Course Description
The annual NYU Seminar in Advanced
Rheumatology is an informative conference for
rheumatologists, orthopedists, trainees in the
rheumatic diseases, internists and other physicians
with a special interest in rheumatology. It will
review and address those areas where recent,
solid knowledge in the treatment of rheumatic
diseases is available, focusing particularly
on the state-of-the-art in rheumatology,
at the leading edge between new clinical
knowledge and its translation into practice.
Rheumatology is a fast-moving field in which
new discoveries are continually being made, and
new treatment modalities developed. Because
the basic and clinical scientific underpinnings of
rheumatologic practice frequently and directly
come to impact upon therapy, this course will also
explore those areas of new science that are likely to
prove important to the near-future management of
patients with rheumatic disease.
Lectures and Year-In-Review updates will provide
participants with the latest information in arthritis,
lupus and other rheumatologic conditions; panel
discussions and optional Meet-the-Professor
sessions will allow participants to raise questions
and participate in dialog with the course instructors
and clinical experts.
Target Audience
Our target audience consists of clinical and
academic rheumatologists, orthopedists, trainees in
the rheumatic diseases and internists with a special
interest in rheumatology; other interested clinicians
are welcome.
Statement of Need
Across the spectrum of rheumatic disease, rapid
accrual of new knowledge and continually evolving
therapeutic advances challenge rheumatologists
to stay abreast and provide their patients with the
best possible care. Lupus management remains a
core performance area in which rheumatologists
require updating. Clinicians need to be aware of
newer approaches to treating lupus, including
the central role of hydroxychloroquine, the recent
introduction of belimumab and the potential for
new agents in the near future. Physicians must be
prepared to incorporate new thinking in regards to
pathophysiology and genetics, and to manage the
co-morbid conditions of SLE, particularly premature
atherosclerosis. Management of pregnancy in
lupus patients also presents unique and evolving
challenges and opportunities. In rheumatoid and
psoriatic arthritis, newly available biologic therapies
and small-molecule signal transduction inhibitors,
as well as the potential for directly managing
bone erosion, provide options and shed light on
the pathophysiology of inflammatory arthritis.
Clinicians must be able to incorporate these new
approaches into treatment paradigms, considering
both safety and efficacy. With the aging of the
population osteoporosis has become an increasingly
prevalent disease, demanding new insights into
disease risk, bone biology and pharmacologic
development. Physicians need to stay abreast of
the current recommendations for osteoporosis
screening and management, including management
of steroid-induced osteoporosis and osteoporosis
in patients who fail or are unable to take standard
therapy. The understanding and treatment of other
rheumatic diseases including vasculitis, osteoporosis
and gout are also advancing rapidly. The NYU
Seminar in Advanced Rheumatology will address
all these, and other conditions, and help clinicians
stay abreast with the state of the art in 2015.
Educational Objectives
After participating in this activity, clinicians should
be able to:
• Be up to date on information regarding the
pathogenesis of lupus and application to
therapy
• Evaluate new and future treatment
approaches for lupus management,
including the management of lupus nephritis
• Apply the most recent recommendations for
screening, following and managing pregnant
lupus patients, their fetuses and offspring
• Recognize the benefits, and manage risks
of newly available biologic therapies for
treatment of rheumatoid arthritis, psoriasis
and psoriatic arthritis
• Develop strategies to determine
when to introduce small-molecule
biological therapies into treatment
• Appropriately select anti-osteoporosis
drugs, including managing potential
toxicities of treatment, and apply
second-line approaches when first-line
agents are ineffective or contraindicated
• Properly diagnose and treat
ANCA-associated vasculitis,
including both the induction and
maintenance of vasculitis remission
• Recognize and manage risk factors for
the progression of osteoarthritis
• Incorporate an understanding
of cardiovascular risk in the
management of gout and SLE

Course Description
The use of musculoskeletal ultrasound (MSKUS) has increased
amongst rheumatologists in the United States over the last
5 years, but we still trail Europeans and others in this regard.
MSKUS complements the physical examination by allowing
for superior visualization of synovitis and erosive changes
compared to conventional radiography, and provides detail
comparable or supplementary to magnetic resonance imaging
(MRI). This modality is also less expensive than MRI and CT
scans, without the claustrophobia or other contraindications,
while uniquely providing dynamic—rather than static—imaging.
While MSKUS enhances our diagnostic and prognostic abilities
in the clinics and at the bedside, rheumatologists skilled in
MSKUS can also aspirate/inject hips and posterior knees
(Baker’s cysts) and improve the accuracy/success of other
routine injections. A growing body of literature for MSKUS in
RA, gout and other diseases is contributing significantly to the
understanding of diagnostic and prognostic utility, longitudinal
assessment, and disease remission. Furthermore, scoring
systems focusing on the patient level rather than individual joint
have been developed, allowing for simplification of exams while
still retaining accuracy and utility. Despite all of this, relatively
few rheumatologists have received training to use this modality.
There are very few MSKUS courses focused on the needs of
rheumatologists and the patients and problems that they
see. We feel that there is a significant need on a regional and
national level for a beginner’s course for rheumatologists and
associated health care professionals.

Educational Objectives
At the conclusion of this activity, participants should be able to:
• Compare the efficacy of MSKUS techniques to conventional
radiography and select the best diagnostic and treatment
modalities to treat their patients
• Demonstrate the correct way to handle the ultrasound probe
and locate and recognize normal anatomy and pathology in
each of the joints using musculoskeletal ultrasound
• Utilize ultrasound to track needles into joints and bursae by
both indirect and direct guidance

The hands-on sessions for this course will
be divided into two tracks: Beginner and
Intermediate. You must choose your track upon

or are familiar with all of the joint scans should

be geared more towards the Intermediate level,
however they will be beneficial to all participants.
