About AltaMed Health Services
AltaMed began as the East Los Angeles Barrio Free Clinic in 1969, a
storefront clinic providing basic medical services. Today, AltaMed Health
Services, a private nonprofit organization, is a major provider of health
and human services in Los Angeles and Orange Counties and the largest
independent Federally Qualified Health Center (FQHC) in the United States.
AltaMed’s mission is to eliminate disparities in health care access and
outcomes by providing superior quality health and human services through
an integrated world-class delivery system for Latino, multi-ethnic and
underserved communities in Southern California.
Message from the CEO
In my 30-plus years with AltaMed, we’ve had more “firsts”
than I can remember. Our first adult day health center,
our first teen program and our first HIV program are just
a few of the amazing opportunities our organization has
undertaken. Our size, innovation and longevity will only
bring more of these opportunities our way in the future.
This summer, we received a significant distinction
from The Joint Commission. AltaMed was awarded
the very first Patient Centered Medical Home
accreditation from this prestigious organization.
This Joint Commission designation recognizes the
hard work AltaMed employees engage in every
day as they put our patients at the center of our organization.
Following these practices also gives us a framework to take our organization to the
next level and helps create a culture of excellence.

Thankfully, support

Promise campaign continues to
grow as we spread the word and welcome
new partners. With this publication, I am very
excited to announce that Monica Lozano, CEO
of ImpreMedia and Publisher of La Opinión, has
joined forces with AltaMed Foundation Chair,
Hector Barreto, and will Co-Chair Keep the
Promise’s Uninsured Services Program. Their
support has already made a substantial difference
in the amount of uninsured families to whom we
can provide care. We would also like to thank Jack
Cline, Jr. and Richard Gomez, Co-Chairs of our
Childhood Obesity Program, for their dedication and
commitment on behalf of this initiative.
Our goal is to raise $4.2 million, and thanks to the
generosity of AltaMed’s supporters, I am proud to tell
you that we have already raised $1.6 million toward
this goal. This year, we will increase our activities to
support Keep the Promise through a host of new
events. I look forward to keeping you informed of

AltaMed Launches its Keep the Promise Campaign
In keeping its promise to provide quality care without exception to the families
in our underserved communities, AltaMed officially launched its Keep the
Promise campaign in November 2011.
The campaign, critical to the health and well-being of thousands, will increase funding to
three of AltaMed’s successful community-based programs:
Childhood Obesity
Type II diabetes, hypertension, and even heart disease are all linked to obesity.
Now, up to 60 percent of children in some of AltaMed’s service areas are obese.
Through Keep the Promise, AltaMed will roll out its proven obesity prevention and
intervention programs to more community sites.
Teen Rescue Programs
The consequences of not supporting our at-risk youth would be tragic. Keep the
Promise will enhance AltaMed’s ability to reduce crime and teen pregnancy in its
communities. Research has shown that, for every $1 invested in our at-risk
teens, the cost savings to taxpayers is $5.34 and the savings to society as
a whole is $21.84! AltaMed is committed to providing better opportunities
for our youth while saving the valuable resources of our state.
Uninsured Services Program
Without regular access to medical care, those living without health insurance are less inclined to seek
preventative care and often delay medical treatment for conditions which often become chronic. Last
year, AltaMed provided 45,000 doctor visits to the uninsured; Keep the Promise will help us provide
over 65,000 in 2012.
Keep the Promise was launched at the Ramona Gardens Housing Project Recreation Center. The event
was well attended by donors, executives from Pepsico and the National Council of La Raza and many
employees from AltaMed. The program featured an award ceremony for the graduates of Teen Rescue’s
Escalera program. Impassioned speeches from supporters and beneficiaries inspired all attendees to
give back and support at-risk members of their communities.
The launch marked the end of the campaign’s “Silent Phase” during which we successfully raised over
$1.6 million. We would like to thank our major donors: Ken Jackson, Jack Cline Jr., Richard
Gomez, Hector Barreto, Bank of America, Dr. Hector Castillo, Dr. Martin Serota and the
Health Resources and Services Administration of Orange County for their generous support.
Throughout this coming year, we are looking forward to spreading the word of the campaign
and how it is affecting lives and providing opportunities for many. We are diligently educating
our constituents, finding new donors and asking for support. We look forward to keeping you
updated on our progress.

become one of L.A.’s premier food and wine tasting events.
Pairing with a Purpose
Pairing gourmet cuisine with fine wines, “East LA Meets Napa” is a unique event that celebrates
the richness of fine cuisine and the success of Latino vintners from Napa Valley.
In addition to the 60 wineries and restaurants anticipated to participate this year, over 1,500

2012 Gala a Great Success
AltaMed Celebrates “A New Era” with 750 Guests
On Friday, March 9, 750 donors, corporate partners, local elected officials,
community members and AltaMed employees came together to raise
$500,000 for the organization.
Emceed by Shayla Rivera, named the "Funniest Latina Comedian" by "Latin
2015," funds raised from the Gala will help support AltaMed's Keep the
Promise campaign which focuses on uninsured services, teen rescue and
childhood obesity. In fact, .in attendance, were several members of our Teen
Rescue Programs; thank you to TELACU for sponsoring their tables.
A great highlight of the evening also included a special guest performance by Grammy-award winning artist, Pepe Aguilar.
“It's really great to know that there are so many folks with a social
consciousness. There will always be people in need, and it is great that some are actually doing something about it,” Pepe said of the event.
AltaMed also presented awards to two individuals who have made significant
changes in the way we serve our patients and deliver care. Gala honorees

