Avondale Golf Club invites you to attend the 42nd Annual Memorial Day Mixed Couples Golf
Tournament to be held Saturday and Sunday, May 23rd and 24th, 2015. Play will be individual team by
divisions. The format is Chapman both days. The Schedule will be:

Awards Presentation
The field will be limited to 60 couples on a first come basis. The entry fee must accompany the entry
form. No Refunds will be given after May 21st, 2015
Handicap maximums are 36 for men and 40 for women, with a maximum 20-stroke spread between the
couple’s handicap. All handicaps must be figured on the USGA System. Established handicaps as of
May 15th, 2015 will be used. GHIN # and USGA Index handicap (not home course handicap) are
required for your entry to be accepted.
