Changes in the health care industry that occurred in the prior year, most notably with the enactment of federal health care legislation, have a significant impact on all health systems and health care providers. The operational environment for health systems in the United States is ripe with change and challenges. Heritage Valley Health System has taken a proactive approach to these changes and challenges, implementing procedures and processes that preceded the national mandates. Outlined as strategic imperatives, essential working objectives for the Health System, much of what is required now has been operational at Heritage Valley for years. For example, • Processes to increase and have resulted in improvements in Centers for Medicare & Medicaid Services quality scores which will be used in the future to determine hospital payment. • efforts to identify and retain highly skilled physicians and employees and solicit feedback from staff have led to improvements in delivery of quality care and customer satisfaction. • Identifying as a priority and implementing processes to increase the accessibility of patient health information for clinicians have led to Heritage Valley’s critical involvement in the region’s first health information exchange. • Strategies for have led to the opening of medical neighborhoods and physician practices that expand access to health care and improve in the Heritage Valley service area. • The ongoing commitment and adherence to , signified by strong financial ratings from Standard & Poor’s (AA-), Fitch Ratings (A+), and Moody’s Investment Services (A1), has enabled growth and expansion of services as well as improvements to existing Heritage Valley locations and equipment. In addition to these strategic imperatives, the Board of Directors, health system management, physicians and employees are guided by the mission, vision and values that are continually reinforced. Heritage Valley’s strong foundation, current proactive leadership, and strategic plans for the future ensure that it will remain a viable community resource for years to come. Heritage Valley Sewickley, a 186-bed acute care hospital located in Sewickley, PA. 5 The beginning of Heritage Valley Health System spans decades and has roots in many communities in Beaver County and western Allegheny County. Early in the 1900’s many communities maintained their own hospital facilities. Established by esteemed citizens that were concerned for the health and well being of people in the community, these hospitals included Beaver Valley General Hospital, Providence Hospital, Rochester General Hospital, Sewickley Valley Hospital, and United Hospitals. The doctors and nurses at these institutions delivered care to persons arriving at their doors who were in need of immediate treatment. As time evolved, so did the organizations and several in Beaver County merged into The Medical Center, Beaver, which opened in 1980. Through the Board of Directors’ vision to bring the highest quality care to the region, Sewickley Valley Hospital and The Medical Center, Beaver consolidated in 1996 into the Heritage Valley Health System of today. After the consolidation, Heritage Valley remained dedicated to giving people easy access to health care services. Efforts to bring primary and specialty care services into the community have led to the implementation of unique programs that help define Heritage Valley as a true “community” health system. Providence Hospital originated in 1909 in a 16-room mansion received from Beaver Falls banker, H. W. Reeves. United Hospitals, Inc., was the result of a merger between Providence and Beaver Valley General Hospitals in 1965. 8 United Hospitals Inc., (formerly Providence Hospital and Beaver County General Hospital) and Rochester General Hospital merged into the Medical Center of Beaver County in 1972. Groundbreaking for a new Medical Center occurred on a 98-acre site in Brighton Township in 1978. The Medical Center of Beaver County was dedicated on June 4, 1980. Now named Heritage Valley Beaver, it is part of Heritage Valley Health System. ...was the only health care. 9 Change is constant in health care. One critical change is the movement away from providing care only at the hospital. Heritage Valley has long embraced this change as it provides expanded access and care for people served by the Health System. Heritage Valley has more than 88 locations in the community. Many of these locations are the result of innovative delivery ideas, such as Heritage Valley Convenient are walk-in clinics that provide immediate care to persons with minor illnesses and injury, and diagnostic and laboratory services providing state-of-the-art testing close to home. Other locations include primary