This is nowhere more evident than in the worldwide economic and medical challenges posed by the metabolic syndrome of obesity and diabetes. The statistics are truly staggering: • • • • • • • 58 million Americans are overweight 40 million Americans are obese 9 million children and teens are obese 26 million Americans have Type II diabetes (8% of the population) Direct obesity-related healthcare costs exceed $70 billion Lost or restricted work resulting from obesity-related illness is also $70 billion Obesity is the #2 cause of preventable death in America Based on these data, it is not only logical for Cleveland Clinic to convene a conference of thought leaders on this topic; it is a professional and ethical responsibility. Cleveland Clinic Innovations (CCI) holds the annual Medical Innovation Summit to promote discourse among key stakeholders and decision makers from all sectors of the healthcare ecosystem. This year’s conference has clearly succeeded. It unites all specialists within one unique, fully integrated model of care aimed at optimizing patient experience. Departments include Gastroenterology and Hepatology, Colorectal Surgery, General Surgery (including hepato-pancreato-biliary surgery, transplant surgery, and breast surgery), and Human Nutrition. The DDI offers the most advanced, safest, and proven treatments performed in the most effective and patientfriendly way, including shorter waits for appointments and more seamless interaction with all specialists. In addition, the institute model enhances opportunities for cutting-edge research and physician education. Throughout the years, DDI physicians have pioneered many new technologies and procedures for treating digestive disorders, such as continent ileostomy/stapled pouch procedures; advancement flaps/sleeves for perianal fistulas and Crohn’s disease; innovative and advanced endoscopic, laparoscopic, and robotic procedures; multi-organ and living donor transplants; multimodality approaches to abdominal malignancies; and the most current medical therapies for a wide variety of gastrointestinal disorders. This rich history of innovation continues today under the direction of DDI Chairman John Fung, MD, PhD, whether through the development of new surgical techniques or participation in clinical trials and operating outcomes research databases or registries. Dr. Philip Schauer: Busy Surgeon, Active Obesity Researcher Obesity has become a public health crisis in the United States. Current research suggests that in this country alone, about 300,000 deaths per year can be linked directly to morbid obesity, which is defined as weighing at least 100 pounds over ideal body weight. Philip Schauer, MD, international obesity expert and the Director of the Bariatric and Metabolic Institute (BMI) within the Digestive Disease Institute, has first-hand experience gained at the frontlines of this ongoing battle that has become a national health and economic crisis. Dr. Schauer reduces the stomach size of patients with a surgical procedure—there are several types—that cures them of their obesity but oftentimes eliminates type 2 diabetes and as many as 20 or more other medical ailments. In 2013, the Bariatric and Metabolic Institute (BMI) was fully integrated within the DDI. Under the leadership of Philip Schauer, MD, five surgeons, four medical doctors, four psychologists, four dieticians, and more than two dozen support staff assist the 700 patients who annually undergo bariatric surgery at Cleveland Clinic. Of these major health problems, type 2 diabetes is one of the most debilitating. Tragically, obesity and diabetes go hand in hand in the vast majority of cases. Bariatric surgeries are on pace to increase by 15% in 2013, and this specialized service has been expanded to two other hospitals in Cleveland as well as Cleveland Clinic Florida, Cleveland Clinic Abu Dhabi, and an allied hospital, Doctors Hospital at Renaissance in Edinburgh Texas, which is the largest physician-owned facility in the United States. “The good news is that in 2012, I proved with my ground-breaking STAMPEDE (Surgical Treatment and Medications Potentially Eradicate Diabetes Efficiently) study that weight-loss surgery can cure type 2 diabetes,” says Dr. Schauer. The Endocrinology & Metabolism Institute Cleveland Clinic’s Endocrinology & Metabolism Institute (EMI) is committed to providing the highest-quality healthcare to patients with diabetes, endocrine, and metabolism disorders, while continuing its nonstop efforts exploring innovative ways to improve their care and best treat these disorders. The EMI formally came together in 2008 and under the leadership of James Young, MD, it has evolved into a paradigm of what Cleveland Clinic was hoping to accomplish with its complete reorganization into 16 distinct clinical “institutes.” The EMI continues to raise the bar. With patient-centered care, cutting-edge treatments, innovative research, scores of clinical trials, and extraordinary state-of-the-art medical technologies, Cleveland Clinic remains as one of the top diabetes and endocrinology centers in the nation. Management of disease is an important focus of EMI. The Diabetes Center and main campus diabetes educators have spearheaded the standardization of diabetes education across Cleveland Clinic. Shared medical appointments are offered for diabetes management with specific providers. Through the Diabetes Center, bariatric surgeon consults are also offered to help patients sort out their options for weight-loss surgery for control of type 2 diabetes. Surgical Treatment Of Obesity And Diabetes: Weighing The Facts Dr. Schauer and his colleagues at Cleveland Clinic reported that after a year 37% to 42% of patients in the STAMPEDE trial who had the bariatric surgery were able to keep their blood glucose levels below the threshold doctors use to diagnose diabetes (HbA1c levels of 7% or less) compared with 12% of those who depended only on the medications. “These one-year results were simply stunning,” says Dr. Schauer, “especially considering the fact that these surgical patients had suffered from uncontrolled type 2 diabetes for eight years or more in spite of regular doctor visits. Losing drastic amounts of weight and curing this disease with weightloss surgery—and eliminating the attendant illnesses that accompany obesity—have nothing to do with vanity. Other theories include the reduction in chronic inflammation in the body caused by the drastic weight loss, and the significant impact that surgery has on taste preferences.” Dr. Schauer has now successfully treated more than 5,000 obese patients with weight-loss surgery, and he knows that it can be transformative, on both a medical and personal level. “As a busy surgeon with over 25 years of experience, I have had extraordinary clinical success with thousands of people who went from obese to significantly lighter, healthier, and happier. I have made the study and refinement of the surgical approach to treating obesity my life’s work. This pursuit has taken me all over the world as a teacher, lecturer, researcher, and surgeon with the dual goal of not only helping some of society’s most vulnerable and in-need people, but also desperately trying to coordinate a medical counterattack against the obesity epidemic that has overtaken our country and is now poised to engulf the rest of the world.” OBSTETRIC AND GYNECOLOGIC Fetal abnormalities and infant mortality Gestational diabetes Infertility Miscarriage GENITOURINARY Stress urinary incontinence Urinary tract infections Even after Dr. Schauer’s groundbreaking STAMPEDE study, no one is sure how or why weight-loss surgery cures type 2 diabetes, although there are now five distinct theories. “Some experts believe that weight loss reduces fat cell mass and that the fat cells in belly fat—visceral fat—are metabolically active and secrete hormones that may have effects on blood sugar,” says Dr. Schauer. “By reducing the number of cells, improvements and reduced insulin resistance may then be triggered. Others believe that it’s the ‘gut hormone theory’ that’s responsible for diabetes reversal. The thinking is that when food is surgically rerouted by the surgery, other hormones are activated because the duodenum (the first section of the small intestine) has been bypassed. These may be hormones we have not discovered yet and their activation along with suppression of other hormones could be what triggers the results.
