Welcome to APM Terminals Maasvlakte II
APM Terminals Maasvlakte II B.V. (hereafter referred to as APMT MVII) is a fastgrowing organisation in which all work undertaken is directed at creating
the container erminal of the future.
Not only will the performance of the terminal be amongst the best in its
class. Inextricably bound up with this are the objectives to not only have
the safest and most sustainable terminal from the start of operations, but
also to construct and install the terminal with equal regard to safety and
sustainability. The terminal expects to open its gates by the end of 2014. Both
in the time towards this moment as thereafter the slogan is “Everybody goes
home safe and unharmed!”.
APM-Terminals’ Policy Statement
APM Terminals Maasvlakte II B.V. (APMT MVII) is a fast growing project organisation, whose main goal is to realise ‘the container terminal of the future’;
meaning the most innovative, highly computerised and most efficient terminal ever.
The policy of APMT MVII is therefore aimed at incident and/or accident prevention as much as possible; the ultimate goal being zero accidents and for
everyone (employees, contractors, suppliers and visitors) to return home safe
and sound.
In order to estimate the various potential risks at all levels and to take timely
controlling measures, APMT MVII’s safety policy is founded on three pillars.
• People and culture: leadership directed at stimulating safe behaviour
• Technical integrity; top-class, certified and reliable equipment with maximum separation of man and machine.
• Management systems; emphasis on risks analysis and risk management with
practicable and effective regulations concerning safety and health.
During preparation, construction, initialising and test and operation phases,
attention to HSSE is of the utmost importance. APMT MVII’s management
expects all the parties involved to work harmoniously with the utmost regard
to safety, health and the environment. All employees, internal and external
are expected to comply with this policy in both word and deed, and they must
act accordingly.

Project description
APMT MVII is developing a new container terminal for the handling of
containers. Containers are loaded/unloaded from deep sea ships/barges/
vessels, trucks or trains. Onward transportation then takes place by sea, rail
or road transport modalities.
APMT MVII will carry out the first operational transactions as a commercial
terminal in 2014. The year 2014 will be devoted to the construction of the
terminal and the construction and testing of the cranes and vehicles.

All main contractors must deliver a health and safety plan (HSSE-plan) to
APMT MVII in advance of the start of the work which points out the risks
associated with operations and the necessary management measures to
minimise these risks.

Project stages
A number of major construction and equipment contractors will be coming to the location to carry out their work at the same time. Besides that
APMT MVII is also testing equipment and systems. The construction of quay
cranes, stack cranes, automatic guided vehicles, camera systems, buildings
and terminal layout will all take place at the same time. This conjunction of
various operations means that a number of risks can
occur and regular activities are planned that require proper coordination.
To achieve sufficient coordination during the construction it is necessary
that everyone is sufficiently informed of the arrangements and the organisational structure. The site manager has an important role in this.
It is of paramount importance that the companies build up a good working relationship and the operations coordinate themselves in such a way
that arrangements with regard to health, safety and the environment are
properly fulfilled adhere to.

Health and safety organisation on site
In order to allow the on-site operations to run as smoothly as possible, an
organisational structure has been set up in which the various participants
are represented, such as health, safety and the environment, site coordination and the different project managers from the equipment/civil engineering focus areas. The various contacts together with the corresponding role/
department are listed in the organisation chart on the right (page 5).

This APMT MVII project is characterised by health and safety. APMT MVII
has committed itself to becoming the safest terminal in the world. This
attention to health, safety and the environment is not only limited to the
operational phase, but also to the construction phase.
Everyone needs to contribute to a safe working environment and to do everything possible to carry out their jobs in a safe and prepared manner.

