Let’s make a difference Lemont!
To kick off Breast Cancer Awareness Month, the
Lemont Park District is teaming up residents to help
with the fight against breast cancer! On October 1, 2016,
the district will be hosting a one-of-a-kind “pink”
challenge to raise funds to combat this terrible disease.
All profits from this event will be donated to the Hope
& Friendship Pink Fund designated to help those in
Lemont living with breast cancer!
This 2-hour event will center around 10 teams completing 10 challenges in a circuit
environment in The CORE gymnasium. Whether it’s basketball, zumba, hockey or
pickleball - each area will focus on fitness and FUN! In addition, prizes will be given
for the best PINK dressed team and the team with the highest score of completion.
Don’t miss the after party to be held at Gelsosomo’s Pizzeria & Pub here in Lemont!
All participants will receive a dry-fit pink shirt and goodie bag!

A Message from the President...
Dear Residents,
Another summer full of memorable events has come
and gone. Our staff and community partners
continue to pleasantly exceed our resident’s
expectations. This is a perfect example of what
happens when a community comes together and
works together. Magical things start to happen. Let’s
take a moment to revisit those magical events that
took place over the summer.
The most popular event in our community remains to
be the Independence Day Extravaganza and
Freedom Days! This week-long series of motivational events include the Freedom 5K run, Sky High
Freedom Kite Fly, the Stars and Stripes Tribute to
our Veterans, Starlight Movie Night, the All-American
Swim all leading up to the Independence Day
Extravaganza held on July 3rd. Because of the spirt
of cooperation and partnership that exists between
the Village of Lemont, Lemont Township and the
Lemont Park District, we are able to bring this
phenomenal event to our residents. Additional
partners and sponsors that made this celebration
possible are the Lemont Park Foundation,
Gelsosomo’s Pizzeria and Pub, Zlan Partners, Citgo
Refinery, Kids Plus Pediatric Dentistry, Lemont Bank
and Trust, Presence Medical Group, Lemont Police
Department, Lemont Fire Protection District, Lemont
Emergency Management and Will County Mutual
Aid. Most importantly, let us not forget the motivated,
dedicated and passionate commitment displayed by
the Lemont Park District staff and members of our
GIVE program in preparation for this event. They
have put in countless hours throughout the week
and weekend to make this all possible for our
community to enjoy. They are truly Makers of
Memories!
As part of our ongoing celebration for our 50th
anniversary we enhanced several programs. Our
movies in the park have become such a hit, it only
made sense to schedule another night out in the
park featuring a family friendly flick! Our joint venture
with the Village of Lemont for the Sunset Soirees
concert series in downtown Lemont continue to grow
in size and popularity. Adding another concert in the
peaceful summer night air provides one the
opportunity and inspiration to truly enjoy what life is
all about. We just couldn’t pass up a unique
opportunity to create an Enhanced Kids Zone with
numerous enjoyable rides for the kids at the Independence Day Extravaganza. The smiles on their
faces told the story. The year isn’t over yet. Watch
for other opportunities to participate in this
celebration.
We continue to proceed forward with our park
replacement program. Northview Park, which is one
of our regional parks, has taken shape quite nicely.
We are looking for a completion date this Fall.
Rolling Meadows and Kensington parks are in
various stages of construction. We anticipate that
both parks will be completed this Fall as well
pending weather conditions. We will continue to
keep our district residents informed on the
progress of all construction along with information
on their grand openings.
It also gives us great pleasure to announce the
appointment of our new Board Commissioner,
Leland Wagner, which took place at the April
Board meeting. A highly motivated married father
of two young children, Leland brings with him a
solid background in Educational Leadership and
Marketing. His energy is not only admirable, but
wonderfully contagious. We are fortunate to have
Leland as part of our team.
As you browse through this edition of our Memory
Maker, you’ll definitely find some great new
programs and some favorite annual activities!
What comes to mind when Lemont thinks of Fall?
How about the ever-so-popular Lemontster Days!
This community-wide celebration will start off, on
October 15, 2016, with Lemont Park District’s Fall
Fest. This fun, family event will feature festive
entertainment for all ages. Also, be sure to check
out the Family Fun Night in November and
Breakfast with Santa and the Frigid 5K in
December. It will also be our honor to host our
men and women who served their country on
Veterans Day with our annual luncheon. After all,
if it wasn’t for their commitment and contribution
to our country, we wouldn’t be who we are today.
We have also implemented several new
programs for all ages. From the Kindergarten
Enrichment program, Archery, Pickleball and
Lacrosse – there is surely something for
everyone! Teens will have a blast at the all-teen
dance and our adults can choose from a variety of
programs such Crockpot Cooking and Beeswax
Candle Making. Our mature residents also have
many choices this season such as Oktoberfest at
Starved Rock and Glass Pumpkin Festival just to
name a few!

