1. This is where your Herbalife Distributor
Identification Number (ID) is located. Please
keep the Registration Card that accompanies
your Distributor application with you at all times.
You may also use this number to place orders
until your Distributor Application is processed.
2. Your Canadian Social Insurance Number.
3. Your last name, first name and middle initial
(this will be the name of your Distributorship,
which must be in the name of an individual).
4. Your address. All correspondence and future
earnings will be mailed to this address.
5. Your main telephone number.
6. Your date of birth (if under 18).
7. Your country of permanent residence (if
other than Canada).
8. Your Sponsor’s name, address, telephone
number and Herbalife ID number. This
information is crucial to properly establish
your Distributorship.

All Orders must be paid in full, processed and accepted for

■ Cash⁄Debit Card - Canadian Dollars only. Cash and Debit
Card may be used for walk-in orders only.
■ Credit Cards - American Express, Visa and MasterCard.
The credit card must be in the name of the purchaser. Credit
cards may be used for all order types.
■ Automatic Payment System (APS) - If you have been a
Fully Qualified Supervisor for at least 30 days; approved
applications (available through the Sales Order Call Centre or
Distributor Relations Department) must be on file at
headquarters, and you must have a 4 digit Personal
Identification Number (PIN). This system automatically
deducts the amount of your order from a designated Canadian
bank account. APS may be used for all order types with the
exception of HAP orders.

completed and the information listed on page -5- readily
available before calling. Again, this will help ensure speedy
and accurate processing of your order, as we are unable to
place orders on hold awaiting information. You may pay for

■ Personal Cheques - You must be a Fully Qualified
Supervisor for 30 days or more to be eligible to use
personal cheques (cheques must be drawn on a Canadian
Bank). Supervisors may use personal cheques only for their
own orders or orders for their downline Distributors. All
personal cheques must be preprinted with the Fully Qualified
Supervisor’s name. Postdated and third-party cheques will not
be accepted. The daily cheque writing limit is $4,500
Canadian Dollars. You must also note your Herbalife ID
Number and order number (if available) on your cheque.

■ Wire Transfers - Your order may be paid by wire transfer to
Herbalife’s bank account (shown above right); it must be a
bank-to-bank transaction. Please note that products will not
be shipped/released until your wire transfer has been received,
confirmed and processed by Herbalife. You must reference
your Herbalife ID Number and order number with your wire
transfer. Wire transfers may be used for telephone, fax or mail
orders.

If you are located near Calgary or Montreal, the fastest way to
receive product(s) is to place and pay for your order in person
at our Sales Centre. Please note that you must present your

avoid any delays. Orders cannot be picked up until full

Shipping Procedures

All orders will be shipped on the next business day, whenever
possible, if all moneys have cleared. You should indicate the
method of shipment applicable, i.e., Greyhound Bus, UPS,
etc on each order.

United Parcel Service - UPS
■ UPX Air Express will deliver to most major cities the next
day within Ontario and Quebec. It will take two to three
working days for delivery to other areas and three to five
working days to the Maritimes.
■ Because UPS delivers to your door, it is recommended that
someone be available at the “ship to” address to accept the
shipment (You may wish to address your shipment to a
neighbour, friend, relative, etc).
■ When your shipment consists of several boxes, they may
not all be delivered at the same time.
■ Missing telephone number or Postal Code, use of P.O. Box
numbers instead of street address, Rural Routes or
incomplete addresses can delay your shipment

UPS Ground
■ All orders shipped to the West will be sent UPS Ground
unless you choose another courier (Greyhound). We also use
this method of shipment for dangerous goods (all Dermajetics
Fragrances and GHD hair sprays).
■ This service will range from overnight delivery to five or
more working days, depending on the distance between the
shipping point and the destination.
■ Door-to-door delivery is provided. Someone must be
available at the “Ship to” address to accept the shipment
(You may wish to address your shipment to a neighbour,
friend, relative, etc).
■ Generally, the complete shipment is delivered at once.
Packaging & Handling Charge
In order to accommodate the costs associated with
packaging, handling and marketing, be sure to add 7%
packaging and handling charge to the full retail price of all
product and art of promotion (AOP) orders.
Tax Information
Please be certain to add to your order the appropriate sales
tax, based on the exact location (province) where the order is
being shipped, e.g. 14.5% for Quebec (7% GST + 7.5%
QST), 15% for Newfoundland, New Brunswick, Nova Scotia
and 7% for the rest of Canada.
Greyhound Bus Service
■ Your order can be shipped via Greyhound if you reside in
Western Canada.
■ Deliveries are from station-to-station (not door-to-door).
■ They offer call-upon-arrival service.
■ Will accept transport of dangerous goods.
■ You may request freight collect.
■ One to six days transit, depending where you are located.
Note: Will Call (Pick-Up) orders are taxed on the location of
the Herbalife Distribution Centre.

■ Any hidden damages discovered after the driver has left
must be reported immediately to the Herbalife Order Support
Department. Keep the damaged items in the original package
as found, including all packing material and the master
container. In the event that highly visible damage is evident
upon receipt, please refuse the shipment. Request a claim
form from UPS and notify the Herbalife Order Support
Department immediately.
Air Shipping
■ All air shipments are made on a pre-paid freight basis, and
offer the fastest means of transportation for Eastern Canada.
When Receiving Your Shipment
All Herbalife products are in perfect condition when the carrier
takes possession of the shipments at our Distribution Centre.
Before signing “Received” for your shipment, here are some
things to remember:
■ Your signature means that you have received your shipment
in a satisfactory manner (unless otherwise noted).
■ Determine the number of boxes your shipment includes,
either by the packing slip or by the number of boxes marked on
top of each box, e.g. 1 of 3, 2 of 3, etc.
■ Only sign “Received” for the number of boxes actually
received.
■ Inspect your shipment for obvious outside damages. Even
though there is no apparent damage on the outside, some
damages might be found inside, which could become part of a
freight claim. Should there be visible damage do not accept
delivery of that box or have the driver wait until you have
inspected your order. Please record any damages and then
contact the Herbalife Order Support Department immediately.
Freight Claims
Air and ground carrier damaged and lost shipment claims must
be placed directly with the Herbalife Order Support
Department, please refer to page -9- for complete information.
Air and motor freight claims, when properly documented and
immediately filed, are usually settled promptly.
Important Reminders
■ UPS lost shipments (complete or partial) must be reported
to Herbalife Canada Ltd. within seven (7) days after estimated
time of arrival.
Placing International Orders
For your convenience, we have a simple ordering process that
will enable you to expand your business internationally.
International orders are available through our Canadian Sales
Order Centre or through any Herbalife Distribution Centre
worldwide (where applicable). Please note that Herbalife can
only accept International Orders for countries in which
Herbalife is officially open.
Each country in which Herbalife is officially open has a specific
range of products, literature and promotional items approved
for that country and approved for international ordering.
Therefore, orders can only be accepted for products and
literature approved for international ordering in the country to
which you are requesting shipment. It is also important to note
that when you purchase products, literature or promotional
items for shipment to a specific country, they must be sold only
in that country.
Information on approved products, literature and promotional
items approved for international ordering for each open
country is readily available through the Sales Order Center or
through the Distributor Relations Department. When placing
your order, please indicate that you are placing an
“International Order.” The Order Representative will assist you
with pricing, which will be based on prices in the country to
which the products are being shipped, then converted to
Canadian Dollars for payment in Canada.