Finding the location
• Follow the A15-N15 in the direction of Maasvlakte
• Take exit “Havens 8210-9900/ Distripark Maasvlakte/ Maasvlaktestrand”
towards Maasvlakteweg
• At Maasvlakteweg, take exit “Havens 8210-8700 / Distripark/ Futureland”
towards Dardanellenstraat
• Follow Dardanellenstraat and after about a few hundred metres, turn left
and follow the signs for “8410”
• After 300 metres, the entrance gate is on your left
Personal protective equipment
Employees/visitors/suppliers that enter the site – with exception
of the sanitation and dining facilities and where indicated – are
obliged to make use of the following protective equipment:
• Safety helmet (to be worn during all operations on the project
location). Safety helmet with no stickers other than approved by
HSSE (without solvents and plasticizers), and only the name and
function. APMT employees are required to wear a Yellow helmet
provided by APMT MVII (ERP Organisation green). Deviations only
with permission and for functional reasons;
• Safety footwear; class S3 or S5.
• High visibility clothing (tabard or safety vest with fluorescent
yellow colour and reflective stripes). Work clothes should also
display the company logo.
• A safety harness is required if operations are being carried out at
height, with danger of falling (> 2.5 metres) or at the locations at
which falling from height could lead to serious injury (even if the
fall height is limited).
• A lifejacket is required when working in the vicinity of water.
• Safety glasses must be worn when there is risk of eye injury.
• Ear protection is required in all situations in which the noise
levels are above 80 dB (A).

Carrying out risk full operations
In order to create a safe workplace, additional requirements
apply to a number of locations.

Working near water
For working near water (quays), supplementary measures to prevent drowning and/or hypothermia apply. As operations must be carried out in the
vicinity of the quay edge, an edge barrier must be placed in order to prevent
fall hazard.
When working on the dock with long-term residence, the employee must
work on a leash. If this is not possible should be made of
​​ a lifejacket. Use
within 2 meters of the quay edge Under long stay work on the quay means

Working near the railway
The APMT MVII terminal is equipped with a rail terminal facility. Trains will
cause significant additional risks, namely a collision danger between person
and rolling stock.
In the construction phase, it is possible that not all safety systems will yet
be working correctly. Explicit permission is therefore necessary in order to
carry out operations within the railway infrastructure.

• Access to APMT MVII is only permitted after registration with the APMT MVII
site security personnel (see also Chapters 29 and 30).
• APMT MVII is subject to the ISPS code, which requires supplementary security rules.
• Any personnel entering the APMT MVII site must hold a valid passport or
other form of official identification.
• Employees from outside Europe or from non-EU member state will only be
admitted if they are able to hand over a valid work permit.
• Any personnel entering the APMT MVII site location – with the exception of
visitors–- must have successfully completed a BVCA course (basic safety), or,
in the case of managers, the VOL-VCA course (operational safety management). A copy of the certificate must be submitted (an exception applies to
vehicle repair companies, see Chapter 31).

Working at height
Working at height (>2.5m) must always be regarded as hazardous, and may
only be carried out using approved equipment. The preference regarding the
work equipment to be used for carrying out work at height must always be
given to stable platforms such as aerial platforms, scaffolding and/or mobile
scaffolding instead of ladders.
Repaiars of (external) vehicles
In case of breakdown of a vehicle from an external driver, always inform the
security. Never perform maintenance on a vehicle on the road without
having done a proper risk assessment and informed the security.

Order and tidyness, waste products
Accidents, incidents and disasters
All (First aid) accidents/incidents must be reportedin CMO and to the APMT
MVII site manager. If aid from professional emergency services (e.g.
ambulance, fire services) is necessary, on-site security must be informed
immediately (see the emergency card on site).
In the case of disaster, all employees must gather at the site’s assembly
point, located at the security lodge Stack 36, and at the visitors parking near
the office.
To prevent incidents from happening again, unsafe situations, incidents and
damages always have to be investigated to determine the cause.
11 Emergency Response Team
In the field of APMT MVII there is an emergency response organization
present. The emergency response organization comes into action when an
event occurs at the terminal. An ERP member is recognizable by its green
helmet.
First aid facilities are available in all offices. There are also several AEDs
(Automatic External Defibrillator) present. The location of the AED’s are
indicated on the map.
Code of Conduct
A number of general rules on conduct, which APMT MVII values highly, apply
on site:
• Offensive language, aggression/violence, sexual intimidation, harassment
and discrimination are not tolerated and will lead to sanctions on the part of
APMT MVII;
• Sexual images/pin-ups are forbidden;
• Communication with newspapers/the media may only take place through
the official channels and never via individual employees (therefore always
refer to the site coordination);
• Filming and photography are strictly prohibited without prior approval.
• Challenge one another immediately in the event of any unsafe behaviour;
• Report unsafe conditions on site directly to the APMT MVII site manager and/
or HSSE coordinator;
• Workwear must be worn at all times, shorts and an exposed upper body do
not provide adequate protection and are therefore not permitted;
• Sanitary and amenities for breaks are available on site. Please keep the facilities clean and tidy and take the basic rules of personal hygiene into account.
• Public urination is forbidden on site; sufficient amenities have been provided;
• Dining is only allowed in the therefore designed area’s.
The site must be kept clean and in good order; packaging material, scraps of
wood, stones and similar must be cleared up immediately.
Waste products (including hazardous waste products) must be sorted according to type in containers, and are periodically sent to a recognised waste
processor.
When stopping operations at the end of a working day, all machines and
electrical,-/ hydraulic tools must be turned off, and as many as possible
must be stored in the materials container. Use by unauthorised personnel
must be prevented.

