COMMUNITY EQUIPMENT SERVICE (CES) COMPETENCIES/GUIDELINES FOR PRESCRIBERS AIM All prescribers/authorisers of equipment will be familiar and competent to use the following information, which is available on the CES pages of [email protected], Tarkanet (North Devon) and DCC website. OUTCOMES • Clear accountability and responsibility of prescribing and authorising at complex care team level. • Clear clinical reasoning to support the request for equipment form the Community Equipment Service that meets the clients need in accordance with standard practice and policies. • Accountability at prescriber/authoriser level to track/review high spend equipment for example mattresses and step up/down as required. • Ensuring clients get the right suitable equipment to support them in the community at the right time and that this is returned if not required. • Prescribing/authorising trends will be audited and reviewed both at complex care tem level and strategically. The FACS criteria must be applied at the time of assessment and when reviewing needs. The framework is based on individuals’ meds and associated risks to independence and includes four eligibility bands – critical, substantial, moderate and low. When placing individuals in these bands, the guidance stressed that councils should not only identify immediate needs, but also needs that would worsen for the lack of timely help. The following criteria for each band describes the seriousness of the risk to independence o other consequences if needs are not addressed. However the Threshold for Services and equipment provision as of July 2004 is: Critical and Substantial only Critical –when • Life is, or will be, threatened; And /Or • Significant health problems have developed or will develop; And/Or • There is, or will be, little or no choice and control over viral aspects of the immediate environment; And/Or • Serious abuse or neglect has occurred or will occur; And/or • There is, or will be, an inability to carry out v8ital personal care or domestic routines; And/or • Vital involvement in work, education or leaning cannot or will not be sustained; And or • Vital social support systems and relationships cannot or will not be sustained; And or • Vital family and other social roles and responsibilities cannot or will not be undertaken. Substantial – when • There is, or will be, only partial choice and control over the immediate environment; And/Or • Abuse or neglect has occurred or will occur; And/Or • There is, or will be, an inability to carry out the majority or personal care or domestic routines; And/Or • Involvement in many aspects or work, education or learning cannot or will not be sustained; And/Or • The majority of social support systems and relationships cannot or will not be sustained; And/Or • The majority of family and other social roles and responsibilities cannot or will not be undertaken. Low –when • There is, or will be, an inability to carry out several personal care or domestic routines; And/Or • Abuse or neglect has occurred or will occur; And/Or • Involvement in one or two aspects or work, education or learning cannot or will not be sustained; And/Or • One or two social support systems and relationships cannot or will not be sustained; And/Or • One or two family and other social roles and responsibilities cannot or will not be sustained; And/Or • One or two family and other social roles and responsibilities cannot or will not be undertaken. Check within Satellite store if equipment is available, if so, issue direct from store b. Prescriber to complete CES form – ensure all fields are completed and email to authoriser with explanation in email need for urgent delivery. Prescriber to contact authoriser by telephone to prompt. For next day delivery emails but have been received by CES by 1pm. c. If agreed authorise to email direct to [email protected] confirming authorisation and copy prescriber in email**** please note decision to supply date is the date authorisation has been gained. d. The rationale for equipment needs to take into account clinical assessment, care provision, risk assessment and professional judgement. BASIC MATRESS/CUSHION For Patients:1. 2. 3. 4. How to order Equipment from the Community Equipment Stores 2222CES Equipment June 2010 Completing the CES order form When completed all order forms to be emailed to [email protected] unless they require special authorisation. Order should only be faxed if the computer is unavailable. You must state the measurements required, for example walking frame to be set at height of ……. Toilet frame to be set at height of…… Please ensure that all the boxes are completed. Requisition date – is the date the order form is completed and sent to CES. The decision to supply date is when the decision to make the order was made. This particularly applies when authorisation is required – the decision to supply date is the date it is authorised and sent to CES These should ideally be the same day or close together. Order raised by - must be the name of an authorised prescriber. Delivery/Collection – Please ensure that this is completed. Collections must be on a separate form from delivery. Please ensure that the correct box is selected to indicate type of delivery or collection – one box only for each item. The delivery priority – box has a drop down menu for 7 working days delivery, 3 working days delivery or authorised urgent. a) 7 working days delivery does not mean that the delivery will take 7 days. Attached is delivery schedule for each area. b) 3 working days delivery costs £36 and is to reduce the use of same day delivery/next day deliver requests. c) Same day/next day delivery costs £90 er and the requests need to be with CES before 1pm the previous day. The loan period is drop down menu for the length of time you require the item to be with the patient. GP Practice code must be entered. Each base has a list of the practice codes for their area. If you are unsure of the code please enter the name of the practice not the GP. The catalogue code must be entered and can be found in the CES catalogue in the Community Equipment Service Section on Infopoint, Tarkanet and DCC Website. Please ensure you use the up to date codes, if the item you are requesting is not in the catalogue then it is a special item. Insert special into the code section. The price band can also be found in the appropriate catalogue page. Special instructions must indicate information such as height required which room the equipment is to be based in, instructions for access to the house etc. Please ensure that the delivery/collection address and contact person fields are completed if different from Client’s address or prescriber. Financial/special authorisation to be completed by authoriser. Deliver means CES deliver the piece of equipment only. Deliver and Fit means CES will deliver piece of equipment and fit to furniture etc. Delivery, Fit and Demonstrate means CES will deliver and fit equipment and demonstrate how to use it. E.g. how a bath lift goes up and down. The driver will not assess the safety of the client using the piece of equipment. If an individual has been assessed as requiring individual instructions or further practice then the prescriber will need to provide this.
