Discontinuation of CMFG Loan Protection Insurance
CMFG (Cuna Mutual Financial Group) Life Insurance Company, has made the
decision to exit the Loan Protection Insurance business currently offered to you
through ODJFS Federal Credit Union. Loan Protection insurance is structured as
a monthly policy where the premium paid only covers the next month rather than
building equity over time.
Per the terms of the CMFG contract, the group policy with ODJFS Federal Credit Union will expire on May 31, 2014. This insurance has been provided to you at
no additional cost to cover outstanding loans with the credit union. This loan
product was paid monthly by ODJFS Federal Credit Union to cover its loan portfolio in the unfortunate event of a borrowing member’s death. Please contact

Details on the 2014 Vacation Club
The 2014 Vacation Club checks will be deposited to your Regular Share Account, and available for you to withdraw by
ATM, MTA, mail or in person on Thursday, May 1. Please call the Credit Union at (614) 466-3416, by
April 30th, if you’d like to have a check mailed to you. You can join the Vacation Club, or make
changes to your current Vacation Club account, by completing the form below and returning it to the
Credit Union prior to May 31.
You do not need to complete the form if you do not wish to make changes to your current club account
– your account will automatically renew.
YOUR 2015 VACATION CLUB CONTRACT
By joining the Vacation Club, I understand that there are no withdrawals during the year. There will be one
deposit credited to my Regular Share Account in the month of May, 2015. I may stop deductions at anytime
during the year, but the account will not close until the Vacation Club account is disbursed in May, 2015.
Should I choose to withdraw from or close my Vacation Club account prior to May, 2015, a $25 fee will be
assessed for each early withdrawal.

pay, excluding Free Pays. I understand that any current payroll deduction I have will be increased by the
amount indicated. I further understand that my Vacation Club account will automatically renew each year unless the Credit Union is otherwise notified by me in writing.
