The Chronic Dialysis Treatent Standards were developed in collaboration beteen the members of the Malaysian Societ of Nephrolog, National Kidney Foundation, Minist of Health (MOH) and MSQH. These standards were developed based on the ISQua Accreditation Federation Council principles and philosophy on standards development. The purose of these standards was to ensure safe medical practice, patient safet and qualit serice at the haemodialysis facilities and serices. The standards cover the following areas: Standard 1 : Goverance Standard 2 : Access to Care Standard 3 : Human Resource Standard 4 : Haemodialysis Treatent Standard 5 : Ethical Practice Standard 6 : Prevention and Contol of Infection Standard 7 : Facilities and Equipment Standard 8 : Qualit Improvement Activities List of Clinics with current Accreditation Status as at 31st March 2015 WILAYAH PERSEKUTUAN IMU Specialist Clinic Klinik Kesihatan Putrajaya, Presint 9 SELANGOR Klinik Waqaf An-Nur Bukit Indah Klinik Wakaf An-Nur Ijok Klinik Waqaf An-Nur Sabak Bernam SARAWAK Klinik Waqaf An-Nur Sarawak BRUNEI Penaga Health Centre, Brunei At the accreditation surey, the sureyors identif and commend on areas of excellence as well as highlight any oppornities for improvement, using curently accepted standards as the benchmark. The obserations of the sureyors, together with individual recommendations on the accreditation stats by each member of the surey team are collated by the Chief Sureyor, who will forard this composite result with a final Chronic Dialysis Treatent Surey Repor to MSQH. The final accreditation stats decision is made based on a review of the Chronic Dialysis Treatent Surey Repor and recommendations of the surey team by a panel of to (2) Councillors, selected from the members of the Malaysian Council of Healthcare Standards (Chronic Dialysis), a body chaired by the National Kidney Foundation Malaysia. This will deterine the Accreditation Stats of the organisation. Three categories of Accreditation awards may be awarded, depending on the level of compliance to standards attained by the healthcare facilit which is undergoing the accreditation surey process 4 YEARS ACCREDITATION NON ACCREDITATION DELAYED ACCREDITATION A four-year Accreditation is awarded to those facilities, which in the opinion of MSQH, substantially comply with the MSQH healthcare standards. It should be noted that the healthcare facilit must achieve substantial compliance in all Core Standards and other Safet Standards, to achieve four (4) - year Accreditation. Accreditation cannot be awarded to a facilit in which the sureyors have obsered and repored that a sigificant number of standards are not complied with. Facilities who are not accredited are encouraged to implement the recommendations made in the Surey Repor and to re-apply for surey. It is recommended that a minimum of telve (12) months should elapse, to allow time for remedial actions and rectification works, before the nex surey is underaken. The final award decision and the recommendations are forarded by MSQH to the Chief Executive Officer (CEO), the owner, or the chair of the govering body of the sureyed facilit. Release of any confidential inforation peraining to the facilit will be subject to written consent from the CEO, the owner or the chair of the govering body. A Delayed Accreditation is awarded to those facilities which have met the requirements of most of the standards. A facilit awarded a delayed accreditation is offered the oppornit to undergo a Re-Assessment within the six (6) months period. During the re-assessment, only those areas of deficiency noted in the initial surey are visited. However, this does not exclude visits to other areas deemed relevant by the sureyors. The facilit should have taken action on the recommendations which were recorded by the sureyors at the initial surey, and should achieve substantial compliance to MSQH standards in order to qualif for the four year award.
