I am thrilled to look back at 2015 as a record
breaking year for Reliant Medical Group
Foundation. It was a remarkable year of growth
and change.
Thanks to the generosity of all our individual
donors, including grateful patients, local
foundations, corporate sponsors and volunteers,
we reached three major milestones in 2015. Our
signature event, The Drive for a Difference Charity
Auction and Golf Tournament, raised over
$500,000 for the first time. We raised $300,000 in
gifts and pledges to the ReadyMED PLUSSM urgent
care capital project. We also awarded more than
$350,000 in health-focused grants this year. These are major accomplishments
and something that we should feel proud about.
As we roll into 2016, we are very excited about further expanding our capacity
to give meaningful relief to those in need within our community. With
the 2015 merger between Southboro Medical Group and Reliant Medical
Group, the Reliant Foundation takes on a new service area. We are excitedly
working with our new Southborough Medical Group colleagues and partners
to link our collective generosity to identified needs within the Metro West
community. We are so fortunate to add their energy and insight as to how we
can together serve the needs of our expanded community.

Our mission in granting priorities remains constant. Reliant Foundation
is committed to improving the health and well-being of individuals of all
ages in the Central and Metro West regions of Massachusetts. We focus our
grant making on community programs that battle childhood obesity and on
resources that increase the care and comfort of Reliant patients beyond what
insurance covers. We continue to make strong investments in programs that
set people on course for healthy living including Let’s Go! 5-2-1-0 and Reach
Out and Read.

What do these have in common? They’re all part of the Let’s Go! 5-2-1-0 experience that
Reliant Foundation funds for pediatric patients and their families.
The goal? To decrease the rate of weight gain by children who are overweight or obese.
How? Teach children and their families to change daily behaviors like decreasing screen time,
increasing minutes in activity, eating breakfast more often, cutting down on sugary drinks, eating less
fast food and eating together as a family more often.
Easier said than done? “Education is so important,” said Martha Waite, MD, who along with Dale St. Lawrence, MD, initiated

the collaborative program with the help of Reliant’s dietitians and the YMCA of Central Massachusetts in 2013. “The kids come away much more
knowledgeable, and we see the results. It’s really exciting!” she said.

Three years into the program, some patients are even surpassing the goals during each eight-week session. “The goal over that time is really to stop gaining
weight or to decrease your rate of gain because it’s really the rate of gain in children that’s a problem,” said Dr. Waite. In a session among kids 9-16 years
old this year, for example, 50 percent lost weight, and all others had a decrease in the rate of weight gain. In the half-mile run, 83 percent increased their
speed from the first to the last week of the program, while one child stayed stable.

“Growing up, my mom worked multiple jobs, and my dad had
worked second shift. After school, I went to the YMCA, became
a life guard and had a life lesson that you can reach high goals if
you are willing to put in the time and effort. Today, I am happy to
give back to the community where I live and work because I know
many families are the same as we were. The Reliant Foundation
supports so many organizations in our community like the YMCAs.
It’s easy to give back, and I trust the Foundation’s choices to help

In each eight-week session, children, siblings and at least one parent attend
a weekly one-hour class. It is typically divided into hands-on nutrition
education led by Reliant pediatric dietitian Kim Minogue, MS, RD, LDN
and fitness, led by a YMCA trainer. Additionally, families have one smallgroup session with a trainer during the week and have access to the Y facilities
throughout the session. Reliant pediatricians refer a patient and family into
the program, which has been run at the YMCA’s branches in Worcester
(Central and Greendale) and Westborough (Boroughs).

“The best part for me is exercising with my patients,” said Dr.
St. Lawrence. “Doing Zumba together is really funny because
they know I’m really bad at it! But I’ve also learned a lot of great
ideas from our dietitian, and I now incorporate that nutrition
information into conversations I have with patients at my office.”
The multi-disciplinary team has modified the program as they
learn more about elements for success. “Our next area to look at in
the program is follow-up, finding ways to keep families engaged,”
said Dr. St. Lawrence. “Keeping the rate of weight
gain down is huge. As they grow,
their BMIs drop over time
if the rate of gain does not
increase. It may take years
to see the effect, so keeping
families engaged will be so
important.”

