schedule an appointment with your doctor. Or just go to a CVS or Walgreens convenience
care clinic, where no appointment is needed. These convenience care clinics do the finger
stick for the fasting blood tests. Just bring your personalized health provider screening form
with you for completion. They will complete the form for you and then you will fax in the
completed form to Optum.

Thank you to all of the candidates. The Board of Directors certified the election on 3/23/2015. The
names in bold signify the members elected to the various positions. A special thank you to Julie
Owen-Chair of the Election Committee for the work she and her committee did for the election. All of
the members of the committee that assisted in the election thank you for a job well done.
IMPORTANT HEALTH INSURANCE INFORMATION
Employees enrolled in a medical plan can save on their medical premium contributions by voluntarily
participating in the Health Rewards Program. Employees who earn 300 points by July 31, 2015 will
receive the Health Rewards discount of $50 per month in 2016. If an employee covers their
spouse/domestic partner on the medical plan, then that spouse/domestic partner must also earn 300
points in order for the employee to qualify for the discount. Your results are confidential. FEDERAL
LAW PROTECTS YOUR PRIVACY. The District is only notified of your participation and if you have
completed all required actions within the program time frames.
STEP 1
The Health Assessment is a confidential, online interactive tool which allows you to assess your lifestyle habits
that are directly linked to health status and health care costs. Log into www.myuhc.com and click on the
Health and Wellness tab, followed by the link for the Health and Wellness info, then the “Health Assessment”
button under the “I Am” section. Follow the directions listed and confirm that your assessment has been
submitted. You get 100 points immediately upon submission.
STEP 2
Complete a Biometric Screening for 100 points. Options include having participated in Onsite Biometric
Screenings or downloading a personalized Health Provider Screening Form (MD Form) for your in-network
physician or Convenience Care Clinic (not an Urgent Care Center) to complete after they do the biometric
screenings. Fax your form to Optum as indicated on the form, and follow up to ensure the form was received,
if you are not contacted within 30 days. To download your personalized Health Provider Screening Form (MD
Form) visit the Health and Wellness Portal on www.myuhc.com . First time users will need to register with
information from their United Healthcare ID card.
STEP 3
If 2 or more of your biometric measurements do not fall within the established ranges you can still earn rewards
by participating in a telephone-based wellness coaching program. To connect one-on-one with a Wellness
Coach, call 1-800-478-1057. Programs may take 8 – 12 weeks for completion of the program. The
program must be completed prior to the rewards credit due date to be eligible (July 31, 2015) for the
reward to begin in January of 2016. You can start the program prior to completing the biometric screening in
order to complete it within the required time periods.
