Introduction
National Otolaryngology (ENT) ST3 recruitment is coordinated by Heath Education Yorkshire
and the Humber (HEYH) and is now in its 7th year as a National process. In 2015 we will be
recruiting to ST and LAT posts across England and Scotland.
This Applicant guide aims to provide all applicants with detailed information regarding all
aspects of the 2015 Otolaryngology recruitment process. General information about applying
to specialty recruitment is available on the Specialty Training Website and in the Specialty
Recruitment Applicant Handbook (we recommend that all applicants read this).
In 2015 applicants will apply, complete their preferences and receive offers through the New
Oriel recruitment portal. This will be the case if you are applying to any Specialty, Dental or
Academic specialty. Oriel will enable all applicants to access all their applications and offers
through a single unified recruitment system.
National ENT will hold 1 round of interviews in Leeds on the 30th and 31st March 2015 at
Elland Road Football Stadium, Leeds.
Academic ENT Applicants please note:- In 2015 all applicants deemed appointable at
Otolaryngology (ENT) Academic Clinical Fellow (ACF) Interviews will need to attend and be
deemed appointable at the National ENT Clinical interviews to be appointed to an ACF
(candidates may be offered an ACF pending the result of a clinical interview), unless they
currently hold a National Training Number (NTN) or Deanery Reference Number (DRN) in the
same GMC Specialty which the NIHR ACF is being advertised in.

Units of Application
A Unit of Application (UoA) is an area to which you can make an application. In most cases the UoA will
be a Local Education and Training Board (LETB) or Deanery.
As of 1st April 2013, LETBs have taken on the functions of Postgraduate Deaneries in England. LETBs are
responsible for educating and training doctors, dentists, nurses and all healthcare professionals at a local
level. Deaneries continue to exist in Northern Ireland, Scotland and Wales.

Oriel Recruitment Portal

Oriel is the single application portal for all postgraduate medical and dental applicants in 2015, providing
a complete start to finish candidate experience. Oriel now combines application, interview booking and
offers functionality.
The Oriel system was piloted by Otolaryngology and 19 other ST3 specialties in 2014 with offers taking
place in a separate system.
With the introduction of Oriel 2015, you will be able to register for specialty training, view vacancies,
apply, book interviews, manage interviews, receive and accept offers, all within the one location. You
will have your own profile and status updates for each vacancy that you have applied to through Oriel.
Providing a single applicant journey and ensuring a consistent experience throughout the specialty
recruitment process by ensuring all relevant specialty training information can be sourced, and
applications can be managed, within the one location.
The process to apply will be seamless; all you need to do is select the ‘apply’ button on the specialty
page and you will be directed to the Oriel system. From this point, you can register then select and
commence the process of filling in your application form. An Oriel applicant user guide has been
developed to assist you with this part of the process.

Application
The National Otolaryngology ST3 application form is built around the National Standard application form
which is used for all specialty training recruitment, and has been tailored for National Otolaryngology
Recruitment by the Lead Clinician in conjunction with the ENT SAC and HEYH.
Applications will only be accepted through the Oriel 2015 online application system. For further
guidance on creating an account, registering and submitting your application using the system, please
refer to the Oriel Applicant User Guide.
Please note: All Otolaryngology ACF appointable applicants will be required to complete and submit a
National Otolaryngology ST3 application form. The application form contains a self-assessment form
which is an integral component of the interviews.
Preferences
Candidates will be given the opportunity to preference the available posts in Oriel. The preferencing
process will take place after applications close and before the offers process commences.
Application Deadline
The deadline for submitting your application is 11th March 2015, 4pm. After this time no applications
will be accepted. There will be no exceptions to this deadline. You are advised to complete and submit
your application 24 hours ahead of the deadline to allow for any unforeseen problems.
When you submit your application, you should receive an automatic confirmation email. If you do not
receive this email within 2 hours you should check to ensure that you have submitted your application.