Medical Director for Pediatrics. “Reach Out and Read promotes early literacy, which is proven to be a key element in terms of developmental progress and
school readiness.”
Reliant Foundation supplements the national Reach Out and Read program in Reliant’s family practice and pediatric departments to help prepare our
youngest patients for healthy lives. This year, Reliant Foundation helped distribute more than 16,000 new books to Reliant patients from six months to
five years, the time when the majority of all brain development occurs.
Reading aloud to children has a major impact on early cognitive, language and literacy skills. Reach Out and Read builds on the unique relationship
between providers and parents to engage them in reading aloud to their children every day. Children whose families engage in Reach Out and Read are
proven to enter kindergarten with larger vocabularies and stronger language skills than children who are not part of the program. Reach Out and Read
children also score three to six months ahead of their peers on vocabulary tests. These early language skills put children on a path to success!
“Reach Out and Read is not about learning to read,” said Dr. Paz-Soldan, MD. “It’s about developing early literacy skills by reading together, sharing
together, and engaging with your child to help them develop to their fullest potential.”
When the provider hands the new book to a parent or child at the beginning of a visit, it opens a conversation about sharing an activity and how to
stimulate your children while reading together. It also gives the provider an opportunity to observe how the parent and child engage and how the child
responds to the book, which helps evaluate developmental progress.
Children receive culturally and age-appropriate books. “We’ve gone out of our way to be sure we have books in several languages, especially Portuguese
and Spanish,” said Beth Wigden, MD, a pediatrician at Southboro Medical Group’s Framingham site. “Parents’ eyes light up when we give them a book
in their own language to read to their child. And when they have books in English to read to their children, they see it as a benefit, a way to help improve
their own English,” she said.

Foundation are living it once again, for the benefit of patients.

“We are patient-centric,” says Raj Hazarika, MD, Chair of Same Day
Services. “And our new project was designed to fill our patients’ needs
for both acute care and for many patients with chronic conditions who
require regular outpatient treatments.”
With the help of generous donors, Reliant Foundation is proud to support
patient-focused capital components of the new, 14,000 square-foot
ReadyMED PLUS, responding to the increasing need for urgent care and
providing more convenient, time- and cost-effective expanded services for
patients. Set to open in March 2016 on Shrewsbury Street in Worcester,
ReadyMED PLUS includes 17 exam rooms, a stat lab, diagnostic space for
X-ray and CT and a completely separate infusion center to accommodate
10 patients at a time.
“Reliant Foundation itself is a rare organization, as the vast majority
of medical practices do not have their own charities,” said Bill
Balcom, MD, Foundation president. “And that fits the personality
of Reliant. When Dr. Fallon established the practice 86 years ago,
we were the first group practice in Central Massachusetts. He truly

pursue new trails, including the creation of an Urgent Care department
more than 30 years ago.” The Urgent Care (now Same Day Services)
department has grown to include five ReadyMED sites, from Auburn to
Hudson, as Reliant was the first to bring physician-supervised acute care
clinics to the region.
With support from Worcester-area private foundations and our generous
donors who made gifts to the “Fund-A-Need” at the Drive for a Difference
Charity Auction, the Reliant Foundation team has raised $300,000 for
this project. Auction donors specifically helped us fund the new, 10-bay
infusion center.
The infusion center will serve patients who come to ReadyMED PLUS
with acute illness, as well as patients with a range of chronic diseases,
whose health depends on regular infusion treatments of high-powered
medication. Those treatments ease debilitating symptoms of diseases
like multiple sclerosis, Crohn’s Disease, rheumatoid arthritis, ulcerative
colitis and many others. Each infusion treatment lasts about two hours.
“Having the infusion center will be so convenient for patients of all ages
who depend on these treatments,” said Dr. Hazarika. “They can schedule
treatments outside of their work or school hours, in the evening or on the
weekends or holidays. We’re open 365 days a year for our patients.”

More employees than we can count helped make the
holidays merry and bright for thousands of individuals
during our annual Holiday Goods Drive. Together, we
donated food to Worcester County Food Bank and food
pantries in Upton, Worcester, Auburn, Southborough,
Hudson and others. Our team fulfilled wish lists for
children—resettled refugees, those living in foster care, kids
living in shelters after fleeing domestic violence, children
in families served by food pantries…and many more. We
shared the basics—warm mittens, hats and gloves, and clean
socks and undergarments—for men served by Veterans, Inc.
and other local shelters. And, much more.

