!Scholarship RequirementsApplications MUST be received by March 31st, 2015 at 5 pm
! 1. Applicant must be the child or grandchild of a member of our organization who has attended at least 3
!
activities (one shall be a meeting) within the past 12 months and has been a member for 12 months
prior to the application submission. Children of associate members that meet the same requirements
may be eligible based on a funding evaluation by the scholarship committee.
2. Scholarships are for high school senior students planning to attend college after graduation, other first
time applicants and past recipients (defined as previous recipient still enrolled in an undergraduate
program).

Awarding of scholarships will depend on funds available for disbursement. Priority will be given to the
high school senior, followed next by other first time applicants and lastly previous recipients. The
eligible high school senior will always have first priority. In the event funds are limited, priority for other
first time applicants will be based on the date of application submittal.
3. Applications must be received by March 31, 2015 at 5 pm for review by the scholarship committee.
4. Once a scholarship is approved proof of college enrollment must be provided to the committee.
5. This information sheet/requirement form must be attached to all scholarship application packets.

6. Awards must be accepted in person. The recipient will receive their award on one of two dates, which
will be scheduled by the scholarship committee.
!! 7. Incomplete or ineligible applications will be rejected upon receipt.
