2014 FWA and Compliance Attestation Instructions
Only independent pharmacies, not affiliated with a PSAO (Pharmacy Services
Administrative Organization) or Chain, should attest through the Catamaran
portal and survey.
Please proceed to Provider Login to access the FWA Attestation Survey. This
can be accessed via the Pharmacy Resources link.
Once you have entered, select “FWA Compliance”.
If you are unable to access the Provider Login, and are an independent pharmacy
provider (non-PSAO/Chain) please request access via the Provider Portal Request form by
clicking the Click Here to Register link inside the login screen.
Please note independent pharmacies attached to a PSAO should attest to
their PSAO and not directly to Catamaran.
Your attestation deadline is December 31, 2014.
