The notice period when a request to cancel a service with the O2TV Business tariff without a commitment and without stating a reason by
the Subscriber is three months and it commences on the first day of the month following the delivery of the request to cancel the service.

7. Common provisions for O2 TV tariffs and channel portfolio packages
Provisions:
A. Discounted prices of O2TV digital television service in combination with O2 Internet connection or O2 Voice service are valid
only if the subscriber pay for O2 Internet connection service in correct amount and time. Otherwise the subscriber shall pay
the price shown in the column „O2TV separate“.
B. The digital O2 TV service (with the exception of the O2TV Business tariff) is designed specifically for a personal use of the
Subscriber. Commercial or business use and any public use of the digital O2TV service (with the exception of the O2 TV
Business tariff) is expressly banned. If the Subscriber uses the digital O2TV service for a public broadcasting and they do not
have the O2TV Business tariff arranged, their tariff will automatically get changed to the O2TV Business tariff.
C. The broadcasting hours of Hustler TV, Daring! TV, Dorcel TV, SuperOne, EroXXX, XMO and Dusk are each day from 22:00 to
05:00 resp. from 22:00 to 06:00 (TV Leo). All channels within the Private plus complementary offer contain erotic and sexual
materials. The Subscriber agrees that he/she does not find the materials offensive, disturbing or harmful and that the order
is made voluntarily and also that he/she will not give this material (or allow access to it) to minors or other people who may
find the sexual content offensive, disturbing or harmful.
D. If a subscriber contract for the O2TV service is terminated earlier than the Subscriber has made all payments for the
hardware all outstanding payments will become due and the Subscriber will be obliged to pay the service charges to O2
immediately. If the Subscriber fails to pay any of the payments for hardware all outstanding payments will become due and
the Subscriber will be obliged to pay the service charges to O2 immediately. When requested by the Subscriber O2 will make
it possible for the Subscriber, during the effective period of the contract of O2TV, to pay the outstanding amount of fees for
the hardware.
E. The price for relocation within a building, moving and transfer of the O2 Internet connection service is included in the
monthly price for tariff services.
F. The O2 TV digital television service can be set up only if the subscriber line has sufficient capacity.
G. Subscribers asking for the installation of the Service may be asked by O2 to deposit a certain amount of cash for buying
goods within the Special offer or another discount event. This deposit will be automatically returned to the subscriber as
long as: all the invoices for services delivered in three (3) consecutive months following the payment of the deposit are paid
in correct amount and time, the subscriber has no outstanding debt and his/her service has not been reduced or suspended
(due to payment delinquency) over the recent 12 months.
H. Subscribers buying services whose value exceeds CZK 500 may be asked to deposit an extraordinary amount of cash of the
maximum of 5 times the debt + price of services purchased in the monitored period. This does not apply to amounts below
CZK 500. The extra deposit will be returned following the relevant terms & conditions specified when placing the request for
the deposit.
I. In the case there is reasonable suspicion that the Subscriber may not fulfil his commitments under the Contract and the
General Terms and Conditions (in particular when recorded in the SOLUS register or a similar register or misusing services),
O2 shall have the right to ask the subscriber to deposit a reasonable amount of cash up to CZK 10,000. The deposit will be
returned based on a written request of the subscriber at the earliest 12 months from the completion of the Specifications;
all this is conditioned by the Subscriber’s clean payment history (no overdue payments, no outstanding debt, no reduced /
suspended service due to payment delinquency in the recent 12-month period).
J. All deposits will be returned by the end of the contract period unless used for the settlement of the Subscriber’s debt to O 2.
K. HD is the name of a television broadcasting format (HD stands for High Definition). A TV set with full HD or HD ready uses an
HDMI cable to connect to a set-top-box. The user guide is available at www.o2tv.cz. The superb quality of the HD technology
depends on the adequate quality of the PBX line. Details on service availability can be found at the website of O2
www.o2tv.cz
L. Misuse of a benefit - granted as part of a marketing campaign - means, but not only, the exploitation of benefit primarily in
order to gain financial or non-financial profit for the subscriber or a third party at the cost of O2 or the usage of benefit in
access of the defined limit. In such a case, O2 may apply standard per-unit rates. Misuse of benefit/s is prohibited.
M. For current subscribers with tariff activated to 8th May 2011 are applied prices and conditions stated in Pricelist of O2
Internet connection from 1st May 2013.

