This resource guide has been developed by the Cabarrus Wellness Coalition to assist you with managing your type 2 diabetes. We have designed this resource guide to make it easier for our community members to get the help they need to lead healthy lives. The Cabarrus Wellness Coalition is a taskforce of Healthy Cabarrus through Cabarrus Health Alliance in partnership with the Cabarrus County Southeastern Diabetes Initiative (SEDI) and Community Transformation Grant Project. The coalition was formed as a result of the 2012 Cabarrus County Community Needs Assessment and partners with local organizations to improve the health and wellness of Cabarrus County residents with a specific focus on obesity and diabetes prevention and treatment. While most of these services are available to the whole community, because SEDI clinical resources are primarily geared to assist underserved populations, we have chosen to focus on increasing awareness of resources that can be used by uninsured, underinsured, and Medicaid/Medicare patients. Insulin helps us use blood sugar for energy. Diabetes can affect many parts of your body, like your heart, mouth, eyes, ears, and feet. Getting regular medical tests can help you and your healthcare team manage your diabetes to help prevent complications (problems) or find issues early when they are most treatable. Working with your doctor You can work with your doctor to make sure that you are getting the right tests to watch your type 2 diabetes and avoid problems. Below are some tests about which you should talk to your healthcare team. These are known as the ABCs of diabetes. Together, you can decide what your targets should be. A1C The A1C test measures your average blood sugar control for the past 2 to 3 months. It measures the percentage of glycated hemoglobin (GLI-kay-ted HE-meh-GLO-bin) or HbA1c, in the blood. It does not replace testing blood glucose daily. Recommended A1C Target: Less than Blood Pressure (BP) Blood pressure is the force of blood flow through your blood vessels. Having your blood pressure checked regularly and taking action to reach your blood pressure target can prevent or delay diabetes problems. High blood pressure causes your heart to work harder. It also raises your risk for heart attack, stroke, eye problems, and kidney disease. High blood pressure is a problem that won't go away without treatment and changes to your diet and lifestyle. Recommended BP Target: Below 120/80 mmHg Cabarrus Wellness Coalition 2014 • www.JustStick2It.org Page | 8 ABCs of Diabetes Cholesterol Cholesterol is a form of fat that is carried through the blood in two kinds of bundles called lipoproteins. It is important to have healthy levels of both. 1. HDL cholesterol - Called the “good” cholesterol. It may lower your risk for heart attack. 2. LDL cholesterol - This is sometimes called the “bad” cholesterol. It can clog arteries and cause heart problems. Triglycerides (try-GLISS-er-ides) are another kind of blood fat that raise your chances for a heart attack or stroke if your levels are too high. Normal artery Recommended Cholesterol Targets HDL Cholesterol: More than 40 mg/dl for men More than 50 mg/dl for women LDL Cholesterol: Less than 100 mg/dl Triglycerides: Less than 150 mg/dl Cholesterol build-up Cholesterol is influenced by blood sugar and blood pressure. If your Narrowed artery blood sugar and blood pressure are high, your cholesterol may be high, too. Some steps that you can take to lower your cholesterol are: If you smoke, get help to quit. Lose weight if needed. Exercise most days of the week. Eat a low-fat, low-cholesterol diet. Increase monounsaturated fats in your diet. These are canola oil, avocado oil, or olive oil. Your doctor may also prescribe cholesterol-lowering medicine. While working with your healthcare team is important in managing your diabetes, it is also important to prepare yourself mentally to deal with it. Diabetes can be scary. It can bring about many feelings like denial, guilt, anger, frustration, or even depression. These feelings are normal. It is okay to feel all of these emotions and many more. Remember that it is never too late to jump start diabetes self-management. The key is to be gentle with yourself because you are your best resource for managing your diabetes. The way that you deal with your emotions is called coping. There are many things that you can do to cope with type 2 diabetes—both negative and positive. Negative coping strategies like smoking, drinking excessive amounts of alcohol, overeating, or avoiding people may cause more health problems. Below are some examples of positive ways to cope. Positive ways to cope include: Having a positive support system like your family, friends, or church group Finding things that make you laugh Focusing on things that make you happy Attending group support sessions with people who understand you Building healthy relationships Increasing physical activity—try riding bikes with a friend or walking your dog Starting a new hobby like reading, cooking, scrap booking, knitting, or journaling Local Coping Services are listed on the next page. It can damage the small blood vessels in your retina, the back part of your eye. This is called diabetic retinopathy (ret-i-nop-ah-thee). Diabetes also increases your risk of having glaucoma (glaw-koh-muh), cataracts (kat-uh-rakts), and other eye