and specialty care physician offices and services that reach the elderly or those with behavioral health needs. At Heritage Valley, health care has truly come out of the hospital, into the neighborhood. In spring 2012, Heritage Valley Health System opened Heritage Valley Chippewa, a facility that offers a variety of health care services in one location. The concept of this “one-stop” health care experience for patients serves as a hub, or neighborhood epicenter of care. Heritage Valley Chippewa has three Heritage Valley Medical Group primary care physician practices, a Tri-State Pediatric Associates location, diagnostic imaging, including a state-of-the-art MRI, and laboratory draw services. A Convenient are walk-in clinic and Signature Rehab outpatient rehabilitation services will be added in early 2013. Additionally, through an agreement with Children’s Hospital of Pittsburgh of UPMC, pediatric endocrinology, gastroenterology, nephrology, and pulmonary medicine are available – bringing high quality pediatric specialty care into Chippewa Township. In addition, Heritage Valley Moon Township was converted into a medical neighborhood location in 2012 and three other medical neighborhood locations are slated to open in late 2012 or early 2013 in Robinson Township, Edgeworth, and Ellwood City. Heritage Valley Chippewa is one of the first of the Health System’s medical neighborhood initiatives to offer diverse health services at one community location. 10 STRATEGIC FUTURE Heritage Valley Health System’s partnership with Lutheran SeniorLife has resulted in the highly regarded and impactful LIFE Beaver County program. In FY 2012, LIFE Beaver County and the new LIFE Lawrence County, which opened in June 2011, served more than 300 persons. Participants in the LIFE Beaver County and LIFE Lawrence County programs receive comprehensive health care services provided by Heritage Valley Health System. The program coordinates their personal, social and medical needs in a way that enables them to remain in independent living situations. Heritage Valley and Lutheran SeniorLife have also partnered on a new, innovative project to restore individuals traditionally in assisted or nursing care settings, back into the community. The GREEN HOUSE® project is supported by the Robert Wood Johnson Foundation and is leading the transformation of long-term care. On the site of the former Aliquippa Hospital, the GREEN HOUSE® project will feature four self-contained residences, designed like a private home, housing up to ten residents per home, needing skilled level of care. Construction on the GREEN HOUSE® project is slated to start in calendar year 2013. Heritage Valley’s commitment to elderly care through a long-standing partnership with Lutheran SeniorLife has translated into comprehensive health care offered in the community at LIFE Beaver County and LIFE Lawrence County. Neighborhood Health Care… 11 Heritage Valley Health System’s successful retail medicine strategy expanded last year with the opening of a Convenient are clinic in the established Heritage Valley Moon Township building on Thorn Run Road. The opening of the new walk-in clinic was in direct response to the Aliquippa-Ambridge Bridge closure which redirected traffic for most of 2012. Operational and clinical leaders at Heritage Valley came together quickly to open the clinic on March 1, 2012. Like other Convenient are locations, the walk-in clinic is open 7 days per week and has extended hours. Unlike other Heritage Valley Convenient are clinics that are located within area Walmart stores, Convenient are Moon Township is part of an established Heritage Valley medical neighborhood facility which offers primary care offices, laboratory draw, diagnostic imaging, and outpatient rehabilitation services. Heritage Valley Health System initiated a mobile MRI strategy to provide the highest quality imaging services in a variety of community locations. The new Espree Open Bore MRI began operating at Heritage Valley Chippewa in June 2012 and expanded usage to the Heritage Valley Women’s Health Center in Center Township, Heritage Valley Moon Township, and Heritage Valley Robinson Township. The new mobile MRI unit provides high power to achieve quality images and an extra-large opening to accommodate and give patients more space during testing. Integral to the medical neighborhood strategy is the provision of high quality health care services in the community. A new, state-of-the-art mobile MRI unit travels to Heritage Valley Chippewa, Heritage Valley Moon Township, and Heritage Valley’s Women’s Health Center in Center Township. 