N. From 8 February 2014 the subscribers (consumers) who terminate the Special offer service earlier than set in the contract by
notice or after a mutual agreement between the parties may be charged by O2 at 20% of monthly fee (according to current
monthly fee incl. VAT) remaining till the end of the agreed end of contract. The subsribers (non-consumers) who terminate
the Special offer service earlier that set in the contract by notice o rafter a mutual agreement between the parties, O2 is
entitled to charge the subscriber lump sum compensation for early termination of service equal to the sum of the monthly
fees (according to current monthly fee incl. VAT) remaining till the end of the agreed end of contract.
O. If the subscriber using both O2TV (digital) and O2 Internet connection on the same access line (including on the xDSL
technological platform provided by another operator) asks for the change of the internet provider while preserving the O2
TV (digital) service, O2 may agree with the subscriber on the interruption of the O2 TV (digital) service for a time interval
needed for the de-installation of the service from the current provider and the installation of the service to the new
provider. During the interruption, no charges will be applied. Once the change has been implemented, the service will be
fully restored in the original setup.
P. Any TV program package from the additional O2 TV channel offer can be ordered for a minimum period of 30 days. In case
of submit an application for cancellation TV program package from the additional O2 TV channel offer before this deadline,
the package will be canceled after its expiration.
Q. The subscriber is entitled to a discount of CZK 50 incl.VAT for fourth and every other even additional package ordered from
an additional channel offer to O2TV Flexi tariff.
R. Each subscriber, who between October 1, 2014 and February 28, 2015, paid the price more than CZK 400 incl. VAT (CZK
330,58 exl. VAT) / billing period for O2TV digital television service provided via fixed line and established an O2TV provided
by other devices service complementary service in the same period, can use an O2TV provided by other devices service
complementary service within this price, when paying the price more than CZK 400 incl. VAT (CZK 330,58 exl. VAT) / billing
period for O2TV digital television service provided via fixed line.
S. The subscribers, who activated O2 TV Start tariff before February 8, 2015, from April 4, 2015 the O2 TV Start tariff terms will
be changed on O2TV Flexi tariff terms with Styl and Dokumenty packages for CZK 199 incl. VAT (CZK 164,46 excl. VAT) /
billing period. If the subcriber, at his own request after the date of change in the previous sentence, change the service
setting on other than O2TV Flexi with Styl and Dokumenty packages, will pay a standard list price for this contracted services.
T. The change of tariff is possible on a request of the subscriber at the earliest 30 days from the last change of tariff. The
commitment is renewed when the tariff with a commitment is swapped for a lower tariff with a commitment.
U. The subscribers of tariffs offered to June 30, 2015 will be gradually migrated to the tariffs offered from July 1, 2015.

8. Short-term marketing events
1. Kino complementary package at CZK 200 (incl. VAT)
Conditions:
A. Each Subscriber (a natural person registered by their birth certificate number) who activates the add-on bundle Kino with the
O2 TV M or O2 TV L tariffs from July 1 until December 31, 2016, will pay CZK 200 with VAT (CZK 165.29 without VAT)/billing
period for the Kino bundle for the subsidized period.
2. HBO GO
Conditions:
A. Each subscriber, who between July 1 and December 31, 2016, uses packages such as Kino and Kino plus or the O2 TV XL tariff
and O2TV Komfort tariff with the O2TV Kino offer, have the opportunity to use a HBO GO service for the monthly price of the
O2TV (digital) service. HBO GO as a service is not subject to separate charges.
B. HBO GO enables the user to access the HBO TV channel and watch by streaming various movies and film series from the HBO
production on an electronic device such as PC, laptop, tablet or smartphone. The HBO GO can be used on up to 4 electronic
devices. The maximum of simultaneously watched TV programs is two. Users can use the O2TV service via the set-top-box while
watching another TV program on the TV screen.
C. Terms and conditions of HBO GO including data connection parameters can be found at www.hbogo.cz (section called
“Conditions of the use of HBO GO”).
D. Access to Internet is not part of the HBO GO service.
E. Users who wish to use HBO GO must be registered at www.hbogo.cz through a validation code. After re-starting the set-topbox, users can get this validation code from the O2TV menu on the TV screen (section Archive, item HBO GO).
3. Special offer - O2 TV M, L, XL tariffs separately for a beneficial price
Conditions:
A. Each current Subscriber (a natural person registered with their birth certificate number), who, between October 1 and 31,
2016, uses this Special offer and newly activates O2TV digital television service without O2 Internet connection with M, L or XL
tariff with 12months-commitment, gets the following bonus:
B. The participant will pay a special fee of CZK 399 with VAT (CZK 329.75 without VAT)/billing period for the use of the O2TV M
tariff, CZK 499 with VAT (412.40 without VAT)/billing period for the use of the O2TV L tariff, CZK 849 with VAT (701.65 without
VAT)/billing period for the use of the O2TV XL tariff for the time when the Special Offer is valid. The monthly price of the service
includes a monthly price for the add-on O2TV provided on other devices service.
C. The Subscriber can buy a Basic DSL WiFi modem for the price of CZK 500 with VAT (CZK 413.22 without VAT) at the same time
when they activate digital O2TV.
D. The Special Offer is conditioned by the commitment to use the service continuously and to pay for it properly and on time for
the period of 12 months. Once the period of 12 months runs out the commitment is automatically prolonged by the same
period (automatic prolonging). Nevertheless the Subscriber has the right to decline this automatic prolonging at any point in
the future. If the Subscriber refuses the automatic prolonging, they lose their right to claim the benefit provided by this Special
Offer when the commitment period runs out and their tariff is automatically changed to a corresponding tariff without a
commitment. The Subscriber is entitled to the automatic prolonging of the Special offer only in case that they have covered all
the due commitments on the Connection Line where the service is provided.
E. Offer based on these conditions cannot be used by Subscribers who cannot have the service set up for technical reasons at the
time when the request for service activation is submitted.
F. The providing of benefits based on this Special Offer based on these conditions is conditioned by the request for service
activation being submitted properly and on time at O2 in the period specified in Article A. The date when a properly completed
request was submitted to O2 is a decisive factor for the application of the claim.
G. The requirement for granting the advantages based on these conditions is that the Subscriber had not used the digital O2
service for at least 3 months prior to submitting the request for service activation.

