Our Mission The Van Wert County Hospital Association serves Van Wert County, Ohio and adjacent communities as a financially independent nonprofit community health system. Van Wert County Hospital Association recognizes the collaborative efforts and contributions of individuals and organizations that strive to meet the health care needs of the communities we serve. The Association will meet community health needs by continually endeavoring to improve the quality of patient care we provide within our resource capabilities. Programs and services sponsored by the Association will serve as an access point to health care within our community. We accept responsibility for providing community leadership. The Association will remain abreast of national, regional and state trends, enabling us to continually assess and meet the needs of the communities we serve. We will be competitively priced while providing an environment of excellence in which all health care professionals strive to identify, provide and maintain an appropriate level of care in a safe and secure environment. 2 The Van Wert County Hospital Association makes fiscally responsible use of resources in carrying out our mission. The Association assures that health care services provided are available to all members of the communities we serve regardless of race, creed, sex, national origin or ability to pay, while at the same time respecting both the rights and responsibilities of our patients. 3 A Message from the President & Chief Executive Officer Mark Minick Thanks to the dedication and teamwork demonstrated by our physicians, employees, board members and volunteers, 2012 is marked with a number of celebrated hospital contributions. This past year has again been one of many accomplishments, furthering our continuum of exceptional care and proactive commitment to improvement. As always, we remain committed to patient satisfaction. We feel satisfied patients are healthier patients. For this reason, patient satisfaction remains paramount in our approach for today and our planning for tomorrow. We continue to evaluate our patient care to ensure our patients are happy and healthy. We are proud to report, to date, our patient satisfaction ranks above the eightieth percentile nationally, largely due to the dedication of our staff and physicians, alike. Our medical imaging department added a new state-of-the-art, 16 slice, CT scanner, equipped with dose reduction software, which provides an increased capability for higher quality, faster and more accurate imaging, while ensuring the least amount of exposure to radiation. The CT scanner has also been relocated to the main medical imaging hallway, to consolidate testing, and the waiting room has been enlarged. Into 2013, continue to watch our growth and progress as we relocate the main hospital registration to improve your hospital experience, as well. We are excited to announce several advances in technology were also made. The hospital expanded its use of its electronic medical record system to provide an electronic backup for important patient health related information and to provide a centralized point of access for providers coordinating patient care. In addition, Van Wert Medical Services began utilizing NextGen management system, connecting them to CliniSync’s statewide health information exchange, to quickly and securely assist in the diagnosis and treatment of our patients. This exchange promises to enhance communication among caregivers ultimately providing safer care for our patients. 4 As we look ahead, trust that we continue to progress and improve in accordance with our mission and vision for you, our community. This includes expanding the range and quality of our clinical services, making investments in technology, evolving our culture, and adding to and improving our facilities. Thank you for allowing us the opportunity to be your preferred healthcare network. 5 A Message from the Board of Trustee’s Chairman Jon Bagley The Van Wert County Hospital’s Board of Trustees exists to provide ongoing strategic governance and oversight while ensuring the long term viability of Van Wert County Hospital. To achieve this, this past year, the board of trustees played a vital role in helping to make changes to our hospital structure to positively improve each patient’s experience. A main achievement of 2012 has been an increased focus on overall patient safety. As a concentration of our efforts, we have launched a new initiative to support our improvement goals and maintain our commitment to our hospital’s mission. Our initiative is Why Van Wert County Hospital Should Fly, based on the book Why Hospitals Should Fly, written by John Nance. Nance, a pioneer for the aviation industry’s safety revolution, outlines professional communication, teamwork and leadership as essential components of the aviation industry and identifies what, within healthcare, should resemble aviation to mirror that model. Concluding much like aviation, the goal aimed at creating a culture of safety is and must remain a collaborative initiative and at the forefront of patient quality standards. Based on his model, our hospital has identified a target goal to bring our initiative to fruition: developing a collaborative partnership between healthcare providers, patients, and families to support and prioritize patient preferences, needs, and values, interdepartmentally and across all medical settings. To accomplish this, you may notice that our hospital has begun to implement patient and family centered rounding. All members of a patient’s health care team, including themselves and family members, if they wish, will be active participants in their health care decisions. Our model demonstrates, we encourage and value input from all participants, to make the best decisions possible, and to create an environment where patient safety and satisfaction are always our priority. 6 We continue to strive toward evaluating our present healthcare system and continually making improvements to the quality of care we provide so you continue to receive the highest and most innovative level of care, right here, in your own community. As president of the Medical Staff, I am pleased to announce the addition of three new doctors, joining our healthcare services, and pledging their commitment to providing you with exceptional care. We are privileged to announce that Van Wert Medical Services welcomes Charles Hoehn, D.P.M., to our growing list of practitioners. Dr. Hoehn joins us as a podiatric surgeon, fielded in foot and ankle surgery, and provides optimum healthcare services to our patients experiencing disorders of the foot, ankle, and lower leg. His expertise and personality are highly regarded, and we are honored to say he is a part of our healthcare community. Carrie Huber, M.D. has joined our team of specialists and provides outstanding care for the wellness of women. Dr. Huber provides obstetrical and gynecological care for adolescents and women at the Van Wert Health Center, and she shares with us her enthusiasm to genuinely know her patients and make a difference in women’s healthcare. Our team of physicians are also excited to welcome Ziad Khatib, M.D. who joins us as a physician of internal medicine, specializing in comprehensive medical care for adults. Dr. Khatib excels at providing patient-centered care both as a practicing physician at the Van Wert Health Center location, and through the management of care for patients who are acutely residing within your Van Wert County Hospital. 