Self-Assessment Questions
There are a number of multiple choice self-assessment questions built in to the application form. For
these questions, you need to select the most appropriate response from a drop-down list. The scores
that you award yourself will then be verified at interview and contribute to your portfolio station score.
A self-assessment guidance document has been made available to candidates and is available from the
HEYH National Otolaryngology Website, this includes general information about the process, the
questions, responses, scoring system, details of suitable evidence and a checklist.
If a candidate’s portfolio is not provided in the specified format at interview and evidence can not be
verified by the assessor during the allocated time, and a score of zero may be awarded for that element
of the portfolio.
Candidates considered to have provided documents that are inaccurate, falsified, or contain private or
confidential patient details will have evidence taken and be referred to a scrutiny panel. The scrutiny
panel will be conducted by Health Education Yorkshire and the Humber and the candidate will be
offered an opportunity to explain their actions and provide supporting documentation. The result of the
scrutiny panel may lead to one or more of, but not limited to, the following.

Candidate removed from the current round of National ENT recruitment.

Candidate being reported to the GMC.

Candidates current Trust and/or LETB/Deanery being notified.
Eligibility and Longlisting
The eligibility criteria for Otolaryngology ST3 applications is displayed in the 2015 Person Specification
available from the specialty training website. It is an applicant’s responsibility to demonstrate that they
meet the eligibility criteria. All applications will be assessed against the eligibility criteria during
longlisting. Any applications which do not meet the eligibility criteria will be longlisted out of the process
and will not progress to the next stage.
In some instances applications may proceed past longlisting so that more information about an
applicant’s eligibility can be obtained. Any applicant can be longisted out of the process or have an offer
of a training program withdrawn at any point if the essential requirements are not met, this includes
after an offer has been made and/or accepted.

MRCS and DOHNS or MRCS (ENT)
Applicants must have successfully completed MRCS and DOHNS or MRCS (ENT) or equivalent at the time
of interview. If you are invited to attend an interview you must provide evidence that you have
successfully completed MRCS and DOHNS or MRCS (ENT) or equivalent when you attend.
If you have not successfully completed MRCS and DOHNS or MRCS (ENT) or equivalent at the time of
application, but are expecting results of a recent examination by the interview date you are eligible to
apply and will be asked to provide relevant information on the application form. If you are invited to
interview, you must email evidence to [email protected] with ENT “your name” and
MRCS/DOHNS in the Subject of the email and bring evidence that you have passed ALL parts with you to
the interview.
Assessment of Competency
Applicants must have achieved CT1 and CT2 competences in surgery by the time of appointment. There
are a number of ways this can be demonstrated:

If you are currently in a UK Core Surgical Training programme, you do not need to provide any
further evidence at the time of application. Any offer of a training post will be conditional on
you achieving a satisfactory ARCP outcome for CT2 by the start date of the post to which you are
appointed.

If you have already completed a UK Core Surgical Training programme you must attach scanned
copies of your ARCP for CT1 and CT2 or a Core Certificate of Completion to your application.

Applicants who are not able to provide evidence of their CT1 and CT2 Core Competences may
provide the following alternative evidence by attaching scanned copies to their application:

o
ARCP or RITA documents showing satisfactory outcome at CT1 and CT2 level.
o
Educational / Clinical Supervisor reports showing satisfactory outcome at CT1 and CT2
level.
o
Completed Certificate C (Alternative Certificate of Core Competence). This can be
downloaded from the HEYH National Otolaryngology Website.
Scanned copies of any evidence must be attached to your application; alternatively you may
email them to [email protected]. Please mark your email ENT “your name” Competency
this must be received no later than 11th March 2015, 5pm.
Criminal Records and Fitness to Practice Declaration
Page 3 of the application form is a Criminal Records and Fitness to Practice Declaration Form containing
several Questions. If you answer Yes to any of the questions in this form you must provide full details by
email to [email protected] this must be received no later than 11th March 2015,
5pm. Please mark your email “CONFIDENTIAL – Otolaryngology”
The declaration Form and any information provided in relation to your responses will be kept securely
and in confidence, and access to it will be restricted to designated persons within Health Education