B. The subscriber, who will, on a same PBX, use the O2 Internet Connection service, shall pay the price of CZK 299 incl. VAT (CZK
247.12 excl. VAT) / billing period for the use of the O2TV M tariff, the price of CZK 399 incl. VAT (CZK 329.75 excl.VAT) / billing

L. One of the conditions for the awarding of benefits and discounts arising from the offer is that the request for service
installation must be submitted to O2 in time and in a correct manner meeting the deadline specified in point A. The “D” date for

Conditions:
A. All the Subscribers to the O2TV M tariff are entitled to a benefit in the form of a discount worth CZK 100 with VAT (CZK 82.64
without VAT) and all the Subscribers to the O2TV L and O2TV XL tariff are entitled to a benefit in the form of a discount worth
CZK 150 with VAT (CZK 123.97 without VAT) to purchase a WiFi repeater Comtrend WAP 5884 between October 1 and 31,
2016. The discount may be utilized via a discount code that can be requested at brand shops, on a customer care line, or at
www.o2.cz. A Subscriber can apply only one discount during the advantageous period.
11. O2TV free for 2 months
Conditions:
A. Each Subscriber registered with their birth certificate number who is a Subscriber to the O2 Internet Connection service and
who between October 1 and 31, 2016, via offices of Czech Post s.p. defined in paragraph I of this conditions and than via O2
Telesales newly activates a digital O2TV service with a selected O2TV tariff with a commitment to use the service without
interruptions and to pay for it for 12 months and who at the same time purchases a set-top box for CZK 1,999 with VAT (CZK
1,652.10 without VAT) in a one-off payment or in 24 monthly installments, gets the following bonuses:
B. From the day of the service activation (“trial period”) the Subscriber gets a 100% discount on the monthly price of the chosen
O2TV tariff.
C. Once the beneficial trial period runs out, the Subscriber pays a standard pricelist fee for the O2TV service and they can draw it
under standard terms and conditions.
D. The offer under these conditions cannot be used by Subscribers who cannot have the service activated for technical reasons at
the time when the request for service activation is submitted.

to pay for it for 12 months and who at the same time buys a set-top box for CZK 1,999 with VAT (CZK 1,652.10 without VAT) in
24 monthly installments via O2 call centres between October 1 and 31, 2016, gets the following bonuses:
B. The Subscriber gets a 100% discount on the monthly fee for the O2TV L tariff for one month from the service activation (“trial
period”).
C. Once the beneficial trial period runs out, the Subscriber pays a standard pricelist fee for the O2TV L service and they can draw it
D. The Subscriber can rent a set-top box free of charge during the trial period.
E. The Subscriber has the option to terminate the digital O2TV service in the trial period without having to pay the lump sum
compensation for not keeping the commitment and with no notice period by returning the set-top box before the end of the