problems. Signs of Problems You may not know there is any damage to your eyes until the problem is very bad. Your doctor can catch problems early if you get regular eye exams. If your doctor finds eye problems early, drugs and other treatments may help prevent them from getting worse. Preventing Eye Problems Control your blood sugar levels Control your blood pressure If you smoke, get help to quit Regular Eye Exams The eye exam may include: Adding special eye drops to enlarge your pupil, called dilating your eyes, so the eye doctor can get a good view At times, special photographs of the back of your eye Your eye doctor may ask you to come more or less often than once a year. Working with existing eye problems If you already have eye problems, ask your doctor if you should avoid some exercises that can strain the blood vessels in your eyes. Also, of the 79 million adults thought to have pre-diabetes, the rate of hearing loss is 30% higher than in those with normal blood sugar. How does diabetes contribute to hearing loss? Hearing depends on small blood vessels and nerves in the inner ear. Researchers believe that over time, high blood sugar levels can damage these vessels and nerves, making it harder to hear. Signs of Problems For most people, hearing loss happens over time. The symptoms can be hard to notice. Quite often, family members and friends notice hearing loss before the person experiencing it. Common signs of hearing loss include: Frequently asking others to repeat what they say Trouble following conversations that involve more than two people Thinking that others are mumbling Problems hearing in noisy places such as busy restaurants Trouble hearing the voices of women and small children Turning up the TV or radio volume too loud for others who are nearby Regular Hearing Exams Your doctor may not always screen for hearing loss during a physical. Even if your doctor does check for hearing loss, you may still "pass" the screening test in a quiet exam room. Ask your healthcare team if you should see an audiologist or EarNose-Throat Specialist (ENT). When you go to see your audiologist or ENT: Tell them that you have diabetes Request an Otoacoustic (o-to-ah-ku-stik) Emissions (e-mish-uns) test (OAE) to check the condition of the small hair cells in the ear Cabarrus Wellness Coalition 2014 • www.JustStick2It.org Page | 17 Ear Care & Hearing Preventing Hearing Problems Follow the 80 for 90 rule: listen to headphones at no higher than 80% of the highest volume for no longer than 90 minutes. Protect your ears in noisy situations with earplugs. Manage your blood sugar level. Clean your ears regularly. Do not swab or insert foreign objects into your ears. Eat healthy meals with vegetables, fruits, whole grains, and lean meats. When diabetes is not controlled, high sugar levels in your saliva help harmful germs grow, called plaque (plak). Plaque comes from eating foods that contain sugars or starches. Some types of plaque cause tooth decay or cavities. Other types of plaque cause gum disease. Gum disease can happen more often, be worse, and take longer to heal if you have diabetes. Having gum disease can make your blood sugar hard to control. Treating your gum disease may make it easier to control your blood sugar. Signs of Problems Check your mouth for signs of problems from diabetes. Some of the first signs of gum disease are swollen, tender, or bleeding gums. Sometimes there are no signs at all. You may not know you have gum disease until you have serious damage, but some signs are: Pain in the mouth, face, or jaw that does not go away Loose teeth Pain when chewing Changes in your taste Dark spots or holes in your teeth Healthy gums Diseased gums A sore, or ulcer, that does not heal Prevent Dental Problems Your best defense is to see your dentist twice a year for a cleaning and checkup. You can keep your mouth healthy by taking these steps: Keep your blood sugar numbers as close to your target as possible. Eat healthy meals with vegetables, fruits, whole grains, and lean meats. Brush your teeth at least twice a day with fluoride toothpaste. Use dental floss at least once a day. If you wear dentures, keep them clean. If you smoke, get help to quit. Talk with your doctor and dentist before the visit about the best way to take care of your blood sugar during dental work. Tell your dentist what medications you take. Bring your diabetes medicines and food with you to the dentist’s office if needed. Delay any non-emergency dental work if your blood sugar is not under control. If a problem gets very serious, it can lead to amputation, or removal of the foot. Checking your feet every day for cuts and other problems is important. Your healthcare team should check your feet during each visit as well. Here are some diabetes foot care tips to follow: 1. Examine your feet each day. 2. Wash and dry your feet daily. 3. If you have dry skin, rub lotion on your feet after you wash and dry them, but do not put lotion between your toes. 4. Take care of your toenails. 