this
only works for patients tall enough to reach the
ground with their feet while seated. This can be
a major inconvenience. Here are three
approaches to solve this important problem.
1. Use an AliMed® Stroke Cushion
The Stroke Cushion fills the sling-seat area
and protects ischial tuberosities with
shear-reducing gel. Stroke cushion nestles
into the concave seat without raising the
seat height. For added comfort, choose
the Stroke Cushion with a T-Foam™ layer.

2. Install a “drop seat”
Not all wheelchairs are adjustable to hemi
height. Drop seats replace wheelchair slingseats to lower the seating surface by
approximately 2". Drop seats install on
the chair frame and require the use of a
wheelchair cushion for protection. When
you install a drop seat, you won’t be able
to fold the chair unless it is removed.

3. Buy a hemi-height wheelchair
A hemi-height wheelchair has a seat that
is 2" lower than a standard wheelchair.
Average cost is $400 to $700. You’ll find
hemi-chairs from all of the wheelchair

Stroke Cushions with Low-Profile Solid Seat
Insert™ (SSI) Rigid closed-cell foam Solid Seat
Insert™ promotes upright sitting posture without increasing
the overall seat cushion height. Concave bottom fills a
wheelchair’s sagging sling seat and allows user’s legs to
reach the floor. Front waterfall edge reduces pressure at
distal edges of thighs. Three options to choose from:
Basic Stroke Cushion with SSI Gel insert in the rear of
the SSI offers coccyx protection. Gel/cushion interface helps
prevent window edema by creating a gradual pressure
transition between the firmness of the extra low-profile SSI
and the soft, pressure-relieving gel insert.
T-Foam™ Stroke Cushion with SSI offers greater
cushioning. Layer of 1"-thick, pressure-relieving Medium
T-Foam Memory Foam lays over SSI for comfort and
pressure distribution. Gel insert in the rear of the SSI
offers coccyx protection.
Pommel Stroke Cushion with SSI assists with
hip abduction. For those experiencing a slight increase in
hip adductor tone. Pommel keeps hips apart, improving
alignment and minimizing tendency to slide forward.

Anti-Thrust Positioning Cushions
have a higher front surface and a lower rear
surface to help prevent forward sliding in the
wheelchair. The sloped region between the
front and rear surfaces is positioned in front
of the ischial tuberosities to keep the pelvis in
a neutral position and help support upright
sitting. Anti-thrust protection can be helpful
when extensor tone is present. Anti-thrust
cushions offer a comfortable slide-control
alternative to pommel cushions, especially for
male patients.
AliMed Anti-Thrust Cushions Flat Base
models have nonslip vinyl covers. Convex Base
models ensure a level seating surface in
wheelchairs with sagging sling seats, and are

Pommel Cushions help reduce the
risk of adduction or internal rotation
contracture while limiting forward sliding
in the wheelchair. The pommel also helps
separate the knees to reduce skin shear
and allow air to circulate.
Pommel cushions are recommended for
residents who require a supportive sitting
surface and adduction control, who have a
tendency to slide forward or require post-op
hip replacement care, and those with lowerextremity spasticity.
AliMed pommel cushions start with a
convex base, filling the sling seat to create a
level sitting surface and promote upright
posture, improved comfort, and increased
sitting tolerance. There are three options
available to meet a range of patient needs:
Solid Seat Insert with Pommel
Choose from three different foam options.
Standard foam is recommended for patients
without skin integrity issues; T-Foam™ offers
excellent pressure redistribution for those with
low-to-moderate skin integrity issues; T-Gel
offers optimal sheer reduction for moderaterisk individuals by mimicking fatty tissue and
moving with the skin.
Sit-Straight Wedge with Pommel
Wedge shape positions knees slightly higher
than hips to discourage forward migration of
the pelvis, making them ideal for those with a
tendency for forward sliding.
Release Pommel Cushions
Standard pommel cushions utilize a
nonremovable pommel, which presents an
obstacle when transferring the patient. Our
revolutionary Release Pommel is attached
to the cushion by a secure retaining strap.
When it is time to transfer, the pommel
can be released and dropped out of the way,
allowing the transfer to be safely performed
with only one assistant.
Solid Seat Insert™ with Pommel An
abduction wedge that also eliminates the
effects of wheelchair seat sag. Firm fixed pommel
resists collapse for secure positioning. Surfaces
that contact user are padded. For enhanced
cushioning and pressure distribution, select a
model with a T-Foam™ or T-Gel™ layer covering the
entire sitting surface. Built-in Solid Seat Insert is
only 11⁄2" thick at the center, and fills sling-seat sag.