service without a notice period and at the same time a withdrawal from the contract of purchase. If the Subscriber decides to
return the set-top box in the trial period and to cancel the digital O2TV service this way, they are obliged to return the set-top
box undamaged and with all its accessories.
F. If the Subscriber returns the set-top box damaged or incomplete in the trial period, O2 is entitled to charge the Subscriber a
contractual fine of CZK 1,999.
G. The contract of purchase for the set-top box between the Subscriber and O2 is signed with a suspensory condition that it
becomes valid if the Subscriber does not use the option to return the set-top box within the trial period and thus withdraw
from the contract of purchase. The ownership right to the set-top box of the Subscriber who does not return the set-top box in
the trial period is transferred at the moment the trial period runs out.
H. The offer under these conditions cannot be used by Subscribers who cannot have the service activated for technical reasons at
the time when the request for service activation is submitted.
I. The providing of benefits based on these terms and conditions is conditioned by the Subscriber not having used the digital O2TV
J. This offer cannot be combined with currently available O2 offers connected with the O2 Internet Connection and digital O2TV.
K. To apply the claim for the benefit based on this offer the date when the request for service activation in paragraph A is the
deciding factor.
L. The Subscriber can use the benefit only once.

14. Internet Bundle + TV with tablet
Conditions:
A. This benefit is designed for the subscribers (natural persons registered based on their birth certificate number) who based on
being approached by O2 newly set up the O2 digital TV services with the O2TV M, O2TV L, and O2TV XL tariffs and an O2 Internet
Connection with the Internet Optimal Plus or Internet Aktiv Plus tariffs from October 1, 2016, until October 31, 2016, at the
latest; they will commit to use both the services with the arranged tariffs without any interruptions and to pay for these
services properly and on time for the period of 12 months, they will purchase a set-top box based on standard terms and
conditions and they will activate electronic billing with billing being sent to their email address. O2 will approach those people
interested in a service who will use the Call Me Back button at www.o2.cz and who will submit their contact phone number or
who will check the availability of the Internet Connection or O2 digital TV services and who will fill in all the requested details.
B. An Alcatel PIXI 3 7“ WiFi tablet worth CZK 1,595 with VAT (CZK 1,318.18 without VAT) will be sent to the subscriber who meets
all the terms and conditions based on articles A, H, I, J, and K within 21 days from activating the services based on article A at
the earliest; the tablet will be sent via a courier service to the Subscriber’s address where we will send the self-installing
package as well. The tablet will be sent to the subscriber based on the condition that at the time the tablet is sent the
subscriber properly uses services based on point A and they have not submitted a request to have them cancelled or to
withdraw from the contract for these services. There are only 600 pieces of the Alcatel PIXI 3 7´´ WiFi tablets in stock, once the
stock is used up the Alcatel PIXI 4 7´´ WiFi of the same value will be sent out.
C. If the courier service does not manage to get hold of the customer, the subscriber has the right to request the tablet by
November 15, 2016, at the latest. The courier service will hand over the tablet based on the presentation of two identification
documents, one of these must be the identification card and the other one can be any document with a photograph issued by
the state or a birth certificate. The handing over of the tablet is conditioned by the subscriber’s agreement to have a copy of
their identification card archived and to pay CZK 1 with VAT (CZK 0.83 without VAT) from the standard price of the tablet. The
tablet cannot be handed over to a subscriber who has submitted a request to cancel the services, a request to lower the service
tariff to the Start tariff, or who has submitted a request to withdraw from the services. The subscriber can receive the tablet
only in person, they cannot be represented by a different person.
D. If the subscriber receives the tablet in agreement with article C and they meet the condition to have a subsidized price
acknowledged based on articles E and G, they pay a subsidized price for the tablet, which is by CZK 1,594 with VAT (CZK
1,317.26 without VAT) lower than the standard price of the tablet.
E. The subscriber is only entitled to the subsidized price if they fulfil the commitment to use both the services with the originally
agreed on or a higher internet tariff without any interruptions and to pay for these services properly and on time for a period of
12 months.
F. If a subscriber who received a tablet infringes their commitment to use both the services without any interruptions and with
the internet tariff agreed on when the services were activated and to pay for these services properly and on time for a period
of 12 months, they lose their right to the subsidized price of the purchased tablet and they are obliged to subsequently pay O 2
the amount corresponding to the price subsidy of CZK 1,594 with VAT (CZK 1,317.26) that was provided to them when they
bought the tablet. The subscriber does not lose their right to claim the subsidized price if they increase the tariff of services.
G. If the data in the subscriber’s identification card does not correspond to the data that the subscriber shared with O2 when
ordering the service according to article A, the subscriber loses their right to claim the benefit.
H. The offer subject to the terms and conditions cannot be delivered for Subscribers whose technical conditions disallow the
installation of the service at time of application.
I. The “D” date for the consumption of the benefit is the date of submitting an application of installation services specified in
point A.
J. Providing of the benefit is conditioned by the fact that the subscriber did not use the services specified in point A for at least 3
months prior to submitting the request to have them activated.
K. This Special offer cannot be combined with other benefits and Special offers provided with services specified in point A
excepted Special offer – O2 Internet bundle with autoprolongation.
L. The O2 Mobil service cannot be used to cover the price of the tablet.
M. The subscriber can use the benefit only once.