Fitness Facility and Free Weights
The CORE is home to a state-of-the-art fitness facility which houses
over 100 pieces of the most innovative fitness equipment and free
weights available, including towel service, iPod docking stations and
video screens on most cardio machines. The cardio fitness equipment
includes cutting edge “Preva Technology” designed to help you set
your goals and track success. The CORE also has Personal
Training available. Adjacent to the equipment and free-weight rooms
is an open area for stretching, warming up and cooling down. Here
Personal Trainers can have one-on-one meeting sessions with their
clients to discuss individual progress and goals. Providing the most
up-to-date equipment and impeccable service, is at the center of The
CORE. All guests ages 11-17 who sign up for a CORE Membership
must provide proof of age and attend an orientation with their parents
before they will be allowed to work out in the fitness center.

Basketball, volleyball, even baseball with
indoor batting cages - you name it, this Gym
has it! The 3-court Gymnasium at The CORE
spans over 21,000 square feet maximizing
Court Time for all. “Open Gym” as well as
an array of other events take place here every
week. Encompassing the entire Gym is a
remarkable elevated indoor track. Extending
a 10th of a mile around, it allows ample
room for all runners and walkers year-round.
Aquatics
Feel like a swim? You can jump
right in the 6-lane swimming pool
inside The CORE. This modern
indoor pool, in addition to the
outdoor Centennial Pool, is ready
to meet Lemont’s aquatic needs
year round. This aquatic facility
measuring nearly 8,000 square
feet plays host to lap swim, water
aerobics, swim meets, open swim
and a variety of other Park District
aquatic events! Come join us, the
water’s perfect!

Moms and Dads can get their
workouts in while kids can get their
fun-time fill! Childcare hours are
Monday-Friday 7:30 am-1:30 pm
and 5:00 pm-8:30 pm (8 pm on
Fridays during the summer), and
Saturdays 8:00 am-12:00 pm. The
cost is $2.75 per hour per child for
residents and $3.75 per hour per
child for non-residents. We also offer

Frequently Asked Quesons
What are the hours at The CORE? The hours for The CORE are
as follows:
Monday – Friday 5 a.m. – 10 p.m.
Saturday and Sunday 7 a.m. – 8 p.m.
Can I pay for my entire year’s membership at once? Yes, CORE
Members can pay for a year’s membership upfront and save.
However, we offer a month-to-month automatic payment
option, as there is not any annual obligation.
Check our website, www.lemontparkdistrict.org for seasonal
and holiday hours.
What are “Group X” classes? Group X is an abbreviation for
Group Exercise classes. Group X classes are included as part
of the CORE-Fit Memberships and can be attended by people
that purchase a Pre-Paid Punch Card.
Can I just purchase Membership to the Fitness Center and not
the Swimming Pool? The Fitness Membership includes access
to the Indoor Swimming pool. The pricing for Membership is
based upon admission into the Fitness Center and is competitive
with area Fitness Clubs that do not have swimming pools.
Including admittance into the pool is simply a bonus that is
being offered to all members.
Do Seniors get a membership discount? Yes, Seniors ages 60 69 receive approximately 10% off membership. Seniors over
70 years of age receive an additional discounted price.
Is there a walking track inside The CORE and if so, what is the
fee for using it? Yes, there is a track inside that equals 1/10th of
a mile in distance. Entry is FREE for all Lemont Residents.
Non-Residents pay a small daily fee. Must have a Resident or
Non-Resident ID Card.
Why do I need a Resident or Non-Resident ID Card? Any guest
who is not a member but uses The CORE facility, needs to
have a resident or non-resident ID card to present each time
you visit The CORE for safety purposes. There is no cost for
the ID card.
I am a Senior Citizen and I usually go away during the Winter
months. Can I freeze my Membership during this time? Yes, we
have a “Snow Birds” policy for Seniors who leave Lemont
during the Winter months for extended periods of time. Please
inquire at the Service Desk about the “Snow Birds” Policy for
complete details.
What do I get for my one-time Enrollment Fee? Each member
will receive a one-on-one Physical Assessment, Facility
Orientation, One half-hour Personal Training Session and a
T-shirt!
What kind of equipment is in the Fitness Center? The Fitness
Facility contains state-of-the-art equipment—some of the most
up-to-date pieces available! Complete with iPod docking
stations and individual video screens, these machines make
working out fun and entertaining!