Yorkshire and the Humber and other persons who need to see it as part of the selection process and
who are authorised to do so.
If you are successfully appointed to a training post, this information will be passed to designated persons
in the UoA to which you are allocated and subsequently to your employing organisation and any
organisations through which you rotate as part of the training programme.
No information relating to your declaration form will be made available to any members of the selection
panel at any time during the recruitment process.
Applicants on the Specialist Register
You are not eligible to apply if you already hold, or are eligible to hold a Certificate of Completion of
Training (CCT) in Otolaryngology Surgery. You are not eligible to apply if you are currently on the
specialist register in any EU member state.
Applying for a Deferred Start Date
Applicants can request deferred entry to specialty training programmes on statutory grounds (e.g.
maternity leave, ill health etc.) only. Deferred entry to training programmes is not permitted for any
other reason. Refer to the Gold Guide for more information about deferring start dates.
If you wish to request a deferred start date you should indicate this on page 1 of your application form.
If you are appointed to a training post, this information will be passed to the LETB/Deanery to which you
are allocated however, we recommend that you also contact them directly to discuss your needs.
The final decision regarding any deferment will be made by the LETB/Deanery to which you are allocated
according to their local policies.
Applying for Less Than Full Time Training
Trainees may train on a Less Than Full Time (LTFT) basis for a variety of well-founded reasons. Refer to
the Gold Guide for more information about Less Than Full Time Training.
All posts offered through this recruitment process will be offered as full time posts. If you wish to
request Less Than Full Time Training you can indicate this on page 1 of your application form. If you are
appointed to a training post, this information will be passed to the LETB/Deanery to which you are
allocated; we recommend that you also contact them directly to discuss your needs. You will only be
able to apply for Less Than Full Time Training once you have accepted a full time post.
The final decision regarding Less Than Full Time Training will be made by the LETB/Deanery to which you
are allocated according to their local LTFT policies.

Shortlisting
If the number of eligible candidates after longlisting exceeds the Interview capacity, applicants will be
Shortlisted using their Self-Assessment score. Please read the Self-Assessment section of this document
for more information.
Shortlisting is a method of reducing the field to a manageable number of candidates, being unsuccessful
at shortlisting does not affect subsequent applications.
Academic Clinical Fellow (ACF) Applications
Candidates wanting to apply to Academic Clinical Fellow (ACF) posts in addition to, or instead of
National ENT Recruitment will need to apply for NIHR ACF posts through the Local Education Training
Board (LETB) where the post will be based. ACF posts were open for applicants to apply from Monday
13th October 2014.
Shortlisted candidates will be invited for ACF interviews organized by the local LETB, which will have an
academic and clinical component, you will be required to pass all components of the interview.
If you do not already hold a Deanery Reference Number (DRN) or a National Training Number (NTN) in
the GMC specialty to which you are applying for, you should submit an application to the National
Clinical Recruitment Process (ENT National Recruitment).
Candidates that are successful at ACF interviews and who do not hold a NTN or DRN in the GMC
specialty to which you are applying for, will be made an offer subject to being deemed appointable at
the National Clinical Interviews.
Please note: All Otolaryngology ACF appointable applicants will be required to complete and submit a
National Otolaryngology ST3 application form. The application form contains a self-assessment form
which is an integral component of the interviews.

Interviews
Booking an interview slot
If your application passes longlisting you will be invited to book an interview slot through your Oriel
account. Interview slots are offered on a first come first served basis. You must do this by the deadline
stated in the invite email. Please refer to the Oriel Applicant Userguide for instructions on booking your
interview slot.
You should receive an automatic confirmation email once you have booked your interview slot. If you do
not receive this email within two hours of booking, please check your Oriel account to ensure that you
did book a slot.

Interview Dates and Venue
The interviews for 2014 Otolaryngology (ENT) will take place on the 30th
and 31st March 2015 at Leeds United Football Club in the East Stand.
The venue is conveniently located for access from the M621, M62 and
M1. Leeds railway station is approximately two miles away and easily
reached by public transport or taxi.