7th Annual Golf Tournament
AltaMed hosted its 7th Annual Golf Tournament on Friday, April 13, 2012
at the SeaCliff Country Club in Huntington Beach, with attendance at
an all-time high. We would like to extend a special thank you to our Title
Sponsor, Travis Mathew, LLC, who generously provided apparel for the
golfers and contributed $10,000 toward the tournament’s total of nearly
$150,000. AltaMed was honored to work with Travis Brasher, Chief
Operating Officer and one of the founders of the event, to raise funds for
the Huntington Beach Community Clinic (HBCC).
Travis Brasher commented, “Travis Mathew was honored to be a part
of this year’s golf tournament and to be able to give back to the local
community by raising money for AltaMed’s Huntington Beach Clinic.”
HBCC offers the highest quality medical and dental care to local families in
Orange County and currently provides over 38,000 patient visits annually.

Duck-A-Thon
The Duck-A-Thon will be celebrating its 20th Anniversary May 18 – 20, 2012
at the Huntington Beach Pier. This year’s theme is “Heart to Heart,” and, as
Duck-A-Thon Founder, Donna Cross, explains it means from our heart to
your heart, which represents the gratitude we feel toward our volunteers and
donors for their generous support over the past 19 years.
A weekend long fundraiser in support of the Huntington Beach Community Clinic, the Duck-A-Thon kicks off with a
wine tasting that includes food from local restaurants, a silent auction, and door and raffle prizes.

The first Corporate Duck to return to shore will be the winner of the traveling Golden Duck Trophy. The winner will keep

In Memory of Marilyn MacAllister
Marilyn MacAllister was a member of the group that originated the Duck-A-Thon in 1992.
She continued to serve on the committee as Secretary, as Chair of the Corporate Duck
Committee, and in any other way asked of her throughout the past 20 years.
She was a valued and much-loved friend to us all and we on the committee, as well as
countless other members of our community, will miss her terribly.

Gold Seal of Approval for AltaMed
AltaMed is proud to announce it has earned The Joint Commission’s Gold Seal of Approval™ in
compliance with The Joint Commission’s standards for Primary Care Medical Home (PCMH). AltaMed is
the first organization in the nation to have earned this designation.
Patient centeredness has been imbedded into AltaMed since its founding.
Last year, AltaMed considered applying for the National Committee for Quality
Assurance (NCQA) PCMH accreditation; however, upon learning of The Joint
Commission’s new designation, we felt it was a better match. “Currently, the
Joint Commission’s approach is more oriented to large-scale enterprises like
AltaMed,” says Dr. Martin Serota, Chief Medical Officer.
“We had a meeting where we reviewed the list of Joint Commission standards
and we quickly realized we were already well on our way,” recalls Jerry
Coil, Vice President of Enterprise Services. Ivonne Magallanes, Director of
Accreditation and Special Projects, agrees. “AltaMed has always wanted to do
the right thing,” says Dr. Serota. “Long ago we adopted many of the principles
that are now bundled under the term PCMH.”

Still, even though AltaMed has
practiced a medical home approach,
PCMH is a relatively new term in the
health care industry. Under the PCMH
model, AltaMed primary care doctors
work closely with an interdisciplinary
team to support the patient and ensure
comprehensive and coordinated care, treatment and services. “For patients with
chronic conditions, this model of care is exceptionally valuable. Now they have
assistance in scheduling referral appointments, getting access to medication
and medical equipment or coordinating with other community resources that will
improve their overall health,” says Dr. Serota. “We’ve seen significant recovery in
many patients who have numerous conditions. This is the way physicians have
always wanted to care for patients, they just didn’t have the resources to do so.”

To earn this designation, AltaMed underwent a rigorous unannounced on-site survey the week of July 11, 2011. A
team of Joint Commission expert surveyors evaluated AltaMed staff for compliance with the standards of care that
continued on page 15

