Credit Union Members Lottery Rules Please deduct the total cost shown below from my credit Union Membership Account 1. The Lottery is only open to members of the credit union over 18 years of age 2. The lottery is drawn on 5th of every month Number of tickets required Lottery Application Form This form can be used to join the Lottery or add more tickets Credit Union: Credit Union Member Number: Total Cost If you win, we will notify you by post. A list of all winners will appear on your Credit Union website and Facebook page. Join our monthly lottery Tickets cost only £1 each Lottery drawn on the 5th of each month Great cash prizes Buy up to 5 tickets a month Just complete this form to give us permission to make deductions from your savings account for your tickets Remember you must have funds in your membership account to be eligible for entry Name: Home Address: Daytime Telephone: Return completed form to: 3. Tickets cost £1 each and members can have up to 5 tickets 4. Prizes are decided by the Lottery Committee. The total prize money will be at least 70% the total income from ticket sales. The balance after costs have been deducted will be donated to the Credit Union to improve products and services. 5. Staff at Credit Union Solutions (CUS) will perform the monthly draw. 6. Members must have sufficient funds in their share account by 5th of each month to cover the ticket cost, otherwise entry for that month will be fore fitted 7. Prize money is paid into the member’s instant access savings account. 8. Any member whose Loan Account is in arrears will be suspended from taking part in the Lottery. 11. The member may terminate their lottery membership at any time 12. Once an application has been received the member will be included in all Lottery draws (provided these rules are satisfied) until the member cancels their lottery application in writing. 13. The CUS administers all Lottery income on behalf of the members and accounts for it separately from any Credit Union funds. 12.
