Port Moody Recreation Centre – 300 Ioco Road, Port Moody, BC
(Games will be played on both the NHL rink and the Olympic rink)
FEES:
$1,375.00 Payable to PMAHA or Port Moody Amateur Hockey Association
 Registration Form must be submitted with cheque for the full amount in order
to reserve a spot

GAMES:
4 game minimum
TEAM ROSTER:
Maximum 19 players including goalies. Please list all participating players and
officials on the Roster Form upon team selection after completion of try-outs.
DEADLINE:
October 10, 2014
ACCEPTANCE:
Registration does not guarantee acceptance, you will be notified by email shortly
after the deadline. Teams cancelling after being accepted will be charged a $500
fee. Refunds will not be given for cancellations within 14 days of the tournament.

Tournament Permission #: 2014-2015-5425
Once accepted we will require a Tournament Permission Number. The Permission Number must be
submitted no later than 48 hours before your first game (if your team is not a Pacific Coast Member
then a letter from your association will be required).
Deadline: Team Rosters will be accepted up to one week before the first day of the tournament,
official BC Hockey roster required.
The Tournament can hold a maximum of 12 teams with the number in each pool yet to be determined.
PMAHA reserves the right to not accept applications due to an imbalance in the pool structure.
By signing this Registration Form, the Team Manager and Coaches release the Port Moody Amateur
Hockey Association (PMAHA) and all Officials and Volunteers associated with the tournament from
any liability for injury or accident which may be incurred by any player or team official while attending
or traveling to, or from, the tournament.