Alcohol and drugs
It is expressly forbidden to consume and/or use alcohol and/or drugs or to be
under the influence of alcohol and/or drugs. APMT MVII applies a limit of 0%!
Should the situation arise, APMT MVII will take (disciplinary) measures.
The client may organise alcohol and drugs checks. Everyone is obliged to
cooperate with these checks.

Sanctioning of behaviour
APMT MVII is of the opinion that punishment is not the most efficient means
of achieving a safe workplace. Should safety, however, be wilfully or knowingly compromised, this will lead to direct consequences for those involved.
Access to the site will be forbidden in the following cases:
• Fraudulent acts (details provided proven to be incorrect);
• Not abiding – also after warning— by the prescribed safety, conduct and
environmental rules;
• Creating unsafe situation(s) which could have led directly to situations harmful to yourself or others;
• Not abiding by the relevant traffic rules;
• Insufficient communication/discussion/coordination of activities/operations
with other parties/APMT MVII;
• Taking too high risks when carrying out operations;
• Not taking part in or organising information sessions, tool box meetings and
safety awareness training;
Offenses will be immediately reported to the police.

Health, safety and environment points
On the site location, health and safety points have been set up – locations
where means are present to give first aid, containing important information
about the location and fire extinguishing equipment.

Smoking
A smoking ban applies to the whole APMT MVII site. Smoking is
only permitted within the designated smoking zones. These zones
are clearly indicated by means of signage (near the health, safety
and environment point).

Equipment, employee and machines
Every contractor must make sure that all equipment is maintained, approved and/or visually inspected on a regular basis. Hired tools also fall
under this obligation; the main contractor must check in advance whether
the tools conform to the periodic test requirements.
Furthermore, the contractor must maintain an overview of equipment
in which all tools, machines and work equipment is registered and must
always have this available on site. All equipment is provided with an inspection sticker indicating the next inspection date.

Tools and equipment
The contractor must fulfil the following requirements relating to the safe
usage and maintenance of tools and apparatus:
• Ensure that all tools and equipment are operable and to maintain them in
such a way that they are in a safe state;
• Ensure that all electronic equipment is periodically checked and the administration is maintained in accordance with legal requirements;
• Ensure that inspection and maintenance of critical apparatus is carried out
in accordance with legal-/statutory requirements;
• Ensure that routine inspections and the daily pre-start checks are carried
out and the apparatus is looked over before use. Defects must be repaired;
• Ensure that all rotating or moving components of all tools or apparatus are
well-protected in order to prevent contact;
• All motorised machines must be equipped with means of stopping the
machine in a safe manner, such that they cannot (immediately) start up
again.
Unsafe operations and the right to stop work
If employees believe that operations could become unsafe to carry out, or
could lead to dangerous circumstances, s/he must immediately inform
his/her own supervisor and if necessary, the APMT MVII site manager. The
super-visor/site manager will then investigate the situation, and, if necessary, take appropriate action.
Employees have the right to stop work whenever they – based on their own
estimation - think it is unsafe to continue work.
The main contractor or subcontractor is obliged to remove his/her own
waste. All main contractors must set up a recycling station in which waste
may be divided or deposited by type. Employees are expected to cooperate
in order to maintain the working environment.

Hoisting and lifting equipment
Contractors must keep a register of all hoisting and lifting equipment.
The register must contain the details of the hoisting and lifting equipment
(inspection data, certification, etc.) The contractor must ensure that all
equipment is tested, checked and certified according to the Legal requirements. They must also ensure the checking of all hoisting and lifting
equipment, and label/mark it with the next inspection date due.
Contractors must prepare and implement relevant working instructions for
the safe hoisting, loading and unloading, securing and implementation of
the hoisting operation.