Facilities:
Photocopying/photographs – there are no facilities on site so please
ensure you have all the copies you require for interview.
Food/drink – Water will be available in the reception areas. There are a number of places to purchase
food within a short walk of the Venue.
Parking is provided in the Stadium Way car park opposite the East Stand.
Applicants with Disabilities
If you have a disability which may require specific arrangements or adjustments to enable you to attend
an interview please indicate this on page 1 of your application form and contact the recruitment
helpdesk to discuss your requirements.
Documents Required at Interview
There are a number of documents that you are required to bring with you when you attend your
interview. Please ensure that all copies provided are on loose A4 white paper, (no plastic wallets,
folders, staples or paperclips). Please bring the original documents and copies as listed below:
• Your Portfolio containing evidence to substantiate your Self Assessment form responses. You must
put these documents in a separate section at the front of your Portfolio and in the order prescribed
in the Self-assessment Guidance Document, with the Checklist page at the front to ensure your Self
Assessment evidence can be easily identified.
• Original passport including front cover, inside cover & signature/photograph page AND 1 COPY
• Original proof of address AND 1 COPY
• Original current GMC Proof of Registration AND 1 COPY
• Original evidence of right to work in the UK AND 1 COPY
• Original primary qualification certificate AND 1 COPY
• Original postgraduate qualification/membership exam certificate AND 1 COPY
• Original proof of CT/ST2 competences where you are applying for ST3 posts (i.e. ARCP if in
core/ST2/ST3 training, or completed alternative certificate of Core Competence certificate) AND 1

These documents will be checked against a checklist and the photocopies will be retained on your
recruitment file. If you are appointed to a training post, these documents will be passed to the
LETB/Deanery in which you will be training.
References
Clinical references will not be requested from consultants until after offers have been accepted.
Candidates therefore do not need to request them personally and/or bring them to interview. We will
use the referee email addresses you supplied as part of your application to request them through the
Oriel recruitment system, we therefore strongly advise that you ensure the email addresses are current
and correct.
If you are appointed to a training programme, your references will be forwarded to the LETB or Deanery
in which you will be training. These will subsequently be forwarded to your employing trust. Your
employer will not be able to issue a contract without references.
Interview Format
The interviews will consist of 5 “stations”, lasting 10 minutes each. There will be a 5 minute gap between
each station. Each station has been designed to assess the selection criteria on the Person Specification.
You will rotate round all 5 stations during the interview process, the order you complete the stations
may be different to below and different to that of other candidates.
Scenario
There will be a short vignette to read in the 5 minutes before the interview, in the room there
will be 2 assessors you will be marked independently by both.
Communication
In the room there will be an actor and 2 assessors, you will be marked independently by the 2
assessors and the actor. There will be a short vignette to read in the 5 minutes before the
interview.
Management
There will be a short vignette to read in the 5 minutes before the interview, in the room there
will be 2 assessors you will be marked independently by both.
Portfolio
Prior to entering the portfolio station, one of the 2 assessors will have spent 30 minutes
reading through your portfolio and validating the Self-assessment scores you awarded
yourself during application.
The station will consist of a review of the Self-assessment scoring of your portfolio and if
necessary the clarification of any discrepancies between your score and that of the assessors,
candidates will then be expected to confirm their amended score with a signature. If a Selfassessment score is not agreed the Quality Assurance panel will be asked to adjudicate and
sign the score sheet on behalf of the Candidate.

Skills
There will be a short vignette to read in the 5 minutes before the interview. You will then
complete a task before being asked some questions. You will be scored independently by 2
assessors.

HEE will advertise all higher training specialties together in a single advert appearing in the BMJ
published 11th February 2015. This advert will contain a link to the Specialty Training Website where
applicants will be able to access the HEYH National Otolaryngology ST3 recruitment webpage.
HEYH will publish an individual National Otolaryngology advert in NHS Jobs providing key information
and a link to our website. Applicants are unable to apply directly through NHS Jobs for these posts.

Scoring and Ranking
Your performance in each station will be scored individually by a minimum of 2 assessors using a
structured scoring system. The scoring system has been developed by ENT Clinicians experienced in
interview and exam methodologies and is mapped to the 2015 Otolaryngology Person Specification.
The scores from each interview station will be combined to produce your overall interview score, this
may include a predetermined weighting on specific stations.
Your appointability will be assessed based upon 3 different factors, 2 of which are determined using the
modified ANGOFF method. The modified ANGOFF method requires each assessor to provide a separate
appointability score for each station, the scores provided from all the assessors for each station are then
averaged, providing an ANGOFF Score for each interview station. A total ANGOFF score is then
calculated by the addition of the individual station’s ANGOFF scores.
The three methods of judging appointability are:-