Rev. Dr. Paul Kennedy has been a respected philanthropic figure in Worcester County for many decades, and
as a valued member of the Board of Trustees of the Reliant Foundation for many years, he remains a Trustee
Emeritus. His positive influence on our charity and on people in need is immeasurable.
A restricted fund established in his name in 2002 made annual grants to organizations in Central Massachusetts
that provided access to health care for vulnerable populations. Over the last several years, Rev. Kennedy directed
annual grants to Children’s Friend’s School Age Mothers’ program until the program was moved in 2014 to the
Worcester Public Schools. With that specific need fulfilled, Rev. Dr. Kennedy chose to close out the remaining
funds by making a final grant to a local nonprofit making an impact on at-risk youth.

That’s the pioneering spirit, and Reliant Medical Group and Reliant
Foundation are living it once again, for the benefit of patients.

“We are patient-centric,” says Raj Hazarika, MD, Chair of Same Day
Services. “And our new project was designed to fill our patients’ needs
for both acute care and for many patients with chronic conditions who
require regular outpatient treatments.”
With the help of generous donors, Reliant Foundation is proud to support
patient-focused capital components of the new, 14,000 square-foot
ReadyMED PLUS, responding to the increasing need for urgent care and
providing more convenient, time- and cost-effective expanded services for
patients. Set to open in March 2016 on Shrewsbury Street in Worcester,
ReadyMED PLUS includes 17 exam rooms, a stat lab, diagnostic space for
X-ray and CT and a completely separate infusion center to accommodate
10 patients at a time.
“Reliant Foundation itself is a rare organization, as the vast majority
of medical practices do not have their own charities,” said Bill
Balcom, MD, Foundation president. “And that fits the personality
of Reliant. When Dr. Fallon established the practice 86 years ago,
we were the first group practice in Central Massachusetts. He truly

pursue new trails, including the creation of an Urgent Care department
more than 30 years ago.” The Urgent Care (now Same Day Services)
department has grown to include five ReadyMED sites, from Auburn to
Hudson, as Reliant was the first to bring physician-supervised acute care
clinics to the region.
With support from Worcester-area private foundations and our generous
donors who made gifts to the “Fund-A-Need” at the Drive for a Difference
Charity Auction, the Reliant Foundation team has raised $300,000 for
this project. Auction donors specifically helped us fund the new, 10-bay
infusion center.
The infusion center will serve patients who come to ReadyMED PLUS
with acute illness, as well as patients with a range of chronic diseases,
whose health depends on regular infusion treatments of high-powered
medication. Those treatments ease debilitating symptoms of diseases
like multiple sclerosis, Crohn’s Disease, rheumatoid arthritis, ulcerative
colitis and many others. Each infusion treatment lasts about two hours.
“Having the infusion center will be so convenient for patients of all ages
who depend on these treatments,” said Dr. Hazarika. “They can schedule
treatments outside of their work or school hours, in the evening or on the
weekends or holidays. We’re open 365 days a year for our patients.”

Medical Director for Pediatrics. “Reach Out and Read promotes early literacy, which is proven to be a key element in terms of developmental progress and
school readiness.”
Reliant Foundation supplements the national Reach Out and Read program in Reliant’s family practice and pediatric departments to help prepare our
youngest patients for healthy lives. This year, Reliant Foundation helped distribute more than 16,000 new books to Reliant patients from six months to
five years, the time when the majority of all brain development occurs.
Reading aloud to children has a major impact on early cognitive, language and literacy skills. Reach Out and Read builds on the unique relationship
between providers and parents to engage them in reading aloud to their children every day. Children whose families engage in Reach Out and Read are
proven to enter kindergarten with larger vocabularies and stronger language skills than children who are not part of the program. Reach Out and Read
children also score three to six months ahead of their peers on vocabulary tests. These early language skills put children on a path to success!
“Reach Out and Read is not about learning to read,” said Dr. Paz-Soldan, MD. “It’s about developing early literacy skills by reading together, sharing
together, and engaging with your child to help them develop to their fullest potential.”
When the provider hands the new book to a parent or child at the beginning of a visit, it opens a conversation about sharing an activity and how to
stimulate your children while reading together. It also gives the provider an opportunity to observe how the parent and child engage and how the child
responds to the book, which helps evaluate developmental progress.
Children receive culturally and age-appropriate books. “We’ve gone out of our way to be sure we have books in several languages, especially Portuguese
and Spanish,” said Beth Wigden, MD, a pediatrician at Southboro Medical Group’s Framingham site. “Parents’ eyes light up when we give them a book
in their own language to read to their child. And when they have books in English to read to their children, they see it as a benefit, a way to help improve
their own English,” she said.