10 With great pleasure, our medical staff looks forward to our future together and to achieving more renowned accomplishments in the upcoming years. These dedicated individuals contribute financially to the hospital through a variety of service projects. Over the years, the Twig Association has donated over one million dollars to the hospital. The Twig Association has been instrumental in providing our community with state-of-the art equipment. There are four Twig groups, each responsible for different projects throughout the community. Twig I and Twig II operate the Hospital Thrift Shop located at 131 East Central Avenue in downtown Van Wert. Twig III members manage the Hospital Gift Shop, located inside the main entrance of the hospital, as well as the yearly Tree of Lights memorial fundraiser. Twig IV contributes to Van Wert County Hospital by hosting a monthly bake sale, book and jewelry fair, and yearly poinsettia sale in November. We are fortunate to have ladies who make various items and to have received several in-kind donations, which were auctioned by a local auctioneer at no cost, to assist us in generating funds. We are grateful for all of our donors as well as 38 active volunteers and 23 emeritus members who make our Twig organization possible. With their dedication and community support combined, we continue to share with Twig II a substantial net profit that enables us to give between $7,000 and $10,000 each year to the Hospital for equipment. LaDonna Allenbaugh Twig II Twig II shares responsibilities of managing the Hospital Thrift Shop with Twig I. The proceeds of the Thrift Shop are used to buy equipment at the Van Wert County Hospital. The Shop is open five days a week: 9:00 am – 5:00 pm, closed on Wednesdays. The community is vital to the success of the Thrift Shop – both as “shoppers” and “donors”. Clothing, jewelry, shoes, purses, baby items, bedding, books, small household goods as well as small appliances are examples of greatly appreciated donations. The Twig II members are ladies who freely give of their time, talents and energy ensuring the profitability of the Thrift Shop. Their efforts are rewarded with self-gratification knowing each has played a role in achieving the goal of the Twig Association. We thank the public for their continued support. Carolyn Garwood Twig III Twig III had another successful year operating the Hospital Gift Shop. The gift shop is open 24 hours a day, 7 days a week to serve the needs of patients, their families and friends, as well as hospital staff. The gift shop is stocked with candy, cards, balloons, stuffed animals, toiletry items, Ohio State and Michigan items and various gift items. In November, Twig III celebrated the holiday season with the 19th Annual Tree of Lights Ceremony to honor and remember family and friends. Since inception, this annual event has raised more than $70,000 in contributions for the hospital. The members of Twig III would like to thank the community and hospital staff for their continued support of these projects. Lisa Dunn Twig IV Twig IV is deeply indebted to the Van Wert Hospital employees. Our success is due largely to their participation in purchases at our events. We try to make our offerings adequately diverse by adding a variety of vendors at our Artisan Sales throughout the year. We entice visits to our display of RADA Cutlery with sweet treats at our Sweet Shoppe on the second Thursday of every month from 11:00 a.m. to 2:00 p.m. (The public is also welcome to “stop and shop” at all events.) In November and December, we sell poinsettias to our local merchants and residents. This year we sold roughly 500 plants— thank you, Van Wert! We welcome new members to join our Twig IV group which currently consists of 13 active members. About the only qualification is that you enjoy baking! Shirley Straley 15 Lady Board of Managers “The Sewing Ladies” When the hospital was first built in 1911, the Lady Board of Managers supervised the hospital operations. Over the years, the role of the Lady Board of Managers has evolved. Today, this group supports the hospital and community by volunteering their time and talents in both the mending and making of new articles for the hospital. Tuesday morning is Sewing Day. Ladies from the city, and the twelve townships, gather to assist the hospital. The workforce can vary from 3-6 ladies. The ladies are always eager to work on any projects. Annually, the Sewing ladies volunteer over 928 hours, making over 3,061 new and 200 mended items. As 2012 ends, we find ourselves in yet another transition. Edie Laukhuf, who served as executive director for the Van Wert Chapter of the American Red Cross, retired as of January 31, 2013. The Red Cross does not plan to hire a new director immediately. In the interim, the Lima office will assist our chapter. The hospital volunteer program is unique to the Van Wert chapter; not many other Red Cross chapters provide this service. Red Cross Hospital Volunteers help to provide the best hospital care possible to members of our community. Many times, volunteers can help relieve a patient’s anxiety, offer comfort to family members or just make visitors feel welcome. Red Cross Volunteers are an important part of the excellent health care provided at Van Wert County Hospital. The volunteers continue to do a great job! In 2012, six new volunteers joined our program, bringing the roster to a total of 47 volunteers. Through the remarkable contributions of all of our Red Cross volunteers, a total of 6,721 hours were worked in 2012. I leave you with this thought from John F. In keeping with our mission, we continuously support our community by reinvesting time, efforts and resources into improving community health. 2012 Community Benefit Charity Care $265,056 Van Wert County Hospital provides quality services with a special concern for the health needs of the poor and disenfranchised. Each year the hospital provides free or discounted health care services to patients with no ability to pay. HCAP $182,477 Hospital Care Assurance Program or HCAP is an Ohio program intended to ensure that uninsured individuals have access to hospital care. 20 Unpaid Costs of Medicaid $2,964,544 Medicaid reimbursements continue to lag behind medical inflation, placing an increasing burden on hospitals because the reimbursement does not cover the cost of providing services to Medicaid recipients. Community Health Outreach Programs $439,654 In an effort to increase the health and wellness of the community, Van Wert County Hospital is proud to host a variety of outreach programs including health fairs, health education workshops, donations, sponsorship and other wellness programs.