12 Convenient are, a Heritage Valley walk-in clinic, opened at Heritage Valley Moon Township in 2012, bringing a new service to the location that already offered lab draw services, diagnostic imaging, and physical rehabilitation services as well as several physician practice offices. Susan Frank, CRNP, (above) was integral to the launch of Heritage Valley Convenient are in Moon Township and was a recipient of the 2012 Cameos of Caring Award from the University of Pittsburgh School of Nursing. …increases access. 13 There are iconic images of hospitals from the past -- the doctor’s bag, the nurse’s white hat, and the X-ray films on a light board. As predictable as the imagery is, so are the general themes of operations that have defined the health care industry. Glance back through annual reports and newspaper clippings and a reader will see repeated references to emphasis on quality care, highly trained and skilled staff, technology improvements, and the need to keep costs low while increasing access to care. One of the greatest changes in the delivery of care is how rapidly innovations and improved treatments are realized and the investment it takes to bring advanced treatments into the community. Heritage Valley’s Board of Directors has continually embraced innovation and the impact it has on patient care. In September 1968, Sewickley Valley Hospital opened a 4-bed Coronary Care Unit located in the Intensive Care Unit of the hospital. Watson Home donated the Electroencephalograph (EEG) machine to Sewickley Valley Hospital in exchange for the hospital providing technical staff to perform the examinations. 16 Beaver Valley Providence General Hospital expanded and improved its emergency room service in 1968. Three beds were added for a total of seven and nursing staff was increased from five to twelve. A new waiting area was added and equipment was purchased at a cost of approximately $12,000. Significant advances in the treatment of disease and injury are realized every day, however, there is often a delay between the development of clinical innovation and its implementation on the community level. It is the goal of Heritage Valley Health System to ensure that the most advanced technology and procedures are available to people in the communities we serve. As such, in fiscal year 2012 Heritage Valley spearheaded initiatives to improve equipment as well as increase the efficiency and quality of care, including: early stages of telemedicine implementation; Heart & Vascular expansion with cardiac computed tomographic angiography at the Beaver and Sewickley hospital campuses, and the addition of a second hyperbaric oxygen therapy chamber in Wound are. These efforts coupled with the solidification of relationships with highly-trained physicians ensure that Heritage Valley will remain competitive in the regional health care environment. Marking a significant change at Heritage Valley Health System, its two largest physician practice organizations, Tri-State Medical Group, Inc. and Sewickley Valley Medical Group, Inc. consolidated into Heritage Valley Medical Group, Inc. in January 2012. The new organization is comprised of 117 employed physicians with fifty office locations throughout southwestern Pennsylvania. Physicians Dr. Thomas Michael, medical director of Heritage Valley Wound are, reviews patient information at the control panel of Heritage Valley’s two hyperbaric oxygen therapy chambers located at Heritage Valley Beaver. 18 STRATEGIC FUTURE represent many disciplines and specialties including family practice and internal medicine physicians as well as specialists in cardiology, rheumatology, general surgery, cardiothoracic and vascular surgery, pulmonary, endocrinology, physiatry, gastroenterology, vein medicine and electrophysiology. Heritage Valley Health System has taken a leadership role in the development of Western Pennsylvania’s first regional health information exchange (HIE), ClinicalConnect. Launched in June 2012 with the pilot implementation at Heritage Valley, ClinicalConnect benefits physicians and patients by providing timely access to vital patient information such as allergies, medications, lab results, past hospitalizations, immunizations and other data. This access leads to more informed clinical decisions and reductions in repeat medical testing. The other members of ClinicalConnect are Altoona Regional Health System, Armstrong County Memorial Hospital, Butler Memorial Health System, Excela Health, Jefferson Regional Medical Center, St. Clair Hospital, The Washington Hospital and UPMC. Heritage Valley emergency medicine physicians were the first to implement ClinicalConnect into practice. The HIE provides access to patient medical records at the eight partnering health systems. Ultimately, ClinicalConnect will be made available to all Heritage Valley physicians, clinicians and