Cal-Learn Program
Cal-Learn is a critical element of AltaMed’s Teen Rescue programs. Through educational support and
mentorship, it gives qualified pregnant and/or parenting teens the opportunity to finish high school
with a diploma or earn an equivalent.
Cal-Learn’s case management is what makes this program so valuable. It provides a safety
net for teens between the ages of 12-17 and has resulted in reducing or preventing:
generational welfare dependence, repeat teen pregnancies, child abuse and neglect,
homelessness and crime. Because a single high school drop-out costs approximately
$260,000 in lost earnings, taxes, and productivity, Cal-Learn has saved the state millions
of dollars. In 2010 alone, Cal-Learn Program impacted over 40,000 teen moms and their
babies throughout California.
Despite this outstanding success, the State of California suspended Cal-Learn for oneyear effective July 1, 2011. During this time, Los Angeles County and First 5 Commission
have sustained a modified program targeting the most at-risk pregnant and parenting
youths. Outcomes from this modified program have actually exceeded expectations over
the previous year! While commendable, 5,697 teens were still left without support from
Cal-Learn.
AltaMed’s Government Relations department, its Youth Services department and LA
County providers are working with the state to restore Cal-Learn for the 2012-13 fiscal
year and are hopeful the program will not be eliminated. We won’t know the outcome
until the Governor releases his revised budget in May. In the meantime, we continue to
meet with legislators and stress the positive impact Cal-Learn has on parenting teens in
the State of California as we strive to build productive citizens and healthy families. d
Pharmacy Opens in Commerce
In early December, AltaMed held a ribbon-cutting ceremony for its second
dedicated pharmacy in the Commerce clinic. Serving both general practice
and our HIV/AIDS patients, the pharmacy offers everything our patients might
need. Staffed by Pharmacy Technicians Christian Ortiz, Rosa Arreguin, and
Pharmacist Vincent Ly, patients now have one more reason to call AltaMed
their medical home.
The opening coincided with World AIDS day. In commemoration, AltaMed
patients, donors and staff walked through the community distributing red
flowers in honor of those lost to the HIV/AIDS epidemic.
HIV/AIDS treatment has evolved greatly in the last 10 years. “People can live with
a normal life expectancy if they do well in treatment,” says Dr. Scott Kim, Medical Director, East LA Commerce.
“We've realized in a lot of ways that HIV is with us and it's here to stay,” he said, “[now we] try to educate people that
HIV is a disease that is preventable through modification of behavior and it's also a disease that once you have, you
can treat and live with.”
AltaMed has been serving HIV/AIDS patients for over 25 years and will continue to lead the charge in this battle.
Several local media stations were on hand to cover the event, including both Telemundo and Univision. d

Magic event, held on Sunday,
December 11 at the Island Hotel
at Fashion Island, Newport
Beach. Proceeds from the event

Obesity Program as well as atrisk youth at the Orangewood
Children’s Foundation and Big
Brothers Big Sisters Orange

local pianist entertaining guests as they
enjoyed a fabulous menu, mingled with
one another, and bid on their favorite
auction items.
The highlight of the event was the live
and silent auctions of 8 foot and 4 foot
Christmas trees elaborately decorated
by artists and designers. In addition, live auction items included a Hawaii
trip with airfare, a stay at an exquisite Manzanillo home, and a bungalow
golf & spa experience from the resort at Pelican Hill. Golf packages, wine
baskets, Christmas items, dining experiences and sports memorabilia were
just some of the other auction items that were available for bidding
at this event.
We raised over $100,000 for last year’s Christmas
Tree Magic and would like to thank all of you who
helped make this event such a success.

At AltaMed, Patients Come First
Patient-centered care means assessing each individual’s needs and getting them the
customized treatment they deserve. It’s not new for our staff, who are accustomed to working
with a range of backgrounds, illnesses, and disabilities. It’s a key part of our Patients Come
First value. Recently, one clinical team exemplified this process by caring for a Chinesespeaking patient who was also extremely hard of hearing.
“We had some unique challenges with this patient because he didn’t speak English,
and he didn’t know sign language,” says Ivonne Leonel de Cervantes, Cultural and
Linguistic Specialist. “What happened with this patient is an exceptional example of what
we can do when we come together and work as a team.”
When the patient first entered the Commerce site, the staff largely relied on body language
to determine what he needed. Working with Dr. Jarantilla, the team instructed the patient
to return for a follow-up visit when they could have Interpreter Services in place. Cristina
Rocha, Front Office Supervisor at Commerce, contacted our Cultural and Linguistics
Department, who then contacted GLAD (Greater Los Angeles Agency for the Deaf) and
Lifesigns, a vendor we use for sign language services. “Because these interpreters are
professionals, and work as a team on a regular basis, we avoid getting a diluted message,”
says Ivonne.
In the meantime, Dr. Puertas was sharing this challenge with David Lin. David, a member
of our HR team, offered to help by providing a technology that would allow him to speak
into a microphone in English and the words would be translated into Chinese for the patient
to read. “Here you have someone who is not clinical staff, not a doctor, and was willing to
help,” Ivonne says. To plan their approach, Clinic Administrator Annette Dreher, David Lin,
Cristina Rocha and Ivonne got on a conference call to work out the details. David arrived at the
clinic one hour in advance of the appointment to set up and test his computer. Once the patient arrived, David
spent time with him getting preliminary information and working on their communication.
“The patient was very happy and appreciative that we went the extra mile to help him,” Ivonne says. “I’m so
proud of our teams. We helped not just a patient, but a fellow human being who really needed medical care;
and that’s what quality care without exception is all about.” d

team and treat the whole patient—it’s the core of our organization and an accreditation we’re extremely proud
to have,” says Dr. Serota. The PCMH designation is a new Joint Commission option for accredited ambulatory
care organizations, enacted July 1, 2011. AltaMed is the first organization to receive this Joint Commission