1) Total modified ANGOFF – If a candidates total score is less than the Total modified ANGOFF
score, the candidate is deemed unappoitable.
2) Communications modified ANGOFF -20% - If a candidate’s communication score is less than
80% of the communication modified ANGOFF score, the candidate is deemed unappoitable.
3) Red Flag – A candidate may be considered unappointable if a serious lack of knowledge, a
dangerous action or a probity issue is identified. In this situation an interviewer may “Red
Flag” a score sheet, the issue is then discussed with the lead clinician and/or with any other
relevant person and a decision made as to whether the candidate is deemed appointable.
After your appointability has been assessed you will be ranked against all other appointable candidates.
Your ranking will be based on your overall interview score. In the event of tied scores, individual station
scores will be used to separate applicants, these will be used in the following order
1) Communication
2) Portfolio
3) Skills
4) Scenario
5) Management
Lay Representatives and Observers
A number of Lay Representatives will be present on each interview day. They may sit in some of your
interview stations to observe the process. They will not score or assess your performance in the station;
they are there to ensure the process is fair and consistent.
There will also be a number of observers present. Again, they may sit in some of your interview stations.
They will predominantly be consultants and their role is to assess the recruitment process, and observe
the interview panels; they are not there to assess your performance.
Confidentiality
You should ensure that the content of the interview assessments remains confidential, the content or
your responses must not be discussed with other applicants or published where they can be accessed by
other applicants. This includes on websites, online forums or discussion groups and all other forms of
social media.
Remember, this is a competitive process and you may reduce your own chances of appointment by
giving an unfair advantage to other applicants.

Applicants attending interviews are entitled to a travel and subsistence allowance. A claim form and
guidance can be downloaded from the HEYH National Otolaryngology Website. You are advised to read
the guidance document in full before incurring any expenses.
Oriel Offers
All offers will now be made within the Oriel recruitment system across all CT/ST Specialty Recruitment,
Academic Recruitment and Dental Recruitment. Candidates will no longer be required to log into and
receive their offers through a separate recruitment system.
Selecting Your Preferences
You will be asked to rank your preferences using the Oriel Recruitment System. You will receive an email
when Preferencing opens.
There are two lists labeled “Preference” and “Not Wanted” by default all preferences will appear in the
“Not Wanted” list. You can move preferences between the two lists by dragging and dropping individual
preferences from one list to the other. Once you have moved preferences to the “Preference” list, you
can move them up and down by dragging and dropping individual preferences. You should put your
preferences in rank order with your most preferred option at the top of the list and your least preferred
option at the bottom.
IF YOU DO NOT COMPLETE YOUR PREFERENCES BEFORE THE DEADLINE THEY WILL REMAIN IN THE “NOT
WANTED” LIST AND YOU WILL RECEIVE NO OFFERS.
Please note: There is no advantage to be gained by ranking preferences with the most posts high up
your preference list. There is also no disadvantage to ranking a preference with few (or even zero) posts
high up your list.
If you would be not prepared to accept a particular post, then you should not rank it. By ranking a
preference you are stating that you would be prepared to accept that preference. If you are offered a
post and you reject it you will receive no further offers. It is important that you think about this
carefully when completing your preferences and leave any posts that you would not be prepared to
accept in the “Not Wanted” list.
Allocation to Posts
Successful applicants will be allocated to posts in order according to their interview ranking.

The highest ranked applicant will be allocated to their first preferenced post.

The second ranked applicant will be allocated to their first preferenced post unless it has already
been allocated to the first ranked applicant in which case they will be allocated their second
preferenced post.