nurses. Dr. Michael Barton, emergency medicine physician at Heritage Valley Sewickley, is able to access the medical records of patients from regional hospitals participating in the ClinicalConnect health information exchange. Quality care through… 19 Heritage Valley is committed to state-of-the-art diagnostic imaging in order to provide the highest level of care. Heritage Valley Sewickley houses the centralized radiology reading services for the entire health system where review and analysis of diagnostic images is provided 24 hours a day, 7 days a week. All Heritage Valley diagnostic imaging locations have the most advanced digital imaging available. Radiologists are able to read the images round the clock and provide immediate feedback in the most emergent situations. As a result of securing new, advanced equipment, endoscopic and endobronchial ultrasound procedures are offered at Heritage Valley Health System. Endoscopic ultrasound (EUS) and endobronchial ultrasound (EBUS) technology provide high-resolution images and information about the digestive and bronchial tracts and the surrounding tissue and organs. Prior to Heritage Valley Health System’s implementation of endoscopic ultrasound procedures, this type of advanced care could only be found regionally at larger Pittsburgh-based hospitals. Heritage Valley’s expansion into these types of specialty care remains a top priority of the Health System, affording patients the convenience of receiving advanced care in their community. Dr. Edward Estrin and Dr. Justin S. Torok, radiologists from Brighton Radiology Associates, read diagnostic images around the clock in order to give physicians timely information for diagnosing patients. 20 Nurses and CT Technicians at the John “Tito” Francona Heart & Vascular Center in Heritage Valley Beaver prepare a patient for testing. In 2011, Cardiac computed tomography, or Cardiac CT, services were made available at Heritage Valley Beaver and Heritage Valley Sewickley. A collaboration between the Heritage Valley Cardiology and Radiology departments, the Cardiac CT is a painless, non-invasive test that uses advanced imaging in order to reveal important structural abnormalities of the heart that may relate to disease. As part of the test, CT angiography is used to evaluate narrowing, or stenosis, of the coronary arteries. Heritage Valley uses 64-slice CT Scanners with the latest technology that includes reduced radiation dosages and high quality image acquisition and processing. …state-of-the-art technology… 21 The Inpatient Rehabilitation Department at Heritage Valley Beaver helps to rehabilitate a variety of patients including individuals recovering from orthopedic surgeries. Heritage Valley Beaver and Heritage Valley Sewickley have been recognized for high quality knee and hip replacement programs. Quality care and customer satisfaction have long been strategic imperatives at Heritage Valley. However, the June 2012 decision by the United States Supreme Court to uphold the Affordable Care Act (ACA) has made these imperatives critical to the survival of all health systems. One of the significant portions of the legislation is the Medicare Value Based Purchasing (VBP) program. Under VBP, hospital payment from the government is no longer based solely on the quantity, or number of services, provided to a patient. The of care is now factored into how hospitals will be paid for services from the federal government. Although this change represents a significant shift in health care payment, Heritage Valley Health System, through the guidance of its board of directors, has been preparing for this reform for years by remaining focused on quality, safety and customer satisfaction. , an internal newsletter was initiated to communicate changes to physicians and employees and a Quality section was added to the Heritage Valley website to share Heritage Valley’s quality information with the general public. magazine featured Heritage Valley Health System as one of the top 100 IMS Integrated Health Network (IHN)s for 2012 as named by IMS Health. Researchers determine the rankings by compiling a health system’s overall score measuring 33 attributes in eight differently weighted performance categories. Overall integration is the most heavily weighted. Other categories include integrated technology, financial stability, hospital utilization and contract capabilities. Services and access, outpatient utilization and physician services are also measured. 