across Victoria & Tasmania
Hume Region program has 2 offices located in Wallan & Chiltern
providing 114 HCP’s across Hume Region covering the following
areas:

What is a Home Care Package?
Government funded program aimed at
• Assisting people to remain living at home for as long as
possible
• supporting the person having choice and flexibility in the
way that care and support is provided at home.

goals
• deep, collective determination to reach an identical
