ocal business owner and automobile aficionado Dennis Ciotti
had passed his yearly physical with flying colors. So, when a
typical day of shoveling snow and moving heavy auto parts
at his Dover shop turned into a critical health situation, it came as
a shock. Dennis had torn an artery. The extra blood pooling in his
heart sent him into cardiac arrest and in a matter of minutes, his life
was in jeopardy.

Expand Clinical Affiliation
M
assachusetts General Hospital and Wentworth-Douglass
Health System (WDHS) recently announced a new
clinical affiliation agreement that will increase the
availability and accessibility of comprehensive, tertiary health care
services on the New Hampshire Seacoast. The agreement identifies
Wentworth-Douglass as the Primary Clinical Affiliate of Massachusetts
General Hospital on the New Hampshire Seacoast.
The burgeoning relationship between the two institutions began
in 2008 to develop multiple centers of excellence. The new clinical
affiliation agreement, approved last month by the WDHS Board
of Directors, combines the expertise of the two hospitals, offering
patients greater access to a broader range of clinical services and
specialty care, ranging from the routine to the very complex.
“Mass General and Wentworth-Douglass have had a successful
working relationship the past five years. It is that success and the
changing needs of health care along the Seacoast that have inspired
this expanded relationship,” says Peter L. Slavin, MD, president of
Mass General. “We welcome this opportunity to further our clinical
efforts with the Wentworth-Douglass community.”
The clinical affiliation agreement includes the creation of a
joint executive committee and a shared director to oversee daily
operations. Several new clinically affiliated programs will be
developed, including surgical services, cardiovascular services,
digestive health, neuroscience/neurosurgery, population health
management and oncology services.

I
n an effort to provide a location for more
cost-effective, outpatient surgical services
for area residents, the WentworthDouglass Health System has formed a new
company called Wentworth Surgical Center,
LLC. The new company will become a joint
venture with qualified investors (physicians,
podiatrists and dentists) and lease a 6,500
sq. ft. building from The Works Family
Health & Fitness Center to be built this year.
Construction is anticipated to begin in midsummer with completion in late 2013 or
early 2014.

surgery center is the result of the insurance
industry directing their members to
ambulatory surgery centers where the cost
of providing certain procedures (that do not
require an inpatient stay) is less than similar
surgery in a hospital where 24/7 staffing
requirements and overhead costs are higher.

Hospital has recently experienced a decline
in outpatient surgery volume due to the
insurance industry’s reaction to healthcare
reform’s strong focus on cost reduction.
In a letter sent to employees Walker
said, “Over the past two years, WentworthDouglass leadership has been speaking
about the impact of healthcare and payment
reform and the need for our organization
to be prepared for change. Change has
come quickly. The Patient Protection and
Affordable Care Act of 2010 and its effects
are being felt in hospitals across the nation,
in New Hampshire, and at WentworthDouglass Hospital.
“Our recent declines in surgical volume

to centers outside our service area for
their care, leaving our Hospital and our
specialists. In an effort to recapture this
lost volume and keep patients and their