“The best part for me is exercising with my patients,” said Dr.
St. Lawrence. “Doing Zumba together is really funny because
they know I’m really bad at it! But I’ve also learned a lot of great
ideas from our dietitian, and I now incorporate that nutrition
information into conversations I have with patients at my office.”
The multi-disciplinary team has modified the program as they
learn more about elements for success. “Our next area to look at in
the program is follow-up, finding ways to keep families engaged,”
said Dr. St. Lawrence. “Keeping the rate of weight
gain down is huge. As they grow,
their BMIs drop over time
if the rate of gain does not
increase. It may take years
to see the effect, so keeping
families engaged will be so
important.”

responsibilities and deadlines that we forget about others who are in need.

What do these have in common? They’re all part of the Let’s Go! 5-2-1-0 experience that
Reliant Foundation funds for pediatric patients and their families.
The goal? To decrease the rate of weight gain by children who are overweight or obese.
How? Teach children and their families to change daily behaviors like decreasing screen time,
increasing minutes in activity, eating breakfast more often, cutting down on sugary drinks, eating less
fast food and eating together as a family more often.
Easier said than done? “Education is so important,” said Martha Waite, MD, who along with Dale St. Lawrence, MD, initiated

the collaborative program with the help of Reliant’s dietitians and the YMCA of Central Massachusetts in 2013. “The kids come away much more
knowledgeable, and we see the results. It’s really exciting!” she said.

Three years into the program, some patients are even surpassing the goals during each eight-week session. “The goal over that time is really to stop gaining
weight or to decrease your rate of gain because it’s really the rate of gain in children that’s a problem,” said Dr. Waite. In a session among kids 9-16 years
old this year, for example, 50 percent lost weight, and all others had a decrease in the rate of weight gain. In the half-mile run, 83 percent increased their
speed from the first to the last week of the program, while one child stayed stable.

“Growing up, my mom worked multiple jobs, and my dad had
worked second shift. After school, I went to the YMCA, became
a life guard and had a life lesson that you can reach high goals if
you are willing to put in the time and effort. Today, I am happy to
give back to the community where I live and work because I know
many families are the same as we were. The Reliant Foundation
supports so many organizations in our community like the YMCAs.
It’s easy to give back, and I trust the Foundation’s choices to help
our community!”

In each eight-week session, children, siblings and at least one parent attend
a weekly one-hour class. It is typically divided into hands-on nutrition
education led by Reliant pediatric dietitian Kim Minogue, MS, RD, LDN
and fitness, led by a YMCA trainer. Additionally, families have one smallgroup session with a trainer during the week and have access to the Y facilities
throughout the session. Reliant pediatricians refer a patient and family into
the program, which has been run at the YMCA’s branches in Worcester
(Central and Greendale) and Westborough (Boroughs).

I am thrilled to look back at 2015 as a record
breaking year for Reliant Medical Group
Foundation. It was a remarkable year of growth
and change.
Thanks to the generosity of all our individual
donors, including grateful patients, local
foundations, corporate sponsors and volunteers,
we reached three major milestones in 2015. Our
signature event, The Drive for a Difference Charity
Auction and Golf Tournament, raised over
$500,000 for the first time. We raised $300,000 in
gifts and pledges to the ReadyMED PLUSSM urgent
care capital project. We also awarded more than
$350,000 in health-focused grants this year. These are major accomplishments
and something that we should feel proud about.
As we roll into 2016, we are very excited about further expanding our capacity
to give meaningful relief to those in need within our community. With
the 2015 merger between Southboro Medical Group and Reliant Medical
Group, the Reliant Foundation takes on a new service area. We are excitedly
working with our new Southborough Medical Group colleagues and partners
to link our collective generosity to identified needs within the Metro West
community. We are so fortunate to add their energy and insight as to how we
can together serve the needs of our expanded community.

Our mission in granting priorities remains constant. Reliant Foundation
is committed to improving the health and well-being of individuals of all
ages in the Central and Metro West regions of Massachusetts. We focus our
grant making on community programs that battle childhood obesity and on
resources that increase the care and comfort of Reliant patients beyond what
insurance covers. We continue to make strong investments in programs that
set people on course for healthy living including Let’s Go! 5-2-1-0 and Reach
Out and Read.
