what makes the unloader so unique?
The Unloader® pioneered osteoarthritis knee bracing. In 1986,
the inventors of a single upright ligament brace noticed that an
increasing number of patients in their orthopaedic clinic had medial
meniscectomies and associated pain and/or instabilities. After
numerous discussions with orthopaedic surgeons, they looked
closely at adapting their ligament brace to unload the medial
compartment of the knee.
The Adjustable Dynamic Joint. In the early ‘90s, it became evident
that if the hinge was adjustable, treatment could be fine-tuned. The
Adjustable Dynamic Joint was created and its use in a custom-made
brace that could be hinged medially or laterally received world-wide
attention. By the mid-‘90s, the new Unloader ADJ brace was being
used around the globe and the name Unloader became
synonymous with OA knee bracing.
The 3-Point Leverage System is born. The theory was that, by
adding a valgus bend to a single, medial upright, the upper and
lower shells of the brace frame would act as leverage arms when
the counter force of a diagonal Dynamic Force Strap was applied.
This would unload weight from the medial compartment of the
knee, relieving pain.
Improving on a classic. The last decade has witnessed the rapid
expansion of the Unloader product line. New models (both custom
and off-the-shelf) and a variety of product improvements have
been added to improve patient comfort and compliance. Today,
there is an Unloader brace for every conceivable activity level
and body type.
Testing the theory. This new application was proposed to a patient
who had medial pain but wished to continue skiing. The idea to alter
the mechanical forces with a valgus hinge bend applied to the medial
side worked! The patient felt immediate pain relief.
The latest technology: Unloader One.® Unloader One is the most
signficant re-design of the Unloader concept since its inception.
Available in both custom and OTS models, Unloader One offers
a unique twist on the traditional 3-Point Leverage System. Instead
of an Adjustable Dynamic Joint and Dynamic Force Strap, Unloader
One features a streamlined, flexible upright combined with two
Dynamic Force Straps. Add a racheting Dynamic Force Control System
and padding that features Össur® Sensil® Silicone and Unloader
One offers superior unloading power in a product that virtually
eliminates migration.
The first study. After further consultation with the medical community,
a groundbreaking scientific study examining the efficacy of lateral
and medial hinge placement for treating medial OA was conducted.
This study concluded that medial valgus bracing resulted in significant
reduction of pain.
With a scientifically proven method and patents in place, these
new custom knee braces were launched. Quickly, this viable option
for treating unicompartmental osteoarthritis of the knee gathered
momentum. Eventually, lateral hinge placement was used to treat
lateral compartment symptoms.

Recent clinical research demonstrates that the Unloader One
provides the following benefits for patients: (1) decreased pain,
(2) improvement in function and (3) reduction in the usage
of pain medication.

effective bracing. That’s why our new Unloader One Plus model
features a wider thigh strap than the standard version, plus a
gastroc strap that helps secure the brace in place. And because

Össur warrants the frame and hinge components to be free
of defects in materials and workmanship for the lifetime of
the original patient. Straps, liners/covers, sleeves, return
cords, hinge coatings and other soft components shall be
warranted free from defects in materials and workmanship
for 6 months.

Custom Brace Alterations or Cancelations
All custom knee braces are custom fabricated for a
unique individual and cannot be restocked. Therefore,
alteration or cancelation of a custom order may result in
a fee if brace fabrication has already commenced. To limit
this possibility, please notify Össur of any custom order
alterations or cancelations as quickly as possible.

overnight to ground.
Custom Knee Braces
Most custom knee braces ship within 3-4 days. Rush
service is also available. Please contact an Össur Customer

allow an additional 48 hours for custom paint or private

Off-The-Shelf Knee Braces
Off-the-shelf product orders received before 12pm Pacific
Standard Time will be shipped the same day.
Össur warrants the frame and hinge components to be
free of defects in materials and workmanship for 2 years.
Straps, liners, return cords, hinge coatings and other soft
components shall be warranted free from defects in
materials and workmanship for 6 months.

2 Photos can be taken with a digital camera or smartphone. If using a smartphone, we suggest
you download one of the free grid camera applications. This will help minimize angle distortion.
• Apply landmarks to the leg with a marker at the medial and lateral joint spaces,1 plus a line
along the tibial crest between the center of the tibial tuberosity2 and 6" (15cm) below. Extra
landmarks can be added for better accuracy during chart analysis (e.g., outline patella).

• With the patient in a weight-bearing position, obtain the M-L measurement at the joint
space and record this value on the patient sticker.3
• Measure circumference at 3" (7.5cm) and 6" (15cm) both above and below the
patella4 and record these values on the patient sticker.
• Apply patient sticker 6" (15cm) above mid-patella for anterior view photo.

• If using a digital camera, adjust the height of the camera stand until the camera is level
with the middle of the patella. If using a smartphone, aim and hold the phone so the lens
is level with the middle of the patella.
• Move the camera stand (or smart phone) approximately 2-3 feet (61-91cm) away from the
patient, capturing roughly 8" (20cm) both above and below mid-patella in the viewfinder.5
Note: Align the camera with the leg to eliminate any internal or external rotation.
• Have the patient turn 90,° reapply patient sticker 6" (15cm) above joint line, and take a
picture of the lateral view.6
• Complete the Online Data Form at www.ossur.com/dms, attach the photos and submit.

• With the patient in a weight-bearing position, use the calipers to obtain the M-L measurement
at the joint space and record the value on the order form.
• Mark the patient’s leg at mid-patella, as well as 3", 6" and 8" above and below mid- patella.1
• With the ankle held at 90 degrees, record the circumference in centimeters at each of the
marked points from Step 1 above on the Custom Knee Brace order form.2

If measuring for a CTi brace:
Measure the patient’s tibial alignment using the standing graduated slider.3 At the mark
3" below mid-patella, bring the tool towards the lateral side of the calf until it just
touches the skin. Slide the horizontal bar down until it just touches the tibial crest. Read
the number on the horizontal bar at the point of intersection with the tibial crest. Repeat this
at the mark 8" below mid-patella. Record these two measurements on the order form next
to the 3” BELOW and 8” BELOW bubbles.

• Center the patient’s leg with mid-patella mark on line 5 of the tracing board.4 You can use
the centerline on the tracing tube to help align the mid-patella directly on line 5.
Note: If the patient has excessive tissue, use the knee center block to raise the knee
off the board. This helps maintain a proper profile of the leg.

• At the top of the board, hold the base of the tracing device flat on the board with the pen tip
facing the leg. Keep in contact with the leg while tracing the profile down to the bottom
of the board.5
• Without moving the leg, repeat on the other side.

• On the Custom Knee Brace order form, record the number where the tracing line
intersects each of the nine coordinates lines for sides A and B.