Center for Advanced Endoscopy offers
specialty services
W
entworth-Douglass Hospital’s Center for Advanced Endoscopy delivers specialized care
to patients suffering from disorders of the gastrointestinal tract. All procedures have
the option for moderate or general anesthesia. Our Center uses the guidelines from the
American Society of Anesthesiologists (ASA).
Advanced Endoscopy Procedures:
Endoscopic Retrograde Cholangiopancreatography (ERCP):
This procedure uses a combination of endoscopic and radiological techniques to visualize the biliary
and pancreatic ducts. Patients presenting with signs and symptoms of a malignancy, pancreatitis,
stones lodged in the common bile duct or bile duct disease are good candidates for this procedure.
The process begins with passing an endoscope into the duodenum followed by passing a cannula into
the pancreatic duct or common bile duct. Radiocontrast material is injected through the cannula in
order to visualize through X-ray images the configuration of the appropriate ductal system. Then
appropriate interventions are undertaken to treat the medical problem.
Bravo® pH monitoring:
This diagnostic tool is used to identify the cause of severe heartburn, or Gastrointestinal Reflux
Disease (GERD). Left untreated, this condition can lead to more serious issues such as difficulty
swallowing (dysphagia), narrowing of the esophagus (strictures), or Barrett’s esophagus. A miniature
pH capsule is placed on the esophagus and measures the frequency and duration of acid coming up
from the stomach. This information is recorded to a receiver that is the size of a pager and worn
on a waistband. The test period is 48 hours and when the receiver is returned, the information is
uploaded to a computer and analyzed by the Gastroenterologist. The capsule will naturally fall off the
wall of the esophagus and pass through the digestive tract.
Endoscopic Ultrasonography:
New technology has combined the endoscope with ultrasound ability to enhance visualization of the
GI tract. This procedure results in better-quality resolution, esophageal wall thickness evaluation, and
visualization of the esophagus, stomach, duodenum, colon and pancreas. It has become a valuable
tool in the identification of gastrointestinal cancers and treatment decisions.
Barrx® Endoscopic Endoluminal Radiofrequency Ablation:
Barrett’s esophagus can lead to high-grade dysphagia (difficulty swallowing) and cancer of the
esophagus. A specialized endoscope and the Barrx® ablation catheter will apply a “burn” which
is limited to 1 mm in depth and removes the affected tissue. Current studies have shown that this
technique is successful in up to 70% of patients.
Endoscopic Mucosal Resection of Esophageal Lesions:
This is a technique performed to remove precancerous or early cancerous lesions from the esophagus
endoscopically.

WDH Surgeons remove gallbladders
through a single belly button incision
using a robotic surgical system
A
surgical team at Wentworth-Douglass Hospital performed the first
da Vinci® Single-Site™ Cholecystectomy in New Hampshire in
February. The patient’s gallbladder was removed through one tiny
incision in the belly button, making the procedure virtually scarless.
Led by Patricia Auty, MD, the team removed the gallbladder using
Single-Site™ instruments on a da Vinci® System. The minimally invasive
procedure is traditionally called a laparoscopic cholecystectomy (removal of
the gallbladder). The standard laparoscopic cholecystectomy uses three to
five small incisions to remove the gallbladder. Single incision laparoscopic
gallbladder removal has also been conducted in the past but is difficult to

perform, often adding time to the length of the procedure. Thus, single
incision laparoscopic cholecystectomy has largely been abandoned. Using Single-Site™ robotic
assistance, surgeons are able to remove the gallbladder through an incision of approximately one inch
without adding to the length of the procedure.
The Food and Drug Administration cleared the specialized Single-Site™ instruments for use with
the da Vinci System in December 2011. The robotic surgical system is widely used in complex
minimally invasive surgery. Patients should discuss all the specific risks and benefits of robotics with
their provider before electing to proceed.
Robotic surgery and single-incision surgery are not new, but combining the two to remove the
gallbladder requires additional training and special equipment.
“When the single incision procedure is performed robotically, the ease and precision is enhanced
over laparoscopic methods due to the state of the art instrumentation that the da Vinci System
provides,” says Dr. Auty. “To be the first hospital in New Hampshire and one of the first in Northern
New England to offer this technically advanced surgery demonstrates Wentworth-Douglass’ leadership
in providing patients with the most advanced options for minimally invasive surgery.”
In addition to Dr. Auty, her partners at Seacoast General Surgery, David Coppola, MD, and Joseph
Rodriguez, MD, also perform this procedure.

