2015 STATE PIN AND T-SHIRT
DESIGN CONTEST
Florida HOSA Wants You!!!
To Design a Logo for the 2015 Florida HOSA Trading Pin and T-Shirt.
Members of Florida HOSA
can vote for the design they like best!
Florida HOSA is extending you the opportunity to submit your creative design exhibiting this year’s
HOSA Theme, “HOSA: Lead!” The full theme must be written as part of the design, as well as the
school year “2014-2015.” The winning design must also have a Florida motif. The criteria for this year’s
competition:
Include designer’s name and school on back of design
 The designer must be a Florida HOSA member in good standing (affiliated/paid member)
 Entries may be submitted on 8½” x 11” white unlined paper, on a computer CD (jpg format), or
sent via e-mail to [email protected] (jpg format)
 Submit one drawing denoting the color scheme or colored correctly
 The design should be drawn at least 5” in size (size will be modified when placed on t-shirt
and pin)
 All entries must have the HOSA theme, “HOSA: Lead!”, the school year “2014-2015”, and a
Florida motif.
 All designs submitted become the property of Florida HOSA, Inc.
All designs must be received in the Florida HOSA State Office on or before December 3, 2014. A
panel of judges will narrow the submitted designs to the top five. From December 6th - 17th, members of
Florida HOSA can go to the Florida HOSA website for directions on how to vote for one of the five
designs. The winner will be announced in the February Florida HOSA Newsletter and at the 2015 State
Leadership Conference during the Recognition Session with a presentation of 10 trading pins and 2 t-shirts.
The trading pins and t-shirts will be sold at the State and National Leadership Conferences with the
proceeds going to Florida HOSA. Please mail entries to:

Dancing with the
State Officers!
During the conference, members will have an
opportunity to dance on stage
with the State Officers during the
Saturday evening General Session for $3.00!
Also, the State Officers will be designing a
Long Sleeve FLDA Commemorative T-shirt!
The price will be $15 each and pre-orders will be taken
through on-line registration for FLDA 2014. Payment in
full for pre-ordered t-shirts must be received by October
22, 2014. If payment is not received by this date, your
order will be cancelled. Some extra t-shirts will be
on sale at the conference available on a
first come, first served basis.
All proceeds will go to the Florida HOSA Foundation Scholarship.