22 This past year, Heritage Valley Health System hospitals were once again named Blue Distinction Centers for Specialty Care®. The designation is awarded by the Blue Cross and Blue Shield companies to medical facilities that have demonstrated expertise in delivering quality healthcare. Heritage Valley Beaver received the designation for Cardiac Care, Heritage Valley Sewickley for Bariatric Surgery and both hospitals were recognized for Knee and Hip Replacement. Aetna, a leading health insurer nationwide, designated Heritage Valley Health System’s hospitals, Heritage Valley Beaver and Heritage Valley Sewickley, as Aetna Institutes of Quality® in January 2012. Heritage Valley Beaver was designated as an Aetna Institute of Quality® Cardiac Care facility for cardiac medical intervention while Heritage Valley Sewickley was designated an Aetna Institute of Quality® Orthopedic Care facility for spine surgery and total joint replacement. Additionally, Heritage Valley Sewickley in collaboration with Hope Bariatrics has been re-certified as a Bariatric Surgery Center of Excellence by the American Society for Metabolic & Bariatric Surgery (ASMBS). The Sewickley campus has a partnership with Hope Bariatrics and its surgeons Geoff Wilcox, M.D. and Mike Felix, M.D. Dr. Richard Begg, interventional cardiologist, in one of the state-of-the-art procedure rooms at the John “Tito” Francona Heart & Vascular Center. More than two dozen interventional and diagnostic cardiologists, cardiothoracic and vascular surgeons and radiologists collaborate on care delivered at Heritage Valley’s Heart & Vascular Center locations. …speed and accuracy of information. 23 In the last century, many hospitals had direct affiliations with religious organizations and missions that focused squarely on helping those most in need. As mergers and consolidations have occurred in recent decades many for-profit and large non-profit health systems have a less obvious focus on this population. For Heritage Valley Health System, the focus to “improve the health and well-being of all people in the communities we serve” has fortunately remained intact. Heritage Valley Health System and the hospitals that preceded it have long been dedicated to the improvement of health for the entire community. Historically, this had meant ensuring that they had the equipment and supplies needed to treat those who came through their doors. Auxiliaries, clubs and foundations comprised of dedicated citizens worked tirelessly to benefit the hospitals. As the decades have passed and technology has advanced, medical care has seen a shift from solely acute care to an increased emphasis on preventive care. Heritage Valley developed a host of community health programs over the years, always examining and listening to the community and then providing services in a way that educate and offer preventive interventions for everyone. In 1905, a Country Fair was held on the grounds of what is now the Sewickley Valley YMCA, directly across from Sewickley Valley Hospital. Funds raised from the fair were donated to the hospital. 24 STRONG FOUNDATIONS Hospital volunteers participated in a variety of programs that improved the health and safety of citizens. Beaver County industries and community members have long supported bringing advanced health care to the region. In 1978, 24 industry gifts, combined with those of the Medical Staff and employees exceeded $2 million toward the construction of the Medical Center of Beaver County. Community bands together… 25 The hospitals had thriving maternity departments throughout the middle decades of the 20th Century. Pictured above is a gathering of nurses and nursing school personnel from 1957. 26 Informational outreach was conducted by the Medical Center of Beaver County at the Beaver Valley Mall in 1974. In 1993, more than 120 local first graders toured the Medical Center, Beaver in an effort to alleviate children’s fears of coming to the hospital. The children toured the emergency, X-ray, and pediatrics departments. …to make hospitals a community asset. 27 As a community health system, the Heritage Valley Board of Directors has put community health at the forefront. The Community Health Services department offers programs, free of charge, to members of the community in need of preventive health care. Funding for these programs comes from the Heritage Valley Beaver Foundation and Heritage Valley Sewickley Foundation or from external grants. The Community Crisis Counseling Service is a new program under Heritage Valley’s Community Health Services Department. The goal of the program is to engage, educate, assist and follow-up with people in the community toward the resolution of a crisis. In FY 2012, 216 participants received support from a trained crisis intervention specialist that provided short term counseling and referral to children, adults and families. For nearly 50 years, Staunton Clinic has provided outpatient and inpatient behavioral health and mental health services