AGREEMENT FOR USE OF BAND UNIFORMS
Please read the following and sign and date below
I,___________________________________, fully understand the importance of keeping the
individual uniform checked out to me taken care of in the best manner possible. My
uniform will always be hung up with the proper care taught to me by the
Quartermasters. If at any time I lose an accessory, such as a button, collar, citation
cord , NO ONE other than the current Quartermasters, Mrs. Evanilla and/or an
available Booster member may fix or replace this accessory. If my pants or jacket
sleeves need to be hemmed, they must be done by a professional tailor and neither
the pant legs nor the jacket sleeves may be cut at any time in the hemming
process. I understand that the cost to have a button replaced is $2 per button, and
the cost for a replacement hangar is $7. Replacement cost for lost citation cords is
$13 per cord.
I further understand that my assigned uniform will only be worn and used for its
only purpose, which is to represent the Arroyo High School Band at any band
function, performance or in band photos. I understand that I may NOT eat any type
of food or drink other than plain water while in uniform. UNIFORMS MUST BE
DRY-CLEANED. If I in any way break this signed agreement, I am aware that
consequences, including but not limited to a lowering of my class grade or removal
from the program, may result. The garment bag, my uniform and all its parts are to
be cared for properly and in a reasonable fashion that will not result in damage to
any part of the garment bag or uniform or uniform parts. Uniform collars may be
machine or hand washed to be maintain their cleanliness and must be removed
from the uniform prior to dry cleaning.
Finally, I COMPLETELY AND FULLY UNDERSTAND that if, at any time, my
uniform and/or its accessories and parts are lost, stolen, burned, stained
beyond repair, or in any way damaged and/or destroyed or cannot be
returned in usable condition, I will pay the exact cost for replacing my missing
or damaged uniform as assessed by the Band Office, and/or pay for any
damaged or missing accessory or part.
