I hereby authorize my child’s participation in the 2015 Little Spiker’s Camp at Middleton High
School. I know of no physical, mental, emotional or behavioral problems which may affect my
child’s ability to safely participate. I understand it is my responsibility to ensure that my daughter
has current and active medical insurance before she may attend and confirm here that she does. I
authorize the camp staff to act on my behalf should any health problem or injury occur. Neither I
nor my daughter will hold Middleton High School or any person associated with this camp liable
for any injuries/illnesses sustained while my daughter is at camp.