How many Group X classes are offered at The CORE? There are
a variety of over 60 classes offered weekly for most of the
year. Also, Group X Specialty classes are offered for an
additional fee.
Are there Fitness Classes that are geared toward Seniors? Yes,
there are Specialty Fitness Classes that are specifically
designed to be “Senior friendly.”
Is there different prices for Non-Residents of Lemont? Yes,
Non-Residents pay more than Lemont Residents. This
substantial, yet reasonable amount helps offset costs for
Lemont Residents but is still inviting for those not from
Lemont to become members.
How old do you have to be to join The CORE? Eleven is the
minimum age requirement for membership. All 11-13 year
olds must be in direct supervision of a parent or guardian, who
is a CORE member, from the same household at all times in
the fitness facility and must provide proof of age.
Is the Pool at The CORE heated? Yes, the pool is heated.
What is the pricing for children who would like to visit the pool
and the gym? There are “Open Gym” and “Open Swim” Daily
Passes that can be purchased for children under 18 years-old.
There are also Annual rates.
Is there a Daily Rate for the Facility? Yes. There is a daily rate in
place for those who would like to just come every so often.
What if I do not want a membership but I still want to participate
in Group X classes? A Pre-Paid Punch Card is available that is
redeemable for 12 or 24 classes on a walk-in basis. Also, for
those who would not like to commit to the punch card or the
6-week class, there is a daily class fee for Group X Classes.
Is there a Corporate Membership Rate? The Lemont Park
District has partnered with area businesses to develop
customized Corporate Membership agreements. For more
information, please contact The Lemont Park District.
Lemont Park District’s Preschool Academy
Terrific Two’s
Preschool Plus
Terrific Two’s is a great way to introduce your child to a preschool
setting. One of the most important aspects of early development is
becoming comfortable with separation, encouraging independence
and fostering communication skills. Classes will follow a monthly
theme and include circle time with songs, a story and socialization
and playtime. Fine and gross motor skills will be enhanced through
games and crafts. Class will meet Tuesday and Friday mornings for
one hour and fifteen minutes.
Preschool Plus is an exciting opportunity to expand your child’s
educational, social & developmental horizons by adding one or two
days to our current programs. Children will experience an extended
curriculum including art, music, science and computers as well as
indoor and outdoor physical activities. Students that enroll in our

Young 3’s
Our Young 3’s classes were developed for children who will be three
years old between September 1st and December 31st. The “Young 3’s
class meet Tuesday and Thursday mornings for one and a half hours.
Three Year-Olds
Our Three Year-Old classes are designed for children who will be
three years old by September 1st. The three year-olds meet twice a
week for two hours.
Four Year-Olds
Our Four Year-Old class is for children who are four years old as of
September 1st. The 4 year old class meet three times a week for two
and half hours each day.
Finally Fives
Our Finally Fives class was developed for children who will turn
five years-old between September 1st and December 31st. Finally
Fives meet Monday, Wednesday and Friday for two and half hours
each day.
Preschool Enrichment Program
The “Preschool Enrichment Program” is available to students
currently enrolled in the Lemont Park District’s 4 Year Old and
Finally Fives preschool program. Students will attend their regular
preschool program during the morning - Monday, Wednesday and
Friday from 9 a.m. to 11:30 a.m. These students are able to extend
their preschool experience until 2:30 p.m. Students who enroll in the
Enrichment Program will experience a balance of Illinois CORE
standards of education in literacy, math and science along with
drama, music, science and creative movement. Students enrolled in
the Preschool Enrichment program will have a 30-minute supervised
lunch between their a.m. and p.m. programs.

collection of fascinating tricks from the "Magic Team of Gary
Kantor"! Amaze family and friends with tricks that involve

may appear difficult, you'll discover that they are quick to learn
and easy to perform. Additionally, you can sign up for this

