2012 Annual Report …to eliminate disparities in health care access and outcomes by providing superior quality health and human services through an integrated world-class delivery system for Latino, multi-ethnic and underserved communities in Southern California. This year’s key accomplishments: * AltaMed moves into our new corporate home * In spite of the program’s elimination, AltaMed successfully converts three ADHC centers to PACE, saving services for hundreds of seniors and jobs for many employees * AltaMed advocates for another extension of the Cal-Learn program for pregnant and parenting teens, a program that would serve 3,200 teens statewide * After receiving recognition as a patient centered medical home from The Joint Commission, AltaMed begins the National Committee for Quality Assurance designation process achieving Tier III for AltaMed Medical and Dental Group – El Monte * AltaMed receives major grants from the Department of Labor, Centers for Disease Control, Blue Shield, California Department of Public Health and First 5 LA among others This year, it was my great We also know that millions of individuals will still not pleasure to celebrate my qualify for health care coverage through this expansion 35 anniversary with the and remain without access to care. It is a dedicated effort to provide resources to programs that combat childhood obesity, support teens in at-risk communities, and provide quality care to the many individuals in our community who remain uninsured. AltaMed operates in communities that are designated In the next three years, we aim to raise a total of to be medically underserved. Cástulo de la Rocha, J.D. President & CEO We promise to We promise to Uninsured Services Salvador has a personality that lights up a room. Looking at him, you’d never know that in the last few years, he’s faced such great challenges. Sal was diagnosed with diabetes years ago but managing a work schedule and caring for his family didn’t afford him the time to keep his condition under control. As the years passed, the disease worsened, he was He’s worked closely with a case manager who missing work and unable to do his job. Ultimately, checks in about his diet and blood sugar levels. His son insisted Sal visit AltaMed’s El Monte location, a clinic he was familiar with since his father-in-law also received services there. well. “In the year we’ve worked together, Sal has gone from high blood sugar levels (A1C levels of a high eight) to a manageable range (a low seven),” she says. This change reflects a great improvement in quality of life and care, AltaMed’s goal is that diabetes patients are no higher than a seven. The For the last three years, Sal has received prescription improvement helps prevent complications of diabetes medication and attended health education classes like kidney failure, nerve damage and eye disease. which have enabled him to get his diabetes much more under control. AltaMed has covered a large percentage of Sal’s medical expenses as part of our commitment to serving the underserved in our community. Each visit averages roughly $116. This year, thanks to the Healthy Way LA expansion, a county step toward health care reform implementation, Sal has some insurance coverage. It’s a good thing too, like many diabetics, Sal garnered a serious infection when trimming his toenail which has required foot surgery, rehabilitative therapy and much more. Ongoing treatment and preventative care are essential to the long term well-being of our patients, but they can be expensive. Left unchecked, Sal may not have made it to the day he received coverage through Healthy Way LA. Even as we see health care reform enacted, we estimate millions of Southern Californians will remain uninsured. We need your support today, please donate to AltaMed’s Keep the Promise campaign and help us ensure Quality Care Without Exception. Teen Rescue Program Nohely had always been a good student. But at just 15 years of age, she found out she was pregnant and assumed that her plans to graduate and enroll in college would need to wait. It wasn’t long before she found out she would have not just one, but two sons to care for in the months to come. “I felt so alone,” she recalls. “I didn’t know if I could finish school. I didn’t know where to find help.” While still pregnant, Nohely learned about one of the teen rescue programs AltaMed offers for Upon her graduation, Nohely wants to help her community and other teens whenever she can, which is why she’s continuing her education and looking toward a career in nursing. pregnant and parenting teens. After talking with AltaMed providers, Mirtalin, Leandro’s mom, knew something had to be done. that he’s within a healthy weight range. And you can tell by the way his eyes light up when he looks at his younger sister that she’s sure to benefit from his example too.