Patient dives to heal her wound
W
entworth-Douglass Hospital’s Hyperbaric Oxygen Treatment Center
offers comprehensive wound care in conjunction with the Hospital’s
Wound Healing Institute. Hyperbaric oxygen therapy (HBO) improves
the benefits of certain antibiotics, activates white blood cells to fight infection and
promotes the healing process in chronic wounds. HBO is a form of medical treatment
that occurs inside of a chamber pressurized with 100% oxygen and up to 2 and ½ times
atmospheric pressure, the equivalent of diving to a depth of 49 ½ feet in the ocean.
After a series of daily dives at the Hyperbaric Oxygen Treatment Center,
Sandra Pombriant is wound-free and more than pleased with her experience.
“I was a bit scared at first when I looked at the chamber. I had to wear a
johnny, take off my glasses and remove a partial plate. I couldn’t have any
perfume or deodorant or jewelry and I lay inside this glass chamber for 2.5
hours everyday for 40 days (M-F). They have a TV on the outside you can
watch or you can listen to music or just sleep. They gave me blankets if I was
cold and I could talk to the tech. He’d make me laugh. I was very comfortable.
I came out great. I’m healed and I don’t have any more problems.”
Sandra developed a hematoma (a blood filled swelling – bruise) that didn’t
heal properly following surgery for breast cancer. She was referred to the
Wound Healing Institute for treatment where Dr. C.W. Bean cleaned her
wound and suggested HBO treatment. The success of her treatment left Sandra
with one thought, “I would do it all over again, no problem,” she said.
HBO is commonly used to heal radiation damage to soft tissue or bone,
compromised skin grafts and surgical incisions, chronic bone infections
(osteomyelitis) and diabetic foot wounds. HBO is a covered benefit with most

Women’s Life Imaging Center
introduces 3-D Mammography
W
omen’s Life Imaging Center (WLIC), a joint venture between
Wentworth-Douglass Hospital and Frisbie Memorial Hospital, is
the first breast screening center in the Seacoast area to offer one of
the most advanced technologies in mammography today.
Called breast tomosynthesis or 3-D mammography, the new system
produces a three-dimensional view of breast tissue acquired through a series
of low-dose images. The system produces thin slices (images) of the breast,
allowing radiologists to view one-millimeter sections at a time, increasing their
ability to detect cancer and reducing the recall rates.
WLIC radiologist Shannon Nedelka, MD, describes tomosynthesis as a stateof-the-art tool in breast imaging. “Tomo allows us to find smaller and earlier
lesions in the breast that might have been difficult to detect particularly in
patients with dense breast tissue,” Dr. Nedelka explained. “I am pleased it is
now available at WLIC for our community.”
Dr. Nedelka also acknowledged fellow radiologist Burns Foley, MD, for
his service at WLIC and his strong support in developing tomosynthesis
technology at the Center. “Dr. Foley has dedicated many years to providing an
exceptionally high quality service for our patients,” she said. “He has been a
leader in bringing the 3-D technology to WLIC and is continuing to provide his
clinical expertise.”
Radiologist and Medical Director David Greatorex, MD, manages the
operational aspects of the program at WLIC for the radiology practice
and concurs with the benefits of 3-D mammography. “WLIC has offered
exceptional quality care for many years and our group is proud to be part of
this advancement in breast imaging technology,” he said.
The screening process is similar to a traditional mammogram. During
tomosynthesis the x-ray arm of the mammography machine makes a quick
arc over the breast, taking a series of breast images at a number of angles.
This only takes a few seconds. The technologist sends the breast images
electronically to the radiologist at WLIC who can see breast tissue detail in a
way never before possible. Instead of viewing the complexities of breast tissue
in a flat image, the radiologists can examine the tissue a millimeter at a time.
Fine details are more clearly visible, no longer hidden by the tissue above and
below.
Beth Beaudin, Operations Manager of Women’s Life Imaging Center,
emphasized the value of the 3-D technology in early detection. “We are
excited to have this new technology which benefits all mammography
patients, not just those with family history or high risk factors,” she said. “Our
goal is to find breast cancer as early as possible and the greater detail of 3-D
mammography provides the radiologists with the best information to do so.”
Women’s Life Imaging Center has been designated a Breast Imaging Center
of Excellence by the American College of Radiology, a recognition of breast
imaging centers that have earned accreditation in mammography,
breast ultrasound, stereotactic breast biopsy and ultrasound-guided breast