How does it work? The base cushion has
the 15°-20° sloped cutout at the front two-thirds
of the cushion surface. The detachable wedge
attaches securely on the non-operative side of
the cushion, but can be removed and
repositioned on the opposite side, depending on
the individual patient needs. The wedge creates
a flat sitting surface for the non-operative leg,
and allows the distal end of the operative leg to
rest slightly lower than the hip. The smooth but
secure hook-and-loop attachments ensure proper
positioning without risk of irritating skin.

SOLID SEAT INSERT
Solid Seat Insert™ (SSI) solves
the problem of "sling-seat sag" by
creating a flat, level surface that provides
postural support, balances the pelvis,
distributes weight evenly on both ischial
tuberosities, and discourages side-to-side tilt.
A sagging, curved sling-seat wheelchair,
on the other hand, leaves the user with a poor
sitting foundation. Common (yet less effective)
approaches involve using a hard surface to
bridge the sagging seat cavity. This means
raising the user too high, resulting in poor
stability. The SSI’s semirigid, self-cushioning
foam block with a convex bottom and flat top
effectively fills this cavity to achieve a flat sitting
surface while maintaining a proper height.
AliMed’s shaped-foam approach offers extra
cushioning, minimum seat rise, and light weight
(81⁄2 oz.). Seat flatness is significantly improved,
limited only by the extent of original seat sag.

Appropriate wheelchair seating is an
important factor when considering a
comprehensive approach to fall prevention.
Over time, an individual sitting in a
wheelchair is affected by muscle fatigue,
balance deficits, and pressure on soft
tissue—all of which can cause shifting or
scooting on the seating surface. The end
result is often a dangerous fall off the
wheelchair cushion. This doesn’t have to

FALL PREVENTION
Cushions can be used as alternatives to belts
or restraints to help prevent forward sliding in a
wheelchair, which can easily result in a fall. A
cushion design that is higher in the front and
lower in the back uses natural body mechanics to
do so by placing the knees slightly higher than the
hips. This position naturally tips the pelvis slightly
forward, decreasing the tendency to slouch and
slide forward on the seat cushion. There are three
basic cushion designs that achieve this: A basic
wedge, a pommel, and an anti-thrust.
Wedge Cushions come in all shapes and
configurations, but the basic principle is the same—
the front-to-back slope encourages an anterior pelvic
tilt. Wedge cushions also lend themselves well to
modifications that assist in skin protection. Options
include coccyx cutouts, gel toppers, and contoured
cushion surfaces.
Pommel Cushions work differently. They use a
center pommel at the front of the cushion that
physically blocks the user from sliding forward.
Pommel cushions have the added benefit of
positioning the hips in slight abduction for patients at
risk for developing hip adduction contracture
secondary to fracture or abnormal tone issues. Our
Sit-Straight™ Wedge with Pommel combines the
wedge and pommel designs for added benefit.
Anti-Thrust Cushions use a similar design
concept to a wedge, but are more effective than a
straight wedge at reducing forward sliding. They utilize
a quick slope in height from a high flat surface in the
front to a lower flat surface in the rear. The sloped
region (known as the pre-ischial ridge) is placed
anterior to the ischial tuberosities to keep the pelvis in
neutral and prevent forward migration. Anti-Thrust
cushions are ideal for patients who actively push their
hips forward on the seat, and are a good alternative to
pommel cushions for many men and heavier women.
Other considerations
When choosing any of these cushions, consider a flat
or convex (solid seat insert) base, the cushion material
(foam type and density, or gel), and cover material. If
you are fitting the cushion for a sling seat wheelchair,
a convex base and nonslip cover fabric with rear safety
ties are always a good bet. All of our wedge, pommel,
and anti-thrust cushions come in flat or convex base
models and most have rear safety ties.