with Android 4+ or iOS7+ or Windows Phone 8.1. and higher). The maximum of simultaneously watched TV programs is two, on all

F. The Subscriber of the services must have available for set-top-box an internet connection with minimal speed of 6 Mbit/s, for
electronic devices with minimal speed of 0,5 Mbit/s. O2 is not responsible for subscribers hardware and software equipment.
Ensuring of internet connection is not part of O2 TV Air M and O2 TV provided by other devices.
G. Detailed conditions of O2TV Air M and O2TV provided by other devices are described in the commercial terms and conditions of the
O2 Videotéka and O2 TV Air M services, which are available at www.o2tv.cz.
H. O2 provides the subscriber a virtual storage space for recording of television broadcasts acquired by the subscriber for his personal
use.
I. The subscriber can enter a recording through the O2TV menu or web site https://www.o2tv.cz/program/ or O2TV application. The
subscriber can record multiple programs at once. Programs on adult channels, HDTV channel, ČT regional channel cannot be
recorded. All recordings are accessible via TV menu. O2 is not responsible for recordings content acquired by subscriber. O2 is
authorized unilaterally to change a storage capacity, storing recordings time or TV program extent. These changes cannot be
considered as a worsening of terms.
J. The subscribers of O2TV Air tariff activated to November 30, 2015 will be migrated to the new O2TV Air M tariff from December 1,
2015. These subscribers will pay a monthly fee of CZK 249 with VAT for O2TV Air M tariff. The O2TV Air M tariff includes the
program O2TV Sport HD available in HD quality and the programs O2 Info, ČT1, ČT2, Nova, Prima, Prima love, Prima Cool, STV1, JOJ
TV, Markíza, JOJ Plus, Barrandov TV, Kino Barrandov, Fanda, TV8, Filmbox, Nova Cinema, Smíchov, Horor film, Spektrum, Discovery

Installation of terminal equipment by technician
Conditions:
A. In the case of the rental or sale of O2 TV terminal equipment to the subscriber, paid installation of the equipment is undertaken
upon the request of the subscriber. The activities which form part of the installation are as follows:
• the siting and installation of the appropriate adaptor; 1 drilled hole in wall, max. thickness 30 cm;
• installation of Basic or Premium DSL modem (max. 2 m from adaptor);
• installation of set-top-box (max. 10 m from xDSL modem);
• delivery of Ethernet cable of RJ 45end connector, depending on package approximately 10 m;
• presentation of the O2 TV connector function.
B. In the case of the rental or sale of O2 TV terminal equipment to the subscriber and the selection of installation of the equipment
in the form of the self-installation package, the subscriber is delivered the terminal equipment by post, and based on subscriber
request the package contains the following:

conditions, especially due to the failure to pay for provided services, the access will be prevented to all TV channels except for
O2 info and all complementary services will be blocked. Provided that the Subscriber uses other O2 services on the connection
line, the re-activation fee is charged only once.
D. Provided that the Subscriber uses other O2 services on the same connection line, the Written Notification/ Reminder of Overdue
Charges is charged only once.
E. Change in the name of the Subrscriber (natural name, corporate name) or in billing address in Czech republic is free of charge.

A basic technical support includes help with basic installation and settings of a telecommunications device, which enables providing

The above-standard users’ support includes expert consultation on setting up Set-top-box (list of channels, channels shifting, HDMI
settings) or devices, which is not supplied as standard with O2TV service (e.g. devices for wireless set-top-box). The price for the
Above-standard users’ support is billed per consultation concerning one device.
Priority handling of a request for a basic technical support (point 16) in case when the subscriber explicitly asks for it is also
considered an above-standard users’ support.