Youth Specialty Classes
Beginning Voice Class (Ages 6 - 13)
If your child is a fan of shows like "The Voice" or if they haven't
stopped singing every song from the movie "Frozen" (word-forword and with the same exact movements of the movie
characters) then this class is the perfect choice for you!
Children will learn ways to improve posture, breathing and
tone, singing songs they already know and learning new
pieces from around the world. Watch your child's confidence
soar in this fun and non-threatening class with friends. Session

Lemont Dolphins Swim Team
(Ages 6 - 18)
The Lemont Dolphins Swim Team is a competitive swim program dedicated
to teaching youth to swim and develop skills for competition on the local and
regional level. The Dolphins are committed to supporting all swimmers at
every level of competition, from dual meets to championship level
competitions. The team is open to anyone ages 6-18 who can swim at least
three lengths of the pool or 75 meters. Once the season begins, the coaching
staff will decide group placement based on the swimmer’s age and swimming
ability. Dolphin team members will be instructed on proper strokes,
mechanics, nutrition and best training practices. Please contact Christine

This program builds basic water safety for both parents and children. It helps
infants and younger children ages six months to three years old become
comfortable in the water so they are willing and ready to learn to swim. These

comfort while maintaining a front or back position in the water and
demonstrating breath control, including blowing bubbles or voluntarily

Fireflies Tumbling Team (Ages 3 - 14)
The Fireflies Tumbling Team tumbles October thru February.
There is no charge to join the Fireflies Tumbling Team. Boys
and girls of all ages and levels are eligible to compete. All
tumblers must be enrolled in park district gymnastics or
tumbling classes to participate on Tumbling Team. Meets are
every other week or may be once a month and there is an
additional cost for each meet. Tumblers compete in tumbling
passes, consisting of 5 major skills for their skill level. For more
information regarding the Tumbling Team, contact your child’s
instructor, ask for a tumbling manual or watch for postings on
the gym window or marquee. Mandatory parent meeting on

The Lemont Fireflies are members of the Illinois Park District
Gymnastics Conference and this is a year round gymnastics
program that will be participating in the Optional Level for the
2016-17 season. The Optional season starts Round meets in
January, 2017. Each gymnast is required to compete in five (5)
Round meets, plus one (1) Sectional meet & (1) State meet,
totaling seven (7) meets and most of them will be on Saturday
or Sunday at an additional cost. If you have any questions
regarding our division, see our coaches. Jr. Bronze division will
have a Conference Meet. Must have coach’s consent to join

Girls gymnastics at a young age provides a strong basis for all
sports. It can help your child increase coordination, strength,
flexibility and self esteem. Each class builds up to the next
level: children must master skill in each class before being
promoted to the next level.
Level 1 - Beginner Stars: This is a very beginner level and
some skill requirements are needed to begin class.
Level 2 - Advanced Stars: Forward roll stand-up (without
using hands), bridge, cartwheel (working on), handstand (with
a spotter).
Level 3 - Shooting Stars: Forward and backward rolls,
cartwheels (right & left), handstand hold.
Level 4 - Super Stars: Handstand to backbend, back
walkover, backbend, (working on) backbend to a stand. This
level will qualify you to move up to PRE-TEAM based on
Coach’s approval. Participants must be 5 years old. (TP)

Fireflies Pre-Team (Ages 8 - 14)
Fireflies Pre-Team program is offered to improve and develop
the skills of the young gymnast. This class is for those who like
gymnastics, want to have fun and improve their skills at a
faster pace than if they participated in one class a week.
Gymnasts in this class are required to compete at the
OPTIONAL level. Gymnasts are required to compete in five (5)

River Valley School Gym
This program will provide boys and girls 3rd – 8th grades with
the opportunity to learn and play basketball competitively.
Participants will have weekly games, starting in January and
can expect to have practice one day a week and a 10-game
schedule. Practices begin in November and the season will
run from January through March, 2017. Parents and adult
volunteers can coach, but they too will receive education and
must volunteer for ASEP certification. Games will be officiated
by certified referees. Schedules and scores will be posted on
our web site: www.lemontparkdistrict.org. Pick up a PLAYER
information packet at the Service Desk for grade level you are
playing and return as soon as possible. Available on-line will
be player information packets, player and parent manuals and