Work permits
Certain methods of working entail additional risks, and require extensive
preparation and coordination. As a result of the heightened risk, a work
permit is required for these operations.
The work permit is an organisational measure aimed at clarifying the various management measures beforehand, and to communicate with all those
concerned.
The work permit applies to the following operations in all circumstances:
• Working with electrical installations, live components and working near
high voltage;
• Carrying out heavy lifting tasks, hoisting with several cranes simultaneously,
deployment of a crane pontoon (from the water);
• Working in confined spaces;
• Carrying out operations that involve a number of different companies (e.g.,
operations that are not solely limited to the area allocated to the contractor)

Traffic rules

Transport routes
• The Road Traffic Act [Wegenverkeerwet] is nearly fully applicable on the
project site (there are some additions).
• The maximum speed is 30 km per hour, or where necessary lower
• Wearing seat belts is required in all vehicles.
• Using a mobile phone or other electronic devices while driving is not permitted.
• Construction routes have been prescribed on site, follow these routes.
• Keep through routes and crossings free.
• Overtaking is not permitted.
• Parking of passenger cars on site is only permitted in the spaces allocated
for this purpose.
It is allowed to use the following vehicles on the asphalt:
• Passenger cars;
• Trucks (drive with reduced speed and not overloaded);
• Wheel mounted cranes and wheel dumpers.

Activities only allowed after consultation of the site management:
• Use of “Point concentrated loads” on the asphalt/outriggers (support of
cranes during hoisting activities);
• Enter an area where (recently) asphalt is applied;
• In case of material damage to the asphalt, do report this immediately to
the site manager (+31 6 1173 6362). Quick response might prevent further
damage;
• In case of repair costs respective companies will be held responsible.

Weather conditions
Maasvlakte II is located at the coast, and will often be exposed to strong
wind and a low wind chill temperature.
Generally speaking, wind speed 6 is the limit for performing hoisting work
(partly dependent on the type of objects being hoisted and the wind type).
In conditions of wind speed 8 or higher, on-site operations will be stopped.
In these weather conditions, consultation is necessary with the site
management regarding ensuring the safety of the various materials and
machines on site (securing equipment/material) and preventing damage.
During operations, employees may have to deal with sand/dust grains. It is
therefore advised that safety glasses are made available.
During the summer, the sun’s ultraviolet radiation can be above average.
Cover bare skin and/or use sunscreen lotion. Also, in combination with
strong winds, dehydration can occur. Always drink plenty fluids.
Monitoring and visitation
• To avoid goods or property to be removed without permisson it is regulary
examined by the security department and ensures that all the requirements
have been met.
• This officer is authorized to check vehicle contents, briefcases, etc.
• Everyone is obligated to cooperate.

Temporary cabin area and parking area
The accommodation cabins have been temporarily set up in stack 36, where
all crane contractors’ accommodation and material containers are located.
There is also parking space. Sanitation, first aid measures including AED are
also located here.

It is not allowed:
• Tracked vehicles (for example bulldozers);
• Short rotating (wringing) traffic;
• Vehicles over 60 tonnes;
• Driving or parking on asphalt edges;
• Leakage/spill of oil on the asphalt.
Heavy transport
• After consultation with APMT MVII, it is possible to attain an alternative
route for exceptional transport such as mobile cranes and heavy transport.
• All heavy transport must be registered in advance with the Site Manager.
• Accompanied exceptional transport takes preference over other traffic on
site.
Traffic in the vicinity of cranes
• It is strictly prohibited to drive under a load of the quay crane or to go under
a mobile crane with a load. With a few exceptions, the spreader is also seen
as a load.
• It is strictly prohibited to enter the automatic stack, AGV area, AGV test area
or areas in which the automatic L-AGV’s are driven. During the commissioning and construction phase, safety procedures may not be fully in place.
Entering the automatic area is especially dangerous and subseted to specific
rules.
• Parking vehicles at the quay must be at sufficient distance from the cranes
and must always be in the parking zone (do not park under the crane).
• It is forbidden to enter the automatic area in which the ARMG cranes are
driven without prior permission.
Activities on the asphalt
To avoid the top layer of the asphalt is damaged during the work on site
there are a few rules that apply specially for (heavy) machinery (like mobile
cranes, trucks, etc) and stowage of machine-parts and tools.