The third ranked applicant will be allocated to their first preferenced post unless it has already
been allocated to the first or second ranked applicant. If it has, they will be allocated their
second preferenced post unless this has been allocated to the first or second ranked applicants
in which case they will be offered their third preference.
This process will continue until all posts are allocated or there are no more appointable applicants.
Any offer made through this recruitment process is an offer of an allocation to a training programme; it
is not an offer of employment. Offers of employment can only be by an employing organisation
following completion of satisfactory pre-employment checks.
Receiving and Responding to Offers
If you are offered a post you will have 48 hours from the time of your offer, including weekends and
bank holidays, to decide whether to accept, reject or hold it. You must respond to the offer through the
Oriel System. No other form of response will be accepted.
IF YOU DO NOT RESPOND TO YOUR OFFER WITHIN 48 HOURS, THE OFFER WILL EXPIRE AND THE POST
WILL BE OFFERED TO ANOTHER APPLICANT. YOU WILL NOT RECEIVE ANY FURTHER OFFERS.
If you are offered a post you have the following five options:
Accept
If you accept an offer without upgrades you are confirming that you wish to accept the post you have
been offered. You will not receive an upgraded offer even if a post further up your preference list
becomes available.
If you accept an ST post, you will not be eligible to receive offers from another recruitment process (e.g.
General Surgery or Otolaryngology in Wales or Northern Ireland) with the exception of offers for ACF
posts.
If you accept a LAT post, you will still be eligible to receive offers for ST, CT and ACF posts from another
recruitment process but you will not be eligible to receive other offers for LAT posts.
Accept with Upgrades
If you are not offered your highest ranked preference, you will be able to choose the ‘Accept with
upgrade’ option. This option will not be available after 16:00 on 22nd May 2015.
By choosing this option, you are confirming that you wish to accept the post you have been offered but
that you also want to be considered for an upgrade to a post higher up your preference list.
If a post further up your preference list becomes available your current offer will automatically be
upgraded. You will be notified of any offer upgrades by email. If your offer is upgraded you will
automatically be deemed to have accepted the upgrade. Your original offer cannot be reinstated.
Upgrades will continue to be made until 16:00 on 22nd May 2015.
You can opt out of upgrades at any time by logging into your Oriel account and selecting the relevant
option from within the specialty.

offers from other recruitment processes.
You will not receive an upgraded offer from this recruitment process even if a post higher up your
preference list becomes available.
You can only hold one offer at a time. If you hold your offer, any other held offers will be rejected. If
you hold your offer and subsequently hold an offer from another recruitment process, your original
held offer will be rejected.
Before the holding deadline (13:00 on 20th May 2015) you must confirm whether you wish to accept,
accept with upgrades or reject the offer. If no action is taken by the hold deadline, the held offer will
expire and will be REJECTED. You will receive no further offers from this recruitment process.
Hold with Upgrades

If you do not wish to accept or hold your offer you can reject it. If you reject an offer, you will receive no
further offers from this recruitment process. You will still be considered for other recruitment processes
that you are taking part in.
LAT Offers
Offers for English LAT posts can only be made once all English ST posts are filled. Offers for Scottish LAT
posts can be made alongside offers for ST posts.
Offers for all ST posts and Scottish LAT posts will be released in the initial round of offers. Offers for
English LATs will not be released until after the upgrade deadline (16:00 on 22th May 2015) has passed.
Applicants who are offered a Scottish LAT and accept with upgrades will be eligible for upgrades to
English LAT posts once they are released.
Please note:- Applicants with more than 24 months experience in LAT posts in the specialty by intended
start date should not preference LAT post and will not be made LAT offers.
Feedback
Feedback will be provided at three stages of the recruitment process:
Longlisting
Applicants whose applications are unsuccessful at longlisting will be contacted with the reasons for this.
Applicants whose applications are longlisted will receive an email inviting them to book an interview.
Interview Ranking
All applicants will be able to view their total interview score and rank online through their Oriel account
prior to offers being released. Please note that a thorough Quality Assurance and validation process is
carried out on all scores following the interviews. No scores will be released until this process is
complete.
Interview Scores
Following the conclusion of the offers process, all applicants will be able to view a breakdown of their
interview scores including the scores they achieved in each interview station online through your Oriel
account.

Evaluation of the Selection Process
Following the conclusion of the 2015 recruitment process Health Education Yorkshire and the Humber
and the Otolaryngology (ENT) Specialty Advisory Committee will carry out an evaluation of the
recruitment process. This will include an analysis of anonymised interview scores, any piloted interview
station scores, appointment data and feedback from a number of groups involved in the process.
HEE will also be conducting an evaluation of the Oriel recruitment system, using feedback from
applicants and administration staff involved in using the system