to the community. Established with a seed grant from the Staunton Farm Foundation in 1963, this arm of Heritage Valley Health System currently serves more than 11,300 mental health and intellectually disabled clients in Allegheny and Beaver counties. On June 19, 2012, 75 members of the Staunton Farm Foundation, many direct descendents of the foundation’s founder Matilda Staunton Craig, “Aunt Daisie,” visited Heritage Valley’s Staunton Clinic facility in Edgeworth. Through the dedication of the Staunton Farm Foundation and Heritage Valley, thousands of people have received much needed behavioral health care. At a time when other health care organizations have walked away from these important services, Heritage Valley Health System remains committed to the delivery of behavioral health services in the communities we serve. Heritage Valley Sewickley’s Maternity Department secured a grant from the Pennsylvania Department of Health to implement an initiative aimed at increasing breastfeeding among mothers that deliver at the hospital. All labor and delivery nurses will receive continuing education regarding the importance of breastfeeding. They will also learn methods to engage new mothers to be successful with the ultimate goal to increase in-hospital exclusive breastfeeding and breastfeeding initiation rates. 28 STRATEGIC FUTURE Dr. Dan Brooks, vice president of community health services, speaks to descendents of Matilda Staunton Craig, founder of Staunton Farm Foundation, during a visit in June 2012. The Staunton Farm Foundation is a benefactor to Heritage Valley Health System’s Staunton Clinic. Susan Pointer, lactation consultant at Heritage Valley Sewickley, educates and provides guidance to mothers that breastfeed their infants. The Heritage Valley Sewickley maternity department received a grant from the Department of Health to enhance staff education on breastfeeding. Integrated health system… 29 Barbara Brummit, mammography specialist at Heritage Valley Women’s Health Center, went beyond her clinical responsibilities and took on the role of breast cancer awareness educator for the community made possible by a grant from Susan G. Komen for the Cure. Heritage Valley Health System was awarded a grant from the Pittsburgh Affiliate of Susan G. Komen for the Cure to expand mammography services to women in Allegheny and Beaver counties. The funding supports Heritage Valley’s All About Breast Care (AABC) program whose goal is to increase breast health and screening mammography knowledge as well as screening mammography compliance for health disparate populations within the Heritage Valley service area. The AABC program provides education to groups regarding the importance of mammography screening. Presentations, delivered by nurse practitioners and nurses, are tailored to each group’s need. The program – education and mammography services – are open to all women, ages 40 to 69, regardless of whether or not they have health insurance, throughout calendar year 2012. Life mart, a program for people at-risk for developing diabetes, received renewed funding from UPMC Health Plan in FY 2012. The program is anchored by a 12-week group lifestyle intervention which teaches participants proper approaches to nutrition and physical activity and helps them realize weight loss that has a significant impact on their health. The evidence-based initiative is based on research done at the University of Pittsburgh Diabetes Prevention Support Center. 30 Life mart, a healthy lifestyle program for adults with pre-diabetes or metabolic syndrome, represents a successful partnership between Heritage Valley Health System and UPMC Health Plan. The support of those attending events enables Heritage Valley to raise funds for facility improvements, purchase of additional state-of-the-art equipment and fund a vast array of Community Health Services programs that are offered to the community at no charge. The following are pictures from annual events such as the Heritage Valley Sewickley Auxiliary Unique Boutique, Heritage Valley Beaver Holiday Ball, Heritage Valley Sewickley 5K “Heart and Sole” Run and Walk, An Evening of Holiday Shopping, the Heritage Valley Sewickley Card Party, as well as, Heritage Valley’s biennial “A Toast to Your Health” Wine and Culinary Event. More than 50 unique artisans and vendors showcased their wares in Heritage Valley Sewickley Auxiliary’s Unique Boutique held on November 12, 2011 at the Edgeworth Club in Sewickley. More than 500 people attended the event and helped raise $15,000 to support Heritage Valley Health System’s Surgical Waiting Room Renovation project. 38 The Heritage Valley Beaver Auxiliary held its annual Holiday Ball on November 12, 2011 at The Club at Shadow Lakes.