GRANT HELPS KIDS WITH HEART DISEASE
Wentworth-Douglass Hospital & Health Foundation (WDH&HF) Board member Tom Levasseur is the
recipient of a 2013 Quality of Life Award from the Million Dollar Round Table Foundation (MDRT).
The MDRT presented a grant of $5,000 to WDH&HF in his honor for volunteerism on the Board and
his support of Camp Meridian, a three-day overnight camp for children with heart disease.
In a letter to Tom, Tammy Scarponi expressed her deep appreciation for the donations to Camp
Meridian that allowed her daughter Samantha (Sam) and other youngsters with heart disease to
benefit from the annual camp free of charge.
“Through her years at Camp Meridian Sam has learned that she can be anything and do anything despite
her heart condition. She has gained confidence in herself and learned how to solve challenges with her team and
achieved personal and team goals through all the fantastic activities provided.”

their homes and families.
The Annual Report for 2012 also provides information about
$23 million in Community Assistance Programs for adults and
children unable to pay for medical and dental care, transportation
and medications - funded by the Hospital and supported, in part,
by our Foundation’s generous donors last year.
Every gift to our Foundation, no matter how small, makes a
difference in our community. Donations help a family get the
dental care they could not afford if we did not have our Dental
Center or help a child with diabetes go to a camp where they can
learn to enjoy life, share hopes and dreams with other children just
like them. Donations buy expensive, diagnostic equipment to help
our cardiologists see inside the hearts of patients to repair damage
and save lives. Donations fund scholarships for our nurses to learn
better ways to care for patients and keep our Care-Vans on the
road traveling thousands of miles every year to bring patients to
the Hospital for vital treatments. Donations are truly life-giving in
so many ways. Thank you.

The 1906 Heritage Society was formed to honor those that have joined with
Wentworth-Douglass Hospital & Health Foundation to help Wentworth-Douglass
Hospital remain the Seacoast’s leading medical center. Society membership is extended
to individuals who include Wentworth-Douglass Hospital & Health Foundation in
their estate plans. Through their bequests, life income gifts and deferred gifts, members
share in Wentworth-Douglass Hospital’s mission of helping individuals and their

Community Assistance Programs
WDH supports local family through hardship
H
e was in tears because he
was so relieved – his family
qualified for WentworthDouglass Hospital’s Financial Assistance.
“Sometimes you have to bite the bullet
and take on the humility,” said David
Green of Somersworth when describing
how difficult it was to admit he needed
financial help.
It began when David’s wife, Tammy,
started having pain in her knees. “I
needed physical therapy and was
having a little trouble making all the
payments,” said Tammy. “I was so
happy when the awesome and kind staff
in the Financial Assistance Department
at Wentworth-Douglass said they could
help. Nanette, Jae and Amanda were
all so easy to work with and so friendly.
They helped me put together a payment
plan so I could make all my payments.
And I did make all my payments.”
Troubles were compounded when
David and Tammy’s 9-year-old son,
Cameron, fell and broke his arm in 2010
(at the age of 6).

“We were almost done paying our
previous bills and, of course, something
unexpected like this happens,” said Tammy.
But that wasn’t the last unwanted surprise for the Green family.
In January of 2011, David was laid off from his management position in a
home improvement store when the business decided to downsize. As the
sole provider for the family, the loss was devastating.
Once David made the call to the Financial Assistance Department, he felt an
immediate release of stress and pressure. The staff were quick to respond and
walk him through the steps to receive financial assistance.
“No one has ever done anything so nice for me before,” said David. “I was
so grateful. I was literally in tears.”
“After I lost my job, the bills were still rolling in and it was getting really
difficult,” said David. “I did everything I could, working part time when the
opportunity presented itself – but it wasn’t enough.”
David is now managing at a different store and the family is happy and
feeling back to normal. As they prepare for the future and the unknown, the
Green family is thankful to have Wentworth-Douglass Hospital nearby.
“He didn’t want to ask for help at first…you know, pride and all,” said
Tammy. She said that he did the best he could and sometimes things happen
to hardworking people that puts them in tough financial situations. “There’s
nothing wrong with asking for help,” she said.

