This includes seeing that the medication your child may have left over will get home in a safe manner. Students will not be allowed to carry medication home. Only a parent or legal guardian may pick up the medication. Any medication left in the building after June 19, 2015 will be destroyed. If your child needs to take medication during school hours next school year, please have your health care provider complete the attached form before school starts in the fall. Bring the completed medication authorization and the medication in a properly labeled container when school begins in the fall. Please do not put the medication in your child’s back pack--the forms and any medications need to be carried in by the parent or guardian. Thank you for your cooperation.