This is for NEW coaches who are not certified. Don’t fall
through the cracks, come sign up and coach one of these
teams. We believe every child’s coach needs a good background. Coaches who take this course will automatically go into
our National Coaches Registry which will entitle them to
discounts on books, videos and DVD’s from ASEP and Human
Kinetics. All Coaches will be held accountable for their
behaviors. It will be the Coach’s and the Assistant Coach’s
responsibility to keep their certification current. This certification
is only for those who want to coach in our Recreational Hoops
League.
If you have coached the past three seasons, your certification
has expired and you need to get re-certified for the next three

Pickleball - Adults (Ages 21 +)
Pickleball is a racquet sport which combines elements of
badminton, tennis and table tennis. It is easy to learn, good
exercise and a fun game. The Open Gym Schedule, which
pickleball is included in, is available at The CORE Service Desk
and online on The CORE page at www.LemontParkDistrict.org.
Rates are $2 per resident and $3 per non-resident. Wear
comfortable clothing, gym shoes required. You must bring your

make-up deadline. For your safety, eye guards are
recommended. All matches do NOT have to be played on

PERMANENT COURT TIME
Permanent court time requests for racquetball, wallyball and
handball will be accepted beginning September 1st at 9:00 am
at Centennial Community Center. Requests for permanent
court time will be taken on a first come, first serve basis for the
Sept. 2016 - August 2017 season. Reservations will be taken
year-round with an automatic end date of August 31st.
Reservations must be made in person, form filled out, and full
payment made at the time of registration. All requests can be
tailored to fit your schedule. Permanent court time may last
from 5 weeks to 52 weeks with discounts offered. Scheduled
leagues and park district programs take precedence over
permanent court time requests.

GREEN
Whether its helping preserve a natural habitat
or making recycling available, the Lemont Park
District is connuously working on GREEN iniaves to help the environment!
What is “Being GREEN?”
It means looking at what you do or don’t do and how it impacts the environment around you.
Recycling is a big part of being GREEN. Below are some recycling guidelines.
METALS
GLASS
1. Recycling one aluminum can saves enough energy to run
a TV for three hours or the equivalent of one half gallon of
gasoline.
2. Once an aluminum can is recycled, it can be part of a new
can within six weeks.
3. During the time it takes you to read this sentence, 50,000
12-ounce aluminum cans are made.
4. An aluminum can that is thrown away will still be a can
500 years from now!
1. Every month, we throw out enough recyclable glass
bottles and jars to fill up a giant skyscraper.
2. The energy saved from recycling one glass bottle can run
a 100-watt light bulb for four hours. It also causes 20%
less air pollution and 50% less water pollution than when a
new bottle is made from raw materials.
3. A modern glass bottle would take 4000 years or more to
decompose and even longer if it’s in the landfill.
PAPER
1. More than 20,000,000 Hershey’s Kisses are wrapped each
day, using 133 square miles of tinfoil. All that foil is
recyclable, but not many people realize it.
2. Every week about 20 species of plants and animals
become extinct!
3. McDonald’s saves 68,000,000 pounds of packaging per
year just by pumping soft drink syrup directly from the
delivery truck into tanks in the restaurant instead of
shipping the syrup in cardboard boxes!
4. Rainforests are being cut down at the rate of 100 acres
per minute!
1. To produce each week’s Sunday newspapers, 500,000
trees must be cut down.
2. Recycling a single run of the Sunday New York Times
would save 75,000 trees.
3. If all our newspaper was recycled, we could save about
250,000,000 trees each year!
4. If all Americans recycled one tenth of their newspapers,
we would save about 25,000,000 trees a year.
PLASTIC
1. Plastic bags and plastic garbage thrown into the ocean
kill as many as 1,000,000 sea creatures every year!
2. Americans throw away 25,000,000 plastic beverage
bottles every hour!
3. Recycling plastic saves twice as much energy as burning it
in an incinerator.
Recycling FUN Facts!
Within 10 years, wind power could provide 20% of
America’s power. North Dakota alone could theoretically
produce enough wind-generated power to meet the needs of
more than one fourth of U.S. electricity demand. Off-shore
wind turbines have the potential to produce as much power
as all of the power plants in the United States.