Temporary power supply

Some locations still have temporary power provided by generators. These
generators may only be operated by the Site Manager or his/her designated
deputy.
All portacabins and containers should be connected to the temporary power
supply. In order to be provided with the necessary electricity in the works
area, you may submit a request to the APMT MVII BV site manager.
Extra distribution points will be placed in the vicinity of construction activities by the cranes.
Please note that the distribution points are only suitable for normal electronic usage (220V and 380V).

By carrying out the instructions and downloading in a registration form,
only the exam must still be completed to allow access to the site for the
first time. On successful completion of the exam, an access card will be
issued and employees may drive in and out each day.
Fixed power supply
As of June, APMT MVII is connected to the fixed power grid. The permanent
electricity supply will be rolled out over the site gradually. As a general rule,
sufficient power will be available to run the machines and cranes at full
capacity. Consult the Site manager for detailed specifications.
It is expressly forbidden to enter the switching areas or to use the controls.
Only those who are designated and have been trained are permitted to use
the controls.

When placing an order, large or small, please have your name and cell phone
number placed on the bill of lading or packing slip. This way the destination
of the goods can be easily traced. You will then be called by security to either
escort the driver to your workstation and back or to pick up the package
yourself at the gate.
Railings and fences
The whole APMT MVII site is fenced off. This allows access to the site to be
well-regulated and to prevent unauthorised access. It is strictly forbidden to
remove railings or fences without permission from the site manager.

Employees of suppliers – drivers who visit for the sole purpose of loading
and unloading – can take an in-company exam. As long as the employer
ensures that employees have been instructed and have followed the port
induction, no port exam need be completed.
Visitor registration
Visitors are, of course, warmly welcomed on site. These visitors must be
registered at least 24 hours in advance at the front gate by a person
authorized to do so..
Visitors may only enter the site when accompanied (offices of the civil contractors and APMT MVII are located close to the access gate and may always
be visited).
Visitors of contractors must be picked up from/brought back to the gatehouse, to prevent visitors (through ignorance or otherwise) from wandering
round the site location (considering all the risks that this would entail).

Vehicle repair instructions
With some regularity, it can occur that employees of repair companies will
visit APMT MVII for the purpose of carrying out repairs to defective vehicles.
Because the employees at these repair companies are not always in possession of the required VCA certificate, an exception is made in this case if the
repairs must be carried out on the site itself.
Works to vehicles and tools must be carried out by those mechanics without
a VCA certificate as follows:
• In workshop or shed (according to preference);
• On location in the field with a margin of 3 metres round the vehicle. An employee with a permanent access card must monitor safety during the work.
Supplementary rules for carrying out repair works:
• The ground underneath must have adequate load-bearing capacity;
• Spills and/or leaks must always be prevented (at risk of leakage, first take
preventative measures such as drip tray/container);
• Make use of the required basic PPEs and any additional PPE associated with
the job;
• Using inspected, reliable tools suited to the task.

Employers/suppliers/contractors/orders
All employees and suppliers who enter the site should, whenever possible,
be registered 3 days in advance. This will help to prevent long queue at the
front gate and allow work to take place more efficiently.
When visiting the site for the first time, all employees must take part in the
APMT MVII proprietary port induction. This instruction will be finished off
with a short test.

The employee(s) coming to carry out the repairs must, with their name
and signature, indicate that they have understood and will abide by these
instructions. Failure to comply with these rules may result in access to the
site being refused.

If the repairs cannot be carried out safely due to further risks in the area
(traffic, hoisting works in the area, etc.), the APMT MVII site manager is to be
informed immediately. They must – in consultation with the other parties –
determine the conditions necessary for carrying out work safely.

REPORTING ACCIDENTS
Always watch your own safety! (If there is no risk to your own life!)
• Give the first aid necessary.
• Report the incident immediately to Security (to enable emergency response
and/or external assistance) via portophone or telephone
• Report the incident to the area manager.
• Report serious accidents immediately to HSSE and the HR dept. of the direct
employer.
• Use the registration form, which can be obtained via the area management,
and return to the HSSE department within 24 hours.
• Serious accidents often create an emotional atmosphere. For this reason, do not
discuss culpability with the press, investigating officer or police. If necessary,
ask for postponement until a formal statement has been made.