VOLUME DISCOUNTS Item numbers, sizes,
colors, Lefts and Rights, etc. cannot be assorted for
a volume discount. One item #, one style per
volume discount order (unless otherwise specified).
SHIPPING We ship FedEx/UPS whenever
possible. Some small packages may be sent via
USPS. Charges are calculated at time of order and
are prepaid and added to invoice. Freight shipments
are freight collect. Shipping quotes available by
phone upon request. COD orders are not available.
Collect shipments are subject to a $7.50 processing
fee per order and can only be processed when
items are shipped direct from AliMed’s warehouse.
Any items shipped from an alternate site will have
charges prepaid and added to the invoice. Shipments to HI, AK, and Puerto Rico are sent by 2nd
Day Air unless otherwise requested. Shipments to
Canada may incur additional charges, without
limitation, duties, taxes, or country-specific fees.
International Shipments (except Canada) require a
freight quote. Contact our International Department
for a quote.
OVERSIZED ITEMS Subject to additional freight
charges. Applies to all orders with oversized items,
including contract orders, items with free ground
shipping, and free shipping promotions.
HAZARDOUS ITEMS Must ship ground and each
item is subject to a $25.00 handling fee.
TRUCK SHIPMENTS
Delivery Inspection Required
IMPORTANT NOTICE Products are delivered to
your receiving dock. Inside delivery and/or liftgate
services can be arranged for additional charges. All
truck shipments must be fully inspected by receiver
before signing for the delivery to ensure there is no
concealed damage. If you cannot inspect
immediately, make a note “pending inspection” with
your signature. Any damaged shipment received
should be refused by the receiver. These guidelines
must be strictly followed in order to receive any
credit on shipments made by truck.
OVERNIGHT TRUCK SHIPMENTS AliMed does
NOT provide overnight delivery on orders that
require delivery by truck.
DAMAGED GOODS/SHORT SHIPMENTS Must
be reported within 10 days of receipt. Be sure to
note any damage or poor box conditions when
signing for packages. If shipment arrives damaged
or with parts missing, contact the carrier
immediately. Be sure to save product packaging.
All shipping charges and risk of loss are F.O.B.
shipping point and are the responsibility of the
purchaser.

ALIMED PRODUCT RETURN POLICY Convenient
express returns are available for orders under
$100.00. See packing slip for details. All other
returns require a phone authorization. Unless
merchandise is defective, returns will be subject to
a 20% minimum restocking fee. Non-stocked items
that have been drop-shipped from our vendor will
be subject to restocking fees that vary by individual
vendor.
The guidelines below must be followed in order to
receive credit:
1. Product must be in new, resaleable, unmarked
condition and in original and unopened*
packaging.
2. Merchandise must be returned within 30 days
of invoice date. To ensure prompt return
processing, please attach the original packing
list, invoice, or return authorization (RA) number
to the return. Refunds will be for merchandise
value only. Please allow three to four weeks for
return processing.
No returns can be accepted for special orders,
custom products, products used for drinking and
eating, shoes, orthotics in opened packages, books,
videos, or toileting products.
• All returns are subject to a 20% minimum
restocking charge.
• Product must be in new, resaleable unmarked
condition and/or in original and unopened*
packaging.
• There are no returns on discontinued items.
• Original packing list or invoice must accompany
the return.
• No returns will be allowed if past 30 days from
invoice dates.
• Please allow 3-4 weeks to process your credit.

• No call tag/pickup slips will be issued. It is your
responsibility to return the merchandise to
AliMed, unless item is a Non Stocked item, in
which case you will be provided different

*Items arriving in bags or shrink wrap cannot be

a soft gel surface to reduce the risk of
skin-breakdown with sturdy support that
helps address “sling seat.” The soft, T-Gel
polymer surface mimics fatty tissue and moves with
the skin as a solid foam convex base supports the
patient and promotes better posture. T-Gel Plus
Cushions include a third middle layer that consists of
slow-recovery T-Foam that evenly allocates coccyx
