Scholarship Application Form
Mornington Communications Co-operative Limited has been providing telephone service to rural
members since 1919. Mornington Communications goal is to provide total quality communications
service and community leadership to its service areas. To demonstrate dedication and commitment to
education and to rural youth, Mornington Communications is offering a total of $4,000.00 in
scholarships to four students who are currently enrolled, or will be enrolled, fulltime at College,
University or a Vocational Institute of higher learning. There will be four scholarships awarded in the
amount of $1,000.00.

Qualifications:
( The Applicant must be currently enrolled/or will be enrolled in fulltime courses at College, University
or Vocational Institute of higher learning which will lead to a diploma of degree.
( The University/College must be a registered Ontario School as per the OUAC & OCAS website. See
links below for a list of all registered schools.

( The applicant must be a resident of Ontario.
( Parent/Guardian must be a member of Mornington Communications Co-operative Limited. Only one
scholarship per individual will be granted.
( Immediate family members of employees and/or immediate family members of current Directors of
Mornington Communications are excluded.
To Apply:
( The Applicant must complete the attached questionnaire and return it to Mornington
Communications no later than 4:00 PM on April 1st, 2015. Responses must be typed and this
original form must be returned. Acceptable means of delivery include: in person, by mail, or by email.
( Any scholarship application not completed in its entirety, with all the requirements attached will be
disqualified.
( Award recipients will be announced at the Annual General Meeting in May 2014 at the Perth East
Recreation Centre.
Scholarship Award:
( The scholarship is for one year and is non-renewable; each applicant granted a scholarship can only
receive the scholarship once.
( The Scholarship winner will be selected by committee. The primary criteria for determining the
winning submissions will be placed on scholastic ability and leadership qualities. All students are
encouraged to apply.
( Scholarships will apply towards tuition at a College/University/Vocational institution
in Ontario. To redeem the certificate, the scholarship recipient must provide Mornington
Communications with a copy of a paid invoice for tuition. Mornington Communications will then
reimburse the winner by issuing a check payable directly to them.
