five key areas known to reduce avoidable readmissions. Hospitals working on this topic
will focus on improving the use of medications for the patient's condition and ensuring

The Owatonna SeniorPlace is seeking “experienced” members of the community that may be interested in
joining our Police Reserve Division. Our police reserve unit is not for pre law enforcement people, nor is it a
physically demanding position. It is a way to help your community, volunteering time, assisting the police
department with a multitude of assignments or light duties. Our volunteers are provided with full police
reserve uniforms and we provide city vehicles for our reserves to drive. The reserve members are tasked
with “police reserve Details”, such as conducting house checks for residents who register with the police
department when they are gone for a period of time, assisting regular officers with light duty prisoner
transports, light traffic control for parades and light security details to name just a few of the duties. Contact
SeniorPlace to get your name on the list today!

Aging Mastery Program (AMP)
The Owatonna SeniorPlace has been
selected to participate in the Aging
Mastery Grant Program®. AMP is a comprehensive and fun approach to
aging well that encourages people to take actions to enhance their health,
financial well-being, social connectedness, and overall quality of life.
Central to the AMP philosophy is the belief that modest lifestyle changes
can produce big results and that people can be empowered and supported
to cultivate health and longevity. Equally important, the program
encourages mastery—developing sustainable behaviors over time.
Participants in AMP go through a 10-week core program followed by
optional elective classes and activities. We are looking forward to rolling
out this NEW program in March of 2016. We hope that you will join us in
taking small steps for lifelong rewards.

The Owatonna SeniorPlace was awarded a Statewide
Health Improvement Grant in partnership with Straight
River Sports. With that funding the SeniorPlace was
able to purchase an electric assist bicycle (above) that
will be available to rent starting Spring 2016. Straight
River Sports is also willing to provide SeniorPlace with
an additional standard bicycle to rent them out as a
pair.

KWIK TRIP Fundraising
There are two programs that are fundraisers through Kwik Trip: Collecting Milk Moola caps, tops of milk bags & the
oval price tag on the tops of glazed donut boxes. As a non-profit agency, it is a great opportunity for members and
the community to help us out. We get .5 cents for each cap & top and .10 cents for each Glazer’s donut price
collected. Drop-off the milk caps or donut price tags at SeniorPlace anytime! Thanks to everyone who helps with
this program!!!
Cooperative Living
The Statewide Health Improvement Program (SHIP)
works to help Minnesotan’s live longer, healthier, lives
by decreasing obesity and tobacco use and exposure,
the leading cause of chronic disease, disability, and
death. SHIP strategies include changing established
systems to make it easier to make healthy choices,
incorporating health into organizational policies and
changing the environments in which we live, work,
learn, and play in.