5. Protect your feet with shoes and socks. How to Examine Your Feet Check the tops and bottoms of your feet. Use a mirror or have someone else look at your feet if you cannot see them. Check for dry, cracked skin. Look for blisters, cuts, scratches, or other sores. Check for redness, increased warmth, or tenderness when touching any area of your feet. Check for ingrown toenails, corns, and calluses. Do not "pop" blisters or sores, but apply a bandage. It is about learning to eat in a healthy way. This may mean making some changes in what you eat now. When you have diabetes, eating well takes practice and planning. Eat healthy meals with vegetables, fruits, whole grains, and lean meats. A balanced diet includes 5 vegetables & fruits each day. Combine that with regular physical activity and your prescribed medications to feel better and stay healthier. Work with your doctor and/or dietitian to create a meal plan that: Works with your schedule Keeps your weight on track Improves your blood sugar, blood pressure, and cholesterol numbers Physical activity is as important as nutrition to prevent and manage diabetes. If you are new to exercise, start small by using the stairs if you normally take the elevator or walking around your living room when you are on a phone call. Then, build up to biking with a friend, walking around the block, swimming, or dancing. It is recommended to be active for 150 minutes each week. You can reach this goal in 10 minute segments or in bigger chunks if you have the time. The largest studies of diabetes prevention, including the US Diabetes Prevention Program, all used a package of both good nutrition and physical activity leading to reduction in the risk of developing type 2 diabetes. For people with diabetes, physical activity is an important part of controlling blood sugar levels as well as reducing the risk of problems such as heart attacks and strokes. Physical activity can also lead to weight loss. This is important for people with diabetes because being overweight can reduce the life expectancy of people with type 2 diabetes by up to eight years! Unfortunately, high blood sugar can cause problems with your sex organs and sexual functioning. Both men and women can experience sexual health problems caused by diabetes. For Men In men, diabetes can contribute to low testosterone levels and/or erectile dysfunction (ED). ED is when a man can no longer have or keep an erection. Compared to men without diabetes, men with type 2 diabetes are twice as likely to suffer from low testosterone. This can lead to diminished interest in sex, ED, reduced muscle mass, depressed mood, or lack of energy. Over time, high blood sugar can damage blood vessels and nerves. This includes the blood vessels and nerves in the penis which can contribute to ED. Smoking, being overweight, and being inactive can also lead to ED. For Women High blood sugar can lead to sexual problems in women including diminished interest in sex, painful intercourse due to vaginal dryness, frequent yeast infections, and fertility issues. In addition, women with type 2 diabetes who are thinking about having a baby will need to work closely with their doctor to make sure their blood sugar is in control the first few weeks of pregnancy all the way until the baby is born. Having tightly controlled blood sugar will help avoid birth defects, a premature baby, or a baby that is larger than normal. It may be difficult to control your blood sugar during times when your hormones are changing, like during your menstrual cycle, pregnancy, or menopause. Talk with your healthcare team to make a plan on how to keep your blood sugar in control during these times. Online resources can assist the patient in finding the right patient assistance program. The online resources include discount cards, health information, and links to government and community agencies. Eligibility for patient assistance programs is different for each company. Contact the companies and complete an application to see if you qualify. Below are some general requirements: US resident Must not have private or public health insurance with prescription coverage (Medicare or Medicaid) Have an income less than 200% - 300% of the Federal Poverty Guidelines Typically, eligibility is denied if: Patient is eligible for Medicare but not enrolled Patient is eligible for Medicare Part D but not enrolled Contact the patient assistance program to see their eligibility requirements. Each program has different eligibility and financial requirements and they change periodically. Some programs need your doctor’s office to help with the application process. Other programs send medications to your doctor’s office and some send them directly to your house. You will need to allow time for application approval and processing. They can give you a list of suppliers who are under contract with Medicare to provide the new prices. If you prefer getting your supplies locally, you can also get your supplies from a pharmacy or store that accepts Medicare assignment. Ask your pharmacist if they accept Medicare assignment before getting your prescription filled. Any pharmacy or store that says it accepts Medicare assignment can only charge you for any unmet deductible (Medicare Part B) and your 20 percent coinsurance. Stores and pharmacies that don't accept Medicare assignment can charge you more. The cost of diabetic supplies will be the same for mail-order suppliers and pharmacies or stores that accept Medicare assignment. The national mail order program applies to people with both type 1 and type 2 diabetes. It covers blood sugar test strips, control solution for the test strips, lancets, the lancing device and batteries for your blood sugar monitor.