Cancellations
Lemont Park District reserves the
right to cancel, postpone any classes
or rentals, or combine classes as
necessary. Classes may be cancelled
due to low enrollment. Don’t wait
until the last minute to register! All
cancellations for classes or rentals
must be made M-F, 9 am-5 pm, at
least 48 hours prior to the start of a
class. A $4 service fee will be
deducted from all refund requests for
each class that is cancelled. All
facility cancellations are subject to
cancellation fees.

As a service to our CORE Members,
childcare is available at The CORE.

Foundation Helping Hand
Award Program
The Lemont Park Foundation
Helping Hand Program is designed
to provide financial assistance to
residents of the Lemont Park District
in need, to enable them to participate
in the Lemont Park District programs
and/or facilities. The Lemont Park
Foundation, working with the
Executive Director of the Park, will
attempt to provide a scholarship to a
needy individual or family.
Applications are confidential and the
recipient parent or guardian is
required to volunteer their time at a
park district or community event.

Foundation Memorial
Bench/Tree Program
Through the Memorial Tree and
Bench Program, friends and family
can provide a unique dedication to
loved ones by placing a tree or bench
in one of the beautiful parks in the
Lemont Park District. This historical
dedication provides a lasting
remembrance in honor of a loved
one and enhances the meaning of a
tree or bench to observers or park
patrons. Learn more on p.46.

LPD publishes program guides three
times a year: Summer, Fall and
Winter/Spring.

Non-Residents
Lemont residents contribute to the

property taxes. Because of this, they
receive a resident discount rate for
classes, programs and memberships

NSF Checks
Checks returned for insufficient funds
will be assessed a $25 service fee.
Observation Policy
We love parents, but.... We ask that
anyone who is not registered for a
class remain outside of the class
room/gym during the program time.
This allows your child to enjoy class
without distraction.
Participant Illness/Attendance
Guidelines
Participants who have a fever or are
sick should not attend classes or
programs. It is the Lemont Park
District’s goal to provide a healthy,
safe place for our participants and
members. The District asks that all
participants and members be free
from the following symptoms for at
least 24 hours before attending the
District. Some examples of illness
are listed below but not limited to
fever, vomiting/diarrhea, excessive
coughing, green or yellow runny
nose, discharge in or around eyes,
questionable rash or any communicable Disease. District staff are not
medical care providers but reserve
the right to send participants home if
they are showing symptoms of being
ill.
Photo Policy
We like to take pictures of people
having fun! By registering for
classes/programs, joining The CORE
and attending special events at LPD,
you agree to let us use the photos in
our publications, advertisements and
on our web site. If you don’t want
your photo used, you must submit a
written request to the park
administration.
Photo Submissions
If you have great photos of you and
your family enjoying LPD events
and activities, we would love to use
them! Send your pictures (identifying
who, what, where & when) to
the Marketing Dept., 16028 127th St.,

can be submitted to [email protected]
Picnic Permit Reservations
Picnic permit reservations will begin
on January 18, 2016, for the 2016
season. All reservations are taken on
a first-come, first-serve basis.
Reservations must be made in
person. All fees are due when
making reservations. Persons using
Park District picnic sites are required
to provide the Park District with
insurance information regarding any
rentals of inflatable apparatus,
carnival games or other rental items
to be placed on District property.
Refunds
No refunds will be issued after a class
has begun. Participants will receive a
full refund if the park district cancels a
program. Refunds are mailed out only
on the second Tuesday of each month.
Refunds for trips can’t be given once
the registration deadline has passed.
Note: If you cancel a class more than
48 hours before it starts, you will
receive a full refund minus a $4
service fee. The $4 fee will be
charged for each class cancelled.
Resident Rate

Vandalism
The Lemont Park District spends
thousands of dollars a year repairing
damage caused by vandals. Although
we take many steps to prevent these
incidents, we still need your help.

The Lemont Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of
participants in high regard. The Lemont Park District continually strives to reduce such risks and insists that all participants follow safety
rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities
contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered
an illness, injury or impairment, to consult a physician before undertaking any physical activity.
Warning of Risk
Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury
when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must
understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, failure of
supervision, premise defects and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Lemont Park District to guarantee absolute safety.
Waiver & Release of All Claims & Assumption of Risk
Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming
the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might
sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation vehicles when provided).
I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily
agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as
a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or
