Shawn A. DeVol, PT, DPT, CDP, CSST is a licensed physical therapist with 16 years of clinical
experience in home health, outpatient, and acute care settings. Dr. DeVol has been a member of
the APTA for 18 years and currently holds membership in their Home Health and Geriatric Sections.
Dr. DeVol co-founded and owns Select Home Care, a licensed Medicare certified home health
agency in Columbus, OH.

Home health agencies, as well as assisted living facilities have seen a significant spike in acuity levels, rehospitalization rates, as well as an increase in the dementia patient population they serve. In the ever-changing
world of healthcare, it is becoming evident that providers of skilled services must create an environment of
collaboration and communication in order to improve the quality of life for their patients and remain viable. Unique
patient-centered multi-disciplinary approaches are on the rise. Utilization of these types of programs is critical in
order to maintain high-quality care standards to maximize patient outcomes and satisfaction.
This workshop is designed to cover a wide range of topics in order to provide the participant with an improved
understanding of the “big picture” of home health rehabilitation. Participants will learn to overcome common
barriers to successful rehabilitation along with development and implementation of dementia and fall prevention
programs that work. This workshop will also include practical strategies for documentation across all levels and
settings to ensure reimbursement. Knowledge is power and being able to identify problems, to collaborate, and
launch effective solutions is the key to being effective healthcare providers.
About Your Program Instructor
Shawn A. DeVol, PT, DPT, CDP, CSST is a licensed physical therapist with 16 years of clinical experience in home
health, outpatient, and acute care settings. Dr. DeVol received her Bachelor of Education degree from Otterbein
College, a Master’s Degree in Physical Therapy from Andrews University, and earned her Doctorate in Physical
Therapy from The University of Montana. Dr. DeVol co-founded and owns Select Home Care, a licensed Medicare
certified home health agency in Columbus, OH. Having the unique perspective of business owner and clinician
affords Dr. DeVol the opportunity to gain a wider vision on quality care standards. She is particularly passionate
about the area of fall prevention. In 2013 she published A Multi-Disciplinary Fall Management Program Design
Using Home Health for Elderly Residents with Dementia Residing in an Assisted Living Setting. Dr. DeVol has been
a member of the APTA for 18 years and currently holds membership in their Home Health and Geriatric Sections.
She is a Certified Dementia Practitioner as well as a Certified Senior Strength Trainer.

SATISFACTORY COMPLETION
Participants must pay tuition fee, sign in, attend the entire seminar, complete an evaluation and sign
out in order to receive a certificate of completion. Participants not fulfilling these requirements will not
receive a certificate. Failure to sign in or out will result in forfeiture of credit for the entire workshop. No
exceptions will be made. Partial credit is not available.

Physical Therapists - IN: As per Indiana regulation 844 IAC 6-8-5(1 and 9) continuing education courses
approved by an affiliated chapter of the American Physical Therapy Association (APTA) meets the state’s
approval criteria. This course has the current approval of the Kentucky Physical Therapy Association for
6.0 contact hours. KY: Summit Professional Education is approved by the Kentucky Physical Therapy
Association as a continuing education provider. 6.0 contact hours, (Class I) will be granted for course
completion and the approved provider number is CS45-2008-KPTA. MI: This workshop meets accepted
standards for continuing competence activities. Save your certificate of completion for the relicensing
