Stallholders Registration Form – 2015 REGISTRATION COMPLETION NOTES Please read carefully before completing Registration GENERAL SECTION Required fields are: o Company/Stall Name o Address Line 1 and Postcode o At least 1 contact name o At least 1 telephone number – preferably a mobile number so that we can contact you by text if need be. o E Mail address STALL SECTION All fields (with exception of “Further Information”) are required. Stall Description - Please describe what your stall will offer/sell at the festival & if possible give examples. Hot food cooked/prepared on day of the festival Children's sports, games, drama, workshops, youth groups (excluding face painting) Face Painting, Cosmetic or Henna applications Tea, coffee or non alcholic drinks - no food or alcohol Preserves, Ice cream, cheese, sandwiches, cakes, savory snacks etc. - no cooking or preparing of hot food on day festival (may include hotproduct, drinks) this includes the likes of Any stall providing leaflet onlyofand data promoting their Local Government, Historical Society, Civic Trusts, Campaign groups, Local Park groups, sports & activity groups, Colleges & Schools, Registered Charities, Community Groups, Home produced items only (including food produce) & no other sales outlet New business idea Providing therapy or treatment e.g. acupunture or massage o If you are not sure which Stall Type applies, please contact us either via the website or by e mailing [email protected] Catering Stalls – all Supplementary questions are required (these questions only appear on the on-line version of the form after “Catering” has been selected from the Type drop down box Catering, Food/Drink and Drink Stalls – supplementary questions: o Volunteer Scheme – each year we allocate food & drink vouchers to our volunteers (normally in £5 and £2 values) to purchase refreshments at any stall who have indicated that they are willing to accept these vouchers & be reimbursed in cash by the Festival Treasurer at the end of the day. Please note that as all documents received from Catering & Food stall registrations need to be lodged with LBE, the Festival cannot guarantee to accept any food or catering stall registration received after 31st July 2015. 1|Page Stallholders Registration Form – 2015 SUPPORTING DOCUMENTATION SECTION Please check which forms you need to supply BEFORE commencing with Registration Document Public Liability Certificate Copy of the front page of the last food hygiene inspection report with a rating of at least 3 A valid Food Hygiene/Level 2 Certificate for operatives who will be handling food on the day Evidence of Food Registration with your local Authority Trade Body Membership Evidence of skills Stall applying ALL Stalls (excluding Kitchen Table Entrepreneurs) Catering & Food Stalls only Catering & Food Stalls only Catering & Food Stalls only Therapy Stalls only Cosmetic & Therapy Stalls only If it is not possible to supply all the required documents at the time of Registration – please tell us why in the relevant field (e.g. current document expires prior to festival date and you will send document in once it renews) and please also give us an indication of when you will be sending in any outstanding documents. PAYMENT SECTION All fields in this section are required o An important field this year is “Payment Reference” We have introduced this as last year we were unable to cross reference some payments with stalls. Please create a payment reference using the first 8 characters of your stall/company name – up to 8 maximum. This is especially important if you choose to pay via electronic transfer or your cheque is not from a company account. This year there are 2 methods of payment: o Postal Cheque Cheques to be payable to: Palmers Green Festival Please write Payment Reference on reverse of cheque Deposits (Catering Stalls only) – please pay this on a separate cheque. This cheque will only be destroyed after the festival if the committee decide that litter or charcoal was responsibly removed from your site Cheques should be sent with Registration Form to – Festival Stall Registration, 15 Osborne Road, Palmers Green N13 5PT o Electronic Transfer On line payment to the following account details: Account Number: 00098354 Account Name: Stall Fees Sort Code: 40-52-40 2|Page Stallholders Registration Form – 2015 You MUST include the Payment Reference otherwise we reserve the right to refuse payment (for example, if we cannot trace which stall the payment is in respect of) Deposits (Catering Stalls only) can be paid electronically and will be returned after the festival if the committee decide that litter or charcoal was responsibly removed from your site Please note that there may be an additional £20 administration levy for any stalls applying and being offered a plot st after 31 July 2015 as Registration closes on that date. The payment is non-refundable unless the Palmers Green Festival Committee decides to decline your application. We operate a no show, no refund policy. PR/SOCIAL MEDIA SECTION To have your stall listed on the Festival’s website, please include the biography description & attach an image or logo to your registration. If either of these is not received, we will be unable to list your stall. If you include your website address, we will include a link to it from the stall listing on the Festival website Facebook and/or Twitter – if you include these, the Festival will like/follow your page and would ask that you reciprocate and like/follow the Festival’s Facebook & Twitter pages. Links to these are on our web site Festival Flyer – this is a 6 page leaflet that is distributed to most homes within Palmers Green and also handed out on the day of the Festival. Any stall which completes registration before this Flyer goes to printing (normally around the beginning of July) will be listed as attending. Please check the box if you would like to be contacted regarding have a paid advert in the Flyer. DECLARATION Registration will not be accepted unless both of these boxes are checked o A copy of the Terms and Conditions can be downloaded and printed from the Stallholders web page o The Festival uses Mail Chimp for all our mailing list and therefore can assure that your contact details will be kept secure and will never be passed to any third party. We ask all registered stalls to join the mailing list so that we can contact you as needed (e.g. to advise plot numbers) 3|Page