Dr. Courtney joins Dental Center team
T

one of the Hospital’s Community
Assistance Projects for low-income adults
and children. Elizabeth Courtney, DMD,
MA, is the newest member of the Dental
Center team including Jay Afrow, DMD,
MHA and Glen Gordon, DMD. A graduate
of Boston University, Dr. Courtney comes
to the Center with a background in public
health where she cared for the dental
needs of adults and children.

team work closely with the Hospital
and the Emergency Department to refer
patients with dental pain as quickly as possible to the Dental Center for care –
often the same day.
The Center encourages parents to bring their children in for a first visit
around the age of one to get them used to going to a dentist. “It’s never too
early,” Dr. Courtney said. “The first teeth are in by the age of one and we like
to get a child used to coming to the dentist and have some fun.”

System Expand Clinical Affiliation (continued from cover)
WDHS President & CEO Gregory Walker, FACHE, describes the clinical affiliation with Mass
General as an important component of the hospital’s efforts to give patients a wider range of expert
care in their home community.
“I believe our strong focus on quality coupled with Mass General’s ability to connect to scientific
research and clinical breakthroughs help us deliver exceptional care for Wentworth-Douglass
patients,” Walker says. “We’ve already seen the multiple benefits of this relationship in the areas
of gynecologic oncology, trauma and stroke.”
Stroke Initiative
The relationship between the two hospitals began with the implementation of Mass General’s
TeleStroke program at Wentworth-Douglass Hospital (WDH) and a joint initiative to improve
the timeliness and quality of care for stroke patients. The success of the initiative led to WDH’s
certification by the Joint Commission as an Advanced Primary Stroke Center in 2012 – the first
NH hospital to receive this designation.
Oncology Programs
The second endeavor brought the expertise of John Schorge, MD, chief of Gynecologic Oncology
at Mass General, to WDH to lead the development of the Seacoast Cancer Center’s Gynecologic
Oncology Program. For the first time, women with ovarian and other gynecologic cancers have the
option of advanced subspecialty care at their community hospital including complex, minimally
invasive surgery.
Additionally, the clinical affiliation offers a Mass General certified genetics counselor to work with
patients and their families to assess cancer risks. Psychiatric consultations may be arranged with Mass
General experts in psychiatric oncology via telemedicine, which allows medical information to be
exchanged through an electronic communication system.
Trauma and Acute Surgery Program
WDH and Mass General Trauma and Acute Care Surgery Program is led by Mass General surgeon
Peter S. Hedberg, MD. Hedberg is a member of the Seacoast General Surgery group in Dover and
also serves as WDH’s director of Trauma and Acute Care Surgery. The WDH relationship with Mass
General provides the benefit of expanded access to the latest guidelines and protocols for trauma and
acute care management.
Thoracic Surgery Program
WDH’s clinical affiliation with Mass General supported the creation of the Wentworth Health
Partners’ Thoracic Surgery Specialists practice, led by James Allan, MD, Mass General thoracic
surgeon and Douglas Mathisen, MD, chief of Thoracic Surgery at Mass General. This team, along
with Hedberg provides outpatient and peri-operative care for thoracic surgical patients receiving
treatment at WDH.
The clinical affiliation agreement also facilitates the patient transfer process for highly complex
surgeries that may require a team of specialists at the Mass General campus in Boston.
Cardiac team’s fast response saves Dover man’s life

“At first I felt a sharp pain, but tried to brush it off by taking a break,” Dennis explained. “Soon, I was
short of breath, sweating and feeling a lot of pressure.”
Dennis was discovered by his girlfriend conscious, but lying on the floor at his Defense
Manufacturers company, trying to relieve some of the pain. Within moments, Dover Fire & Rescue
Paramedics arrived, connecting Dennis to monitors that transmitted real-time images directly to the
Cardiovascular Interventional Suite at Wentworth-Douglass Hospital while the cardiovascular team
prepped for his arrival.
Dennis said the situation took a turn for the worse at the hospital when the team rolled him
over and the monitor revealed the torn artery. An expert in high performance products for racing and
sports cars, Dennis knew he was in a race against time… a race for his life.
Dennis was told it was only eight minutes from the time the paramedics had him in the ambulance
until he arrived in the Interventional Suite at Wentworth-Douglass Hospital. Cardiologist Christopher
Lawson, MD, was able to insert a stent and make the vital repairs. Dennis recovered to see one of
the most amazing stories unfold on a monitor. The same technology that helped save his life, gave
him the opportunity to review, with his cardiologist, exactly what had happened. “I actually watched
the video of my heart and artery refilling with blood,” he said. “It was the moment they knew I was
going to make it.”
Dennis’s recovery consisted of three months of cardiac rehabilitation, growing stronger, learning to
eat and exercise right. The day he graduated from “cardio college” he was down 27 pounds. Today,
Dennis has lost 40 pounds and is back to work, feeling great. He doesn’t hesitate to talk about his
respect for the teams at Wentworth-Douglass Hospital.
“They exceeded my expectations for care,” Dennis said. “Everyone was knowledgeable and kind.
They took care of me, and they took care of my family. We were always kept informed and we made
decisions as part of the team. They saved my life—I can’t say enough.”
Shrimp & Cod Stew
Serves: 6 • Total time: 25 minutes
INGREDIENTS:

3 tsp olive oil, divided
1 lb shell-on shrimp (24 to 30 shrimp),
peeled and deveined, shells reserved
2 1/2 cups low-sodium chicken broth
2 shallots, minced
1 1/2 tbsp brown-rice flour
Pinch each ground cayenne pepper, sea salt
and fresh ground black pepper
8 baby red potatoes, scrubbed well and halved
1 12-oz boneless, skinless cod fillet, cut into
1-inch pieces
1 cup low-fat milk
2 plum tomatoes, diced
2 tbsp chopped fresh parsley for garnish
INSTRUCTIONS:
1. In a small saucepan, heat 1/2 tsp oil on
medium. Add shrimp shells and sauté,
stirring occasionally, until pink, about 2
minutes. Add broth and bring to a boil;
reduce heat to medium-low and simmer
for 5 minutes. Drain over top of a large
bowl, reserving broth; discard shells.
2. In a large sauté pan with a tight-fitting lid,
heat remaining 2 1/2 tsp oil on medium high. Add shallots and sauté, stirring
occasionally, until soft and translucent,
about 3 minutes. Stir in flour, cayenne, salt
and black pepper; cook for 2 minutes.
Gradually add reserved broth, whisking
constantly. Add potatoes and bring to a boil;
reduce heat to a simmer, cover and cook for
4 more minutes. Add cod, cover and
simmer for 5 minutes. Add shrimp, cover
and simmer until cod is opaque and shrimp
is pink, 5 to 6 minutes. Add milk and cook
until heated through, about 2 to 3 minutes;
add tomatoes. Spoon into serving bowls and
garnish with parsley, dividing evenly.

an Ambulatory Surgery Center for the Strafford
County area to be built in Somersworth.”
The Wentworth Surgical Center will feature
two operating rooms to provide a wide array
of surgical and procedural services. Cases
performed at ambulatory surgery centers are
strictly regulated, generally are under 90 minutes
with anticipated recovery times less than four
hours. The overall health of the patient is also a
factor in determining eligibility for surgery at an
ambulatory surgery center.
The next steps in the project include finalizing
the design and approval of required permits from
the City of Somersworth.
In the conclusion of his letter to employees
Walker said, “With the help and support of team
members like you, I believe our health system
is well prepared to adapt to ambulatory surgery
and other changes in the healthcare marketplace
while maintaining our leadership role in the
Seacoast for cost-effective, quality care, and
exceptional patient service.”

with speaker Michael Lapsley of South Africa. Michael is the Founder
and Director of the Institute for Healing of Memories in South Africa.
He is an internationally-recognized speaker who offers workshops on
healing that have helped people in Cambodia and Rwanda, as well as
other high-tension areas. He is an Anglican priest who served as chaplain
for the African National Congress. In 1990, he received a letter bomb in the
mail that tore off his hands and blinded him in one eye – out of that chaos
came the Institute for Healing of Memories.
